1.A case-control study of nephrocalcinosis in children with X-linked hypophosphatemic rickets

Objective:To investigate potential factors associated with nephrocalcinosis in children with X-linked hypophosphatemic rickets (XLH).Methods:A case-control study was conducted involving XLH children who were regularly followed up at Children′s Hospital of Nanjing Medical University, from January 2016 to January 2024.Patients diagnosed with nephrocalcinosis were assigned to the case group, and those without nephrocalcinosis served as controls.A retrospective analysis was performed to examine the correlation between nephrocalcinosis and laboratory parameters, as well as the diagnostic value of relevant factors.Multivariable Logistic regression was used to identify factors associated with nephrocalcinosis in children with XLH.Receiver operating characteristic (ROC) curves were plotted for the significant factors, and the areas under the curve (AUC) were compared using Delong′s test to evaluate differences in predictive performance.Results:Univariate analysis revealed significant differences between the case and control groups in serum calcium, urinary calcium, urinary calcium/creatinine ratio, urinary phosphorus, and renal tubular maximum reabsorption rate of phosphate/glomerular filtration rate (TmPi/GFR) (all P<0.05).However, multivariate logistic regression analysis identified only urinary calcium ( B=0.489, P=0.001) and TmPi/GFR ( B=-0.886, P=0.007) as independent predictors, while the urinary calcium/creatinine ratio was not statistically significant.The sensitivity, specificity, optimal cutoff value, and AUC for urinary calcium in predicting nephrocalcinosis were 0.842, 0.833, 0.614 mmol/L, and 0.851 ( P<0.01), respectively.For TmPi/GFR, the corresponding values were 0.900, 0.877, 0.573 mmol/L, and 0.875 ( P<0.01).The Delong test showed no significant difference in AUC between the combined indicator and TmPi/GFR ( Z=-1.555, P=0.120) or urinary calcium alone ( Z=-1.598, P=0.110). Conclusions:Urinary calcium and TmPi/GFR are significantly associated with nephrocalcinosis in children with XLH and demonstrate good predictive performance for early detection.The combination of these two indicators does not outperform either individual marker in diagnostic accuracy.

3.A survey on anxiety status in patients over 60 years old from one community health service center in Shanghai

Objective To investigate the prevalence and related risk factors of anxiety symptoms in elderly patients from one community health service center in Shanghai.Methods Patients aged 60 to 85 years visiting Kongjiang Community Health Service Center from August to December of 2016 were included in the survey.The patients were evaluated with Generalized Anxiety Disorder Scale-7 (GAD-7),Sociodemographic and Health-related Variables Questionnaire,Mental Health Knowledge Scale and Lubben Social Network Scale-6 (LSNS-6).Results Total 2 006 questionnaires were distributed and 1 981 valid questionnaires were returned with a recovery rate of 98.75%.The overall GAD-7 score was 0 (0,2),and 13.5% (267/1 981) participants had positive anxiety symptoms.Multivariate logistic analysis showed that 6 variables were related to anxiety symptoms,including educational background,exercise frequency,percentage of medical costs over total household expenses,life satisfaction,the amount of chronic somatic disease,mental health knowledge awareness (P < 0.05).Conclusion The prevalence of anxiety symptoms in the study population is relatively high.Anxiety symptoms are associated with multiple factors,to which the attentions should be paid.
4.The protective effect of ulinastatin on the intestinal mucosal barrier in rats after intestinal ischemia-reperfusion

Objective To investigate the protective effect of ulinastatin on the intestinal mucosal barrier in rats with ischemia-reperfusion.Methods Totally 24 SD rats were randomly divided into 3 groups after clamping superiormesenteric artery for 1 h and reperfusion for 1 h: blank control group,control group and treatment group.Rats in the treatment group were injected with ulinastatin(50 000 U/kg) through vena dorsalis penis after ischemia-reperfusion model was induced.The control group underwent laparotomy with only the manipulation of the intestine and the same dose of saline was used through the same way.Specimens were obtained after ischemia-reperfusion model was induced.Dynamic turbidimetry was used to evaluate the effect of ulinastatin on the intestinal endotoxin translocation in rats.Apoptosis of mucosal cells was detected by TUNEL method,and the expressions of Bax and Bcl-2 mRNA were determined by semi-quantitative RT-PCR.Results The serum endotoxin level and apoptotic index of mucosal cells were evidently lower in blank control group than in control group(P
5.The expression of E-cadherin and its relation with invasive ability of basal membrane in tumor cells

Objective To study the effect of down-regulation of E-cadherin on the invasive ability of tumor cells. Methods Human pancreatic carcinoma cell lines JHP-1, PANC-1, and MIA PaCa-2 were selected. The immunocytochemistry, Western blot and invasive-MTT assay were used to observe the expression and the contents of E-cadherin in the tumor cells. Results Immunocytochemistry showed that E-cadherin expression was on cell membrane. The expressions of E-cadherin were preserved in JHP-1, reduced in PANC-1, and showed negative in MIA PaCa-2. Western blot analysis showed that the protein content was the highest in JHP-1 and lowest in MIA PaCa-2.
