1.Expression and prognosis of miR-29b-2-5p and STX16 in hepatocellular carcinoma

Objective To investigate the expression and prognostic value of microRNA-29b-2-5p(miR-29b-2-5p)and syntaxin 16(STX16)in hepatocellular carcinoma(HCC).Methods The cancer tissues and adjacent tissues of 88 HCC patients diagnosed in Nantong Tumor Hospital from January 2013 to September 2020 were collected.The expressions of miR-29b-2-5p and STX16 in cancer tissues and adjacent tissues were analyzed by real-time fluorescent quantitative PCR.The expression of STX16 protein in cancer tissues and adjacent tissues were analyzed by immunohistochemistry.Pearson correlation analysis was used to analyze the correlation be-tween miR-29b-2-5p and STX16.Non-parametric χ2 test was used to analyze the relationship between the ex-pression of the two proteins and clinicopathological parameters,and Kaplan-Meier survival curve was used to analyze the relationship between the expression of the two proteins and prognosis.Results The relative ex-pression level of miR-29b-2-5p was 0.780(0.351,1.708)in cancer tissues and 1.014(0.458,3.124)in adja-cent tissues in 88 HCC patients.The relative expression level of miR-29b-2-5p in cancer tissues was signifi-cantly lower than that in adjacent tissues(P=0.012).The relative expression level of STX16 mRNA was 0.775(0.406,0.946)in cancer tissues and 0.368(0.080,1.301)in adjacent tissues in 88 HCC patients.The relative expression level of STX16 mRNA in cancer tissues was significantly higer than that in adjacent tissues(P<0.01).Immunohistochemical results showed that STX16 was expressed in the cytoplasm,and the expres-sion of STX16 in cancer tissues was higher than that in adjacent tissues(P<0.05).The expression of miR-29b-2-5p in cancer tissues was related to cancer stage(P<0.05),and the expression of STX16 was related to cancer stage and alpha-fetoprotein(P<0.05).Patients with low miR-29b-2-5p expression had a shorter over-all survival(OS)than those with high miR-29b-2-5p expression,and patients with high STX16 expression had a shorter OS than those with low STX16 expression(P<0.05).Conclusion The expression of miR-29b-2-5p is low and STX16 is high in HCC tissues.The combined detection of miR-29b-2-5p and STX16 can effectively evaluate the prognosis of HCC patients.
2.Mechanism of levetiracetam on juvenile rats with febrile seizures

Objective To investigate mechanism of levetiracetam(LEV)on rats with febrile seizures(FS)based on the dipeptidyl peptidase-4(DPP4)/glucagon-like peptide-1(GLP-1)/gluca-gon-like peptide-1 receptor(GLP-1 R)signaling axis.Methods SD juvenile rats were randomly di-vided into blank control group,model group,low-dose LEV group,medium-dose LEV group and high-dose LEV group,with 10 rats in each group.The behavioral indicators of seizures in rats were observed.Hematoxylin-eosin(HE)staining was used to detect histopathological damage in the hippo-campus.Nissl staining was employed to observe the number of Nissl bodies in hippocampal neurons.Enzyme-linked immunosorbent assay(ELISA)was utilized to measure the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),glutamate(Glu)and γ-aminobu-tyric acid(GABA)in the hippocampus.Western blot was applied to detect the protein expressions of glial fibrillary acidic protein(GFAP),inducible nitric oxide synthase(iNOS),CD86,arginase 1(Arg1),CD206,brain-derived neurotrophic factor(BDNF),DPP4,GLP-1 and GLP-1 R in the hippocampus.Results Compared with the blank control group,the model group showed histopatho-logical changes in the hippocampus,with a decreased number of Nissl bodies.The relative protein expressions of GFAP,BDNF,iNOS,CD86 and DPP4,as well as the levels of TNF-α,IL-1β,IL-6 and Glu in the hippocampus were increased,while the relative protein expressions of Arg1,CD206,GLP-1 and GLP-1R,and the level of GABA were decreased,with statistically significant differences(P<0.05).Compared with the model group,the seizure latency in the low-dose,medium-dose and high-dose LEV groups was prolonged in a dose-dependent manner,and the incidence of grade 3 to 5 seizures was reduced in a dose-dependent manner.The histopathological damage in the hippo-campus was alleviated,the number of Nissl bodies increased in a dose-dependent manner,the rela-tive protein expressions of GFAP,BDNF,iNOS,CD86 and DPP4,and the levels of TNF-α,IL-1β,IL-6 and Glu were decreased in a dose-dependent manner,while the relative protein expres-sions of Arg1,CD206,GLP-1 and GLP-1 R,and the level of GABA were increased in a dose-de-pendent manner,with statistically significant differences(P<0.05).Conclusion LEV can effec-tively alleviate seizure attacks and histopathological damage in the hippocampus of FS rats,and re-duce hippocampal inflammatory responses and neurotransmitter imbalance.Its mechanism may be re-lated to the inhibition of DPP4 signaling and the activation of the GLP-1/GLP-1R signaling pathway.
3.The relationship between blood cell-related inflammatory markers and diabetic retinopathy: a study from Beichen Eye Study

