1.A predictive model for bowel preparation quality and mild adverse events in colonoscopy

Objective To construct a predictive model to explore the factors influencing bowel preparation for colonoscopy and the risks of mild adverse events during colonoscopy.Methods A total of 573 patients under-going colonoscopy at the digestive endoscopy center of the First Affiliated Hospital of the Air Force Military Medical University from July 2021 to June 2023 were enrolled in this prospective study.The patients were divided into an adequate group(n=112)and an inadequate group(n=461)based on bowel preparation assessed with the Boston Intestinal Readiness Score Scale(BBPS).Again,they were divided into an occurrence group(n=106)and a non-occurrence group(n=467)based on minor adverse events within 30 days after colonoscopy.Multivariable logistic regression was utilized to identify independent predictors of inadequate bowel preparation and minor adverse events after colonoscopy;model discrimination was quantified with receiver-operating characteristic(ROC)curve analysis,and the derived coefficients were used to construct a clinically applicable risk-scoring system.Results The adequate group achieved a significantly higher BBPS total score than the inadequate group(P<0.05).The 30-day cumulative incidence of minor adverse events after colonoscopy was 18.5%.Inadequate bowel preparation was associated with a markedly higher event rate(36.61%)than adequate preparation(14.10%;P<0.05).Multivariable logistic regression identified the following independent predictors of inadequate bowel preparation:age≥60 years,body-mass index≥28 kg/m2,history of constipation,diabetes mellitus,concomitant calcium-channel-blocker use,and Bristol Stool Form Scale types 1~2(all P<0.05).ROC analysis confirmed that age,constipation history,diabetes,calcium-channel-blocker use,Bristol stool type,and their combined model were all significant predictors(P<0.05),whereas BMI alone was not(P>0.05).Independent risk factors for post-colonoscopy minor adverse events were age≥60 years,presence of≥1 comorbidity,performance of biopsy or polypectomy,and warfarin use(all P<0.05).ROC analysis demonstrated significant predictive value for age,number of comorbidities,biopsy/polypectomy,warfarin use,and their combined model(P<0.05).Odds ratios derived from the multivariable models were converted into weighted scores;the resulting composite risk scale ranged from 0 to 100.In the validation cohort,this combined score predicted both inadequate bowel preparation and subsequent minor adverse events(P<0.05),with an area under the curve of 0.880(95%CI not shown),sensitivity of 0.829,and specificity of 0.707.Conclusion Inadequate bowel preparation and post-colonoscopy minor adverse events are inter-related.Independent predictors of inadequate preparation are age≥60 years,BMI≥28 kg/m2,constipation,diabetes mellitus,concurrent calcium-channel-blocker use,and Bristol Stool Form Scale types 1-2.Independent predictors of minor adverse events are age≥60 years,≥1 comorbidity,biopsy or polypec-tomy,and warfarin use.Targeted preventive measures against these factors should be implemented to improve clinical outcomes.
2.Effects of periodontal ligament stem cells-derived exosomes on biological characteristics of periodontal ligament stem cells in an inflammatory environment

