1.Determination of residual solvents in amidoxime EN chelating agents by headspace gas chromatography

Objective To establish an analytical method for the determination of methanol and ethanol solvent residues in amidoxime EN chelating agents by headspace gas chromatography.Methods The samples were analyzed in a GC-2030 gas chromatograph system using a Shimadzu HS-20 fully automated headspace injector and quantified by an internal standard method.The samples were separated and detected by the DB-WAX capillary column with a flame ionization detector(FID).The thermostatic furnace temperature was 70 ℃ and the equilibrium time of the sample bottle was 30 min.The sample flow path temperature was 150℃ and the carrier gas was high pure nitrogen.As temperature programming,the initial column temperature of 40 ℃ lasted 9 min and increased to 200 ℃ at 40 ℃/min for 2 min.Results The peak areas of the residual solvents in EN chelator showed good linearity(R2>0.999)in the mass concentration range investigated,the detection limits of solvent methanol and ethanol were 0.25 μg/mL and 0.30 μg/mL respectively,and the limits of quantification were 1.25 μg/mL and 1.50 μg/mL,respectively.The recovery of residual solvent addition ranged from 97.10%to 102.23%,and the RSD value was less than 5.00%.Conclusion This method is sensitive enough to be used for the determination of solvent residues in amidoxime EN chelating agents.
2.Efficacy of ultrasound-guided thyroid injection of dexamethasone in the treatment of subacute thyroiditis

Objective:To investigate the clinical efficacy and safety of ultrasound-guided thyroid injection of dexamethasone in the treatment of subacute thyroiditis (SAT).Methods:This is a randomized controlled study involving 32 patients with SAT who received treatment at Endocrinology Clinic of Heilongjiang Provincial Hospital between June 2022 and March 2023. The patients were randomly divided into an observation group and a control group ( n = 16 per group) using a random number table method. The observation group received ultrasound-guided injection of dexamethasone (5 mg per injection, once a week) into the thyroid lesion, while the control group was treated with oral prednisone acetate tablets (5 mg per dose, three times a day). Both groups were treated for 4 weeks. The clinical efficacy, time taken for body temperature to return to normal, time for pain resolution, time for goiter regression, time for thyroid function to normalize, as well as recurrence rates and adverse reactions, were compared between the two groups. Results:The response rate in the observation group was 100% (16/16), which was significantly higher than that in the control group [75% (12/16), χ2 = 4.57, P = 0.033]. The time taken for body temperature to return to normal, the time for goiter regression, and the time for goiter regression in the observation group were (3.5 ± 3.6) days, (7.4 ± 2.5) days, and (11.6 ± 7.4) days, respectively, which were significantly shorter than those in the control group [(6.1 ± 3.2) days, (9.9 ± 3.5) days, (16.9 ± 6.8) days, t = -2.16, -2.33, -2.11, all P < 0.05]. After 4 weeks of treatment, the observation group showed significantly lower body mass index, fasting blood glucose, and serum triglyceride levels compared to the control group ( t = -2.07, -2.46, -2.13, all P < 0.05). The recurrence rate in the observation group was 0%, which was significantly lower than that in the control group [25% (4/16), χ2 = 4.57, P = 0.033]. Conclusions:The efficacy and safety of ultrasound-guided local thyroid injection of dexamethasone for the treatment of SAT are superior to those of the oral treatment regimen.

4.Experience in building a cooperative research system for rare diseases: a European perspective

Objective:This study aimed to overview the typical practies made by Europe on rare diseases cooperative researches and provide reference for the construction of rare disease research system in China.Methods:Through literature reviews and official website information, this study systematically summarized the contributions and outputs made by the framework of Europe cooperation in rare disease field.Results:Focusing on the needs of patients, the aim of international cooperation was to improve the ability of ″diagnosis, treatment, research and education″. inclnding multi-sourced special funding and project topic screening, expert networks setup and patient impower, research network construction and mutual promotion, information system modernization, etc.Conclusions:The eco-system of rare disease research in China is still under-construction. It is recommended to combine the characteristics of China′s healthcare system, optimize input strategies, empower patients, innovate mechanism, and speed up bioinformation technology.
5.Research on the model construction of medical consumables replenishment strategy and inventory control strategy based on supply-process-distribution mode

