OBJECTIVE: To construct and validate a predictive model for the therapeutic effect of acupuncture at Tongdu Yangxin prescription (acupoint prescription for promoting the circulation of the governor vessel and nourishing the heart) on insomnia, so as to develop an open-access interactive artificial intelligence (AI)-assisted decision-making platform. METHODS: Clinical data of 139 insomnia patients treated with Tongdu Yangxin acupuncture therapy were included. All the patients had received acupuncture at Baihui (GV20), Yintang (GV24⁺), bilateral Shenmen (HT7), and bilateral Sanyinjiao (SP6); and electric stimulation was attached to Baihui (GV20) and Yintang (GV24⁺), using a continuous wave and a frequency of 2 Hz. The treatment was delivered once every other day, 3 treatments a week, and for 2 consecutive weeks. Patients with Pittsburgh sleep quality index (PSQI) score reduction rate <50% were classified as the "no response group", and those with ≥50% were as the "response group". Outliers were addressed using the 1.5×IQR rule, and missing values were imputed via predictive mean matching. Key features were selected by intersecting the feature importance results from eXtreme Gradient Boosting (XGBoost) and random forest algorithms. After balancing class distribution using the Synthetic Minority Over-sampling Technique (SMOTE), 20% of the data was reserved as a validation set. The remained data underwent the stratified sampling iterations to generate 200 pairs of 3∶1 training-test sets, which was employed for training and internal validation of 8 machine learning algorithms. The optimal algorithm and data partitioning strategy were selected to construct the final model, followed by external validation. The best-performing model was deployed online via Streamlit to create an interactive AI platform. RESULTS: Key predictive features for model construction included insomnia duration, the total PSQI score, PSQI sleep efficiency subscore, the proportion of N1 and N2 sleep stages in total sleep duration, and the maximum pulse rate during sleep. The CatBoost-based model achieved an AUC of 0.92, the average precision of 0.77, and accuracy, average recall, and average F1-score of 0.75 on the test set. On the validation set, it attained an AUC of 0.84, with accuracy, average precision, average recall, and average F1-score all at 0.72, demonstrating robust predictive performance. An interactive AI platform was subsequently developed (https://tdyx-catboost.streamlit.app/). CONCLUSION This study successfully establishes and validates a CatBoost-based efficacy prediction model for Tongdu Yangxin acupuncture therapy in treatment of insomnia. The developed AI platform provides data-driven decision support for acupuncture-based insomnia management.

To investigate the effectiveness and safety of ultrasound-guided microwave ablation (MWA) in treatment of primary hyperparathyroidism (PHPT). A total of 12 PHPT patients with parathyroid adenoma were treated with MWA in Nanjing University of Chinese Medicine Affiliated Hospital of Integrated Traditional Chinese and Western Medicine from May 2019 to February 2021. The patients were followed up once every 3 months for 3-12 months. Levels of serum parathyroid hormone (PTH), calcium and phosphorus were detected before and 20 min, 4h and 1day after ablation, and during follow-up period. The volume and volume reduction rate of parathyroid lesion were compared before the treatment and at the end of follow-up. The technical and clinical success of MWA were assessed as well. At the end of follow-up, median serum PTH [66.60 (42.21,80.03) ng/L vs.169.90 (89.01,396.50) ng/L] and calcium [2.39 (2.32,2.49) mmol/L vs. 2.75 (2.57,2.96) mmol/L] levels in 12 patients decreased significantly (all P<0.05). A complete response in terms of PTH and calcium levels was achieved in 6 of the 12 patients, while 4 of the patients had slightly elevated PTH levels just above the upper limit of normal reference range, and 2 of the patients remained abnormal PTH and calcium levels. The clinical cure rate was 50%. The volumes of all lesion after ablation were significantly decreased ( P<0.05), with the technical success rate reaching 92.3%. No serious complications were observed. Ultrasound-guided MWA, thus, is safe and effective in the treatment of PHPT.

By analyzing the main problems existing in the current management of medical devices for clinical trials, this study proposes a feasible management model and specific requirements for acceptance, distribution, storage and recovery combining with the characteristics of medical consumable equipment and diagnostic reagent, which provides a favorable guarantee for the authenticity and reliability of clinical trials.