BACKGROUND:In recent years,the application of exosomes of periodontal ligament stem cells in periodontal tissue regeneration engineering has been widely studied,but the effect of exosomes on periodontal ligament stem cells derived from inflammatory environment is still unclear.OBJECTIVE:To investigate the effects of exosomes secreted by periodontal ligament stem cells from healthy and inflammatory environments on the proliferation and differentiation of periodontal ligament stem cells from inflammatory environments.METHODS:Human periodontal ligament stem cells from healthy and inflammatory tissues were isolated and cultured by enzyme digestion method.Exosomes were extracted from two kinds of periodontal ligament stem cells using ultracentrifugation.Passage 3 periodontal ligament stem cells derived from inflammatory tissue were selected and cultured in three groups.Cells in the blank group were cultured routinely.The healthy exosome group was added with exosomes secreted by peripheral ligament stem cells derived from healthy tissue.The inflammatory exosome group was added with exosomes secreted by human periodontal ligament stem cells derived from inflammatory tissue.Cell proliferation and cloning were detected.The expression of alkaline phosphatase,the formation of mineralized nodules,and the expression of mRNA and protein of genes related to osteogenesis were detected under osteogenic differentiation.RESULTS AND CONCLUSION:(1) CCK-8 assay and clonal formation test showed that compared with the blank group,two kinds of exosomes could promote the proliferation and colony formation of periodontal ligament stem cells from inflammatory tissue (P<0.05),and the effect of the healthy exosome group was stronger than that of the inflammatory exosome group (P<0.05).(2) Alkaline phosphatase and alizarin red staining showed that compared with the blank group,the two kinds of exosomes could promote the expression of alkaline phosphatase and the formation of mineralized nodules in periodontal ligament stem cells from inflammatory tissue,and the promoting effect of the healthy exosome group was stronger than that of the inflammatory exosome group.RT-PCR and western blot assay showed that compared with the blank group,the two kinds of exosomes could promote the expression of alkaline phosphatase,RUNX2,and type Ⅰ collagen mRNA and protein in periodontal ligament stem cells from inflammatory tissue (P<0.05).The promoting effect of the healthy exosome group was stronger than that of the inflammatory exosome group (P<0.05).(3) The results showed that exosomes secreted by human periodontal ligament stem cells could promote the proliferation and osteogenic differentiation of periodontal ligament stem cells derived from inflammatory environments,and the promoting effect of exosomes secreted by human periodontal ligament stem cells derived from healthy tissues was better than that from human periodontal ligament stem cells derived from inflammatory tissues.

Objective To observe the ultrasonic manifestations of aggressive angiomyxoma(AAM).Methods Four patients with newly diagnosed AAM and 3 with recurrent AAM confirmed by pathology were retrospectively enrolled,and the ultrasonic manifestations were analyzed.Results Among 7 cases of AAM,the lesions located in subcutaneous regions of perineum in 3 cases,in both perineum and pelvic cavity in 2 cases,in pelvic cavity and in cervix each 1 case,with a median maximum diameter of 7.6 cm.The lesions mainly manifested as heterogeneous hypoechoic,scattered cord-like moderate/moderate-hyperechoic inside,regular or lobulated shape with finger-like protrusions,with clear boundary or unclear boundary with adjacent vagina/rectum.Layered structure could be observed with gray-scale ultrasound and CDFI.Conclusion AAM mainly affected pelvis and perineal region,often manifested as large lesion tended to infiltrate surrounding areas and form finger-like protrusions,most with clear boundaries and interior heterogeneous hypoechoic regions,also the characteristic stratified structures.
4.Assessment of the predictive value of ultrasound imaging characteristics combined with clinical indicators for the prognosis of pancreatic ductal adenocarcinoma

Objective:To explore the value of ultrasound imaging characteristics combined with clinical indicators in assessing the prognosis of patients with pancreatic ductal adenocarcinoma (PDAC).Methods:A retrospective analysis was conducted for patients who underwent pancreatic contrast-enhanced ultrasound (CEUS) from September 2017 to October 2023 at Peking Union Medical College Hospital and were diagnosed with PDAC based on pathological findings. Various parameters were recorded, including CA19-9 levels, tumor size, location, morphologic features, echogenicity, presence of internal cystic components, dilatation of the main pancreatic duct, peripheral vascular invasion, CEUS characteristics, presence or absence of liver metastasis, and treatment methods. In April 2024, patient survival information was obtained through telephone follow-up or review of medical records. Based on the results of the cox regression model analysis, a nomogram model of the risk of death was developed. The receiver operating characteristic (ROC) curves were applied to evaluate the predictive efficacy of the model. The calibration curves were plotted to evaluate the accuracy of the model, and clinical decision curves were used to evaluate the clinical benefit of the model.Results:This study included a total of 207 patients with PDAC. As of April 2024, 71 patients were alive and 136 died, with a median survival time of 14 months (95% CI: 12 -17). Multivariate analysis confirmed that the elevated CA19-9 ( HR=1.689, 95% CI: 1.102-2.588), tumor size >4 cm ( HR=1.641, 95% CI: 1.159-2.322), taller-than-wide shapes ( HR=1.450, 95% CI: 1.019-2.065), incomplete hypo-enhancement ( HR=1.618, 95% CI: 1.100-2.380), and liver metastasis ( HR=1.687, 95% CI: 1.175-2.423) were independent risk factors for survival in patients with PDAC. A nomogram model was further constructed for 6-month, 12-month and 3-year survival of patients with PDAC. The areas under the ROC curve were 0.679, 0.705 and 0.815, respectively. The calibration curves suggested that the model was more accurate, and the clinical decision curves showed that the model had a better clinical benefit. Conclusion:The combined use of ultrasound imaging characteristics and clinical indicators could effectively predict the prognosis of PDAC patients. Specifically, tumor size >4 cm, taller-than-wide shapes, incomplete hypo-enhancement, elevated CA19-9, and the presence of liver metastasis are correlated with poorer survival outcomes. The nomogram model constructed on the basis of these factors can be used to assess the survival of patients with PDAC.