Objective:To observe the correlation between blood cell-related inflammatory markers and diabetic retinopathy (DR).Methods:A cross-sectional study. From June 2020 to February 2022, the phase Ⅰ data of Beichen Eye Study in Tianjin Medical University Eye Hospital were included in the study. The research contents included questionnaires, routine systemic and ocular examinations, and laboratory blood cell-related indicators including mean platelet volume (MPV), platelet distribution width (PDW), neutrophils, and lymphocytes were performed. Neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) were calculated. The diagnosis and classification of DR referred to the international clinical classification standard of DR. Monocular or binocular DR was defined as DR patients. Participants were categorized into different groups based on whether they had diabetes and whether they had DR. The groups included the no-diabetes group, the diabetes without DR group, and the DR group. The Kruskal-Wallis H test was used for the comparison of quantitative data among multiple groups. Wilcoxon test was used for comparison between the two groups. The χ2 test was used to compare the categorical variables between groups. The variables was adjusted step by step, an unadjusted univariate model was built and the different parameters of the model Ⅰ, Ⅱ, Ⅲ were adjusted. The correlation between MPV, PDW, NLR, PLR, and DR in different models was analyzed by logistic regression. The receiver operating characteristic (ROC) curve was used to analyze the diagnostic efficacy of different NLR models for DR. Results:A total of 3 328 subjects were recruited. Among them, 1 121 (33.68 %, 1 121/3 328) were males and 2 207 (66.32 %, 2 207/3 328) were females. The median age of the included participants was 61.84 (6.05) years. The no-diabetes group, the diabetes without DR group, and the DR group were 2 679, 476, and 173, respectively. There was no significant difference in MPV and PLR among the three groups ( H=5.98, 1.94; P=0.051, 0.379). However, compared with no-diabetes group and the diabetes without DR group, PDW and NLR in the DR group showed an upward trend. In model Ⅲ with completely adjusted related factors, NLR was an independent risk factor for DR in no-diabetes group and DR group [odds ratio ( OR)=1.440, 95% confidence interval ( CI) 1.087-1.920, P=0.041], diabetes without DR group and DR group [ OR=1.990, 95% CI 1.440-2.749, P<0.001]. The results of ROC curve analysis showed that the diagnostic efficiency of NLR model Ⅲ was the highest, the area under the curve was 0.751 (95% CI 0.706-0.796, P<0.001), the optimal cutoff value was 0.390, and the sensitivity and specificity were 74.3% and 64.8%, respectively. Conclusions:The NLR of the DR group is significantly higher than that of the no-diabetes group and diabetes without DR group. NLR is an independent risk factor for DR.
4.Decrease of lncRNA-RMRP expression inhibits proliferation and invasion of human lung cancer cell line A549

Objective To investigate the effect of inhibiting the expression of RNA component of mitochondrial RNA processing endoribonuclease(RMRP)on the proliferation and invasion of human lung cancer cell line A549 in order to provide evidence to support the research on NSCLC mechanism.Methods A total of 122 cases of patients who underwent surgical treatment were selected in Jiaozuo People's Hospital from March 2016 to March 2021.In the same period,50 healthy people from physical examination center were selected as the control group.RT-qPCR was used to detect the expressions of RMRP in blood and tissues.Human lung cancer cell line A549 was cultured and divided into si-RMRP group,siRNA-NC group and blank group.RT-qPCR,CCK-8 and Transwell assays were used to detect the expression of RMRP,proliferation activity and cell counting of invasive cells.Results The rel-ative expression level of RMRP in the blood of NSCLC patients was significantly higher than that of control group(P<0.001).The relative expression level of RMRP in NSCLC tissues was significantly higher than the adjacent tissues(P<0.001).The relative expression level of RMRP in blood and tissues of patients with poorly differenti-ated,lymph node metastasis and TNM stage Ⅲ compared with moderate to highly differentiated,no lymph node metastasis and TNM stage Ⅰ-Ⅱ were significantly increased(P<0.05).The relative expression level of RMRP in the cells of the si-RMRP group was lower than that of blank and the siRNA-NC group(P<0.001).Compared with the blank group and the siRNA-NC group,the absorbance(A)value of cells at 24,48,72 and 96 h in the si-RMRP group was decreased(P<0.05).The number of invasion cells in the si-RMRP group was lower than that in the blank group and the siRNA-NC group(F=27.765,P<0.001).Conclusions The relative expression levels of RMRP in blood and tissues of NSCLC patients are increased.Down-regulation of the expres-sion of RMRP gene in A549 cells can inhibit cell proliferation and reduce cell invasion.

