1.Exploring the mechanism and treatment principles of testicular radiation injury from the perspective of "the struggle between vital qi and pathogen" theory

Testicular radiation injury is a structural and functional abnormality of the testes caused directly or indirectly by radiation, which disrupts spermatogenesis and compromises male fertility. The development of effective preventive and therapeutic interventions is essential because of the high prevalence of this condition in clinical settings and its profound effect on patients′ reproductive health and overall well-being. The concept of "the struggle between vital qi and pathogen" is first seen in the Treatise on Cold Pathogenic Diseases. It denotes the dynamic struggle between vital and pathogenic qi. The occurrence, development, and sequelae of all diseases reflect this ongoing conflict. In this context, this study defines the "vital qi" of the testis as its capacity to generate and preserve the essence of reproduction and to resist damage. The pathogenic qi associated with testicular radiation injury is categorized into two types: ionizing poison and retaining evil. The pathogenesis of testicular radiation damage is delineated into three stages by integrating the characteristics of vital and pathogenic qi: the injury, adhesion, and recovery phases. Based on the theoretical framework advanced by this study, the therapeutic approach for testicular radiation injury should adhere to the fundamental principle of strengthening vital qi and eliminating pathogenic factors. Although the primary focus of treatment should be on strengthening vital qi, it should also be complemented by strategies to eliminate pathogenic influences. This paper aims to provide a novel perspective and strategic approach to the traditional Chinese medicine diagnosis, prevention, and treatment of testicular radiation injury. By elucidating the process of testicular radiation injury and its corresponding treatment principles, it seeks to offer valuable insights for clinical practice.
2.Plasma miRNA testing in the differential diagnosis of very early-stage hepatocellular carcinoma: a multicenter real-world study

Objective To explore the application of plasma 7 microRNA (miR7) testing in the differential diagnosis of very early-stage hepatocellular carcinoma (HCC). Methods This study is a multicenter real-world study. Patients with single hepatic lesion (maximum diameter≤2 cm) who underwent plasma miR7 testing at Zhongshan Hospital, Fudan University, Sir Run Run Shaw Hospital, Zhejiang University School of Medicine, Anhui Provincial Hospital, and Peking University People’s Hospital between January 2019 and December 2024 were retrospectively enrolled. Patients were divided into very early-stage HCC group and non-HCC group, and the clinical pathological characteristics of the two groups were compared. The value of plasma miR7 levels, alpha-fetoprotein (AFP), and des-gamma-carboxy prothrombin (DCP) in the differential diagnosis of very early-stage HCC was evaluated using receiver operating characteristic (ROC) curves and area under the curve (AUC). In patients with both negative AFP and DCP (AFP<20 ng/mL, DCP<40 mAU/mL), the diagnostic value of plasma miR7 for very early-stage HCC was analyzed. Results A total of 64 528 patients from 4 hospitals underwent miR7 testing, and 1 682 were finally included, of which 1 073 were diagnosed with very early-stage HCC and 609 were diagnosed with non-HCC. The positive rate of miR7 in HCC patients was significantly higher than that in non-HCC patients (67.9% vs 24.3%, P<0.001). ROC curves showed that the AUCs for miR7, AFP, and DCP in distinguishing HCC patients from the non-HCC individuals were 0.718, 0.682, and 0.642, respectively. The sensitivities were 67.85%, 43.71%, and 44.45%, and the specificities were 75.70%, 92.78%, and 83.91%, respectively. The pairwise comparison of AUCs showed that the diagnostic efficacy of plasma miR7 detection was significantly better than that of AFP or DCP (P<0.05). Although its specificity was slightly lower than AFP and DCP, the sensitivity was significantly higher. Among patients negative for both AFP and DCP, miR7 maintained an AUC of 0.728 for diagnosing very early-stage HCC, with 67.82% sensitivity and 77.73% specificity. Conclusions Plasma miR7 testing is a potential molecular marker with high sensitivity and specificity for the differential diagnosis of small hepatic nodules. In patients with very early-stage HCC lacking effective molecular markers (negative for both AFP and DCP), miR7 can serve as a novel and effective molecular marker to assist diagnosis.

