1.Analysis of the changes in liver volume after splenectomy combined with devascularization for patients with cirrhotic portal hypertension

Objective:To investigate the changes of the liver volume in patients with cirrhotic portal hypertension after splenectomy combined with devascularization, and to analyze the related causes.Methods:Clinical and imaging data of 94 patients with cirrhotic portal hypertension who underwent surgical treatment at the People's Hospital of Ningxia Hui Autonomous Region between January 2014 and May 2024 were included and analyzed before and after surgery. The cohort comprised 61 males and 33 females, aged (47±12) years. The preoperative and postoperative liver volumes were compared, and the patients were divided into two groups based on the changes in postoperative liver volume: the volume increase group ( n=51) and the volume decrease group ( n=43). Clinical data were collected including liver volume, hepatic artery diameter, spontaneous portosystemic shunt (SPSS), portal vein thrombosis (PVT), and hepatic artery dilation. Results:Compared with the volume decrease group, the postoperative liver volume [(1 157±237) cm 3 vs. (977±271) cm 3] and incidence of hepatic artery dilation [92.2%(47/51) vs. 67.4%(29/43)] in the volume increase group both increased, while the incidence of newly formed SPSS [3.9%(2/51) vs. 18.6%(8/43)] and PVT [5.9%(3/51) vs. 34.9%(15/43)] after surgery both decreased, and the differences were statistically significant (all P<0.05). After splenectomy combined with devascularization, patients with decreased liver volume exhibited aggravated conditions of serrated or wavy changes at the edge of the liver and widening of liver fissures, while patients with increased liver volume had significant reduction or disappearance of serrated or wavy changes at the edge of the liver and narrowing of liver fission. Conclusion:Splenectomy combined with devascularization can increase the postoperative liver volume in patients with cirrhotic portal hypertension, and decrease in postoperative liver volume may be related to the occurrence of SPSS and PVT.
2.Value of Sonazoid contrast-enhanced ultrasound for preoperatively evaluating pathological grade of hepatocellular carcinoma

Objective To analyze the correlation between the characteristics of Sonazoid contrast-enhanced ultrasound(CEUS)and pathological differentiation in hepatocellular carcinoma(HCC).Methods A total of 64 patients with HCC diagnosed pathologically by CEUS examination were included,and a total of 64 lesions were divided into the high,medium and low differentiation groups(6,48 and 10 cases,respectively)according to the degree of pathological differentiation.The enhancement pattern,enhancement level and enhancement pattern of CEUS arterial stage in HCC with different pathological differentiation were compared.Results The enhancement pattern of arterial phase was divided into the uniform enhancement and the uneven enhancement.All lesions in the low differentiated group and 58.3%in the middle differentiated group showed uneven and high enhancement.In the highly differentiated group,lesions showed homogenous hyperintensification,homogenous isointensification and non-homogenous hyperintensification.At arterial stage,all lesions in the middle and low differentiated groups and 66.7%lesions in the highly differentiated group showed high enhancement,and the enhancement levels of HCC with different differentiation degrees were significantly different(P<0.01).At the portal stage,16.7%,25.0%and 70.0%lesions in the high,medium and low differentiated HCC groups subsided to low enhancement,and the enhancement levels of HCC with different differentiation degrees were significantly different(P<0.05).In the delayed stage,75%lesions in the medium-differentiated group and all lesions in the low-differentiated group showed low enhancement,and 66.7%lesions in the highly differentiated group showed equal enhancement.Enhancement levels of HCC with different differentiation degrees were significantly different(P<0.01).At the Kupffer stage,all lesions in the low differentiated group and 95.8%of the moderately differentiated group showed low enhancement,while 50%lesions in the highly differentiated group still showed equal enhancement,and there were significant differences in the enhancement levels of HCC with different differentiation degrees(P<0.01).The highly differentiated group showed multiple CEUS patterns,the moderately differentiated group mainly showed"fast advance and fast retreat"and"fast advance and slow retreat"patterns,and 90.0%of the low differentiated group showed"fast advance and fast retreat"patterns.There were significant differences in CEUS patterns between HCC with different degrees of differentiation(P<0.01).Conclusion Sonazoid-CEUS has certain value in evaluating the differentiation degree of HCC.