6.Preoperative corticosteroids use as an adjunctive treatment for rhegmatogenous retinal detachment associated with choroidal detachment: a network meta-analysis

Objective:To systematically review the efficacy of preoperative corticosteroids use as an adjunctive treatment for rhegmatogenous retinal detachment associated with choroidal detachment (RRDCD).Methods:A evidence-based medicine study. The National Library of Medicine's PubMed, Web of Science, CNKI, and WanFang database were searched. Clinical controlled studies were selected the study object was RRDCD patients and the interventions were preoperative corticosteroids used as an adjunctive treatment. The search was conducted from January 2000 to January 2022. Duplicated, incomplete, or irrelevant articles were excluded. The conventional meta-analysis was used to evaluate the efficacy of corticosteroids used before surgery. The network meta-analysis was used to directly or indirectly compare the efficacy of oral corticosteroids or intravenous dexamethasone, peribulbar injection of glucocorticoids, prednisolone acetate eye-drops, intravitreal injection of triamcinolone acetonide (TA) and posterior sub-tenon injection of triamcinolone acetonide. Publication bias was evaluated by funnel plot.Results:According to the search strategy, 43 articles were initially retrieved, and 929 eyes of 13 articles were finally included for analysis; 6 and 10 articles were included in the traditional meta-analysis and the network meta-analysis. Among the 6 studies included in the conventional meta-analysis, 5 studies were retrospective and 1 study was a randomized controlled trial, involving a total of 575 eyes. The analysis results showed that there was no significant difference in the primary retinal reattachment rate between the corticosteroids group and the control group [odds ratio ( OR)= 1.53, 95% confidence interval ( CI) 0.67-3.53, P=0.314]. Among the 10 studies included in the network meta-analysis, 7 studies were retrospective trials, 2 studies were randomized controlled trials, and 1 study was prospective trial, involving a total of 575 eyes. The analysis results showed that there were significant differences in the primary retinal reattachment rate between the triamcinolone acetonide intravitreal injection group and the no corticosteroid treatment group ( OR=4.09, 95% CI 1.06-15.79). Sub-tenon injection triamcinolone acetonide had a higher incidence rate of ocular hypertension than oral glucocorticoid or intravenous dexamethasone ( OR= 4.47, 95% CI 1.42-14.13). Conclusions:Triamcinolone acetonide intravitreal injection before surgery can improve the primary retinal reattachment rate in RRDCD patients. Patients with the posterior sub-tenon injection of triamcinolone acetonide should be alert to elevated intraocular pressure.
7.Symptom clusters and sentinel symptoms in patients with indwelling tracheal cannula after head and neck cancer surgery

Objective:To understand the symptom clusters of patients with indwelling tracheal cannula after head and neck cancer surgery, and to identify the sentinel symptoms of different symptom clusters.Methods:A total of 197 patients with head and neck cancer who underwent surgical treatment in Department of Head, Neck and Thyroid Surgery in Cancer Hospital Affiliated to Zhengzhou University (Henan Cancer Hospital) from June 2020 to December 2021 were selected as the research objects by the convenient sampling method. On the 7th day after surgery, general information questionnaire and Anderson Symptom Inventory, Head and Neck Module were used, and exploratory factor analysis was used to extract symptom clusters. Apriori association analysis was used to identify sentinel symptoms in the symptom clusters.Results:There were 4 symptom clusters in patients with indwelling tracheal cannula after head and neck cancer surgery, including head and neck cancer-specific symptom cluster, pain-digestive tract symptom cluster, voice/swallowing disorder symptom cluster and fatigue-sleep-emotional symptom cluster. Oral and throat mucus was the sentinel symptom of head and neck cancer-specific symptom cluster, nausea was the sentinel symptom of pain-digestive tract symptom cluster, numbness and needling sensation were the sentinel symptom of voice/swallowing disorder symptom cluster, sadness was the sentinel symptom of fatigue-sleep-emotional symptom cluster.Conclusions:Patients with indwelling tracheal cannula after head and neck cancer surgery have more symptoms, which are combined into symptom clusters, and sentinel symptoms exist in each symptom cluster, suggesting that medical workers should take sentinel symptoms as the entry point for symptom cluster management to achieve effective management of symptoms.
8.Symptom cluster characteristics of postoperative patients with head and neck cancer based on the theory of unpleasant symptoms and its influencing factors