4.Research progress on effects of spaceflight environmental factors on immune function and corresponding Traditional Chinese Medicine protection

With the advancement of space exploration missions,space station operations have entered a routine phase,where single-flight missions last up to six months.Previous studies indicate that the spaceflight environment severely compromises the immune system,increasing the susceptibility to diseases.As primary responders to pathogenic challenges,immune cells exhibit exceptional sensitivity to gravitational alterations and background ionizing radiation in space,with their dysfunction being a critical health risk.For innate immunity,the complex space environment disrupts macrophage polarization and phagocytosis,neutrophil chemotaxis and killing,natural killer(NK)cell cytotoxicity,and dendritic cell maturation,leading to functional impairment of innate immune defenses.For adaptive immunity,microgravity and radiation induce cellular immune dysregulation by suppressing T cell proliferative capacity and perturbing Th1/Th2/Treg subset balance,while simultaneously undermining humoral immunity through interference with B-lymphocyte protein synthesis,blockade of developmental maturation,and reduction of effector B cell populations.This review summarizes recent advances in understanding space environment-induced immune perturbations and protection options using traditional Chinese medicine,focusing on microgravity and radiation.It not only deciphers molecular mechanisms underpinning immune cell dysfunction but also provides a theoretical foundation and drug-targeting strategies for developing countermeasures against spaceflight-specific immune dysfunction.

Identifying the compound formulae-related xenobiotics in bio-samples is full of challenges.Conventional strategies always exhibit the insufficiencies in overall coverage,analytical efficiency,and degree of automation,and the results highly rely on the personal knowledge and experience.The goal of this work was to establish a software-aided approach,by integrating ultra-high performance liquid chromatography/ion-mobility quadrupole time-of-flight mass spectrometry(UHPLC/IM-QTOF-MS)and in-house high-definition MS2 library,to enhance the identification of prototypes and metabolites of the compound formulae in vivo,taking Sishen formula(SSF)as a template.Seven different MS2 acquisition methods were compared,which demonstrated the potency of a hybrid scan approach(namely high-definition data-independent/data-dependent acquisition(HDDIDDA))in the identification precision,MS1 coverage,and MS2 spectra quality.The HDDIDDA data for 55 reference compounds,four component drugs,and SSF,together with the rat bio-samples(e.g.,plasma,urine,feces,liver,and kidney),were acquired.Based on the UNIFI? platform(Waters),the efficient data processing workflows were estab-lished by combining mass defect filtering(MDF)-induced classification,diagnostic product ions(DPIs),and neutral loss filtering(NLF)-dominated structural confirmation.The high-definition MS2 spectral li-braries,dubbed in vitro-SSF and in vivo-SSF,were elaborated,enabling the efficient and automatic identification of SSF-associated xenobiotics in diverse rat bio-samples.Consequently,118 prototypes and 206 metabolites of SSF were identified,with the identification rate reaching 80.51%and 79.61%,respectively.The metabolic pathways mainly involved the oxidation,reduction,hydrolysis,sulfation,methylation,demethylation,acetylation,glucuronidation,and the combined reactions.Conclusively,the proposed strategy can drive the identification of compound formulae-related xenobiotics in vivo in an intelligent manner.

Bamboo jar therapy of Zhuang medicine is a combination of cupping therapy,acupuncture and hot compress of medicine,which is simple,convenient and cost-effective.There are differences in the production of bamboo cans(species,acquisition cycle,diameter,tank length,wall thickness),liquid medicine,and operation process among different regions.These three aspects hinder its wide application and promotion.Therefore,it is particularly important to formulate unified practice standards and norms.Based on the combination of literature research and clinical experience,this paper proposes a standardized draft to respond to the above three perspectives.The authors would like to provide a reference for doctors to understand and master this therapy,so that patients can enjoy this therapy.
7.Mechanism of electroacupuncture on improving corneal inflammation in dia-betic dry eye rats by regulating Toll-like receptor 4-mediated inflammatory signaling pathway

