ObjectiveTo construct a competency model for "dual-qualified" teachers in vocational undergraduate rehabilitation therapy programs based on the World Health Organization's rehabilitation competency framework (RCF) and relevant domestic and international standards, and explore the training pathways for rehabilitation therapy teachers in China. MethodsLiterature analysis and comparative research methods were employed to systematically review RCF, physiotherapist-education-framework-FINAL, the Minimum-Standards-for-the-Education-of-Occupational-Therapists, and the "dual-qualified" teacher standards in vocational education for China. Focusing on the characteristics of vocational undergraduate talent cultivation, it took the integration of the "educator-clinical rehabilitation therapist" dual roles as its starting point, combining rehabilitation industry competence with educator standards as its basic principle, and using RCF as its foundational dimension. It then integrated advanced competency dimensions required of vocational undergraduate teachers to form a dimensional framework. Following the systematic curriculum development concept of the work process, the core activities of each dimension were designed by combining, improving and expanding the concept integration method, and describing the ability characteristics in a graded and progressive manner, to construct a model. ResultsThis model was guided by the core concepts of three types of values ​​and four types of professional beliefs. Vertically, it encompassed 28 activities across eight competency dimensions, including teaching practice, rehabilitation practice, digital literacy, professionalism, learning and development, management and leadership, applied research, and social service. Horizontally, it encompassed 53 corresponding competency characteristics across three advanced levels (beginner, intermediate and advanced). ConclusionThis model, through its vertical dimension design and horizontal progressive development, clearly describes the advancement of vocational undergraduate rehabilitation therapy teachers from newly hired teachers to backbone teachers and professional leaders. It not only aligns with the general requirements of the national standards for "dual-qualified" teachers in higher vocational colleges in China, but also incorporates an international perspective on rehabilitation education and practice, and possesses the characteristics of vocational undergraduate education that integrates multiple abilities and roles. At the theoretical level, it provides core theoretical support for establishing standardized and internationalized "dual-qualified" teacher standards for vocational undergraduate rehabilitation therapy programs in China; at the practical level, it serves as an "action map" for teacher professional development, assessment and evaluation, providing a key reference for constructing a systematic high-end skilled rehabilitation talent training system.

Neonatal diabetes mellitus （NDM） is a rare monogenic disorder primarily caused by insufficient insulin secretion resulting from mutations in the KCNJ11 and ABCC8 genes. Sulfonylureas， represented by glibenclamide， have become the standard therapy for this type of NDM by precisely closing the mutated ATP-sensitive potassium channels in pancreatic β cells， thereby restoring insulin secretion. Clinical studies confirm that sulfonylureas enable over 90% of patients to successfully transition from insulin to oral treatment， achieving long-term stable glycemic control and improving neurological outcomes to a certain extent. In terms of safety， severe hypoglycemia induced by sulfonylureas is relatively rare and gastrointestinal reactions are mild； moreover， sulfonylureas show good long-term tolerability， and have no adverse effects on child growth and development. In the future， by further refining the full-chain management pathway of “rapid genetic diagnosis-early intervention-specialized dosage forms-long-term follow-up”， the clinical application of sulfonylureas is expected to provide NDM patients with an optimized treatment regimen and maximize their health benefits.

Small nuclear RNAs (snRNAs) refer to a class of highly abundant and functionally important non-coding small RNAs that are localized in the eukaryotic nucleus. These snRNAs are highly conserved in different eukaryotes during evolution and form complexes with specific chaperones to fulfill critical biological functions, including precursor messenger RNA (pre-mRNA) splicing and ribosomal RNA (rRNA) modification. Consequently, the regulation of snRNA gene expression is a crucial biological process for plants. In plants, the transcription and processing of snRNAs are regulated by RNA polymerase (Pol), snRNA-activating protein complex (SNAPc), defective in snRNA processing (DSP), and specific cis-elements in the snRNA promoter regions. Proper regulation of snRNA expression is essential for normal plant growth, development, and stress responses. This review summarizes the classification, structures, transcriptional regulation, and biological functions of plant snRNA genes, while outlining future research directions for snRNAs.

