ObjectiveTo investigate the potential mechanisms underlying the therapeutic effects of Bushen Huoxue Formulation (补肾活血方, BSHXF) in Parkinson's disease (PD) from the perspectives of synaptic vesicle cycling and intestinal barrier function. MethodsForty-eight male SD rats were randomly divided into four groups, control group, model group, Madopar group, and BSHXF group, with 12 rats in each group. Except for the control group, rats were administered rotenone by gavage once daily for 4 consecutive weeks to establish a PD rat model. After successful modeling, rats in the BSHXF group received concentrated BSHXF solution by gavage at a dose of 13.5 g/(kg·d), while rats in the Madopar group received Madopar tablet suspension at 2 g/(kg·d). Rats in the control group and the model group were administered distilled water by gavage at 10 ml/(kg·d). All groups received continuous gavage treatment for 2 weeks. Motor function was evaluated using the open field test (total distance traveled, average speed, and total distance traveled in the central zone), rotarod test (latency to fall), and balance beam test (traversal time). Histopathological changes in colon tissues were observed by HE staining. Western Blotting was used to detect the protein levels of α-synuclein (α-syn), phosphorylated serine 129 α-synuclein (pSer129-α-syn), vesicle-associated membrane protein 2 (VAMP2), and synaptophysin (SYP) in the substantia nigra and colon tissues. The mRNA expression levels of α-syn, VAMP2, and SYP were detected by RT-PCR, and α-syn expression was examined by immunohistochemistry. Immunofluorescence staining was performed to assess the levels of tight junction protein 1 (ZO-1) and occludin in colon tissues, as well as tyrosine hydroxylase (TH) levels in substantia nigra tissues. ResultsCompared with the control group, rats in the model group showed reduced total distance traveled, average speed, and central-zone distance in the open field test, decreased latency to fall in the rotarod test, and prolonged traversal time in the balance beam test. The protein levels of pSer129-α-syn and α-syn, as well as α-syn mRNA expression were increased in the substantia nigra and colon tissues, whereas the protein and mRNA expression levels of VAMP2 and SYP were decreased. The relative integrated optical density of α-syn in the substantia nigra and colon tissues was increased, while the relative fluorescence intensities of ZO-1 and occludin in colon tissues and TH in the substantia nigra were decreased (P<0.05). HE staining showed focal necrosis in the mucosal layer of colon tissues, disappearance of some intestinal glands, extensive inflammatory cell infiltration, and proliferation of fibrous connective tissue in the submucosa. Compared with the model group, both the Madopar group and BSHXF group showed varying degrees of improvement in the above indicators (P<0.05). Pathological damage in colon tissues was also alleviated, with only mild fibrous tissue proliferation and limited diffuse inflammatory cell infiltration observed in the mucosal layer. Compared with the Madopar group, rats in the BSHXF group exhibited increased average speed, central-zone distance, and latency to fall in the rotarod test. The levels of pSer129-α-syn and α-syn proteins in the substantia nigra were decreased, while VAMP2 protein expression was increased. In colon tissues, SYP protein levels and mRNA expression were elevated. The relative fluorescence intensity of ZO-1 in colon tissues and TH in substantia nigra tissues was also increased (P<0.05). ConclusionBSHXF may improve motor dysfunction in PD by regulating synaptic vesicle cycling, restoring intestinal barrier integrity, and improving neuronal synaptic function through modulation of synaptic vesicle-associated protein expression in the substantia nigra and colon tissues.

AIM: To compare the differences in intraocular pressure and anterior segment biometric parameters among Tibetan populations permanent residing at different altitudes in Xizang autonomous region, and to explore the impact of high-altitude environments on ocular characteristics.
METHODS:This was a retrospective cross-sectional study. Data were collected from Tibetan patients who visited the People's Hospital of Xizang Autonomous Region from September 2019 to October 2021. Participants were divided into two groups based on their permanent residence altitude: a higher altitude group(2 500-3 750 m)and a very high-altitude group(>3 750 m). The eye with better best corrected visual acuity was selected for each participant. Intraocular pressure, refractive error, and anterior segment biometric measurements \〖white-to-white corneal diameter(WTW), central corneal thickness(CCT), anterior chamber depth(ACD), lens thickness(LT), axial length(AL)\〗 were recorded and compared between the two groups.
