1.Effects of Bushen Huoxue Formulation (补肾活血方) on Synaptic Vesicle-Associated Proteins in Neurons and the Intestinal Barrier in Parkinson's Disease Model Rats

ObjectiveTo investigate the potential mechanisms underlying the therapeutic effects of Bushen Huoxue Formulation (补肾活血方, BSHXF) in Parkinson's disease (PD) from the perspectives of synaptic vesicle cycling and intestinal barrier function. MethodsForty-eight male SD rats were randomly divided into four groups, control group, model group, Madopar group, and BSHXF group, with 12 rats in each group. Except for the control group, rats were administered rotenone by gavage once daily for 4 consecutive weeks to establish a PD rat model. After successful modeling, rats in the BSHXF group received concentrated BSHXF solution by gavage at a dose of 13.5 g/(kg·d), while rats in the Madopar group received Madopar tablet suspension at 2 g/(kg·d). Rats in the control group and the model group were administered distilled water by gavage at 10 ml/(kg·d). All groups received continuous gavage treatment for 2 weeks. Motor function was evaluated using the open field test (total distance traveled, average speed, and total distance traveled in the central zone), rotarod test (latency to fall), and balance beam test (traversal time). Histopathological changes in colon tissues were observed by HE staining. Western Blotting was used to detect the protein levels of α-synuclein (α-syn), phosphorylated serine 129 α-synuclein (pSer129-α-syn), vesicle-associated membrane protein 2 (VAMP2), and synaptophysin (SYP) in the substantia nigra and colon tissues. The mRNA expression levels of α-syn, VAMP2, and SYP were detected by RT-PCR, and α-syn expression was examined by immunohistochemistry. Immunofluorescence staining was performed to assess the levels of tight junction protein 1 (ZO-1) and occludin in colon tissues, as well as tyrosine hydroxylase (TH) levels in substantia nigra tissues. ResultsCompared with the control group, rats in the model group showed reduced total distance traveled, average speed, and central-zone distance in the open field test, decreased latency to fall in the rotarod test, and prolonged traversal time in the balance beam test. The protein levels of pSer129-α-syn and α-syn, as well as α-syn mRNA expression were increased in the substantia nigra and colon tissues, whereas the protein and mRNA expression levels of VAMP2 and SYP were decreased. The relative integrated optical density of α-syn in the substantia nigra and colon tissues was increased, while the relative fluorescence intensities of ZO-1 and occludin in colon tissues and TH in the substantia nigra were decreased (P<0.05). HE staining showed focal necrosis in the mucosal layer of colon tissues, disappearance of some intestinal glands, extensive inflammatory cell infiltration, and proliferation of fibrous connective tissue in the submucosa. Compared with the model group, both the Madopar group and BSHXF group showed varying degrees of improvement in the above indicators (P<0.05). Pathological damage in colon tissues was also alleviated, with only mild fibrous tissue proliferation and limited diffuse inflammatory cell infiltration observed in the mucosal layer. Compared with the Madopar group, rats in the BSHXF group exhibited increased average speed, central-zone distance, and latency to fall in the rotarod test. The levels of pSer129-α-syn and α-syn proteins in the substantia nigra were decreased, while VAMP2 protein expression was increased. In colon tissues, SYP protein levels and mRNA expression were elevated. The relative fluorescence intensity of ZO-1 in colon tissues and TH in substantia nigra tissues was also increased (P<0.05). ConclusionBSHXF may improve motor dysfunction in PD by regulating synaptic vesicle cycling, restoring intestinal barrier integrity, and improving neuronal synaptic function through modulation of synaptic vesicle-associated protein expression in the substantia nigra and colon tissues.

