1.Clinical study of CT-guided hematoma puncture drainage surgery combined with high-dose urokinase in treating hypertensive intracerebral hemorrhage

Objective:To observe the clinical effect of computed tomography(CT)-guided hematoma puncture drainage surgery combined with high-dose(≥50 000 U)urokinase in treating hypertensive intracerebral hemorrhage.Methods:The case data of a total of 90 patients with hypertensive intracerebral hemorrhage who underwent treatment in Zhangjiakou First Hospital from January 2022 to January 2023 were retrospectively selected as the study subjects.They were divided into a control group and an observation group using the average method,with 45 cases in each group.The conventional group received CT-guided hematoma puncture drainage surgery combined with conventional dose urokinase(20 000 U),while the observation group received CT-guided hematoma puncture drainage surgery combined with high-dose urokinase(50 000 U),with a 6-month follow-up for all patients.The changes of hematoma volume,length of hospital stay,levels of inflammatory indicators,the levels of hemodynamics,clinical efficacy,and incidence of postoperative complications after treatment between two groups were analyzed and compared.Results:After treatment,the hematoma volume at the 1st day[(34.64±5.53)mm3]and the 7th day[(34.64±5.53)mm3]post surgery of observation group were significantly lower than those[(44.57±6.85)mm3 and(16.64±3.24)mm3]of conventional group,and the differences of them between the two groups were statistically significant(t=7.567,10.570,P<0.05),respectively.The length of hospital stay of observation group[(16.86±6.63)d]was significantly lower than that of the control group[(23.47±10.34)d](t=3.610,P<0.05).The levels of matrix metalloproteinase-9(MMP-9),interleukin-6(IL-6),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)at 7th days post surgery of observation group were significantly lower than those of conventional group,and the differences of them between the two groups were statistically significant(t=5.826,10.202,7.661,2.724,P<0.05),respectively.The blood flow levels of middle cerebral artery(MCA),anterior cerebral artery(ACA),and posterior cerebral artery(PCA)after surgery in observation group were significantly higher than those in conventional group(t=2.833,3.329,3.973,P<0.05),respectively.The effective rate of treatment in observation group(95.56%)was significantly higher than that in conventional group(80.00%),and the difference was significant(x2=5.075,P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:CT-guided hematoma puncture drainage surgery combined with high-dose urokinase can significantly increase the dissolution rate of hematoma,and reduce the body's inflammatory response,and improve cerebral hemodynamic indicators,and effectively enhance clinical efficacy in treating hypertensive intracerebral hemorrhage,which have better treatment safety.
2.Construction of an automatic optic disc and cup segmentation and cup-to-disc ratio calculation system for ocular fundus image and its application in glaucoma screening

Objective:To develop a deep learning-based automated analysis system for precise segmentation of the optic cup and disc in fundus images and automatic measurement of the vertical cup-to-disc ratio (CDR) for early risk assessment and screening of chronic glaucoma.Methods:The proposed automated system comprised three modules: a dual coding-attention U-net (DCoAtUNet) segmentation network for optic cup and disc segmentation, a conditional random field (CRF) post-processing module, and a CDR measurement and glaucoma screening module based on the segmentation results.The system was designed to enhance boundary detection accuracy and measurement stability and its performance was evaluated on the publicly available Drishti-GS dataset.The dataset was divided into a training set and a test set in a 1∶1 ratio.Dice coefficient and intersection over union (IoU) were used to quantify segmentation accuracy and regional consistency, while accaracy, precision, recall, and F1-score were employed to assess glaucoma screening performance.Results:The DCoAtUNet combined with CRF post-processing achieved Dice coefficients of 0.976 0 for the optic disc and 0.908 1 for the optic cup, with corresponding IoU values of 0.953 4 and 0.837 9, demonstrating high segmentation precision and stability in boundary identification and region overlap.In glaucoma screening, the system achieved an accuracy of 0.843 1, precision of 0.840 9, recall of 0.973 7, and F1-score of 0.902 4, indicating good diagnostic sensitivity and accuracy.Conclusions:By integrating high-precision segmentation and automated measurement strategies, the DCoAtUNet+ CRF model significantly improves the accuracy and stability of CDR evaluation.It effectively assists in identifying high-risk individuals during early glaucoma screening and shows strong potential for clinical application in computer-aided diagnosis workflows.

Objective:To establish a sensitive and rapid nucleic acid detection method for 2019-nCoV that can be promoted in primary laboratories.Methods:The detection method for 2019-nCoV nucleic acid was established based on the Bubble Mediated Nucleic Acid Denaturation (BMD)-PCR technology with 2019-nCoV specific primers and probes. To validate this method, the laboratory-confirmed clinical samples (including 60 positives and 68 negatives of 2019-nCoV) were used and meanwhile the result were compared with the commercialized 2019-nCoV nucleic acid detection kits. The specificity of this method was validated by using 11 respiratory viruses and enteroviruses which cover common genotypes.Results:The BMD-PCR method showed good specificity in the detection of 2019-nCoV nucleic acid. It had no cross-reactivity with 11 common respiratory viruses and enteroviruses, and the limit of detection was less than 600 copies/ml. In addition, the qualitative result of 128 clinical samples were 100% consistent with the commercialized detection kits. Furthermore, the detection time can be shortened to 28 minutes.Conclusions:BMD-PCR for 2019-nCoV nucleic acid rapid detection could quickly diagnose and identify clinical samples, and could meet the current urgent needs of large-scale population and high-throughput screening which has important public health significance.
7.A case of ichthyosis follicularis, atrichia and photophobia syndrome caused by a novel mutation in the MBTPS2 gene

A case of ichthyosis follicularis,alopecia and photophobia syndrome caused by a novel mutation c.1165C>T in the membrane-bound transcription factor protease site 2 (MBTPS2) gene was firstly reported.The proband presented with dry skin,congenital hairlessness,follicular keratotic papules,photophobia,epilepsy,and mental and motor retardation.Next-generation and Sanger sequencing analysis confirmed that the proband and his mother both had a c.1165C>T (p.pro389Ser) mutation in exon 9 of the MBTPS2 gene.According to the clinical manifestations of the patient and genetic characteristics of the MBTPS2 gene mutation,the patient was diagnosed with ichthyosis follicularis,alopecia and photophobia syndrome.
8. A case of multiple carboxylase deficiency presenting with skin lesions as the initial symptom induced by a novel mutation in the holocarboxylase synthetase gene

A male patient, who was aged 3 months and 12 days, presented with well-circumscribed erythema and scales on the scrotum, perineum, buttocks and perianal region at 1 month after birth. The lesions gradually involved the perioral and axillary regions, flexor aspect of the elbow, popliteal fossa and neck. Shortness of breath, crying, dysphoria and vomiting occurred without fever and cough 3 days before hospitalization. Laboratory examinations at admission showed metabolic acidosis, hyperlactacidemia, hyperammonemia and organic aciduria. Second-generation sequencing and Sanger sequencing of the holocarboxylase synthetase gene revealed a known mutation c.1522C>T in exon 9 and a novel mutation c.1796_1814del in exon 11. According to a guideline from the American College of Medical Genetics and Genomics, this novel mutation was ranked as a pathogenic mutation. The patient was diagnosed as multiple carboxylase deficiency. His clinical symptoms were improved after oral biotin treatment, no neurological symptoms or signs were observed.
