Objective:The Social Isolation Scale for people with type 2 diabetes mellitus (T2DM) was developed and tested for reliability and validity, which provided an effective tool for measuring the social isolation level of patients with T2DM.Methods:The initial scale was developed through literature review, qualitative interviews, and expert consultation. Convenience sampling method was used to select T2DM patients who met the inclusion and exclusion criteria for questionnaire survey. The item analysis method was used to select the items of the scale. Exploratory and confirmatory factor analyses were used to evaluate construct validity. The content validity of the scale was evaluated by the scale-level content validity index and the item-level content validity index. The criterion validity was verified by using the Lubben Social Network Scale-6 and the De Jong Gierveld Loneliness Scale. Reliability was tested through internal consistency and test-retest reliability. Results:Through literature review and qualitative interviews, 30 items of the scale were selected to form the first version scale. After two rounds of inquiries from 16 experts (expert authority coefficient = 0.897), the second version of the scale was formed. A total of 407 questionnaires were distributed in two stages using the second version scale. Among the 407 patients, there were 214 males and 193 females. Exploratory factor analysis extracted 4 common factors, with a cumulative variance contribution rate of 61.338%. The final scale was determined to include 4 dimensions and 16 items, with the dimensions being "social support network", "frequency of social participation", "satisfaction with interpersonal relationships", and "diabetes-related sense of isolation".The average content validity index at the scale level was 0.929, the content validity index at the item level was 0.830 - 1.000, and the criterion validity was - 0.647 and 0.681. The Cronbach′s α coefficient of the total scale was 0.822, and the test-retest reliability was 0.858.Conclusions:The Social Isolation Scale for people with T2DM has good reliability and validity. It is a reliable and valid tool for assessing social isolation in T2DM patients.

Objective:To explore the clinical phenotype, pregnancy outcome and follow-up of fetuses with 15q11.2BP1-BP2 microdeletions in order to provide a basis for prenatal and reproductive consultation. Methods:From March 2019 to December 2023, 20 fetuses who were diagnosed with 15q11.2BP1-BP2 microdeletion syndrome at the Prenatal Diagnosis Center of General Hospital of Ningxia Medical University were selected as the study subjects. Results of genetic testing and ultrasound examination, outcome of pregnancy, and postnatal follow-up were retrospectively analyzed. This study has been approved by the Ethics Committee of General Hospital of Ningxia Medical University (No. [2020]0520B).Results:None of the 20 fetuses was found to have chromosomal abnormality, whilst all were found to harbor a 15q11.2 BP1-BP2 microdeletion by low-depth whole genome sequencing (CNV-seq). The range of deletions was determined as 0.26 ~ 0.87 Mb, and all were rated as pathogenic CNVs. Three fetuses had abnormal ultrasound findings, including 1 with widened renal pelvis, 1 with agenesis of corpus callosum, and 1 with nuchal fold thickening. Parental verification in 10 couples verified that two fetal deletions were de novo, whilst the remaining eight were inherited from a phenotypically normal parent. Following genetic counseling, three couples had opted to terminate the pregnancy, whilst the remaining 17 had continued with the pregnancy until delivery. The 17 liveborns were followed up for 2 months to 5 years, with no obvious abnormality in growth and development noted. Conclusion:CNV-seq plays an important role in the prenatal diagnosis of 15q11.2 BP1-BP2 microdeletions. Such deletions may not always lead to disease phenotypes. Individualized consultation and long-term follow-up, in combination with intrauterine ultrasound and parental verification are necessary.

