1.Treatment Principles and Paradigm of Diabetic Microvascular Complications Responding Specifically to Traditional Chinese Medicine

To explore the advantages of traditional Chinese medicine (TCM) and integrative TCM-Western medicine approaches in the treatment of diabetic microvascular complications (DMC), refine key pathophysiological insights and treatment principles, and promote academic innovation and strategic research planning in the prevention and treatment of DMC. The 38th session of the Expert Salon on Diseases Responding Specifically to Traditional Chinese Medicine, hosted by the China Association of Chinese Medicine, was held in Beijing, 2024. Experts in TCM, Western medicine, and interdisciplinary fields convened to conduct a systematic discussion on the pathogenesis, diagnostic and treatment challenges, and mechanism research related to DMC, ultimately forming a consensus on key directions. Four major research recommendations were proposed. The first is addressing clinical bottlenecks in the prevention and control of DMC by optimizing TCM-based evidence evaluation systems. The second is refining TCM core pathogenesis across DMC stages and establishing corresponding "disease-pattern-time" framework. The third is innovating mechanism research strategies to facilitate a shift from holistic regulation to targeted intervention in TCM. The fourth is advancing interdisciplinary collaboration to enhance the role of TCM in new drug development, research prioritization, and guideline formulation. TCM and integrative approaches offer distinct advantages in managing DMC. With a focus on the diseases responding specifically to TCM, strengthening evidence-based support and mechanism interpretation and promoting the integration of clinical care and research innovation will provide strong momentum for the modernization of TCM and the advancement of national health strategies.
2.Effects of the fat mass and obesity-associated gene on apoptosis and the inflammatory response of chondrocytes in osteoarthritis

Objective:To explore the effects of the fat mass and obesity-associated gene(FTO)on apoptosis and the inflammatory response of chondrocytes in osteoarthritis(OA).Methods:Differences in FTO expression between normal human cartilage tissue samples and OA cartilage tissue samples were examined.Primary OA chondrocytes were isolated and cultured, and a rat OA model was constructed.The expression of FTO was detected in clinical, animal and cellular samples.Cells were treated with an FTO knockdown lentivirus vector(sh-FTO)and an m 6A methylation inhibitor(cycloleucine). The amount of m 6A and the expression levels of inflammatory cytokines, interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α), were detected.Flow cytometry was used to detect apoptosis in OA chondrocytes, and Western blot was used to detect the expression levels of B-cell lymphoma 2(Bcl-2)and Bcl-2-associated X protein(Bax). Results:Compared with the normal control group, FTO mRNA and protein expression in human OA cartilage tissue, rat OA cartilage tissue and OA chondrocytes was significantly increased(all P<0.05). After FTO knockdown, the level of m 6A increased, the levels of IL-6 and TNF-α decreased considerably, the apoptosis rate decreased, the expression of the proapoptotic protein Bax decreased considerably, and the expression of Bcl-2 increased considerably in primary OA chondrocytes.However, cycloleucine intervention clearly reduced the level of m6A, increased the levels of IL-6 and TNF-α, promoted cell apoptosis and the expression of apoptosis-related proteins, and reversed the effect induced by the FTO knockdown lentivirus in OA chondrocytes(all P<0.05). Conclusions:FTO may be involved in mechanisms related to the action of m 6A to promote OA chondrocyte apoptosis and the inflammatory response, thus accelerating the progression of OA.
3.Qualitative research on experience of caregivers of palliative care for minors: a Meta synthesis

Objective:To systematically review the qualitative research on the care experience of caregivers of palliative care for minors, in order to provide a reference for formulating a care plan that is more in line with the needs of palliative care for minors and their families.Methods:The Cochrane Library, Australia Joanna Briggs Institute Evidence-based Health Care Center Library, Embase, PubMed and Web of Science, CINAHL, China National Knowledge Infrastructure, Wanfang Database, VIP Database and China Biology Medicine Disc were searched by computer for literature on qualitative research on care experience of underage hospice care caregivers from the establishment of the database to December 30, 2023.Results:A total of 11 articles were included and 70 research results were extracted, which were summarized into 7 new categories and formed 2 integrated results: professional caregivers and relative caregivers had multiple care burden, and need multiple care support; professional caregivers and relative caregivers can shoulder the care responsibility of children together, actively respond to the difficulties in care, gain personal care experience and growth.Conclusions:Hospice care for minors has its own characteristics, and caregivers have various care burdens and needs. Special attention should be paid to the emotional experience and care demands of caregivers, and targeted support and protection should be provided to promote the construction of hospice care team for minors in China and improve the level of hospice care for minors in China.
4.Rapid progressive dementia and involuntary movement caused by vitamin B12 deficiency:a case report