Objective To observe the effect of electroacupuncture on corneal Toll-like receptor 4(TLR4)-mediated inflammatory signaling pathway in diabetic dry eye rats and to explore the mechanism of electroacupuncture in the treat-ment of diabetic dry eye.Methods A type 2 diabetic rat model was established in 32 healthy male Sprague-Dawley(SD)rats by intraperitoneal injection of streptozotocin(30 mg·kg-1)for 12 weeks after feeding with high-sugar and high-fat di-et for 4 weeks.Twenty-five successfully modeled diabetic dry eye rats were randomly divided into the model group(non-in-tervention),electroacupuncture group(the"Jingming","Cuanzhu","Sizhukong","Taiyang"and"Tongziliao"acupoints were treated with acupuncture,and then"Cuanzhu"and"Tongzilao"acupoints were treated with electroacupuncture,15 min for each time,once a day),sham acupuncture group(blunt-tip needle pricking was performed at the same acupoints as the electroacupuncture group),and fluorometholone group(1 g·L-1fluorometholone eye drops were used in both eyes at 8 o'clock,13 o'clock,and 18 o'clock,1 drop each time),with 6 rats in each group,lasting for 2 weeks.Another 6 healthy male SD rats were selected as a blank group.Random blood glucose,tear film breakup time(BUT),tear secre-tion,corneal fluorescein staining(FL)score,and corneal touch threshold(CTT)of rats in each group were detected be-fore modeling,after modeling,and after the intervention.Hematoxylin and Eosin(HE)staining was performed to observe the corneal morphologic changes in each group.Immunofluorescence histochemical staining was adopted to detect the cor-neal TLR4-positive expression in each group.The TLR4,phosphorylated nuclear factor-kappa P65(P-NF-KB P65),inter-leukin(IL)-1β,and IL-18 expression levels in the cornea were detected by Western blot.Results After modeling,com-pared with the blank group,BUT,tear secretion and CTT decreased and FL increased in all experimental groups,and the differences were statistically significant(all P<0.01).After the intervention,compared with the model group,FL de-creased,and BUT,tear secretion and CTT increased in the electroacupuncture group,and the differences were statistically significant(all P<0.05).Corneal HE staining showed that after the intervention,the corneal surface of rats in the model group and sham acupuncture group was not smooth,and the corneal epithelial cells were thickened and disorganized;the corneal surface of rats in the electroacupuncture group and fluorometholone group was smooth,and the corneal epithelial cells were arranged neatly.After the intervention,compared with the blank group,the corneal TLR4 expression of rats in all other groups was elevated,and the differences were statistically significant(all P<0.05);compared with the model group,the corneal TLR4 expression of rats in the electroacupuncture group and fluorometholone group was reduced(both P<0.01).Compared with the blank group,corneal TLR4,P-NF-κBP65,IL-1β and IL-18 protein expressions increased in the model group and sham acupuncture group(all P<0.05);compared with the model group,these expressions decreased in the electroacupuncture group and the fluorometholone group(all P<0.05).Conclusion Electroacupuncture can im-prove the ocular surface signs and inhibit the expressions of TLR4,P-NF-κB P65,IL-1 β,and IL-18 in the cornea of type 2 diabetic dry eye rats,and its mechanism of action may be related to the regulation of TLR4-mediated inflammatory signaling pathway,which inhibits ocular surface inflammation in diabetic dry eye rats.
8.Computer-aided diagnosis for prostate cancer detection in multiparametric MRI:a multireader multicase study

