1.Association of monocyte-to-high-density lipoprotein cholesterol ratio with white matter hyperintensities and its spatial distribution

Objective:To investigate the association of monocyte-to-high-density lipoprotein cholesterol ratio (MHR) with the severity of white matter hyperintensities (WMHs) and its spatial distribution.Methods:Patients admitted to the Department of Neurology, Jiangning Hospital Affiliated to Nanjing Medical University due to various chronic diseases or physical examinations between January 2023 and December 2024 were included retrospectively. Past medical history, clinical and imaging data were collected. The Fazekas scale was used to assess the severity of WMHs. According to the scoring results of periventricular WMHs (PVWMHs) and deep WMHs (DWMHs), WMHs were divided into no/mild group (0-1 points) and moderate/severe group (2-3 points). Multivariate logistic regression analysis was used to determine independent correlation factors for the severity of WMHs, PVWMHs, and DWMHs. Results:A total of 357 patients were included, aged 65.42±9.95 years, with 198 males (55.5%). There were 193 patients (54.1%) in the no/mild group and 164 (45.9%) in the moderate/severe group. Univariate analysis showed that the proportion of patients with hypertension, diabetes, history of cerebral infarction and cerebral hemorrhage, carotid plaque, and age, serum creatinine, monocyte count and MHR in the moderate/severe group were significantly higher than those in the no/mild group (all P<0.05). Multivariate logistic regression analysis showed a significant positive correlation between MHR and the severity of WMHs (odds ratio 3.138, 95% confidence interval 1.042-9.451; P=0.042). Further analysis showed a significant positive correlation between MHR and PVWMHs (odds ratio 3.384, 95% confidence interval 1.111-10.305; P=0.032), but no independent correlation with DWMHs. In addition, age and hypertension, diabetes, history of cerebral infarction and cerebral hemorrhage were significantly positively correlated with the severity of WMHs, PVWMHs and DWMHs. Conclusion:MHR is correlated with the severity of WMHs, and higher MHR is significantly associated with PVWMHs, but not with DWMHs.
2.Clinical Efficacy Observation of Shujin Tougu Wash Recipe in the Treatment of Knee Osteoarthritis Patients and Preliminary Exploration of Its Metabolomics Mechanism

Objective To observe the clinical efficacy of Shujin Tougu Wash Recipe in the treatment of knee osteoarthritis and to analyze the changes in patients'serum metabolites before and after treatment.Methods A total of 30 patients with knee osteoarthritis were randomly divided into the observation group and the control group.Patients in the control group were treated with Glucosamine Hydrochloride Tablets.Patients in the observation group were treated with Shujin Tougu Wash Recipe.Two groups were treated for 3 weeks.The clinical efficacy was evaluated by observing the changes in the Visual Analogue Scale(VAS),Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),and knee joint activity before and after treatment.Serum was collected from five patients from the observation group before and after treatment,and the changes in serum metabolites were detected by UPLC-MS untargeted metabolomics technology.Metabolic pathways were analyzed through the KEGG database.Results Compared with before treatment,the VAS and WOMAC scores of patients were reduced after treatment(P<0.05),and the activity of the knee joint was increased(P<0.05).Tthe Observation Group is better than the control group.There were 15 differential metabolites before and after treatment,among which the content of valproic acid,N-methylundecanamide,arachidonamide,spironolactone,propylene glycol,lysophosphatidylcholine,dehydroisoproporphyrinogen,and marcoticin increased after treatment,while the content of arginine,glycine,aspartic acid,pipecolate,fexofenadine,perindopril,alfuzosin,2-aminohexanoic acid,and all-trans-heptaprenyl diphosphate decreased.The metabolic pathways involved were mainly choline metabolism,retrograde endocannabinoid signaling pathway,linoleic acid metabolism,glycerophospholipid metabolism,α-linolenic acid metabolism,and arachidonic acid metabolism.Conclusion The treatment of knee osteoarthritis patients with Shujin Tougu Wash Recipe can significantly improve the clinical symptoms of patients,improve the quality of life of patients,and can regulate the levels of arginine,valproic acid,and lysophosphatidylcholine metabolites in patients'serum,affecting multiple metabolic pathways,and improving the inflammatory level of patients through correcting d lipid metabolism,and playing an analgesic effect.