4.Luteolin Protects Cardiomyocytes Cells against Lipopolysaccharide-Induced Apoptosis and Inflammatory Damage by Modulating Nlrp3

Purpose:
In this article, we aimed to investigate the influences of luteolin on inflammatory injury to cardiomyocytes induced by lipopolysaccharide (LPS).
Materials and Methods:
H9c2 cells were pretreated with different concentrations of luteolin (10, 20, and 50 μM) for 12 h and then stimulated with 10 μg/mL LPS or no LPS for 6 h. Cell viability was detected by CCK-8 assay. Cell apoptosis was determined by flow cytometry. QRT-PCR and Western blotting were utilized to examine mRNA and protein levels. ELISA was used to determine the levels of monocyte chemoattractant protein-1, tumor necrosis factor-alpha, interleukin (IL)-6, IL-1β, and IL-18 in cell supernatants among different groups of H9c2 cells. Immunofluorescence was applied to evaluate reactive oxygen species formation in H9c2 cells. M-mode images of echocardiography, the ejection fraction test, fractional shortening test, end-systolic volume test, and end-diastolic volume test of mouse heart function were obtained by ultrasonic electrocardiogram.
Results:
Luteolin could alleviate inflammatory damage and inflammatory factor expression among LPS-induced H9c2 cells. Additionally, we found that luteolin decreased LPS-stimulated inflammatory damage in H9c2 cells by down-regulating NOD-like receptor family pyrin domain containing 3 (Nlrp3). Luteolin also improved myocardial function in mice treated with LPS and reduced myocardial relaxation. Luteolin reversed myocardial histological abnormalities in mice and reduced inflammation and cardiomyocyte apoptosis. Additionally, luteolin inhibited oxidative stress-mediated myocardial and systemic tissue damage in mice. Finally, luteolin reduced LPS-induced inflammatory damage in mouse cardiomyocytes by down-regulating Nlrp3.
Conclusion
We found that luteolin could reduce inflammatory damage to cardiomyocytes induced by LPS by down-regulating Nlrp3.
5.Clinical study on neutrophil to lymphocyte and platelet ratio with acute kidney injury in elderly patients with sepsis

Objective:The present study aimed to explore the clinical value of neutrophil to lymphocyte and platelet ratio (NLPR) for acute kidney injury (AKI) in elderly patients with sepsis.Methods:This was a retrospective analysis of 360 elderly patients with sepsis or septic shock who were admitted to Intensive Care Unit (ICU) of Zhongnan Hospital of Wuhan University. They were divided into AKI and non-AKI groups based on the KDIGO-AKI criteria. The independent risk factors of AKI were identified via logistic regression analysis, and the calculation of the receiver-operating characteristic (ROC) curves were used to evaluate the diagnostic capability of NLPR for AKI and the short-term outcome of sepsis patients.Results:Total of 195 (54.2%) patients were attributed to the AKI group, while 165 (45.8%) patients were allocated to the non-AKI group. The median of NLPR was significantly higher in the AKI group compared with the non-AKI group ( Z=8.640, P<0.001). Compared with the non-AKI group, the in-hospital death and the length of ICU stay increased (all P<0.05). Patients with AKI required more vasoactive drugs, mechanical ventilation, and renal replacement therapy (all P<0.05). After adjusting the demographic and clinical variables, multivariate logistic regression analysis showed that NLPR was an independent risk factor of AKI ( OR=1.016, 95% CI 1.002-1.030, P=0.027). The ROC curves showed the excellent clinical value of NLPR and which was significantly higher than the neutrophil to lymphocyte ratio (NLR) and serum creatinine. In addition, the present study revealed that the NLPR was also positively correlated with the stage of AKI ( r=0.525, P<0.001). Conclusions:The NLPR is derived from a complete blood cell count, as a new comprehensive inflammatory parameter that is simple and easily available, it is an independent risk factor for AKI in elderly sepsis patients, and should be paid attention in the clinical practice.
6.Effects of T-2 toxin on chondrocyte proliferation and cell cycle

