1.Early predictive value of dynamic contrast-enhanced MRI radiomics combined with Magee equation 3 for pathological response of HR+/HER2-breast cancer after neoadjuvant chemotherapy

Objective To investigate the predictive value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)radiomics combined with Magee equation 3(ME3)for pathological response following neoadjuvant chemotherapy(NAC)in patients with hormone receptor-positive/human epidermal growth factor receptor 2-negative(HR+/HER2-)breast cancer.Methods A ret-rospective analysis was performed on 325 breast cancer patients diagnosed with HR+/HER2-in two hospitals.Patients from the first hospital were randomly divided into training and internal validation sets at a ratio of 7∶3.Patients from the second hospital served as external validation set.The volume of interest(VOI)of breast cancer was delineated on DCE-MRI images.Then the rele-vant radiomics features were extracted and selected,and the Radiomics score(Radscore)was calculated.The statistically significant clinical and pathological features and Radscore were incorporated into a multivariate analysis.The combined radiomics-clinical model and nomogram were established,and the prediction performance was evaluated by using the receiver operating characteristic(ROC)curve,calibration curve,and decision curve analysis(DCA).Results In the training set,seven radiomics features were selected to construct the radiomics model.ME3 score emerged as the independent factor for pathological complete response(pCR)(P<0.001).By integrating ME3 score and Radscore,a combined radiomics-clinical model was developed.This model demonstrated robust predictive accuracy,with area under the curve(AUC)of 0.88[95%confidence interval(CI)0.80-0.95],0.90(95%CI 0.82-0.98),and 0.82(95%CI 0.70-0.93)in the training,internal validation,and external validation sets,respectively.The nomogram construc-ted from the combined model exhibited excellent discrimination and calibration,with P-values of 0.455,0.312 and 0.062 in the train-ing,internal validation,and external validation sets.Conclusion DCE-MRI radiomics combined with ME3 can be used to predict the pathological response of NAC in HR+/HER2-breast cancer.

3.Effects of targeted surveillance combined with cluster management on isolation protector use rate and hand hygiene qualification rate in caregivers of patients with respiratory tract multidrug-resistant bacterial infections

Objective:To investigate the targeted surveillance combined with cluster management on isolation protector use rate and hand hygiene qualification rate in caregivers of patients with respiratory tract multidrug-resistant bacterial infections.Methods:Second Hospital of Shanxi Medical University began to perform targeted surveillance combined with cluster management among caregivers of patients with multidrug-resistant bacterial infections in July 2019. The caregivers of 50 patients with respiratory tract multidrug-resistant bacterial infections who were admitted before targeted surveillance combined with cluster management were included in the pre-implementation group. The caregivers of 50 patients with respiratory tract multidrug-resistant bacterial infections who were admitted after targeted surveillance combined with cluster management were included in the post-implementation group. Multidrug-resistant bacterial infections in caregivers, isolation protector use rate, environmental disinfection and hand hygiene qualification rate were compared between before and after 6 months of targeted surveillance combined with cluster management. The application value of targeted surveillance combined with cluster management in the prevention of respiratory tract multidrug-resistant bacterial infections was analyzed.Results:The number of caregivers with respiratory tract multidrug-resistant bacterial infections in the post-implementation group was lower than that in the pre-implementation group (5 vs. 13, P < 0.05). The proportion of caregivers who wear a mask (94.11%), isolation clothes (80.39%) and gloves (98.03%) in the post-implementation group was significantly higher than that in the pre-implementation group (70.00%, 62.00%, 78.00%, χ2 = 10.027, 4.911, 9.683, all P < 0.05). The proportion of caregivers who were qualified in terms of hand hygiene (82.35%), object surface (76.47%), and keeping the air fresh in the post-implementation group was significantly higher than that in the pre-implementation group (64.00%, 58.00%, 64.00%, χ2 = 4.341, 3.915, 5.450, all P < 0.05). Conclusion:Targeted surveillance combined with cluster management can effectively help prevent respiratory tract multidrug-resistant bacterial infections in caregivers and increase isolation protector use rate and hand hygiene qualification rate.
4.Clinical features and risk predictors of autism and intellectual disability in patients with Lennox-Gastaut syndrome