Objective To investigate the relationship between histogram analysis of DCE-MRI quantitative parameters and clinical stage of nasopharyngeal carcinoma (NPC).Methods 70 patients with NPC confirmed by pathology underwent MRI examination and staging.NPC tumors were measured by full-volume ROI setting method,and the obtained DCE-MRI quantitative parameters were analyzed by histogram.Spearman correlation coefficients were obtained to evaluate the potential correlation between the DCE-MRI histogram quantitative parameters and NPC clinical stages.Results The histogram-based Ktrans (mean,10 th,75 th,90 th),Kep (mean,10 th,kurtosis),and Ve (mean,90 th,skewness)had correlation with T stage (P＜0.05,respectively).The histogram-based Ktrans (mean)and Ve (mean,90 th) showed correlation with N stage (P＜0.05,respectively).The histogram-based Kep (kurtosis)and Ve (mean)had correlation with M stage (P＜0.05,respectively).The histogram-based Kep had no correlation with N stage,and Ktrans had no correlation with M stage. The histogram-based Ktrans (mean,10 th,75 th,90 th),Kep (10 th,75 th,kurtosis)and Ve (mean,75 th,90 th)had correlation with overall stage (P＜0.05,respectively).Conclusion The histogram analysis of DCE-MRI quantitative parameters showed that the multiple parameters associated with NPC overall stages.DCE-MRI quantitative parameters non-invasively reflect the aggressiveness and progression of NPC.The histogram analysis of DCE-MRI quantitative parameters may play a role in clinical stage of NPC.

ObjectiveTo explore the effect of psychological intervention on the cancer-related fatigue and hope level of patients with gastric cancer under the perspective of positive psychology. MethodsBy convenience sampling, 140 patients with gastric cancer admitted from February 2016 to October 2018 in Department of Gastroenterology, Rui＇an People＇s Hospital of Zhejiang Province were selected and divided into the control group and observation group with 70 patients in each group based on random number table. Routine nursing care was given in both of the two groups while the psychological intervention based on positive psychology was carried out in the observation group. The Piper Fatigue Scale(PFS) and the Herth Hope Index(HHI) were adopted to evaluate and measure cancer-related fatigue and hope level of patients. The satisfaction rates of patients were compared by using Satisfactory Questionnaire. ResultsAfter intervention, the dimensional scores of behavior, emotion, physics, cognition and total score of PFS in the observation group were higher than those before intervention and the control group; each dimensional score and the total score of HHI in the observation group were all higher than those before intervention and the control group with statistical differences (P＜0.05). The nursing satisfaction rate in the observation group was 92.86%, higher than that of the control group (82.86%) with statistical difference (P＜0.05). ConclusionsPsychological intervention in patients with gastric cancer from the perspective of positive psychology can alleviate cancer-related fatigue and improve patients＇sense of hope.

Objective To evaluate the role of the diffusion kurtosis imaging(DKI)in the differential diagnosis of breast lesions. Methods Seventy five breast lesions(32 benign and 43 malignant)in 72 patients confirmed by histopathology were studied. All patients underwent 3.0 T MR examinations, including T1WI, T2WI, T2WI-spectral adiabatic inversion recovery, 4b diffusion-weighted imaging, and dynamic contrast-enhanced MR imaging(DCE-MRI). Data were post-processed by mono-exponential and diffusion kurtosis models for quantitation of ADC, apparent diffusion for non-Gaussian distribution(D), and apparent kurtosis coefficient(K). All breast lesions were described with the classification by breast imaging report and data system(BI-RADS). Lesions with BI-RADS class 4B or above were rated as malignancy. Independent sample t test was used to compare the ADC, D, and K value differences between benign and malignant lesions . ROC analysis was performed to assess the role of ADC, D, K value, and BI-RADS in the differential diagnosis of breast lesions. The morphological characteristics, time-signal curve(TIC)type, and other differences between benign and malignant lesions were analyzed with Chi-square test. Results ADC and D values were significantly lower in malignant than in benign lesions(P<0.01). Conversely, K value was significantly higher in malignant lesions than in benign ones(P<0.01). The shape of the benign and malignant breast lesions, edge, enhancement mode, TIC, and BI-RADS classification difference had statistical significance(P<0.05, respectively). The areas under the ROC curve of ADC, D, K, DCE-MRI, and DCE-MRI combined with K value were 0.857, 0.884, 0.949, 0.806, and 0.958, respectively. DCE-MRI combined with K value had the highest diagnosis efficiency. At a cutoff value of K= 0.856, the sensitivity and specificity were 83.7% and 93.8%, respectively. Conclusions DKI model showed higher diagnostic efficiency than that of traditional DWI model. DCE-MRI combined with K value can increase the diagnostic efficiency in breast lesions.

