1.The relationship between SERPINA4 expression and prognosis in metastatic color-ectal cancer

Purpose To investigate the relationship between SERPINA4 expression and clinicopathological fea-tures and prognosis in metastatic colorectal cancer(CRC).Methods EnVision two-step immunohistochemical method was used to detect the expression of SERPINA4 protein in 104 cases of primary and metastatic tumor paraffin tissues of metastatic colorectal cancer and adjacent intestinal normal mucosa,and its relationship with clinicopathological features of patients was analyzed.qRT-PCR was used to detect the expression of SERPINA4 mRNA in 11 pairs of metastatic CRC metastases and paired primary tumor paraffin-embedded tissue samples.The survival of patients was followed up,and the relationship between SERPINA4 expression and patient prognosis was analyzed by Kaplan-Meier method.Re-sults Immunohistochemical results showed that the strong positive expression rate of SERPINA4 was 97.10%in nor-mal mucosa,71.20%in primary lesions,and 39.40%in metastatic lesions.The difference between the three groups was statistically significant(P<0.001).qRT-PCR results showed that SERPINA4 mRNA was lowly expressed in me-tastatic lesions of metastatic CRC(P<0.05).The expression of SERPINA4 in metastatic lesions was significantly cor-related with histological grade,N stage and M stage of the tumor(P<0.05),but not significantly correlated with age,gender,tumor location and T stage of patients(P>0.05).The survival time of patients with metastatic colorectal cancer with high expression of SERPINA4 was longer than that of patients with low expression.Conclusion The ex-pression of SERPINA4 is of great significance in the survival and prognosis of patients with metastatic CRC.

3.Effects of quality of life in patients with intermediate and advanced lung cancer undergoing different treatment regimens

Objective:
To examine the impacts of chemotherapy/radiotherapy, targeted therapy, and combined treatment on the quality of life in patients with intermediate and advanced lung cancer.
Methods:
The patients with intermediate and advanced lung cancer undergoing chemotherapy/radiotherapy, targeted therapy, and combined treatment for the first time were recruited from a tertiary hospital in Weifang City, Shandong Province, using a quota sampling method in September 2023. Basic information was collected using a general information questionnaire, and the quality of life was assessed using the Chinese version of Functional Assessment of Cancer Therapy-General. The investigation started on the 7th day of treatment, and the follow-ups were conducted at 3 and 6 months. The quality of life in patients with different treatment regimens and at different treatment time were compared using repeated measure analysis of variance.
Results:
There were 26 chemotherapy/radiotherapy patients, 32 targeted therapy patients, and 95 combination therapy patients. There were no significant differences in age, gender, place of residence, education level, self-rated economic status, medical insurance, pathological type and disease stage among the three treatment regimens (all P>0.05). The repeated measure analysis of variance showed an interaction effect between time and group among patients receiving the three treatment regimens (P<0.05). The quality of life scores of patients receiving combination therapy decreased with extended treatment time (all P<0.05). The quality of life scores of patients receiving targeted therapy at 3 and 6 months were lower than those treated for 7 days (both P<0.05). No significant differences were observed in quality of life scores among chemotherapy/radiotherapy patients with different treatment durations (all P>0.05). At 3 and 6 months, patients receiving combination therapy had lower quality of life scores compared to those receiving chemotherapy/radiotherapy or targeted therapy (all P<0.05).
Conclusion
The decline in quality of life for patients with intermediate and advanced lung cancer undergoing chemotherapy/radiotherapy and targeted therapy is less than that for patients receiving combined therapy.
4.Clinical application value of intracavitary PRP infusion combined with IVF-FET in patients with chronic endometritis