Objective To investigate the co-occurrence incidence,clinical features and risk predictors of autism and intellectual disability in patients with Lennox-Gastaut syndrome (LGS).Methods Sixty-four patients with LGS were recruited in our Epilepsy Center from June 2012 to June 2018.Autism Behavior Checklist (ABC) and Childhood Autism Rating Scale (CARS) were performed to evaluate autism,while Chinese Wechsler Intelligence Scale for Children (C-WISC) and Gesell Developmental Scale were applied to estimate intelligence.The influences of different clinical factors in autism and intellectual disability were analyzed in patients with LGS.Results Among 64 patients with LGS,only three (4.7%) were diagnosed as having autism,and their average ABC and CARS scores were 80.0 and 40.0,respectively.The average ABC and CARS scores were 40.9±26.7 and 26.0±8.9 in thepatients with onset age<one year,which were significantly higher than those in other two groups,respectively (P<0.05).The average ABC and CARS scores in the patients accepted antiepileptic drugs (AEDs) ≥ 3 were 27.8±22.8 and 22.2±8.7,which were significantly higher than those in the patients accepted one or two kinds ofAEDs (P<0.05).In addition,the ABC and CARS scores showed significant differences in the groups with different seizure frequency and in the groups with or without symptomatic etiologies (P<0.05).Fifty patients (78.1%) presented different levels of intellectual disability;severe intellectual disability was the leading type,which accounted for 31.3% (20/64);12(18.8%),7(10.9%),and 11 (17.2%) patients were with mild,moderate or profound intellectual disability,respectively.As compared with patients without intellectual disability,patients with intellectual disability had younger onset age,higher proportion of slow background activity on EEG and higher proportion of symptomatic etiologies,with significant differences (P<0.05).Conclusion Patients are in higher risk of autism when they have earlier epilepsy onset age,higher frequency of epilepsy seizure attack,administration of AEDs ≥3 and symptomatic etiologies;early onset age is an independent risk predictor for intellectual disability of patients with LGS.
5.Effect of dronedarone on HCN channel mRNA and protein expression in neonatal rat ventricular myocytes

Objective To explore the effect of dronedaronel on hyperpolarization-activated cyclic-nucleotide-gated(HCN) channel expression by detecting the change of HCN channel mRNA and protein level before and after giving dronedarone in neonatal rat ventricular myocytes.Methods Neonatal rat ventricular myocytes were separated and digested by type Ⅱ collagenase,and then single ventricular myocytes were collected through differential sticking wall separation method.According to the concentrations (0.1,0.5,1.0,5.0,10.0,20.0 μmol/L of dronedaronel for treating myocytes for 48 h) and time(10 μmol/L of dronedaronel for treating myocytes for 1,6,12,24,48 h)the gradient grouping was conducted.The levels of HCN2 and HCN4 channel mRNA and protein level were determined by real-time PCR and Western blot.Results The HCN2 mRNA and HCN4 mRNA expression levels in concentration gradient group and time gradient group were lower than those in the control group(P<0.05);compared with the control group,the protein level in the 10 umol/L dronedaronel treatment for 12 h group was significantly down-regulated(P< 0.01).Conclusion Dronedaronel could inhibit the expression of HCN2/HCN4 channel mRNA and protein,moreover its action shows the concentration dependency and reaches the maximum at 12 h after medication.
6.CT Diagnosis of Splenic Lesions:An Analysis of 33 Cases

Objective To evaluate the value of CT scan in diagnosis of the splenic lesions.Methods A retrospective analysis was made on the CT scans of 33 patients with surgically and pathologically proven splenic lesions,which included plain CT scan(n=33),biphasic contrast-enhanced CT scan(n=28)and contrast delay scanning(n=6).Results In 33 cases,non-neoplastic lesions were 6 cases,including tuberculosis(n=2),hematomas(n=3)and inflammatory pseudotumor associated with a cavernous hemangioma(n=1).Benign tumors were 13 cases,including cysts(n=5),hemangiomas(n=4),lymphanioma(n=1)and lymph-hemangiomas(n=3).Malignant tumors were 14 cases,including malignant lymphoma(n=8),metastases(n=4),malignant myofibroblastoma and hemangioendothelial sarcoma(n=1,respectively).Conclusion CT is of significant value in diagnosis of splenic lesions.
7.Construction and evaluation of bone metastasis of breast cancer in nude mice model

Objective To establish nude mice model of bone metastasis of breast cancer,and study the best method to evaluate the model.Methods The human breast cancer cell,MDA-MB-231PEGFPN,was injected into left ventricule of nude mice to construct the bone metastasis of breast cancer animal model.Methods of X-ray,whole body optical imaging and pathology were used to evaluate the bone damage after six weeks.Results After six weeks,the result of pathology showed that breast cancer cells had already spread to brain,bone marrow and lung of nude mice.The degree of bone destruction was seen clearly in X-ray,and location of breast cancer cells was observed by whole body optical imaging system.Conclusions The results showed that combined application of X-ray,whole body optical imaging and pathology are the best modalities to evaluate bone metastasis of breast cancer in nude mice.
8.Imaging Diagnosis of Pituitary Stalk Interruption Syndrome

Objective To analyze the CT and MRI features of pituitary stalk interruption syndrome, so that to improve the diagnostic knowledge of this disease.Methods The imaging and clinical data of 4 patients with pituitary stalk interruption syndrome were retrospectively analyzed. Results In all of the 4 cases, the pituitary stalk were not shown by CT in 2 and by MRI in 4,the high signal intensity of the posterior lobe disappeared on T1WI,while a small nodule in infundibulum with high signal intensity on T1WI was found. Conclusion MRI is the most valuable imaging modality for the diagnosis of pituitary stalk interruption syndrome.