Objective To investigate the effect of computer-aided diagnosis(CAD)as a"second reader"integrated in prostate cancer(PCa)multiparametric magnetic resonance imaging(mpMRI)on the diagnostic performance.Methods Sixty-four patients were recruited retrospectively.Six readers were divided into 3 groups according to the experience.Unknown the pathologic diagnosis,readers graded according to prostate imaging reporting and data system(PI-RADS)v2.1 and accepted or rejected the revision of PI-RADS v2.1 scores by CAD.Using pathology as the gold standard,the diagnostic efficiency,diagnostic confidence and reading time of PI-RADS v2.1 were compared before and after CAD assistant.Results On the level of lesions,without CAD,the area under the curve(AUC)of reader A1 was higher than B1,C1 and C2,and the AUC of reader A2 was higher than C1 and C2,the difference was statistically significant(P<0.05).While the difference of AUC among other readers were not significant(P>0.05).With CAD,the difference of AUC among all readers were not significant(P>0.05).On the level of patients,the mean AUC of all readers with CAD[0.903,95%confidence interval(CI)0.848-0.958]was higher than that without CAD(0.863,95%CI 0.800-0.929),the differ-ence was statistically significant(F=6.52,P=0.040).The difference of AUC between without and with CAD assistance was-0.040(95%CI-0.078--0.003),standard deviation was 0.016,the difference was statistically significant(t=-2.55,P=0.040).The diag-nostic confidence of readers was improved significantly with CAD(P<0.05).Besides,the average reading time of each case required an additional 0.8 min.Conclusion After integrating CAD into PCa mpMRI diagnosis as a"second reader",the performance of less experienced readers can be improved and show similar diagnostic efficiency with experienced readers.

Purpose/Significance To explore the ways for hospital libraries to provide subject services based on mapping knowledge do-mains.Method/Process The paper summarizes subject services implemented by hospital libraries based on mapping knowledge domains from aspects of reference consulting service,information attainment education service,discipline resource construction service,scientific re-search support service,management and decision-making support service.It elaborates on concrete practice of the library of The Fourth Hospital of Hebei Medical University based on mapping knowledge domains,including improvement of professional attainment of discipline librarians,information attainment education of readers,and scientific research support service.Result/Conclusion Hospital librarians should improve their ability to interpret the mapping knowledge domains,and comprehensively use the visual analysis function of database documents and artificial intelligence scientific research assistance tools to ensure high-quality development of subject services.
10.Accuracy of inferior vena cava parameters measured by ultrasound in predicting post-induction hypotension in patients undergoing general anesthesia: a meta-analysis

Objective:To evaluate the accuracy of inferior vena cava (IVC) parameters measured by ultrasound in predicting post-induction hypotension(PIH) in patients undergoing general anesthesia using a meta-analysis.Methods:Databases such as China National Knowledge Infrastructure, Wanfang, China Science and Technology Journal Database, PubMed, Cochrane Library and Embase were searched for diagnostic clinical trials that involved IVC parameters such as IVC collapsibility index (IVCCI), maximum and minimum IVC diameters (dIVCmax, dIVCmin) measured by ultrasound that predicted PIH from inception to May 2022. Primary outcomes were sensitivity, specificity, and the area under the receiver operating characteristic curve of each parameter. The meta regression analysis and subgroup analysis were used to explore the sources of heterogeneity.Results:Nineteen articles were ultimately included ( n=1 750). The sensitivity and specificity of IVCCI in predicting PIH were 0.73 and 0.82 respectively, and the area under the summary receiver operating characteristic curve(AUSROC) was 0.85, and the prediction threshold of IVCCI was (42.5±6.8)%. The sensitivity and specificity of dIVCmax in predicting PIH were 0.46 and 0.74 respectively, and the AUSROC was 0.64. The sensitivity and specificity of dIVCmin in predicting PIH were 0.69 and 0.66 respectively, and the AUSROC was 0.72. In subgroup analysis, the AUSROC of IVCCI in predicting PIH in etomidate subgroup and non-immediate measurement before induction subgroup was 0.92 and 0.71 respectively. The AUSROC of IVCCI was all between 0.8 and 0.9 in propofol group, fentanyl group, sufentanil group, ARB/ACEI group, unlimited surgical types group, and combined with vascular diseases group and other subgroups. Conclusions:IVCCI measured by ultrasound can moderately predict PIH, and dIVCmax and dIVCmin have low accuracy in predicting PIH. Immediate measurement before induction and use of etomidate can improve the predictive accuracy of IVCCI. The predictive accuracy of IVCCI is not affected by the type of opioid drugs, type of surgery, use of ARB/ACEI antihypertensive drugs or vascular diseases.