Objective:To explore the symptom cluster characteristics of early postoperative patients with head and neck cancer and analyze its influencing factors.Methods:From June 2020 to June 2022, convenience sampling was used to select 230 head and neck cancer patients who underwent surgery in the Head and Neck Thyroid Surgery of Henan Cancer Hospital as the research subject. The questionnaire survey was conducted using the Demographic and Disease Characteristics Questionnaire, Chinese version of M.D. Anderson Symptom Inventory-Head & Neck, Self-perceived Burden Scale for Cancer Patient, and Medical Outcome Study Social Support Scale. Exploratory factor analysis and multiple linear regression analysis were used for statistics.Results:A total of 230 questionnaires were distributed, and 215 qualified questionnaires were collected, with an effective rate of 93.48%. Postoperative patients with head and neck cancer had four symptom clusters, including pain-gastrointestinal symptom cluster, fatigue-sleep-emotional symptom cluster, head and neck cancer-specific symptom cluster, and vocal/swallowing disorders symptom cluster. Medical social support, per capita monthly income of families, type of head and neck cancer, self-perceived burden and complications were the influencing factors of pain-gastrointestinal symptom cluster ( P<0.05). Medical social support, disease staging, complications, self-perceived burden, and per capita monthly income of families were the influencing factors of fatigue-sleep-emotional symptom cluster ( P<0.05). Medical social support, complications, treatment methods, and type of head and neck cancer were the influencing factors of head and neck cancer-specific symptom cluster ( P<0.05). Medical social support, complications, treatment methods, course of disease, and per capita monthly income of families were the influencing factors of vocal/swallowing disorders symptom cluster ( P<0.05) . Conclusions:Postoperative patients with head and neck cancer have multiple symptom clusters and are influenced by psychological, social, and disease factors. Medical and nursing staffs should pay attention to the symptom cluster of postoperative head and neck cancer patients with low levels of social support, heavy self-perceived burden, and complications, and scientifically formulate interventions to reduce the burden of patient symptom cluster and promote rehabilitation.

Monkeypox is a zoonotic disease.Previous studies have shown that children are vulnerable to monkeypox and are also at high risk for severe disease or complications.In order to improve pediatricians′ understanding of monkeypox and achieve early detection, early diagnosis, early treatment and early disposal, the committee composed of more than 40 experts in the related fields of infectious diseases, pediatrics, infection control and public health formulate this expert consensus, on the basis of the latest clinical management and infection prevention and control for monkeypox released by the World Health Organization (WHO), the guidelines for diagnosis and treatment of monkeypox (version 2022) issued by National Health Commission of the People′s Republic of China and other relevant documents.During the development of this consensus, multidisciplinary experts have repeatedly demonstrated the etiology, epidemiology, transmission, clinical manifestations, laboratory examinations, diagnosis and differential diagnosis, treatment, discharge criteria, prevention, case management process and key points of prevention and control about monkeypox.

Since December 2019, severe acute respiratory syndrome coronavirus 2(SARS-CoV-2) infections have raged globally for more than 2 years.China has always adopted scientific and effective prevention and control measures to achieved some success.However, with the continuous variation of SARS-CoV-2 cases and imported cases from abroad, the prevention and control work has become more difficult and complex.With the variation of the mutant strain, the number of cases in children changed, and some new special symptoms and complications were found, which proposed a new topic for the prevention and treatment of SARS-CoV-2 infection in children in China.Based on the third edition, the present consensus according to the characteristics of the new strain, expounded the etiology, pathology, pathogenesis, and according to the clinical characteristics and experience of children′s cases, and puts forward recommendations on the diagnostic criteria, laboratory examination, treatment, prevention and control of children′s cases for providing reference for further guidance of effective prevention and treatment of SARS-CoV-2 infection in children in China.