Objective:To investigate the induction effect of different concentrations of glucose on the polarization of Raw264.7 macrophages in vitro.Methods:The Raw264.7 cells cultured in DMEM medium were divided into control group(5.5 mmol·L-1 glucose),different high-glucose groups(15.0,25.0,35.0,and 45.0 mmol·L-1 glucose),and positive control group[lipopolysaccharide(LPS)].The cells were cultured for 3,6,and 9 h,respectively.Cell morphology was observed in various groups.Cell counting kit-8(CCK-8)method was used to assess the survival rates of cells in various groups.Real-time fluorescence quantitative PCR(RT-qPCR)method was used to detect the expression levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and interleukin-10(IL-10)mRNA in cells in various groups.Enzyme-linked immunosorbent assay(ELISA)was used to measure the levels of IL-6,TNF-α,and IL-10 in the cell supernatants in various groups.Flow cytometry was used to determine the percentages of M1 and M2 macrophage markers,CD86+and CD163+cells,in various groups.Results:In control group,theRaw264.7 cells adhered to the culture dish,with a predominantly round morphology.The cells in 35.0 mmol·L-1 high-glucose group and positive control group showed elongated shapes and pseudopodium formation,indicating inflammatory changes.Compared with control group,the survival rates in different high-glucose groups at 6,12,24,and 48 h after treatment were increased(P＜0.05).Compared with control group,after 3 h of treatment,the expression levels of IL-6 and IL-10 mRNA in the cells in 35.0 mmol·L-1 high-glucose group were increased(P＜0.05 or P＜0.01),while no significant changes were observed in IL-6,TNF-α,and IL-10 levels in the cell supernatants(P＞0.05);after 6 h of treatment,the expression level of TNF-α mRNA in the cells in 35.0 mmol·L-1 high-glucose group was increased(P＜0.001),and the levels of IL-6,TNF-α,and IL-10 in the cell supernatants were significantly increased(P＜0.05 or P＜0.001);after 3 h of treatment,the percentages of macrophage markers CD86+cells and CD163+cells in 35.0 mmolL-1 high-glucose group were significantly increased(P＜0.01 or P＜0.001).Conclusion:A certain high concentration of glucose may induce the polarization of Raw264.7 macrophages towards the M1 subtype in vitro.

Objective:To develop and validate a clinical warning model for severe pediatric community-acquired pneumonia (CAP) using electronic health records.Methods:A retrospective cohort study was conducted, analyzing clinical data of 15 750 children hospitalized for CAP at Wuhan Children′s Hospital between January 1, 2019, and December 31, 2023. Patient data were randomly split into training and testing sets at a 7∶3 ratio. Six supervised machine learning models were constructed in the training set, optimized using five-fold cross-validation, and evaluated in the testing set. Model performance was assessed using ROC-AUC, sensitivity, specificity, positive predictive value, negative predictive value, calibration curves, and clinical decision curve analysis at optimal thresholds. The best-performing model was selected, and SHapley Additive exPlanations (SHAP) were used to interpret feature importance. A program interface was developed based on the model results, enabling integration into clinical decision support systems for automated early warning.Results:A total of 15 750 participants, ranging in age from 28 days to 18 years, were included in the study. The median age was 2 years [interquartile range (IQR): 0-4 years], with 9 555 males (60.67%) and 6 195 females (39.33%). Among them, 2 211 (14.04%) developed severe pneumonia. In the prediction models, XGB outperformed other models with an ROC-AUC of 0.884 (95% CI: 0.870-0.898), sensitivity (0.803, 95% CI: 0.772-0.832), specificity (0.828, 95% CI: 0.816-0.839). Calibration analysis showed strong agreement between predicted and observed risks (Brier score: 0.081, 95% CI: 0.075-0.086). The analysis based on the SHAP method revealed that respiratory rate, heart rate, T-lymphocyte subsets, and red blood cell volume distribution width-SD are predictive factors for severe progression of community-acquired pneumonia (CAP) in children. Conclusion:An interpretable machine learning model was developed for the early detection and personalized treatment planning of severe CAP in children, providing valuable support to clinicians.