However, the concept of "coronary plaque burden" does not accurately reflect the complex pathological progression of coronary artery disease. In this review, various aspects of the total coronary atherosclerosis burden are present, including its mechanics, geometrical characteristics, plaque morphology, coronary artery calcium deposition, and coronary inflammation, to provide a complete view. Different tools used to evaluate the coronary atherosclerosis burden are also assessed according to the most recent studies. 6.Efficacy and safety of autologous hematopoietic stem cell transplantation in patients with multiple myeloma

Objective:To investigate the efficacy and safety of autologous hematopoietic stem cell transplantation (ASCT) in the treatment of multiple myeloma (MM).Methods:The clinical data of 64 MM patients who received ASCT in the Second Affiliated Hospital of Xi'an Jiaotong University from October 2015 to March 2022 were retrospectively analyzed. The clinical characteristics, therapeutic effects and adverse reactions of the patients were summarized.Results:Of the 64 patients, 42 were male and 22 were female; the median age was 54 years old (37-69 years old). The median number of CD34 + cells collected from 46 patients in the CE (cyclophosphamide, etoposide) regimen mobilization group and 17 patients in the plerixafor mobilization group were 7.50×10 6/kg [(1.15-24.73)×10 6/kg] and 4.54×10 6/kg [(0.75-10.40)×10 6/kg], and the difference was statistically significant ( Z = 3.02, P = 0.024). Hematopoietic reconstitution was successful in all 64 patients, and the median time for white blood cell and platelet engraftment was 11 d (8-13 d) and 11 d (8-15 d), respectively. The patients' pretreatment regimens were all high-dose melphalan, the median white blood cell and platelet engraftment time of 29 patients in the oral group were 11 d (8-13 d) and 11 d (8-15 d), respectively, the median white blood cell and platelet engraftment time of 35 patients in the intravenous infusion group were 11 d (8-12 d) and 11 d (8-15 d), respectively, and there were no statistical differences (both P > 0.05). The ≥CR rate was 48.4% (31/64) before transplantation and 70.3% (45/64) three months after transplantation, and the difference was statistically significant ( χ2 = 6.35, P = 0.012). The median follow-up time of all patients was 27 months (2-67 months). The 3-year OS and PFS rates were 77.6% and 54.9%, and the median OS and PFS time were 67 and 52 months. The median hospitalization time was 20 d (15-37 d). There was no transplantation-related mortality, and the main adverse reactions were gastrointestinal reactions (100.0%, 64/64), grade 4 thrombocytopenia (98.4%, 63/64), grade 4 neutropenia, and agranulocytosis with fever (40.6%, 26/64). Conclusions:ASCT is effective for MM patients suitable for transplantation, which can further improve the remission rate and remission depth, prolong the PFS and OS time of patients, and the adverse reactions are controllable.
7.Research advances in nonalcoholic fatty liver disease-related hepatocellular carcinoma

The incidence rate of nonalcoholic fatty liver disease-related hepatocellular carcinoma (NAFLD-HCC) tends to increase worldwide, while its pathogenesis remains unclear. With reference to the literature in recent years, this article summarizes the role of adipose tissue inflammation, oxidative stress, gut microbiota, and insulin resistance in the pathogenesis of NAFLD-HCC and the advances in the prevention and treatment of the above mechanisms, so as to provide new ideas for the treatment of NAFLD-HCC.
8.Discussion on the Inhibitory Mechanism of Lanthanum Chloride on Vascular Smooth Muscle Cell Calcification Induced by High Phosphorus Based on NF-κB Signaling Pathway