Objective:To evaluate the efficacy and adverse reaction caused by Capecitabine compared with S-1 as maintenance treat-ments for patients with advanced gastric cancer (AGC) after first-line induction chemotherapy. Methods:A total of 130 AGC patients who did not suffer disease progression after first-line chemotherapies, including XELOX (four to six cycles), SOX (four to six cycles), and mFOLFOX6 regimen (six to eight cycles), were randomized into three groups. The Capecitabine group (Cap) received maintenance che-motherapy with Capecitabine (1 000 mg/m2 twice daily for 14 days, 21 days/cycle), while the S-1 group (S1) received S-1 (40, 50, or 60 mg according to the body surface area and orally administered twice a day for 14 days, 21 days/cycle). The control group was consid-ered as the observation group. Patients with maintenance treatments received drugs until disease progression or observation of intol-erant toxicity. Results:A total of 44, 33, and 53 patients received XELOX, SOX, and mFOLFOX6 regimens, respectively. The overall DCR was 63.1%. Among the 82 patients, 35, 28, and 19 belonged to the Cap, S1, and observation groups, respectively. The comparison be-tween the efficacy of treatments in the Cap and S1 groups did not show statistically significant differences (P=0.678). The median time of progression was 8.5 months in the Cap group and 9.0 months in the S1 group (P>0.05). Both groups showed better responses than the observation group, which demonstrated a median progression of 6.0 months (P<0.001). The median overall survivals were 14.5, 15.0, and 14.0 months in the Cap, S-1, and observation groups, respectively (P=0.188). The most common adverse effects observed among the patients with maintenance treatments included myelo-suppression, gastrointestinal reaction, fatigue, hand-foot syndrome, and stomatitis. No death occurred in relation to the therapy. Conclusion:The effectiveness of Capecitabine and S-1 as maintenance chemotherapies in AGC patients after the first-line induction chemotherapy are similar, and both can prolong the time of disease pro-gression with low toxicity.

Objective To explore the influences of guided education training on quality of life of long-term inpatients with schizophrenia. Methods A total of 79 inpatients ( at last one year for being hospitalized) with schizophrenia were divided into control group ( routine treatment and nursing ) and observation group ( routine treatment and nursing combined with guided education rehabilitation training ) according to random number table method from April 2013 to March 2014. Patients between two groups were evaluated with insight and treatment attitude questionnaire ( ITAQ) , scale of social function for psychosis in-patients ( SSFPI) , social support rating scale ( SSRS) , generic quality of life inventory ( GQOLI) one day before intervention and twelve, twenty-four weeks after intervention. Results Twelve weeks after intervention, quality of life of patients in observation group was higher than that in control group with a significant difference (P<0. 05), while there were no significant difference among other aspects(P>0. 05). Twenty-four weeks after intervention, the scores of ITAQ, SSFPI, SSRS, GQOLI in observation group were better than those in control group with significant differences (P<0. 05). Conclusions The guided education training can effectively improve the quality of life of long-term inpatients with schizophrenia.

Microalgae is a single-cell organism with the characteristics of high light energy utilization rate, fast growth rate, high-value bioactive components and high energy material content. Therefore, microalgae has broad application prospects in food, feed, bioenergy, carbon sequestration, wastewater treatment and other fields. In this article, the microalgae biotechnology development in recent years were fully consulted, through analysis from the literature and patent. The progress of microalgal biotechnology at home and abroad is compared and discussed. Furthermore, the project layout, important achievements and development bottlenecks of microalgae biotechnology in our country were also summarized. At last, future development directions of microalgae biotechnology were discussed.

Objective:To establish the control charts for early warning of diarrhea based on the syn-dromic surveillance data from enteric clinic in Beijing .Methods:The outpatient data from enteric clinic of a Grade Three General hospital in Haidian district , Beijing from April 1 to Oct.31, 2009 and from May 1 to Nov.10, 2010 were collected, according to the moving average method , the baseline calcula-ted, the value of probability αand μα, the early warning value based on the formula “w=Xj +μαSj”calculated and the early warning control charts drew at last .Results:According to the harmfulness , the severity and controllability of diarrheal diseases , the value of probability αwas determined as 0.01, thenμα( unilateral) as 2, based on the early warning value , the control charts of diarrheal diseases , bacillary dysentery and other infectious diarrhea were established .Conclusion:The enteric clinic requires to fur-ther collect baseline data to evaluate and continuously adjust the established control charts for the best early warning model in accordance with the enteric clinic .