RESULTS: A total of 43 Tibetan patients(43 eyes)were enrolled in this study, comprising 23 males and 20 females, with a mean age of 57.88±13.90 y. There were 19 patients(19 eyes)in the very high-altitude group and 24 patients(24 eyes)in the higher altitude group. The mean intraocular pressure in the very high-altitude group was 11.79±1.72 mmHg, which was significantly lower than 13.44±2.43 mmHg in the higher altitude group(P=0.019). CCT in the very high-altitude group was 482.70±23.76 μm, significantly thinner than 503.00±31.09 μm in the higher altitude group(P=0.012). AL in the very high-altitude group was 22.96±0.96 mm, significantly shorter than 23.64±1.10 mm in the higher altitude group(P=0.020). No statistically significant differences were found between the two groups in WTW, ACD, LT, or refractive error(all P>0.05).
CONCLUSION:Among the Tibetan population in Xizang autonomous region, as the altitude of residence increases, intraocular pressure significantly decreases, CCT becomes thinner and AL shortens. These morphological changes suggest possible adaptive remodelling of the eye in very high-altitude environments, and a thinner cornea may be an important factor contributing to the lower measured intraocular pressure values.

BackgroundThe professional self-concept of nurses is a crucial indicator for their personal growth, development and career planning. Previous studies have shown that work stressors during the internship period may lead to a decrease in the level of professional self-concept among nursing students. Given the existing differences in social roles between nursing students and clinical nurses, the influencing factors of professional self-concept in clinical nurses and its relationship with work stressors require further research. ObjectiveTo explore the relationship between work stressors and professional self-concept among nurses, so as to provide references for improving the level of professional self-concept of clinical nurses. MethodsFrom December 2022 to February 2023, a stratified random sampling method was employed to select 260 in-service nurses from Mianyang Hospital of Traditional Chinese Medicine as the study subjects. Chinese Nurses Stressor Scale (CNSS) and Professional Self-concept of Nurses Instrument (PSCNI) were used for assessment. Pearson correlation analysis was conducted to examine the correlation between CNSS scores and PSCNI scores of nurses. Multiple linear regression analysis was utilized to explore the factors influencing the professional self-concept of nurses. ResultsA total of 238 nurses (91.54%) completed valid questionnaires. PSCNI total score yielded a statistical difference among nurses with different marital statuses (F=8.947, P<0.05). PSCNI total scores were significantly higher in nurses with emergency medical service experience than those without such experience (t=2.208, P<0.05), and were significantly lower in nurses with abnormal physical examination findings in the past year than those without abnormal findings (t=-2.584, P<0.05). Correlation analysis revealed that CNSS total score and subscale scores were negatively correlated with PSCNI total score (r=-0.275~-0.169, P<0.01). Multiple linear regression analysis indicated that work expectation-related stressors, marital status and emergency medical service experience might be influencing factors of their professional self-concept (β=-0.350, 0.345, 0.183, P<0.01). ConclusionNurses' work stressors are closely correlated with their professional self-concept, and high levels of stressors related to nurses' expectations may lead to a decrease in their professional self-concept levels. Chinese Medical Journal 2025;138(17):2130-2138
BACKGROUND:
The purpose of this study was to evaluate the safety and efficacy of subsequent radiotherapy (RT) following first-line treatment with durvalumab plus chemotherapy in patients with extensive-stage small cell lung cancer (ES-SCLC).
METHODS:
A total of 122 patients with ES-SCLC from three hospitals during July 2019 to December 2021 were retrospectively analyzed. Inverse probability of treatment weighting (IPTW) analysis was performed to address potential confounding factors. The primary focus of our evaluation was to assess the impact of RT on progression-free survival (PFS) and overall survival (OS).
RESULTS:
After IPTW analysis, 49 patients received durvalumab plus platinum-etoposide (EP) chemotherapy followed by RT (Durva + EP + RT) and 72 patients received immunochemotherapy (Durva + EP). The median OS was 17.2 months vs . 12.3 months (hazard ratio [HR]: 0.38, 95% confidence interval [CI]: 0.17-0.85, P = 0.020), and the median PFS was 8.9 months vs . 5.9 months (HR: 0.56, 95% CI: 0.32-0.97, P = 0.030) in Durva + EP + RT and Durva + EP groups, respectively. Thoracic radiation therapy (TRT) resulted in longer OS (17.2 months vs . 14.7 months) and PFS (9.1 months vs . 7.2 months) compared to RT directed to other metastatic sites. Among patients with oligo-metastasis, RT also showed significant benefits, with a median OS of 17.4 months vs . 13.7 months and median PFS of 9.8 months vs . 5.9 months compared to no RT. Continuous durvalumab treatment beyond progression (TBP) prolonged OS compared to patients without TBP, in both the Durva + EP + RT (NA vs . 15.8 months, HR: 0.48, 95% CI: 0.14-1.63, P = 0.238) and Durva + EP groups (12.3 months vs . 4.3 months, HR: 0.29, 95% CI: 0.10-0.81, P = 0.018). Grade 3 or 4 adverse events occurred in 13 (26.5%) and 13 (18.1%) patients, respectively, in the two groups; pneumonitis was mostly low-grade.