Human herpesvirus-6B (HHV-6B) reactivation is a significant contributor to nonrelapse mortality following allogeneic hematopoietic stem cell transplantation (allo-HSCT). This retrospective analysis describes three cases of HHV-6B reactivation following allo-HSCT, all of which achieved viral clearance with maribavir treatment following failure or intolerance to first-line antiviral therapy with foscarnet sodium. One patient diagnosed with HHV-6B encephalitis recovered without neurological sequelae. No adverse drug reactions to maribavir were observed. These findings provide preliminary evidence that maribavir may serve as an effective and safe salvage therapy for HHV-6B reactivation in patients receiving allo-HSCT.

The cooccurrence of NPM1, FLT3-ITD, and DNMT3A mutations (i.e., triple mutation) is related to dismal prognosis in patients with acute myeloid leukemia (AML) receiving chemotherapy alone. In this multicenter retrospective cohort study, we aimed to identify whether allogeneic hematopoietic stem cell transplantation (allo-HSCT) could overcome the poor prognosis of DNMT3A^mutNPM1^mutFLT3-ITD^mut AML across four transplant centers in China. Fifty-three patients with triple-mutated AML receiving allo-HSCT in complete remission were enrolled. The 1.5-year probabilities of relapse, leukemia-free survival, and overall survival after allo-HSCT were 11.9%, 80.3%, and 81.8%, respectively. Multivariate analysis revealed that more than one course of induction chemotherapy and allo-HSCT beyond CR1 were associated with poor survival. To our knowledge, this work is the largest study to explore the up-to-date undefined role of allo-HSCT in patients with triple-mutated AML. Our real-world data suggest that allo-HSCT could overcome the poor prognosis of DNMT3A^mutNPM1^mutFLT3-ITD^mut in AML.

Objective To analyze the antiviral effect of molnupiravir against influenza virus in vitro and in vivo.Methods The antiviral ability of molnupiravir against influenza virus strain H1N1 PR8 was detected in vitro and in vivo.H uman non-small cell lung cancer cell line(A549 cells)was used as an in vitro model of PR8 infection.The antiviral effects of molnupiravir on virus replication,protein synthesis,and viral particle formation were analyzed using real-time fluorescence quantitative polymerase chain reaction(qRT-PCR),Western blot(WB)assay,and plaque assay.PR8 nose-dripping infected C57BL/6 female mice were used as an in vivo infection model.The antivi-ral ability of molnupiravir was evaluated by detecting viral load,pathological changes,and immunohistochemistry in the control group,administration group,inoculation group,and inoculation-administration group.Results In vitro test results showed that molnupiravir had no significant inhibitory effect on influenza virus replication,protein syn-thesis,and virus particle formation(all P＞0.05).In vivo test results showed that compared with mice infected with H1N1 alone,the viral load in the lung tissue of mice treated with molnupiravir declined significantly,and the extent of pathological changes was milder.Immunohistochemical detection showed a significant weakening in nuclear protein(NP)antigen signal,and the expression levels of interferon(IFN)-α,interleukin(IL)-4,and IL-6 in the lungs were lower(all P＜0.01).Conclusion As a precursor with antiviral activity,molnupiravir can not exert anti-viral activity in lung-derived cells cultured in vitro,but can be transformed into an active form in the host,which significantly decreases the ability of H1N1 influenza virus to proliferate in the lungs and thereby alleviates the da-mage of virus to lung tissue.