[Objective] To evaluate the clinical application value of intrauterine perfusion with platelet-rich plasma (PRP) combined with in vitro fertilization-frozen-thawed embryo transfer (IVF-FET) in patients with chronic endometritis (CE). [Methods] A randomized controlled trial (RCT) was conducted, enrolling 60 CE patients undergoing artificial cycle frozen embryo transfer at our hospital from January 2022 to January 2024. Participants were randomly divided into three groups: Group A (routine frozen embryo transfer, n=20), Group B (routine frozen embryo transfer + one PRP intrauterine perfusion, n=20), and Group C (routine frozen embryo transfer + two PRP intrauterine perfusions, n=20). Endometrial thickness during the transformation and transplantation phases, uterine artery pulsatility index (PI), resistance index (RI), systolic peak velocity/end-diastolic velocity (S/D) ratio during transplantation, serum levels of IL-2, IL-4, IL-6, IL-10, and TNF-α during transplantation, as well as biochemical pregnancy rate, clinical pregnancy rate, live birth rate, and early miscarriage rate were compared across groups. [Results] No significant differences in endometrial thickness were observed among the three groups during the transformation phase (P>0.05). During the transplantation phase, endometrial thickness in Groups C and B was significantly higher than in Group A[9.54 (8.96-10.22) and 8.90 (8.34-9.72) vs 8.37 (7.89-8.75) mm, P<0.05], with Group C showing greater thickness than Group B (Z=3.733, P<0.05). Endometrial thickness in Groups C and B during transplantation was significantly increased compared to their respective transformation phases (Z=2.191, 2.462; P<0.05). Groups C and B exhibited lower PI, RI, and S/D values than Group A[PI:1.87 (1.77-1.97), 1.94 (1.88-2.15) vs 2.43 (2.35-2.49); RI:0.75 (0.73-0.77), 0.78 (0.75-0.81) vs 0.84 (0.83-0.86); S/D:2.61 (2.33-3.42), 3.01 (2.20-3.93) vs 3.72 (3.06-4.49); P<0.05]. Group C demonstrated lower PI and RI than Group B (P<0.05). IL-2 levels in Groups C and B were higher than in Group A[3.88 (2.71-5.01), 3.59 (2.73-4.38) vs 3.16 (2.11-3.25) ng/L, P<0.05], while IL-4, IL-6, IL-10, and TNF-α levels were significantly lower (IL-4: Z=1.428, 2.421; IL-6: Z=1.754, 2.435; IL-10: Z=1.754, 2.854; TNF-α: Z=1.961, 1.765; P<0.05). Group C had lower IL-6 levels than Group B (Z=3.976, P<0.05). Biochemical pregnancy rate, clinical pregnancy rate, and live birth rate in Group C were significantly higher than in Group A (75% vs 40%, 70% vs 35%, 60% vs 20%, P<0.05). No significant differences in early miscarriage rates were observed among the groups (χ2=3.750, P>0.05). [Conclusion] Intrauterine autologous PRP perfusion in CE patients enhances pregnancy and live birth rates, improves pregnancy outcomes post-FET, and demonstrates superior efficacy in endometrial repair and receptivity with two PRP perfusions compared to a single perfusion. This provides a safe and effective therapeutic option for optimizing outcomes in CE patients with prior implantation failure.
5.Visualization analysis of research progress on carbapenem-resistant Gram-negative bacteria based on VOSviewer and CiteSpace

Objective:Bibliometric analysis was performed to map scientific knowledge landscape, so that to explore the research status and future trends in the field of carbapenem-resistant Gram-negative bacteria (CRGNB) over the past decade.Methods:Literature on CRGNB published between January 1st, 2015 and December 31st, 2024 was retrieved from the China National Knowledge Internet (CNKI) database and Web of Science Core Collection (WoSCC). VOSviewer and CiteSpace were used for bibliometric analysis.Results:A total of 3 340 Chinese and 10 761 English publications were included in this study. The annual Chinese publications remained stable, while English publications exhibited a linear growth. It was anticipated that the English publications would decline in the forthcoming years, although remaining high. China contributed the highest number of publications, and Zhejiang University was the institution with the largest number of publications. Bonomo RA, Chen L, etc. were high-impact authors in the field of CRGNB and had formed a stable cooperative group. Antimicrobial Agents and Chemotherapy was the journal with the largest number of publications. High-frequency keywords in the domain of CRGNB were comprehensively categorized into four distinct clusters, including carbapenem resistance mechanisms and gene transmission, antimicrobial drugs and combination therapy, management of critically ill patients, and infections and colonization. It was imperative to acknowledge the significance of all of these research areas. Burst word analysis suggested that carbapenem-resistant Enterobacterales virulence genes as well as new isoforms of Klebsiella pneumoniae carbapenemase (KPC) had become a research hotspot. Conclusions:The issue of carbapenem resistance remains a significant concern. Current research focus on the resistance mechanisms and antimicrobial agents, highlighting its significant academic advancement and practical applications. Fostering international collaboration through academic exchanges between research teams worldwide is imperative to establish robust cooperative relationships, facilitate multidisciplinary cooperation, and promote high-quality research.
6.Diagnostic value and influencing factors of endoscopic ultrasonography for rectal neuroendocrine neoplasms