Neurogenic bladder(NB)is one of the most challenging urinary system disorders,with spinal cord injury(SCI)being an important etiological factor.Animal models provide crucial tools for investigating the pathogenesis,therapeutic strategies,and novel drug screening for NB subsequent to SCI.We reviewed and synthesized recent literature on NB animal models after SCI from both domestic and international sources.This review summarizes and analyzes research advancements using these models in terms of animal species,SCI segments,modeling techniques,and evaluation indicators,with the aim of offering insights and guidance for future experimental research based on animal models of NB following SCI.

This article presents a case study of a patient who visited the Geriatric Department of Peking Union Medical College Hospital due to "palpitations, shortness of breath for more than 2 years, limb weakness for 6 months, edema, and nocturnal dyspnea for 2 months". The patient exhibited decreased muscle strength in the limbs and involvement of swallowing and respiratory muscles, alongside complications of heart failure and various arrhythmias which were predominantly atrial. Laboratory tests revealed the presence of multiple autoantibodies and notably anti-mitochondrial antibodies. Following a comprehensive multidisciplinary evaluation, the patient was diagnosed with anti-mitochondrial antibody-associated inflammatory myopathy. Treatment involved a combination of glucocorticoids and immunosuppressants, along with resistance exercises for muscle strength and rehabilitation training for lung function, resulting in significant improvement of clinical symptoms. The case underscores the importance of collaborative multidisciplinary approaches in diagnosing and treating rare diseases in elderly patients, where careful consideration of clinical manifestations and subtle abnormal clinical data can lead to effective interventions.
9.Evidence-based practices for exercise management in patients with metabolic associated fatty liver disease

Objective To analyze challenges in translating exercise management evidence for patients with metabolism-associated fatty liver disease(MAFLD),develop actionable strategies,and evaluate the application of best evidence.Methods Utilizing the evidence translation model,the best evidence was implemented for MAFLD patients in 4 phases:evidence acquisition,baseline practice review,intervention,and outcome evaluation.We compared the knowledge of exercise management evidence,implementation rates of review indicators,completion of exercise programs,BMI,liver stiffness measurement,controlled attenuation parameters,and patient satisfaction among medical staff at a tertiary hospital in Fujian Province during baseline(March-May 2023),mid-practice(June-August 2023),and late-practice(September-November 2023)phases.Results A total of 88 patients were included at baseline review,95 during mid-practice,and 107 in late-practice.Significant improvements were observed in the implementation rates of 21 review indicators,nurses'knowledge,completion rate,BMI,and controlled attenuation parameters compared to the data at baseline(P<0.05).Conclusion The application of best evidence in exercise management for MAFLD patients enhances nurses'knowledge,standardizes nursing practices,and reduces patients'BMI and controlled attenuation parameters.
10.Research progresses in functional and quantitative imaging for evaluating and predicting efficacy of neoadjuvant therapy of pancreatic cancer

Pancreatic cancer has a concealed onset and poor prognosis,and surgical resection is the main treatment method.Neoadjuvant therapy may improve the rate of negative resection margin and prolong overall survival of patients with pancreatic cancer.Functional and quantitative imaging can non-invasively reflect tumor microenvironment characteristics related to chemotherapy resistance,providing a new perspective for evaluating and predicting the efficacy of neoadjuvant therapy for pancreatic cancer.The research progresses in functional and quantitative imaging for evaluating and predicting the efficacy of neoadjuvant therapy of pancreatic cancer were reviewed in this article.