3.Clinical features of hepatic myelopathy: An analysis of 126 cases

ObjectiveTo analyze the clinical data of 126 patients with hepatic myelopathy (HM), to summarize their clinical manifestations and prognostic features, and to provide a reference for clinical diagnosis and treatment. MethodsRelated clinical data were collected from 126 HM patients who attended The First Hospital of Lanzhou University from January 1, 2016 to June 30, 2025, and according to their muscle strength on admission, they were divided into grade<3 group, grade 3 group, and grade 4 group. The three groups were compared in terms of the clinical data including etiology, symptoms, clinical signs, past history, personal history, laboratory test results, imaging findings, treatment, and outcome. An analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Bonferroni correction method was used for further comparison between two groups; the chi-square test was used for comparison of categorical data between multiple groups. ResultsAll 126 patients with HM were comorbid with liver cirrhosis, with a male/female ratio of 2.7∶1, an age of 23 — 73 years, a mean age of 53.9±9.3 years, and a median age of 54 years. The top three etiologies of liver cirrhosis were hepatitis B in 78 patients (61.9%), hepatitis C in 9 patients (7.1%), and alcoholic hepatitis in 8 patients (6.3%). During the course of the disease, all 126 patients with HM showed weakness in both lower limbs, among whom 9 patients (7.1%) also had the symptoms of lower limb pain and numbness. Among the 126 patients, 3 (2.4%) had grade 0 muscle strength, 4 (3.2%) had grade 1 muscle strength, 9 (7.1%) had grade 2 muscle strength, 39 (31.0%) had grade 3 muscle strength, and 71 (56.3%) had grade 4 muscle strength. Comparison of related data between the three groups of patients with varying grades of muscle strength showed that as for comorbidities, there were significant differences between the three groups in the constituent ratios of overt hepatic encephalopathy (OHE) (χ2=6.254, P=0.044), ascites (χ2=6.248, P=0.044), and liver failure (χ2=7.704, P=0.029); as for laboratory markers, there were significant differences between the three groups in red blood cell count (RBC) (F=5.334, P=0.006), hemoglobin (HGB) (F=4.627, P=0.012), aspartate aminotransferase (AST) (H=6.866, P=0.032), alanine aminotransferase (H=6.444, P=0.040), albumin (Alb) (F=6.331, P=0.002), cholinesterase (ChE) (H=11.064, P=0.004), total cholesterol (TC) (F=4.843, P=0.009), prothrombin activity (PTA) (F=3.937, P=0.022) and international normalized ratio (INR) (H=6.448, P=0.040). Compared with the grade 4 group, the grade<3 group had significantly lower levels of RBC, HGB, and ChE (all P<0.05) and a significantly higher level of AST (P<0.05); compared with the grade 3 group, the grade<3 group had significant reductions in Alb, TC, and PTA (all P<0.05) and a significant increase in INR (P<0.05). At discharge, 27 patients (21.4%) achieved improvement, and 99 patients (78.6%) showed no response, among whom 2 patients died. ConclusionHM has the main clinical manifestation of symmetrical spastic paraplegia of both lower limbs, and it is more common in men. The main causes of HM include hepatitis B, hepatitis C, and alcoholic hepatitis. Muscle strength in HM patients is associated with the comorbidity of OHE, ascites, or liver failure, and patients with weak muscle strength tend to have low levels of RBC, HGB, Alb, ChE, TC, and PTA and high levels of AST and INR, suggesting that the changes in these indicators may be correlated with the degree of muscle strength impairment. HM often has a poor prognosis, and there are currently no effective treatment methods.
4.Development of the Adolescent Mental Health Computerized Adaptive Test

Objective:To develop the Adolescent Mental Health Computerized Adaptive Test(AMH-CAT)for assessing psychological crises in adolescents using item response theory(IRT).Methods:A total of 8 679 students,aged 9 to 19 years,were sampled from 14 primary and secondary schools.The item bank was constructed by examining unidimensionality,independence,discrimination,and Differential Item Functioning(DIF).The AMH-CAT algorithm was developed using R software and validated for reliability and validity through real and simulated data.The School Psychological Interview Evaluation Form served as the criterion tool to assess the predictive validi-ty of AMH-CAT in evaluating psychological crises levels.Results:The item bank,consisting of 43 items,demon-strated unidimensionality and independence,with discrimination values>0.80 and no DIF across genders.Under va-rious stopping rules,AMH-CAT showed ABS values<0.40,RMSE values<0.50,and a correlation(r)>0.87(P<0.001)between estimated and true psychological crises levels.Real participants answered an average of 10 items under the SE(θ)≤0.4 stopping rule,with a reliability of 0.89,and the scores were positively correlated with the complete item bank scores(r=0.95,P<0.001).The predictive accuracy for Level 1 and Level 2 concerns was 0.89 and 0.90,respectively.Conclusion:The Adolescent Mental Health Computerized Adaptive Test(AMH-CAT)demonstrates excellent validity,reliability and assessment efficiency for evaluating adolescent psychological crises.

Objective To explore the sleep quality and mental fatigue level in elderly patients with cerebrovascular small disease(CSVD).Methods A total of 222 patients aged over 65 years old hospitalized due to chronic diseases in Department of Neurology of the Affiliated Jiangning Hospi-tal of Nanjing Medical University from August 2022 to June 2024 were recruited prospectively and continuously.According to the CSVD score,they were divided into a CSVD group(CSVD score≥1,148 cases)and a non-CSVD group(CSVD score=0,74 cases).All the patients were evaluated by sleep quality,fatigue and neuropsychological scale when they were fully cooperated and in good condition.Subsequently,the patients in the CSVD group were further assigned into a good sleep subgroup(117 cases)and a poor sleep subgroup(31 patients).Results The CSVD group had significantly higher total score of Pittsburgh sleep quality index(PSQI),sleep quality score,sleep disturbance score,total score of self-rating fatigue,and mental fatigue score than the non-CSVD group(P<0.01).The sleep quality score,sleep disturbance score,and mental fatigue score were risk factors for CSVD(P<0.05).The mental fatigue score was significantly higher in the CSVD patients with poor sleep than those with good sleep(4.13±1.15 vs 2.50±1.92,P<0.01).Conclusion Elderly CSVD patients were more likely to have decreased sleep quality and mental fatigue,and among them,those with poor sleep quality are prone to having mental fatigue than those with good sleep.
6.Etiological composition and clinical analysis of hypertension in 74 infants