Objective:To investigate the diagnostic value and influencing factors of endoscopic ultrasonography (EUS) for detecting rectal neuroendocrine neoplasms (R-NENs).Methods:A retrospective case-control study was performed on data of patients with suspected R-NENs by white light endoscopy who underwent endoscopic diagnosis and treatment or surgical operation and obtained pathological diagnosis at the Affiliated Hospital of Qingdao University from March 2016 to June 2023. Clinical data, EUS characteristics and pathological results were statistically analyzed, and the diagnostic accuracy of EUS for R-NENs were obtained by comparing the EUS results with the pathological results. Influencing factors affecting accuracy were analyzed by using the binary logistic regression model.Results:A total of 317 patients were included. The sensitivity, the specificity, the positive predictive value and the negative predictive value of EUS in diagnosing R-NENs were 98.03% (249/254), 34.92% (22/63), 85.86% (249/290) and 81.48% (22/27) respectively. The accuracy was 85.49% (271/317) and the Jorden index was 0.33. Tumor size ≤5 mm ( P=0.002, OR=2.892, 95% CI: 1.464-5.713), absence of surface vascular dilation ( P=0.019, OR=2.613, 95% CI: 1.170-5.837), normal tumor coloration ( P=0.001, OR=3.460, 95% CI: 1.645-7.279) and erythematous surface appearance ( P=0.048, OR=7.242, 95% CI: 1.015-51.680) were independent risk factors affecting the accuracy of R-NENs diagnosis by EUS. Depth assessment accuracy of EUS was 76.77% (195/254), with echo heterogeneity ( P<0.001, OR=4.008, 95% CI: 1.980-8.113) and surface depression ( P=0.035, OR=2.664, 95% CI: 1.073-6.615) emerging as significant factors affecting invasion depth evaluation. Conclusion:EUS demonstrates substantial clinical utility for R-NENs assessment, with diagnostic performance being significantly associated with tumor morphology and sonographic features. Macroscopic characteristics including tumor size, vascular patterns, and chromatic features influence diagnostic accuracy, while echo-textural heterogeneity and surface depression affect invasion depth precision. These findings underscore the clinical relevance of comprehensive EUS evaluation in R-NENs management.
7.Analysis of influenza vaccination intention and influencing factors among urban and rural residents aged ≥50 in Zhejiang Province, in 2024

