1.Association between socio ecological risk factors, physical activity developmental trajectory and depressive symptoms among junior and senior high school students

Objective:
To explore the relationship between socio ecological risk factors (SERF), physical activity (PA) developmental trajectories, and depressive symptoms among junior and senior high school students in Tianjin, in order to provide theoretical support for comprehensive interventions for adolescent depression.
Methods:
A longitudinal follow up design was adopted. In September 2022, a baseline survey was conducted using a stratified cluster random sampling method in two junior high schools and two senior high schools in Tianjin, collecting data on students basic information, SERF, PA, and depressive symptoms. Two follow up surveys were conducted in September 2023 and 2024, yielding 588 valid participants. Latent class growth analysis (LCGA) was used to identify PA developmental trajectory categories among junior and senior high school students. Logistic regression was applied to examine the associations between depressive symptoms and SERF as well as PA trajectories.
Results:
The detection rates of depressive symptoms among Tianjin junior and senior high school students over the three years were 26.53%, 20.24%, and 21.26 %, respectively. Depressive symptoms were positively correlated with SERF ( OR=1.04, 95%CI=1.03-1.05, P <0.05). The highest risks were observed in the individual dimension and family dimension ( OR =1.28, 1.21, both P <0.05). LCGA identified three PA trajectory groups:persistently low level (80.65%), persistently high level (4.58%), and slowly increasing group ( 14.77 %). Multivariate regression analysis showed that compared with the persistently low level PA, the slowly increasing PA significantly reduced the risk of depressive symptoms ( OR=0.44, 95%CI =0.20-0.88), while SERF still increased the risk of depressive symptoms ( OR=1.04, 95%CI =1.03-1.05) (both P <0.05).
Conclusion
SERF are risk factors for depression symptoms among junior and senior high school students, whereas slowly increasing PA development trajectory demonstrates a protective effect.
2.Observation of the efficacy of dupilumab for treatment of atopic dermatitis in the elderly.

OBJECTIVE:
To evaluate the efficacy and safety of dupilumab in the treatment of atopic dermatitis in the elderly.
METHODS:
In this study, elderly patients with atopic dermatitis treated with dupilumab for at least 16 weeks in the Department of Dermatology and Venereology of Beijing Anzhen Hospital from January 2021 to October 2023 were retrospectively collected. Clinical indicators were compared before, during and after treatment, including pruritus numerical rating score (PNRS), eczema area and severity index (EASI), dermatology life quality index (DLQI) score, and the incidence of adverse events was recorded. The expression of interferon γ (IFN-γ), interleukin-4 (IL-4) and interleukin-6 (IL-6) in peripheral blood were compared before treatment and after 16 weeks of treatment.
RESULTS:
A total of 90 elder patients with atopic dermatitis were included, EASI, PNRS and DLQI scores all showed a gradual downward trend during the treatment period, which was manifested as a rapid decline in the first 4 weeks after starting treatment, and then the decline gradually leveled off. The results of point-to-point comparison showed that EASI, PNRS and DLQI scores in 4 weeks after treatment were significantly lower than those before treatment (P < 0.001); In the 16th week after treatment, the scores of the above therapeutic indicators were further reduced, and the difference was statistically significant compared with the 4th week (P < 0.01); The EASI score was significantly lower at each time point than the previous time point, indicating that the patients' skin lesions continued to improve significantly. The overall efficacy of dupliumab was evaluated. After 4 weeks of treatment, 62.89% of patients achieved EASI-50 (EASI score decreased by ≥50%), and 74.4% of patients' DLQI score decreased by ≥4 points. After 16 weeks of treatment, 57.8% of the patients achieved EASI-75, 32.2% achieved EASI-90, and the PNRS and DLQI scores of all the patients decreased by ≥4 points. After 16 weeks of treatment, the expression levels of IL-4 and IL-6 were (31.62±6.23) ng/L and (14.36±2.25) ng/L, respectively, which were significantly lower than those before treatment (P < 0.001), and the expression level of IFN-γ was (15.37±3.14) ng/L, which was higher than before treatment (P < 0.001).The main adverse reactions were conjunctivitis (2 cases), injection site reaction (3 cases) and multiple bacterial folliculitis of the back (2 cases), which could be alleviated by symptomatic treatment, and no serious adverse reactions occurred.
CONCLUSION
Dupilumab has shown good efficacy in the treatment of elderly atopic dermatitis, which can effectively improve clinical symptoms such as itching and skin lesions, improve the quality of life of patients, and no serious adverse reactions occurred during treatment, so it is safe and worthy of clinical promotion.