Objective:To build a medical consumables demand prediction model to predict the demand for medical consumables,and to reduce the occurrence of emergency replenishment and shortage of medical consumables.Methods:The medical consumables demand prediction model was established using support vector regression(SVR)algorithm,the replenishment strategy and inventory control strategy optimization model was established using multiple population genetic algorithm(MPGA)to build a medical consumables demand forecasting model,and the replenishment strategy and inventory control strategy were automatically generated.The relevant data of supply-processing-distribution(SPD)mode introduced by Benxi Central Hospital for medical consumables management from January 2019 to May 2023 were selected for model training,model validation test(test set)and model application prediction(application set),respectively.The average absolute prediction error,peak prediction error and trough of prediction error of the model were evaluated,and the average ratio of consumption per day to inventory cost,the average number of urgent orders per month,the average number of out of stock per month,the average number of non-urgent orders per month and the decreasing range of the index weekly average were compared.Results:There was no statistically significant difference in the average absolute prediction error between the model test set and the application set(P>0.05).The average absolute prediction error of the model was 0.033 5±0.024 5,the peak prediction error was 0.071 7 and the trough of prediction error was-0.0090.After the application of the model,the ratio of average daily consumption to inventory cost,the average number of monthly emergency order frequency,the average number of shortage of stock,and the average number of monthly non-emergency orders were(0.457 5±0.060 3),(23.95±6.04),(5.58±2.17),and(20.68±2.77),respectively.The ratio of daily average consumption to inventory cost was higher than that before the application,the average number of monthly emergency order,shortage of stock,and monthly non-emergency order were lower than those before the application,the difference was statistically significant(F=371.912,88.486,124.472,142.138,P<0.000).After the application of the model,the weekly average inventory amount decreased by 43.66%,and the average number of emergency replenishment decreased by 53.76%,the average number of shortages of stock decreased by 76.95%,and the average shortage of stock normal replenishments decreased by 34.41%.Conclusion:The medical consumables demand prediction model can predict the demand for medical consumables,optimize replenishment and inventory control strategies,reduce the cost of medical consumables inventory,reduce occurrence of emergency replenishment and shortages of stock,and reduce the number of normal replenishment.
6.Effects of Bushen formulae on bone metabolism and ENaCα,NCC and ClC-3 expression in ovariectomized rats with high salt intake

AIM:To investigate the effect of Bushen formulae(BHF)on bone metabolism and its possible mechanism in ovariectomized rats with high salt intake.METHODS:According to the random number table method,80 SPF-grade Sprague-Dawley rats were divided into sham group,ovariectomy(OVX)group,medium-high-salt diet(MSD)group,high-salt diet(HSD)group,BHF group,BHF with normal saline(BHF+NS)group,BHF+MSD group,and BHF+ HSD group,with 10 rats in each group.After modeling,different diets and BHF formula interventions were administered,and the concentrations of sodium chloride added to MSD group and HSD group were 2%(w/w)and 8%(w/w),respective-ly.The dose of BHF was 7.8 g·kg-1·d-1,once a day,and the treatment lasted for 12 weeks.Bone density,bone microar-chitecture,bone parameters,bone metabolism biomarkers,bone histopathological changes,the expression of epithelial sodium channel α(ENaCα),Na-Cl cotransporter(NCC),and voltage-gated chloride channel 3(ClC-3)proteins in bone tissue were detected in each group.RESULTS:Compared with sham group,the rats in OVX group had reduced bone density and destroyed bone microstructure.Compared with OVX group,the bone microstructure in MSD and HSD groups was more significantly damaged,while the levels of bone formation markers,bone glycoprotein(BGP)and type Ⅰ procolla-gen N-terminal peptide(PINP),were significantly increased in HSD group(P<0.05).Moreover,the levels of bone re-sorption markers,such as amino-terminal cross-linked telopeptides of type Ⅰ collagen(NTX),carboxy-terminal cross-linked telopeptides of type Ⅰ collagen(CTX)and tartrate-resistant acid phosphatase(TRACP),were significantly in-creased(P<0.05),indicating that bone metabolism was in high-conversion state.High-salt diet accelerated the structural destruction of bone trabeculae,and Western blot results showed that high-salt diet caused decreases in the protein expres-sion levels of ENaCα and ClC-3 and an increase in the protein expression level of NCC in femoral tissues(P<0.05).After BHF intervention,the expression of relevant ion channels caused by high salt could be regulated to different degrees.CONCLUSION:Bushen formulae could differentially regulate the expression of relevant ion channels ENaCα,ClC-3,and NCC induced by high salt to different degrees,which has certain ameliorative and therapeutic effects on the imbalance of bone metabolism.