Objective:To analyze the intention to receive influenza vaccination and its associated factors among urban and rural residents aged≥50 in Zhejiang Province, China, in 2024.Methods:This cross-sectional study was conducted between March and May 2024 through a multi-stage intercept survey in 35 counties (cities, districts) selected from a total of 90 in Zhejiang Province, involving 175 communities/villages. The survey targeted residents visiting local hospitals and community health service centers/community health clinics. The questionnaire included basic demographic information, knowledge related to influenza prevention and treatment, chronic disease status, history of influenza vaccination, and intention to vaccinate. Urban and rural residents were classified based on their registered residence, according to the "Rules for Compilation of Statistical Regional Code and Urban-Rural Division Code" issued by the National Bureau of Statistics. Chi-square tests, ANOVA, and multivariate logistic regression models were used to analyze the factors influencing the intention to receive the influenza vaccine.Results:A total of 10 500 participants were surveyed, comprising 4 885 rural residents (46.52%) and 5 615 urban residents (53.48%). Of these, 3 430 rural residents (70.21%) and 3 718 urban residents (66.22%) expressed intention to receive the influenza vaccine. Multivariate logistic regression analysis revealed that for rural residents aged≥50, younger age groups (50-59 years: OR=1.747, 95% CI: 1.290-2.366; 60-69 years: OR=1.838, 95% CI: 1.401-2.411), history of influenza vaccination ( OR=6.721, 95% CI: 5.152-8.767), doctor′s recommendation for vaccination ( OR=3.788, 95% CI: 3.078-4.662), and higher scores on knowledge related to influenza prevention and treatment ( OR=1.090, 95% CI: 1.054-1.128) were significant promoting factors for vaccination intent. For urban residents aged≥50, belonging to the 60-69 age group ( OR=1.264, 95% CI: 1.023-1.563), history of influenza vaccination ( OR=5.392, 95% CI: 4.309-6.748), doctor′s recommendation for vaccination ( OR=5.307, 95% CI: 4.420-6.371), and higher scores on knowledge related to influenza prevention and treatment ( OR=1.051, 95% CI: 1.021-1.082) were significant promoting factors for vaccination intent. Conclusions:The intention to receive the influenza vaccination is notably high among residents aged≥50 in both urban and rural areas of Zhejiang Province, yet there remains potential for improvement. Age, history of influenza vaccination, doctor recommendation, and knowledge related to influenza prevention and treatment are significant factors influencing the intention to receive the influenza vaccination.
8.Pneumococcal vaccination rate and influencing factors among residents aged≥50 in Zhejiang Province

Objective:To analyze the pneumococcal vaccination rate and the influencing factors among residents aged≥50 in Zhejiang Province.Methods:This cross-sectional study was conducted between March and May 2024. A multi-stage intercept survey was used to intercept and survey 10 500 residents who visited or underwent physical examinations at 175 community health service centers/clinics in 35 counties (cities, districts) of Zhejiang Province. The questionnaire included basic demographic characteristics, history of pneumonia, chronic disease conditions, pneumococcal vaccination status, doctor recommendation, reasons for vaccination/non-vaccination, and channels for obtaining pneumococcal vaccine information. Participants were grouped based on demographic characteristics, chronic diseases conditions, history of pneumonia and doctor recommendations. The chi-square test was used to assess differences in vaccination rates among demographic groups while binary logistic regression models were used to identify factors affecting the vaccination rate.Results:Among the 10 500 surveyed participants, 1 724 (16.42%) had received pneumococcal vaccination. Binary logistic regression analysis revealed significantly higher vaccination rates among those who received a doctor recommendation compared to those without (26.25% vs 7.59%; OR=4.414, 95% CI: 3.851-5.059). Participants benefiting from the free vaccination policy showed higher vaccination rates than those who didn′t (47.59% vs 5.18%; OR=12.527, 95% CI: 10.723-14.634). The top three reasons for getting vaccinated against pneumonia were the free vaccination policy (65.43%), community promotion (52.15%), and family recommendations (42.34%). The top three sources through which respondents learned about the pneumococcal vaccine were television (64.64%), doctors (59.81%) and family members (50.92%). Conclusion:The pneumococcal vaccination rate among residents aged≥50 years in Zhejiang Province is relatively high but still needs further improvement, with vaccination rates closely associated with factors such as free vaccination policy and doctor recommendation.
9.Efficacy of CEA, CA125, CA199, CA724, and G-17 in diagnosing early gastric cancer