Objective To study the effect of T-2 toxin on proliferation and cell cycle of rat chondrocytes,in order to provide a new idea in molecular mechanism of T-2 toxin-induced chondrocyte damage.Methods Primary chondrocytes of neonatal Wistar rats were isolated and stained by toluidine blue staining and type Ⅱ collagen immunofluorescence staining.The effects of different concentrations of T-2 toxin [0 (control),1,5,10,20,50,100 μg/L)] on proliferation of chondrocytes for 24 h were detected by cell counting kit-8 (CCK-8) method,and control,1 (low dose),5 (medium dose),and 10 μg/L (high dose) T-2 toxin were selected for subsequent experiment;cell cycle changes were detected by flow cytometry;Real-time PCR and Western blotting were used to detect the effects of T-2 toxin on mRNA and protein expressions of proliferating cell nuclear antigen (PCNA) and Cyclin D1 in chondrocytes.Results With increase of T-2 toxin concentration (control,1,5,10,20,50,100 μg/L),the cell survival rates [(100.00 ± 0.00)%,(93.12 ± 1.66)%,(77.12 ± 1.11)%,(59.44 ± 4.09)%,(46.64 ± 3.86)%,(38.15 ± 3.37)%,(33.79 ± 0.99)%] were decreased,and the differences were statistically significant (F =139.21,P <0.05).The percentages of quiescent phase/pre-DNA synthesis phase (G0/G1 phase) ceils in 1,5,10 μg/L T-2 toxin groups [(22.03 ± 0.42)%,(30.54 ± 2.61)%,(36.01 ± 1.51)%] were significantly higher than that in control group [(13.79 ± 1.65)%,P < 0.05];the percentages of DNA synthesis phase (S phase) cells [(60.27 ± 3.53)%,(53.88 ±4.38)%,(49.55 ± 2.49)%] were significantly lower than that in control group [(76.72 ± 4.24)%,P < 0.05].The differences of mRNA levels of PCNA and Cyclin D1 between groups were statistically significant (F =46.80,17.97,P < 0.05),and 5,10 μg/L T-2 toxin groups (0.77 ± 0.13,0.79 ± 0.08,0.60 ± 0.07,0.56 ± 0.05) were lower than the control group (0.99 ± 0.02,1.01 ± 0.01,P < 0.05).The expressions of PCNA protein in 5,10 μg/L T-2 toxin groups (0.69 ± 0.03,0.49 ± 0.03) were lower than that in control group (0.92 ± 0.05,P < 0.05);the expressions of Cyclin D1 protein in 1,5,10 μg/L T-2 toxin groups (0.80 ± 0.06,0.60 ± 0.07,0.33 ± 0.13) were lower than that in control group (0.95 ± 0.07,P < 0.05).Conclusion T-2 toxin can inhibit the proliferation of chondrocytes,which may be worked through influencing the expression of cell cycle protein,causing cell cycle arrest,thereby inhibiting DNA synthesis.

Objective To investigate the protective mechanism of MEK1/2 inhibitor PD98059 on ox-LDL induced injury of human umbilical vein endothelial cells (HUVEC),and its influence on the expression of LOX-1.Methods HUVEC damage models were established by using ox-LDL and were treated with PD98059 later,divided into the negative control group,the ox-LDL group,the positive control group and the PD98059+ox-LDL group.The effect of inhibition of MEK1/2 on ox-LDL induced HUVEC damage was measured.Results Compared with the negative control group,the levels in the ox-LDL group of LOX-1,pMEK1/2,RhoA,ROCK1,ROCK2,TNF-α and IL-6 were increased significantly,the proliferations of HUVEC and the productions of NO were decreased (P<0.05).Compared with the ox-LDL group,the levels in the positive control group and the PD98059+ox-LDL group of pMEK1/2,RhoA,ROCK1,ROCK2,TNF-α and IL-6 were decreased,the proliferation of HUVEC and the production of NO were increased (P<0.05).Conclusion PD98059 inhibit the MEK1/2 signaling pathway to suppress the ox-LDL induced damage of HUVEC by decreasing the expression of LOX-1.
8.Clinical outcomes of the vulnerable plaque sealing with drug-eluting stents for the treatment of intermedi-ate coronary stenosis