Objective:To analyze the etiological composition and clinical characteristics of infant hypertension,and provide reference for its diagnosis and treatment.Methods:This is a retrospective case-control study.Retrospective investigation and analysis were conducted on the clinical data of infants discharged from Beijing Children's Hospital Affiliated to Capital Medical University with a diagnosis of "hypertension" from June 1,2016 to September 30,2021,including clinical manifestations,auxiliary examinations,treatment plans,and prognosis.Results:A total of 74 eligible children were collected,including 42 male infants(56.8%)and 32 female infants(43.2%).A total of 67 cases(90.5%)had clear secondary factors,including 35 cases of kidney disease(47.3%),12 cases of connective tissue disease(16.2%),and 9 cases of hematological tumor disease(12.2%).At the beginning of the disease,cardiac ultrasound showed that 54 cases(73.0%)had ventricular wall thickening,including mild thickening in 31 cases(57.4%),moderate thickening in 11 cases(20.3%),and severe thickening in 12 cases(22.2%).After grouping by etiology,the incidence of proteinuria and severe hypertension in the renal hypertension group,as well as those receiving multiple antihypertensive drugs,was significantly higher than that in the non-renal hypertension group( χ 2=28.493, P<0.001; χ 2=17.283, P<0.001; χ 2=17.358, P<0.001);Renal disease was risk factor for severe hypertension in infants according to univariate and multivariate logistic regression analysis respectively( OR=11.176,95% CI:2.882~43.339, P<0.001; OR=11.669,95% CI:2.921~46.624, P<0.001).Thirty-one children had follow-up records for 6 months or more,and 13(41.9%)still required antihypertensive treatment,of whom 26(83.9%)were no longer recorded as having elevated blood pressure. Conclusion:Infant hypertension is mainly secondary,with a high proportion of renal factors and predisposition to severe hypertension,which requires multiple antihypertensive drugs for control.Active antihypertensive treatment and removal of secondary factors during the acute phase are helpful for controlling hypertension in infants,but further research is needed on treatment options and long-term prognosis.
7.Development of the Adolescent Mental Health Computerized Adaptive Test

9.Cognitive frailty in older patients with chronic heart failure and its influencing factors

Objective:To investigate the characteristics of cognitive frailty and its influencing factors in older patients with chronic heart failure.Methods:In this cross-sectional study, 300 older patients hospitalized for chronic heart failure in a tertiary hospital in Shandong Province between September 2021 and September 2022 were selected.A general information questionnaire, the mini-nutritional assessment scale-short form(MNA-SF), the athens insomnia scale(AIS), the ulca loneliness scale, the geriatric depression scale-5 item version(GDS-5), and the social support rating scale(SSRS)were used for assessment and influencing factors were identified by univariate and Logistic regression analysis.Results:Among 300 older patients with chronic heart failure, the prevalence of cognitive frailty was 75.3%(226 cases). Logistic regression analysis showed that age between 70-79 years( OR=0.543, 95% CI: 0.299-0.987), education level( OR=3.644, 95% CI: 1.780-7.461), weekly intellectual activity( OR=2.168, 95% CI: 1.082-4.334)and loneliness( OR=1.101, 95% CI: 1.032-1.175)were factors influencing cognitive frailty in older patients with chronic heart failure. Conclusions:The prevalence of cognitive frailty in older patients with chronic heart failure is high, and age, education level, weekly intellectual activity and loneliness are influencing factors, with education level having the greatest impact on older patients with chronic heart failure.
10.A consensus on the management of allergy in kindergartens and primary schools

Abstract
Allergic diseases can occur in all systems of the body, covering the whole life cycle, from children to adults and to old age, can be lifelong onset and even fatal in severe cases. Children account for the largest proportion of the victims of allergic disease, Children s allergies start from scratch, ranging from mild to severe, from less to more, from single to multiple systems and systemic performance, so the prevention and treatment of allergic diseases in children is of great importance, which can not only prevent high risk allergic conditions from developing into allergic diseases, but also further block the process of allergy. At present, there is no consensus on the management system of allergic children in kindergartens and primary schools. The "Consensus on Allergy Management and Prevention in Kindergartens and Primary Schools", which includes the organizational structure, system construction and management of allergic children, provides evidence informed recommendations for the long term comprehensive management of allergic children in kindergartens and primary schools, and provides a basis for the establishment of the prevention system for allergic children.