Objective:To explore the clinical values of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), carbohydrate antigen 199 (CA199), carbohydrate antigen 724 (CA724) and gastrin 17 (G-17) detections in the early diagnosis of gastric cancer patients.Methods:A retrospective case-control study was conducted. A total of 120 patients with gastric diseases who were admitted to General Hospital of Xuzhou Mining Group from January 2019 to December 2021 were selected. According to the pathological results of gastroscopy examination, the patients were divided into gastric ulcer group (23 cases), atrophic gastritis group (58 cases) and early gastric cancer group (39 cases). The healthy control group consisted of 30 healthy individuals who underwent physical examination during the same period. All participants were detected for serum levels of CEA, CA125, CA199, CA724, and G-17. The levels of various indicators in each group were compared. Using the pathological results of gastroscopy as the gold standard, the receiver operating characteristic (ROC) curve was used to analyze the efficacy of various indicators in distinguishing early gastric cancer from healthy individuals, gastric ulcer and atrophic gastritis.Results:The serum levels of CA125 [ M ( Q1, Q3)] [19.94 (8.29, 22.99) U/ml vs. 6.03 (4.07, 10.48) U/ml, 7.49 (4.96, 12.19) U/ml, 7.54 (6.20, 11.91) U/ml], CA199 [32.09 (15.68, 41.97) U/ml vs. 19.19 (10.01, 30.05) U/ml, 21.00(16.01, 32.71) U/ml, 18.95 (13.90, 32.76) U/ml], CA724 [19.55 (3.91, 26.25) U/ml vs. 4.61 (3.06, 5.24) U/ml, 4.09 (3.37, 5.00) U/ml, 4.88 (3.92, 5.46) U/ml] and G-17 [21.01 (14.67, 24.00) pmol/L vs. 11.80 (10.07, 16.58) pmol/L, 12.74 (11.09, 14.69) pmol/L, 12.08 (8.40, 15.10) pmol/L] in the early gastric cancer group were higher than those in the gastric ulcer group, atrophic gastritis group and healthy control group, and the differences were statistically significant (all P < 0.05). The CEA level in the early gastric cancer group, gastric ulcer group and atrophic gastritis group was all higher than that in the healthy control group [4.38 (3.22, 7.56) ng/ml, 4.51 (3.37, 5.51) ng/ml, 4.49 (4.13, 5.09) ng/ml vs. 3.95 (2.16, 4.44) ng/ml], and the differences were statistically significant (all P < 0.05). ROC curve analysis showed that among all indicators, the area under the curve (AUC) of G-17 for distinguishing early gastric cancer from healthy individuals was the largest [0.825 (95% CI: 0.728-0.922)], the optimal critical value was 18.21 pmol/L, and the specificity was the highest (96.7%); the AUC of CA125 and CA724 was also relatively high, with values of 0.768 (95% CI: 0.653-0.884) and 0.744 (95% CI: 0.622-0.866), respectively, and the optimal critical values were 15.41 and 39.60 U/ml, respectively, with the corresponding sensitivities of 71.8%, which was the highest among several indicators. Among all indicators, CA125 had the largest AUC for distinguishing early gastric cancer from gastric ulcer, which was 0.829 (95% CI: 0.729-0.930), with an optimal critical value of 11.60 U/ml, and the corresponding sensitivity was also the highest (74.4%); the AUC of CEA and CA724 was 0.534 (0.391-0.677) and 0.786 (0.668-0.903), respectively; the optimal critical values were 7.20 ng/ml and 6.34 U/ml, respectively; the corresponding specificity was 100.0%. Among the various indicators, the AUC of G-17 for distinguishing early gastric cancer from atrophic gastritis was the largest [0.813 (95% CI: 0.710-0.915)]. The specificity of each indicator at the optimal critical value was relatively high (≥ 94.8%), among which the optimal critical values of CEA and CA125 were 7.170 ng/ml and 15.55 U/ml, respectively, with the corresponding specificity of 100.0%; the AUC of CA724 was 0.783 (95% CI: 0.671-0.895), the optimal critical value was 6.40 U/ml, and the corresponding sensitivity was 71.8%, which was the highest among the several indicators. Conclusions:CEA, CA125, CA199, CA724, and G-17 have high sensitivity and detection rate in the differential diagnosis of early gastric cancer, gastric ulcer and atrophic gastritis, and have certain clinical values in the diagnosis of early gastric cancer and potential for clinical auxiliary diagnosis.