Background and purpose:Human papilloma virus(HPV)infection status is crucial for diagnosing cervical precancerous lesions and classifying cervical cancer.High-risk(HR)HPV is often linked to P16 protein overexpression,so P16 detection via immunohistochemistry(IHC)is commonly used to assess HPV infection. However, the differences between HPV status and P16 expression remains unclear. An in-depth study of the correlation between HPV and P16 is essential for clinical guidance. Methods:We retrospectively collected clinical and pathological data of cervical lesions from 618 patients diagnosed at the Department of Pathology,Fudan University Shanghai Cancer Center from January 2020 to December 2023(Ethical number:050432-4-2307E).Polymerase chain reaction(PCR)reverse dot hybridization was used to detect HPV including HR and low-risk(LR)subtypes,and immunohistochemistry was used to detect P16 for comparative analysis.Based on different clinical and pathological diagnoses, the sensitivity and specificity of P16 expression in evaluating HPV infection were evaluated.Among the 618 cases of cervical lesions, there were 92 cases of cervical squamous cell carcinoma,257 cases of cervical adenocarcinoma,79 cases of high-grade squamous intraepithelial lesions(HSIL),105 cases of low-grade squamous intraepithelial lesions(LSIL),and 85 cases of chronic cervical inflammation. Results:According to clinical diagnosis, the HR-HPV positive rate in cervical squamous cell carcinoma was 88.0%(81/92),the P16 positive rate was 91.3%(84/92),and the overall consistency rate between P16 and HPV detection was 90.2%(88/92);for HR-HPV infection, the sensitivity and specificity of P16 were 96.3%and 45.5%.The positive rate of HR-HPV in adenocarcinoma was 54.5%(140/257),the positive rate of P16 was 58.8%(151/257),and the overall consistency rate between P16 and HPV detection was 82.5%(212/257);for HR-HPV infection, the sensitivity and specificity of P16 were 87.9%and 76.1%.In HSIL,the HR-HPV positive rate was 75.9%(60/79),the positive rate of P16 was 70.9%(56/79),and the overall consistency rate between P16 and HR-HPV detection was 82.2%(65/79);for HR-HPV infection, the sensitivity and specificity of P16 were 85.0%and 73.7%.In LSIL,the HR-HPV positive rate was 73.3%(77/105),the positive rate of P16 was 8.5%(9/105),and the overall consistency rate between P16 and HR-HPV detection was 33.3%(35/105);for HR-HPV infection, the sensitivity and specificity of P16 were 10.4%and 96.4%.In chronic cervical inflammation, the HR-HPV positive rate was 20%(17/85),the positive rate of P16 was 0.0%(0/85);for HR-HPV infection, the sensitivity and specificity of P16 were 0.0%and 100.0%.There was a significant positive correlation between P16 positivity and HPV16/18 in cervical squamous cell carcinoma, adenocarcinoma, and HSIL(P=0.000),while there was no significant correlation in LSIL and chronic cervical inflammation(P>0.05).Conclusion:In cervical squamous cell carcinoma and adenocarcinoma, the consistency of P16 expression and HPV DNA positivity are high, especially in HPV16/18 subtype. There is a good concordance between HR-HPV positivity and P16 protein overexpression. The positive expression of P16 in HSIL may initially reflect HPV infection status. However,in LSIL and chronic cervicitis,P16 expression may not accurately correlate with HPV infection. The inconsistency between P16 and HPV DNA testing could be influenced by multiple factors, including HPV subtypes, histopathological categories, specimen quality, and technical limitations. In clinical practice,it is recommended to conduct comprehensive analysis or employ multiple diagnostic methods to confirm HPV infection status for precise evaluation.

Chinese Journal of Preventive Medicine 2025;59(6):916-924
Objective:To analyze and summarize the clinical characteristics of perioperative pneumonia (POP) in elderly patients with hip fractures and to explore the related factors of the occurrence of POP and severe pneumonia. Methods:A cross-sectional study was conducted. The research subjects were hip fracture patients aged 80 years and above who were hospitalized and underwent surgery in Beijing Jishuitan Hospital, Capital Medical University, from January 1, 2014, to December 31, 2022. The basic information and clinical data of the research subjects were collected through the hospital information system. A multivariate logistic regression model was used to analyze the influencing factors of the occurrence of POP and its progression to severe pneumonia. Results:A total of 1 326 elderly patients who underwent hip fracture surgery were included, with an age of [ M ( Q1, Q3)] of 88 (84, 93) years. About 681 patients (51.4%) developed POP, of which 114 patients progressed to severe pneumonia (16.7%). The results of multivariate logistic regression analysis showed that advanced age ( OR=1.056, 95% CI:1.027-1.086), male ( OR=1.416, 95% CI:1.141-1.758), smoking history ( OR=1.159, 95% CI:1.132-1.166), a longer interval between fracture occurrence and surgery ( OR=1.097, 95% CI:1.070-1.125), general anesthesia for surgery ( OR=1.152, 95% CI:1.112-1.175), a history of diabetes ( OR=1.751, 95% CI:1.234-1.983), and higher fasting blood glucose ( OR=1.164, 95% CI:1.061-1.276) were associated with a higher risk of POP ( OR=1.010, 95% CI:1.002-1.017). Patients with a higher proportion of peripheral blood lymphocytes ( OR=0.981, 95% CI:0.971-0.993) and higher albumin levels ( OR=0.966, 95% CI:0.947-0.985) had a lower risk of POP. Further analysis revealed that POP patients with a history of diabetes ( OR=2.316, 95% CI:1.385-2.870), a higher proportion of peripheral blood neutrophils ( OR=1.843, 95% CI:1.790-1.900), higher B-type natriuretic peptide ( OR=1.213, 95% CI:1.197-1.324), and higher procalcitonin ( OR=1.228, 95% CI:1.134-1.329) were more likely to progress to severe pneumonia. However, POP patients with a higher proportion of peripheral blood lymphocytes ( OR=0.806, 95% CI:0.750-0.866) and higher albumin levels ( OR=0.933, 95% CI:0.898-0.969) had a lower risk of progressing to severe pneumonia. Conclusion:The incidence of POP is relatively high in elderly patients with hip fractures aged ≥80 years in the hospital. Age, gender, smoking history, the interval between fracture occurrence and surgery, surgical method, creatinine level, history of diabetes, fasting blood glucose level, the proportion of peripheral blood lymphocytes, and albumin level are influencing factors for the occurrence of POP. History of diabetes, fasting blood glucose, the proportion of peripheral blood neutrophils, B-type natriuretic peptide, procalcitonin, the proportion of peripheral blood lymphocytes, and albumin level are influencing factors for POP patients to progress to severe pneumonia.