CONCLUSION
Addition of RT after first-line immunochemotherapy significantly improved survival outcomes with manageable toxicity in ES-SCLC.

OBJECTIVE To investigate the regulatory effect of vitamin D3(VD3)-mediated TIPE2 on macrophage function and phenotype in chronic rhinosinusitis with nasal polyps(CRSwNP).METHODS Peripheral blood samples were collected from 100 CRSwNP patients and 100 patients with simple nasal septum deviation.RT-qPCR and Western blot were used to measure the expression of TIPE2 and Arg-1,and chemiluminescence was used to measure VD3 concentration. In the J774A.1 macrophage cell line,TIPE2 was silenced, and in CRSwNP model mice,TIPE2 was overexpressed, followed by treatment with overexpressed TIPE2 combined with VD3.Western blot was used to measure the expression of TIPE2 and Arg-1,and ELISA was used to measure the expression of IL-4 and IL-13.RESULTS In the CRSwNP group,VD3 levels decreased, while TIPE2 and Arg-1 levels increased(all P<0.05).Arg-1 and TIPE2 were positively correlated(r2=0.994,P<0.05),and both were negatively correlated with VD3 levels(r2=0.652,P<0.05;r2=0.745,P<0.05).Silencing TIPE2 in J774A.1 cells led to a decrease in Arg-1,IL-4,and IL-13 levels(all P<0.05).In CRSwNP mice, overexpression of TIPE2 resulted in decreased VD3 and increased Arg-1,IL-4,and IL-13 levels, which were reversed by VD3 treatment(all P<0.05).CONCLUSION VD3 inhibits the expression of Arg-1,IL-4,and IL-13 in CRSwNP by downregulating TIPE2 expression.

Objective:To analyze the etiological composition and clinical characteristics of infant hypertension, and provide reference for its diagnosis and treatment. Methods:This is a retrospective case-control study.Retrospective investigation and analysis were conducted on the clinical data of infants discharged from Beijing Children's Hospital Affiliated to Capital Medical University with a diagnosis of "hypertension" from June 1,2016 to September 30,2021,including clinical manifestations, auxiliary examinations, treatment plans, and prognosis. Results:A total of 74 eligible children were collected, including 42 male infants(56.8%)and 32 female infants(43.2%).A total of 67 cases(90.5%)had clear secondary factors, including 35 cases of kidney disease(47.3%),12 cases of connective tissue disease(16.2%),and 9 cases of hematological tumor disease(12.2%).At the beginning of the disease, cardiac ultrasound showed that 54 cases(73.0%)had ventricular wall thickening, including mild thickening in 31 cases(57.4%),moderate thickening in 11 cases(20.3%),and severe thickening in 12 cases(22.2%).After grouping by etiology, the incidence of proteinuria and severe hypertension in the renal hypertension group,as well as those receiving multiple antihypertensive drugs, was significantly higher than that in the non-renal hypertension group( χ 2=28.493, P<0.001; χ 2=17.283, P<0.001; χ 2=17.358, P<0.001);Renal disease was risk factor for severe hypertension in infants according to univariate and multivariate logistic regression analysis respectively( OR=11.176,95% CI:2.882~43.339, P<0.001; OR=11.669,95% CI:2.921~46.624, P<0.001).Thirty-one children had follow-up records for 6 months or more, and 13(41.9%)still required antihypertensive treatment,of whom 26(83.9%)were no longer recorded as having elevated blood pressure. Conclusion:Infant hypertension is mainly secondary, with a high proportion of renal factors and predisposition to severe hypertension, which requires multiple antihypertensive drugs for control.Active antihypertensive treatment and removal of secondary factors during the acute phase are helpful for controlling hypertension in infants, but further research is needed on treatment options and long-term prognosis.