Objective To evaluate the one-year clinical outcomes in patients with the vulnerable plaque sealing with drug-eluting stents for the treatment of intermediate coronary stenosis. Methods 327 patients with an-giographically intermediate lesions(diameter stenosis 50%~70%)with the vulnerable plaque which were detected by 64 slice coronary CT were prospectively enrolled. Patients were divided into medical therapy group (n = 160) and sirolimus-eluting stent group group(n=160). The incidences of one-year major adverse cardiovascular events (MACE)was evaluated(cardiac death,myocardial infarction ,revascularization). Results The MACE tended to be lower in the sirolimus-eluting stent group than medical therapy group(3.13%vs. 10%,log-rankχ2=6.62,P=0.01). The incident of cardiac death and myocardial infarction were lower in the sirolimus-eluting stent group than medical therapy group(1.25%vs. 5.63%,log-rankχ2=4.61,P=0.03). Conclusion The treatment of the siroli-mus-eluting stent can reduce MACE for the paitents with the vulnerable plaque of intermediate coronary stenosis than medical therapy only.
9.Exploration on the Origin of Qilu Modern Nursing and Some Thoughts

In the early 20th century,the term Nursing was introduced into China.In 1884,with the arrival of the first batch of missionary nurse,modern nursing initiated by Nightingale was introduced into China and greatly changed the way of care.The missionary nurses were full of enthusiasm for the spreading Gospel of Christ,the love for nursing care and the empathy for Chinese people;and encountered the lack of staff and material,the mis-understanding and resistance from the local people and other multiple tests.With the untiring effort of them and the trained local nurses,China had led the international care in nursing education and practice standard.Based on this,nursing in Shandong gradually rose and had an unprecedented development as a new medical subject.By clearing up and consulting Chinese and English historical materials,this paper reviewed the 60 years of the mission-ary nurses in China,saw the early development of nursing in our country from the budding and development of Shandong nursing and provided more insights for understanding and solving the complex problems contemporary Chinese nursing faces.
10.Multivariate analysis and countermeasures of attracted subglottic secretion volume

Objective To explore the factors related to the amount of suction under glottis, look for effective nursing counter measures in order to improve the suction effects. Methods A retrospective analysis of the clinical data of 71 patients with subglottic secretion drainage from 1st January 2013 to 1st May 2014 was performed. The secretion viscosity, posture, the endotracheal tube cuff pressure, patients restlessness, suction pressure, and acute physiology and chronic health evaluationⅡscore were set as independent variables. A multiple linear regression analysis was performed to screen factors that may lead to reduction of suction volume of subglottic secretion and nursing counter measures were planned and implemented accordingly. Results Secretion viscosity, supine, endotracheal tube cuff pressure and patient restlessness were the main reasons that influence the amount of suction under glottis(P<0. 05). Multiple linear regression equation:amount of suction of subglottic secretion/2 h(ml) =4. 868 +2. 470 × restlessness-1. 624 × secretion viscosity+0. 874 × endotracheal tube cuff pressure-0. 933 × posture. Conclusions Quantity of suction of subglottic secretion were positively correlated to patients′restlessness, secretion viscosity and endotracheal tube cuff pressure. Subglottic attract amount for the most when take left lateral position. Strengthening the control and intervention of related factors plays a very important role in the care of suction under glottis.