Mycophenolic acid (MPA), the active moiety of both mycophenolate mofetil (MMF) and enteric-coated mycophenolate sodium (EC-MPS), serves as a primary immunosuppressant for maintaining solid organ transplants. Therapeutic drug monitoring (TDM) enhances treatment outcomes through tailored approaches. This study aimed to develop an evidence-based guideline for MPA TDM, facilitating its rational application in clinical settings. The guideline plan was drawn from the Institute of Medicine and World Health Organization (WHO) guidelines. Using the Delphi method, clinical questions and outcome indicators were generated. Systematic reviews, Grading of Recommendations Assessment, Development, and Evaluation (GRADE) evidence quality evaluations, expert opinions, and patient values guided evidence-based suggestions for the guideline. External reviews further refined the recommendations. The guideline for the TDM of MPA (IPGRP-2020CN099) consists of four sections and 16 recommendations encompassing target populations, monitoring strategies, dosage regimens, and influencing factors. High-risk populations, timing of TDM, area under the curve (AUC) versus trough concentration (C0), target concentration ranges, monitoring frequency, and analytical methods are addressed. Formulation-specific recommendations, initial dosage regimens, populations with unique considerations, pharmacokinetic-informed dosing, body weight factors, pharmacogenetics, and drug-drug interactions are covered. The evidence-based guideline offers a comprehensive recommendation for solid organ transplant recipients undergoing MPA therapy, promoting standardization of MPA TDM, and enhancing treatment efficacy and safety.

4.Techniques and performance of biominerals for fabricating bone tissue engineering scaffolds

BACKGROUND:In recent years,methods for incorporating biominerals into bone tissue engineering scaffolds have been extensively studied,including solvent casting,freeze-drying,and 3D printing.However,the diverse types and complex compositions of biominerals lead to varying impacts on scaffold performance and differing requirements for fabrication processes,necessitating systematic research on their applicability.OBJECTIVE:To explore biomineral grinding and screening processes,and evaluate their solution rheological properties,hydrophilicity,mechanical properties,and biocompatibility when prepared as composite materials with polymer materials.METHODS:Five representative biominerals were selected,including turtle shell,eggshell,cuttlebone,deer antler,and pearl.These were ground into powders and screened,then mixed with polycaprolactone in specific proportions to prepare composite materials.The feasibility of applying biominerals in bone tissue engineering was explored by testing the elemental composition and particle size distribution of the powder,as well as the solution rheological properties,hydrophilicity,mechanical properties,and biocompatibility of the composite material.RESULTS AND CONCLUSION:(1)Most biomineral powders followed the rule that the longer the grinding time,the smaller the particle size.The desired particle size range could be obtained through methods such as sieving.Elemental mapping and Fourier Transform Infrared spectroscopy analyses indicated that the main inorganic mineral components of these five biominerals were calcium carbonate and calcium phosphate,containing the elements C,O,P,and Ca.(2)The method of dissolving polycaprolactone in 1,4-dioxane and adding biomineral powders to prepare scaffolds did not significantly alter the composite material composition and did not reduce biocompatibility.The addition of biomineral powders improved the hydrophilicity and 3D printability of polycaprolactone materials but decreased their mechanical properties.(3)These findings suggest that when applying biomineral powders to bone tissue engineering scaffolds,the proportion of powder added should be carefully chosen to balance hydrophilicity,printability,and mechanical properties.
5.Impact of residual intracanal calcium hydroxide and the root canal filling techniques on apical sealant integrity