A 41-year-old male was presented with rapidly progression memory impairment for 2 months and episodic limb shaking for 2 weeks as the main manifestations.Physical examination showed verbal disadvantage with decreased memory,attention,comprehension,and orientation.Serum vitamin B12 levels decreased,serum anti gastric parietal cell antibodies and anti-intrinsic factor antibodies were positive.Blood analysis showed macrocytic anemia,neuropsychological scale showed functional impairment in multiple cognitive domains,electrophysiological examination showed peripheral nerve damage,cerebrospinal fluid and imaging examination showed no abnormalities.The patient was diagnosed as having vitamin B12 deficiency dementia,vitamin B12 deficiency related involuntary movement and pernicious anemia.Supplementing with B vitamins and folic acid significantly improved cognitive impairment and eliminated symptoms of limb shaking.The purpose of this case report is to enhance the understanding of clinical doctors about dementia and involunting movement caused by vitamin B12 deficiency,in order to diagnose and treat it early.
5.Research Progress in Imaging Investigation of TERT Promoter Mutations in Gliomas

Somatic mutations in the promoter region of telomerase reverse transcriptase(TERT),a critical mechanism for telomerase reactivation,play a key role in tumorigenesis.The status of TERT promoter mutation serves as a crucial molecular biomarker for glioma assessment and classification,and is essential for early diagnosis of glioma subtypes,guiding treatment decision-making,and improving patient prognosis.With the recognition of the importance of molecular subtyping of gliomas,there has been a surge in research on non-invasive prediction of key molecular biomarkers based on preoperative imaging of gliomas,with a particular focus on TERT promoter studies using radiomics approaches.This article presents a comprehensive review of research on TERT promoter mutations in gliomas and imaging-related studies,with the goal of providing insights for future studies on non-invasive prediction of TERT promoters status and offering important references for the precision diagnosis and treatment of glioma patients.
6.Genetic analysis of two novel variants in a Chinese pedigree affected with intellectual disorder.

OBJECTIVE:
To explore the clinical phenotype and genetic characteristics of two siblings with intellectual disability.
METHODS:
Clinical data and peripheral blood samples were collected from the proband, his younger sister and parents whom had presented at Wenzhou Central Hospital in February 2024. Low-coverage massively parallel copy number variation sequencing (CNV-seq) and whole exome sequencing (WES) were carried out for the family. Candidate variants were verified by Sanger sequencing. Prenatal diagnosis was performed on a fetus upon the couple's subsequent pregnancy. The study was approved by the Medical Ethics Committee of Wenzhou Central Hospital (Ethic No. L2024-07-001).
RESULTS:
The proband was a 12-year-old boy who had presented with mental retardation and language delay. His 10-year-old sister also manifested delayed mental and motor development. Whole exome sequencing revealed that the proband and his sister had respectively harbored a novel heterozygous c.3549_3550del (p.Glu1183Aspfs*29) variant of the TRIP12 gene and a novel heterozygous c.99del (p.Ser34Alafs*38) variant of the GRIN2B gene. Sanger sequencing confirmed that both variants had a de novo origin. Based on the guidelines from the American College of Medical Genetics and Genomics (ACMG), both variants were classified as pathogenic (PVS1+PS2_Supporting+PM2_Supporting). Neither variant was found to be carried by the fetus upon prenatal diagnosis.
CONCLUSION
Above variants probably underlay the mental disorders in the two siblings, and the concurrent occurrence of two novel pathogenic variants in a family has been extremely rare.

7.Efficacy and safety of tolvaptan for hyponatremia:an overview of systematic reviews/meta-analysis

OBJECTIVE To reevaluate systematic reviews/meta-analysis of efficacy and safety of tolvaptan for hyponatremia. METHODS Retrieved from CNKI, Wanfang Data, VIP, CBM, PubMed, Embase and the Cochrane Library database, systematic reviews/meta-analysis about tolvaptan for the treatment of hyponatremia were included from the inception to June 15, 2022. After screening literature and extracting data, the PRISMA statement, AMSTAR 2 scale and GRADE method were used to evaluate the reporting quality, methodological quality and evidence quality of the included literature, respectively. RESULTS A total of 6 articles were included, of which 1 was systematic review and 5 were meta-analysis, including 56 outcome indicators. All of the 6 studies had PRISMA scores ranging from 15.0 to 20.5, and the quality of them was moderate. Results of the AMSTAR 2 scale showed that the methodological quality of 5 literatures were very low, and the quality of 1 literature was low. The quality of GRADE evidence showed that there were 6 moderate-quality indicators, 13 low-quality indicators, 35 very low-quality indicators, and 2 indicators that could not be assessed due to missing data. The main factors causing degradation were limitations, inconsistency, imprecision and publication bias. In terms of efficacy, tolvaptan could effectively increase the level of serum sodium, increase urine volume, reduce body weight, reduce abdominal circumference, relieve edema, and reduce alaninetransaminase level. In terms of safety, the incidence of total adverse drug reaction induced by tolvaptan was controversial; it may increase the risk of dry mouth, thirst, frequent urination or excessive correction of serum sodium. CONCLUSIONS Tolvaptan has great efficacy in the treatment of hyponatremia, but serum sodium overcorrection should be avoided in terms of safety. Relevant systematic reviews/meta-analysis have shortcomings of low reporting quality, methodological quality and evidence quality, which may reduce the reliability of the results, so the results should be treated with caution.
8.Red blood cell distribution width and white matter hyperintensities