OBJECTIVE:To discuss the inhibitory effect of lanthanum chloride on the calcification of vascular smooth muscle cells(VSMCs)induced by high phosphorus and its mechanism. METHODS :On the basis of screening the action concentration and time of lanthanum chloride by MTT method ,human VSMCs were divided into control group (1 mmol/L phosphorus solution ), lanthanum chloride high concentration control group (1 mmol/L phosphorus solution+ 60 μmol/L lanthanum chloride),model group (3 mmol/L phosphorus solution ),sodium chloride group (3 mmol/L phosphorus solution+ 180 μmol/L sodium chloride),nuclear factor κB(NF-κB)signaling pathway agonist+lanthanum chloride group (3 mmol/L phosphorus solution+ 1 μg/mL lipopolysaccharide+ 60 μmol/L lanthanum chloride),positive control group (3 mmol/L phosphorus solution+ 100 μmol/L sodium pyrophosphate),and lanthanum chloride low ,medium,and high concentration groups (3 mmol/L phosphorus solution+ 15,30,60 μmol/L lanthanum chloride). Alizarin red S staining and Von Kossa staining were used to detect cell calcification in each group after treated with phosphorus solution for 6 d and relevant medicine for 2 d. Western blot assay was used to detect the protein expression of TNF-α receptor associated protein 6(TRAF6),nuclear factor κB inhibitor protein α(IκBα),NF-κB p65,bone morphogenetic protein 2 (BMP-2),smooth muscle 22 α(SM22α)and Runt related transcription factor 2(Runx2). Real-time fluorescence quantitative polymerase chain reaction was used to detect mRNA expression of TRAF 6,IκBα,BMP-2,SM22α and Runx2. RESULTS : Compared with control group ,no cell calcification was observed in the lanthanum chloride high concentration control group ,while obvious cell calcification and significant increase of OD value were observed in model group and sodium chloride group (P< 0.01);protein and mRNA expression of TRAF 6 and BMP- 2 in cytoplasm as well as mRNA expression of Runx 2,protein expression of NF-κB p65 and Runx 2 in nucleus were significantly increased (P<0.01);protein and mRNA expression of IκBα and SM22α as well as protein expression of NF-κB p65 in cytoplasm were significantly decreased (P<0.01). Compared with model group,cell calcification was significantly improved in lanthanum chloride groups and positive control group ,while OD values were significantly reduced ;the expression levels of the above-mentioned protein and mRNA were reversed to varying degrees (P<0.05 or P<0.01). Compared with lanthanum chloride high concentration group ,obvious cell calcification was observed in NF-κB signaling pathway agonist + lanthanum chloride group ,and OD value was significantly increased ;the above indexes were significantly reversed in cytoplasm and nucleus (P<0.05 or P<0.01). CONCLUSIONS :Lanthanum chloride can inhibit the calcification of VSMCs induced by high phosphorus ,and its mechanism may be related to the inhibition of NF-κB signaling pathway activation.
9.Implementation status of nursing for lymphedema in Jiangsu

Objective? To investigate the training, operation and examination of nursing care for lymphedema in Jiangsu Province. Methods? We selected 99 nurses provided nursing care for lymphedema at 13 class Ⅲ hospitals of prefecture-level city in Jiangsu Province as subjects by purposive sampling. All of nurses were investigated with the self-designed questionnaire involving general information, application and training of nursing care for lymphedema, nursing operation and examination of lymphedema. Results? Among 99 nurses, 68 (68.69%) nurses could carry out training regularly, and 59 (59.60%) nurses participated in training on nursing care for lymphedema. The main causes failing to could carry out training regularly included the teaching staff and lack of related content. Conclusions? At present, the training of nursing care for lymphedema needs to be strengthened. There is a pressing need to formulate the unified standardized operation and examination standards so as to improve the quality of work and professional connotation of nursing care for lymphedema.

The widespread application of next generation sequencing (NGS) in clinical settings has enabled testing, diagnosis, treatment and prevention of genetic diseases. However, many issues have arisen in the meanwhile. One of the most pressing issues is the lack of standards for reporting genetic test results across different service providers. The First Forum on Standards and Specifications for Clinical Genetic Testing was held to address the issue in Shenzhen, China, on October 28, 2017. Participants, including geneticists, clinicians, and representatives of genetic testing service providers, discussed problems of clinical genetic testing services across in China and shared opinions on principles, challenges, and standards for reporting clinical genetic test results. Here we summarize expert opinions presented at the seminar and report the consensus, which will serve as a basis for the development of standards and guidelines for reporting of clinical genetic testing results, in order to promote the standardization and regulation of genetic testing services in China.