Objective·To compare the effects of calcium hydroxide(CH)residues in root canals and root canal filling methods on apical closure,and to provide reference for clinical selection of root canal sealing drugs and filling methods.Methods·Seventy-five permanent mandibular premolar teeth with single root canals that were extracted due to orthodontic treatment or periodontal problems were collected from the Department of Oral and Maxillofacial Surgery of the Affiliated Hospital of Qingdao University.The crowns were removed,the root canals were prepared,and the specimens were randomly assigned into 3 groups:a water-soluble calcium hydroxide group(Group A,n=30),an oil-soluble calcium hydroxide group(Group B,n=30),and an unsealed control group(Group C,n=15).After 2 weeks of sealing,Groups A and B underwent manual preparation with a#35 K-file,followed by ultrasonic agitation and irrigation of the root canal to ensure that the calcium hydroxide residue was located roughly at the root apex and that the residue was 15%to 20%.Groups A,B,and C were randomly divided into 3 groups:the iRoot SP single-tip group(Group 1),the hot compression group(Group 2),and the cold compression group(Group 3),and root canals were filled using the iRoot SP single-tip method,the hot adhesive vertical compression filling method,and the cold adhesive lateral compression filling method,respectively.A dye penetration test was used to evaluate apical microleakage,and scanning electron microscopy was used to observe the microscopic morphology of the dentin-root canal sealer interface in each group.Statistical analysis was performed using two-way ANOVA.Results·Both root canal sealing drugs and root canal filling methods affected the apical sealing,and there was an interaction between them.The effects of residual calcium hydroxide on apical closure were analyzed.The difference between Group B and Group C was statistically significant only in Group 1.There were statistically significant differences between Group A and Group C in Group 2 and Group 3,and statistically significant differences between Group A and Group B regardless of the root filling method.Among the three root filling methods,there was a statistically significant difference between Group 1 and Group 3 in Group C(P=0.013).In Group A and Group B,there were statistically significant differences between Group 2,Group 3 and Group 1(P<0.001).Conclusion·The residual water-soluble calcium hydroxide in the root canal has a negative effect on the apical closure,but the residual oil-soluble calcium hydroxide has a small negative effect on the apical closure.iRoot SP can reduce the negative effect of residual water-soluble calcium hydroxide on root canal closure.
6.Health economic analysis and medical cost analysis of children with severe hepatitis B in China: A retrospective study from 2016 to 2022

Background::Hepatitis B poses a heavy burden for children in China, however, the national studies on the distributional characteristics and health care costs of children with severe hepatitis B is still lacking. This study aimed to analyze the disease characteristics, health economic effects, and medical cost for children with severe hepatitis B in China.Methods::Based on patient information in the Hospital Quality Monitoring System, cases with severe hepatitis B were divided into four groups according to age, and the etiology and symptoms of each group were quantified. The cost of hospitalization was calculated for cases with different disease processes, and severity of disease. The spatial aggregation of cases and the relationship with health economic factors were analyzed by Moran’s I analysis. Results::The total number of children discharged with hepatitis B from January 2016 to April 2022 was 1603, with an average age of 10.5 years. Liver failure cases accounted for 43.48% (697/1603) of total cases and cirrhosis cases accounted for 11.23% (180/1603). According to the grouping of disease progression, there were 1292 cases without associated complications, and the median hospitalization cost was $818.12. According to the spatial analysis, the aggregation of cases was statistically significant at the prefectural and provincial levels in 2019, 2020, and 2021 (all P <0.05). The number of severe cases was negatively correlated with gross domestic product (Moran’s I <0) and percentage of urban population (Moran’s I <0), and positively correlated with the number of pediatric beds per million population (Moran’s I >0). Conclusion::The number of severe hepatitis B cases is low in areas with high gross domestic product levels and high urban population ratios, and health care costs have been declining over the years.

Cerebral ischemia-reperfusion injury can lead to secondary brain damage through com-plex pathological processes such as ion imbalance,inflammation,mitochondrial dysfunction,oxidative stress,and increased vascular permeability,serious-ly affecting the prognosis and quality of life of isch-emic stroke patients.How to effectively reduce and prevent neurological damage caused by CIRI has become a key and hot topic in CIS research.In re-cent years,many studies have confirmed that the active ingredients of traditional Chinese medicine can alleviate oxidative stress,inhibit autophagy,regulate cell apoptosis,control neuroinflammation,reduce cell apoptosis index,alleviate neuronal his-topathological damage,promote neuronal survival,enhance synaptic plasticity,and play a therapeutic role in CIRI through the PI3K/Akt signaling pathway.In summary,this article provides a review of the PI3K/Akt signaling pathway and its role in CIRI,as well as the research on the regulation of PI3K/Akt signaling by active ingredients in traditional Chi-nese medicine for the treatment of CIRI.The aim is to provide a theoretical basis for the clinical use of active ingredients in traditional Chinese medicine to alleviate CIRI damage.
8.Dynamic changes of CD4+T lymphocytes in peripheral blood of patients with bronchial asthma at different periods and its correlation with control level