9.Exposure to specific far-infrared ray based on gra-phene film promotes exercise capacity and glucose metabolism via AMPK

OBJECTIVE AMPK activator,act as exer-cise mimetics,effective in preventing or ameliorating met-abolic diseases,including obesity and diabetes.Systemic activating of AMPK represents an important therapeutic strategy to treat metabolic diseases.However,whether far-infrared(FIR)hyperthermia therapy could be used as exercise mimetic to realize wide-ranging metabolic regu-lation,and its underling mechanisms remain unclear.METHODS The mice were subjected to hyperthermia in the FIR chamber(30±1)℃for 14 d.Exercise endurance was determined using a treadmill.Blood flow were mea-sured by the laser speckle contrast imaging.Combina-tion of microbiomic and metabolomic analysis,diversity of microbiota and metabolic profiling in muscle were detected.The microbiota disorder model via treatment with different cocktails of antibiotics(ABX).RESULTS The material characterization shows that the graphene synthesized by chemical vapour deposition(CVD)is dif-ferent from carbon fi ber,with single-layer structure and high electrothermal transform efficiency.The emission spectra generated by graphene-FIR device would maxi-mize matching those adsorbed by tissues(≈8.0 μm).Gra-phene-FIR improves core and epidermal temperature,and increases blood flow in femoral muscle and abdo-men.The diversity of gut microbiota was increased by graphene-FIR exposure.Graphene-FIR reduced the bac-teroidetes/firmicutes(B/F)ratio and increased the abun-dance of short-chain fatty acids(SCFA)-producing bac-teria,including Allobaculum,Blautia and Anaerostipes.Additionally,graphene-FIR stimulated the expression of SCFAs-sensing receptor(GPR 43),p-AMPK Thr172 and GLUT4,and increased the AMP/ATP ratio,thus enhanc-ing muscle glucose uptake.Metabolomic analyses revealed the significant changes in 25 metabolites,with twenty increased(eg.creatinine and phosphate)and five decreased(eg.lactic acid),and the marked impact of five metabolic pathways,including galactose metabo-lism,glycolysis,gluconeogenesis,fatty acid biosynthesis,butanoate metabolism,pyruvate metabolism.Further-more,a microbiota disorder model also demonstrates that the graphene-FIR effectively restore the exercise endurance with enhanced p-AMPK and GLUT4.CON-CLUSION Our results provide convincing evidence that graphene-based FIR therapy promoted exercise capacity and glucose metabolism via AMPK in gut-muscle axis.These novel insights into graphene-FIR therapy suggest a potential as an exercise mimetic for the treatment of metabolic disease in clinical.
10.Severe poisoning caused by mistaken ingestion of European waterhemlock root ( Radix Archangeliae Decurrentis)

A 61-year-old man developed nausea, vomiting, unconsciousness, and limb convulsions after 1 hour of mistaken ingestion of European waterhemlock ( Radix Archangeliae Decurrentis). Laboratory tests showed white blood cell count (WBC) 21.5×10 9/L, alanine aminotransferase (ALT) 246 U/L, total bilirubin (TBil) 35.3 μmol/L, creatine kinase MB (CK-MB) 20.8 μg/L, lactate dehydrogenase (LDH) 525 U/L, serum creatinine (Scr)143 μmol/L, uric acid 1 338 μmol/L, and blood ammonia 45 μmol/L. Chest computed tomography showed multiple ground glass density lesions in bilateral lungs. The patient was diagnosed as having acute and severe European waterhemlock poisoning, toxic encephalopathy, aspiration pneumonia, and pulmonary edema. The treatments including ventilator assisted breathing, analgesia and sedation, anti-infection, expectorant and antiasthmatic, inhibition of gastric acid secretion, rehydration hydration, promotion of awakening, liver protection, and myocardial nutrition were given. In addition, the patient received 2 times of hemoperfusion. Five days later, the patient′s condition was improved. Laboratory tests showed WBC 12.0×10 9/L, ALT 94 U/L, TBil 28.2 μmol/L, CK-MB 2.7 μg/L, LDH 452 U/L, Scr 61 μmol/L, uric acid 514 μmol/L, blood ammonia 38.0 μmol/L. Then the patient was transferred to the Department of Emergency Internal Medicine for further treatment.