Dementia is one of the age-related diseases with the fastest increase in incidence. With the increasingly severe aging problem of the population, it further exacerbates the demand and burden of the healthcare system. Red blood cell distribution width (RDW) is an easily obtainable blood routine indicator that reflects the variability of red blood cell size. As an early marker of dementia risk, researches have shown an association between white matter hyperintensities (WMHs) and RDW. This article reviews the relationship between RDW and WMHs.
9.Effectiveness and safety of generic and original drugs of atorvastatin :a systematic review

OBJECTIVE To systema tically evaluate the effectiveness and safety of gen eric and original drugs of atorvastatin , and to provide the latest evidence-based reference for drug selection in clinic. METHODS Retrieved from PubMed ,Cochrane Library,Embase,CNKI,VIP and Wanfang database ,intervention trials and observational studies about generic and original drugs of atorvastatin were collected during the inception to Apr. 2021. After data extraction of literatures met inclusion criteria ,the Cochrane risk bias evaluation tool 5.1.0 was used to evaluate the quality of intervention trials ;Newcastle-Ottawa Scale (NOS)was used to evaluate the quality of observational studies. RevMan 5.4 software was used to conduct meta-analysis ,and descrptive analysis was performed at the same time. RESULTS A total of 24 studies were included ,involving 21 randomized controlled trials (RCTs)and 3 retrospective cohort studies (RCSs),with 20 001 patients involved. There was no statistical significance difference in total incidence of adverse drug reaction (ADR)[OR=1.08,95% CI(0.85,1.37),P=0.55] and the incidence of other ADR(P>0.05). The results of subgroup analysis showed that the reductions of TC and TG of generic drugs produced by Beijing Jialin Pharmaceutical Enterprise (hereinafter refer to Jialin generic drugs )were less than those of the original drug ,and the difference was statistically significant ;compared with original drugs ,there was no significant difference in other indexes or all indexes of the generic drugs from other manufacturers. Compared with original drugs ,the reductions of TC and TG in 20 mg/d group of Jialin generic drugs were less than original drug group ;the degree of TC reduction at 12 and 24 weeks of follow-up and TG reduction at 24 weeks of follow-up were less than those of the original drugs ,the difference was statistically significant ;there was no significant difference in other indexes. The qualitative description of RCS showed that for elderly patients with death/acute coronary syndrome ,there was no statistical difference between the two groups in terms of cardiovascular events or serious side effects. For the adult patients who switched from original drugs to generic drugs ,the effect of generic drugs instead of original drugs would not be reduced ,but the increase of HDL-C was less than that of original drug. CONCLUSIONS In terms of effectiveness,generic drugs of atorvastatin can replace original drugs and caution should be taken on the levels of HDL-C ,TC and TG for long time use ;in terms of safety ,generic drugs are similar to the original drugs.
10.Angiodynamic and optical coupling analysis of skin tissue model under finite pressure.

The pulse amplitude of fingertip volume could be improved by selecting the vascular dense area and applying appropriate pressure above it. In view of this phenomenon, this paper used Comsol Multiphysics 5.6 (Comsol, Sweden), the finite element analysis software of multi-physical field coupling simulation, to establish the vascular tissue model of a single small artery in fingertips for simulation. Three dimensional Navier-Stokes equations were solved by finite element method, the velocity field and pressure distribution of blood were calculated, and the deformation of blood vessels and surrounding tissues was analyzed. Based on Lambert Beer's Law, the influence of the longitudinal compression displacement of the lateral light surface region and the tissue model on the light intensity signal is investigated. The results show that the light intensity signal amplitude could be increased and its peak value could be reduced by selecting the area with dense blood vessels. Applying deep pressure to the tissue increased the amplitude and peak of the signal. It is expected that the simulation results combined with the previous experimental experience could provide a feasible scheme for improving the quality of finger volume pulse signal.