Objective:To explore dynamic changes of CD4+T lymphocytes in peripheral blood of patients with bronchial asthma at different periods and its correlation with control level.Methods:A total of 108 patients with bronchial asthma who received medical treatment in Beijing Jishuitan Hospital,Capital Medical University from September 2020 to October 2022 were selected as study objects,and CD4+T level in peripheral blood of patients with asthma at different periods were compared.According to control level,patients were divided into control group(n=68)and uncontrolled group(n=40).Factors affecting control level of patients with bronchial asthma were analyzed,and prediction model of column graph was constructed and evaluated.Results:CD4+T level in periph-eral blood of patients with acute attack of bronchial asthma was significantly lower than that in chronic duration stage and clinical control stage,CD8+T and CD3+T levels were significantly higher than that in chronic duration stage and clinical control stage(P<0.05).Allergic rhinitis,smoking history,acute respiratory tract infection and residential air pollution were uncontrolled risk factors of patients with bronchial asthma,CD4+T and regular use of inhaled corticosteroids were protective factors(P<0.05).The above indexes were used to construct a line chart model.After internal verification(Bootstrap resamplage 1 000 times),it was found that AUC of this model was 0.845(95%CI:0.734~0.885),sensitivity and specificity were 85.7%and 88.6%,respectively.Conclusion:CD4+T lymphocytes in peripheral blood of patients with acute asthma are significantly lower than patients with chronic asthma and healthy people.Allergic rhinitis,smoking history,acute respiratory tract infection and residential air pollution are uncontrolled risk factors of patients with bronchial asthma,CD4+T and regular use of inhaled corticosteroids are protective factors.
9.Exploring the mechanism of Qiwei Tangmaishu capsules in the treatment of type 2 diabetes based on network pharmacology and animal experiment

Objective To observe the therapeutic effect of Qiwei Tangmaishu capsules on type 2 diabetes mice,and explore the mechanisms of its treatment of type 2 diabetes based on network pharmacology.Methods TCMSP,ETCM databases were used to query all components and of Qiwei Tangmaishu capsules and their targets.OMIM and DrugBank databases were used to search for targets of type 2 diabetes.The targets of type 2 diabetes and Qiwei Tangmaishu capsules were intersected by Venny 2.1.0.to perform GO and KEGG pathway enrichment analysis on those intersecting targets using the Metascape website.Then,a mouse model of type 2 diabetes was established,and Qiwei Tangmaishu capsules were given to low,medium,and high dose groups(234,468,and 936 mg/kg,respectively),and metformin(MET)group(200 mg/kg)for 2 weeks.The weight of each mouse was measured before and after treatment,and fasting blood glucose was also measured.After the 2 weeks,fasting insulin was measured;ELISA was used to detect levels of inflammatory factors IL-1β,TNF-α,IL-6,TLR4,and NF-κB in serum;Hematoxylin eosin staining was used to observe the morphology of pancreatic islets;and Caspase 3 and INS immunofluorescence were used to detect apoptosis of pancreatic islet cells and the number of pancreatic beta cells.Western Blot assay was used to detect the expression levels of pancreatic tissue proteins such as p-Akt,Akt,p-PI3K,PI3K,Bax,Bcl2.Results 1260 active ingredient targets were identified in Qiwei Tangmaishu capsules;1205 targets of type 2 diabetes were found.Of these,312 targets were intersected by Venny,with core targets involving Akt1,TNF,IL-6,TLR4,among others.Enrichment analysis identified 240 KEGG pathways,among which"insulin resistance""PI3K/Akt signaling pathway"were the key pathways enriched.The animal experiment result showed that compared with the model group,the intervention of Qiwei Tangmaishu capsules and metformin significantly improved blood glucose and insulin resistance;the content of inflammatory factors in serum decreased,and the apoptosis rate of pancreatic islet cells significantly decreased;the number of pancreatic beta cells significantly increased;the expression of pro-apoptotic protein Bax decreased,the expression of anti-apoptotic protein Bcl2 significantly increased,and the expression of p-PI3K and p-Akt was upregulated.Conclusions Qiwei Tangmaishu capsules can significantly reduce blood glucose levels,restore insulin sensitivity,and reduce islet cell apoptosis in type 2 diabetic mice.The mechanism may be related to the activation of the PI3K/Akt signaling pathway.
