1.Early ambulation may avert atrial fibrillation after cardiac surgery

Objective:To explore any impact of early ambulation on new-onset atrial fibrillation (NOAF) after cardiac surgery.Methods:This was a retrospective study covering 364 patients who had survived cardiac surgery. They were divided into an early-activity group (114 cases) whose rehabilitation began within 24 hours after their surgery and a control group (250 cases) whose rehabilitation training began later. Cox proportional hazards regression models were evaluated seeking evidence of any association between early ambulation and postoperative NOAF.Results:The incidence of NOAF during hospitalization was significantly lower in the early-activity group (14.9%) compared to the control group (32.8%). Multivariate analysis revealed that ambulation within 24 hours after surgery may be a protective factor against NOAF. Additionally, the early-activity group exhibited shorter durations of endotracheal intubation and aortic cross-clamping, and tended to have shorter ICU and total hospitalization stays, on average.Conclusions:Initiating ambulation within 24 hours after cardiac surgery predicts less risk of NOAF and improves short-term prognosis, suggesting a protective and preventive effect. However, further studies are needed to validate these findings.
2.Application value of constructing a prediction model based on nutritional status combined with clinical data for infectious complications after radical gastrectomy for gastric cancer

Objective To explore the application value of predictive model based on nutritional status combined with clinical data in infectious complications after radical gastrectomy for gastric canc-er.Methods The clinical data on 394 gastric cancer patients who underwent radical gastrectomy at Suzhou Ninth Hospital Affiliated to Soochow University from January 2017 to July 2024 were retrospec-tively collected.According to whether the patients developed infection after surgery,they were divided into infection group(n=73)and non-infection group(n=321)based on whether they developed postoperative infections or not.The least absolute shrinkage and selection operator(LASSO)method was used to obtain the optimal related features associated with infectious complications after radical gas-trectomy for gastric cancer.Multivariate Logistic regression analysis was conducted to screen for risk factors of infectious complications after radical gastrectomy.R software was used to randomly select 70%of data as training set(for establishing the nomogram model)and 30%of data as test set.The area under the curve(AUC)of the receiver operating characteristic(ROC)curve,Bootstrap sampling validation,and decision curve analysis(DCA)were used to assess the value of the predictive model.Results of the 394 gastric cancer patients,73 developed postoperative infectious complications,in-cluding 53.42%with lung infections,17.81%with intra-abdominal infections,16.44%with uri-nary tract infections,and 12.33%with wound infections.Age,diabetes mellitus,preoperative nu-tritional risk,high preoperative PIV,combined organ resection,and prolonged operation time were identified as risk factors for infectious complications after radical gastrectomy(P<0.05).In the training set,the AUC of the nomogram model was 0.898,and the AUC validated in the test set was 0.891.Bootstrap sampling validation with 1,000 iterations showed that the average errors between the predicted probabilities and actual values in the training and test sets were 0.019 and 0.024,re-spectively.DCA results indicated that the model had clinical utility.Conclusion Infectious com-plications after gastrectomy for gastric cancer are common,and the predictive model based on nutri-tional assessment combined with clinical data has high value in predicting the risk of infectious com-plications after gastrectomy.
3.Development and preliminary application of a one-step RT-qPCR method for de-tection of porcine epidemic diarrhea virus based on immunomagnetic beads

In this study,the carboxylated magnetic beads were coupled with bivalent nanobodies a-gainst porcine epidemic diarrhea virus(PEDV)M protein to construct immunomagnetic beads(IM-NBs-Ⅱ),The capture and enrichment function of IMNBs-Ⅱ was verified by using PEDV propaga-ted in Vero cells.A one-step RT-qPCR detection method for PEDV was established by combining the characteristics of IMNBs-Ⅱ with the detection advantages of reverse transcription fluorescence quantitative polymerase chain reaction(RT-qPCR).Specific analysis found that this method has no cross reactivity with swine fever virus,porcine reproductive and respiratory syndrome virus,por-cine parvovirus,porcine circovirus,indicating that it has good specificity.Sensitivity analysis re-vealed that the detection sensitivity of the RT-qPCR based on IMNBs-Ⅱ was increased 10 times compared to traditional RT-qPCR methods.Detection of the clinical samples confirm that the RT qPCR method based on IMNBs-Ⅱ is suitable for rapid and accurate detection of clinical feces and tissue samples.The method established in this study effectively avoids contamination issues during nucleic acid extraction,simplifies experimental procedures,and saves detection time,which pro-vides a method for efficient detection of PEDV.

7.Research Progress in Environment-responsive Nano-drug Delivery System Based on Traditional Chinese Medicine Monomer in Tumor Therapy

Traditional Chinese medicine monomer has certain curative effects in tumor treatment,but its low targeting and difficulty in gathering at the tumor site limit its application.As a new type of drug delivery method,environmentally responsive nano drug delivery system has the advantages of targeted drug delivery,reducing drug side effects,and environmentally responsive release,and has been widely used in tumor treatment.In this paper,by consulting and sorting out the relevant literature,the general situation of traditional Chinese medicine monomers in the treatment of tumors,the classification of environment-responsive traditional Chinese medicine monomer nano drug delivery system,and its application in tumor treatment are summarized in three aspects,to provide ideas for solving the limitations of traditional Chinese medicine monomer in the treatment of tumor.
8.Correlation Between Expression of HIF-1α and Prognosis of Arsenic-containing Chinese Herbal Compound in Treatment of Myelodysplastic Syndrome

ObjectiveMyelodysplastic syndromes (MDS) is a group of clonal hematopoietic stem cell disorders,and this study aims to investigate the expression of hypoxia-inducible factor-1α(HIF-1α) in the bone marrow cells of patients with MDS and its correlation with the clinical features of MDS,the therapeutic efficacy of arsenic-containing Chineseherbal compound,and the survival prognosis. MethodAccording to the inclusion and exclusion criteria,27 MDS patients treated with arsenic-containing Chinese herbal compound in the Department of Hematology,Xiyuan Hospital,China Academy of Chinese Medical Sciences from January 2022 to September 2022 were included,and their bone marrow samples were collected by myelotomy. HIF-1α expression level in bone marrow cells was detected by real-time polymerase chain reaction (PCR) to analyze its correlation with clinical features,and logistic and Cox regression was used to analyze the risk factors affecting the efficacy and prognostic survival of MDS patients. ResultThe HIF-1α mRNA expression level was lower in bone marrow cells of MDS patients than in healthy subjects. HIF-1α was positively correlated with the degree of myelodysplasia(r=0.384,P<0.05) and bone marrow granulocytic system%(G%)(r=0.560,P<0.01). Logistic regression showed that HIF-1α was a risk factor for the prognosis in the follow-up of the efficacy of treatment(P<0.05)and Cox regression showed that HIF-1α was an independent factor affecting the survival prognosis of MDS patients [odds ratio(OR)=398.968,95% confidence interval(CI)(1.281,116 858.743),P<0.05]. ConclusionThe level of HIF-1α expression in bone marrow cells of MDS patients was closely related to the degree of clinical myelodysplasia and G%,and HIF-1α was a risk factor for the efficacy for and survival prognosis of MDS patients.
9.Free flap combined with super Flow-through technique in replantation of multiple amputated digits in one hand with destructive palm injury: a case report

A 19-year old male patient with multiple traumatic amputations of all digits and destructive injury in one hand caused by a cement mixer was admitted in the Department of Orthopaedics, 920 Hospital, Joint Logistics Support Force, in February 2017. In the emergency surgery, the severed thumb and a bundle of severed index, middle and ring fingers were successfully replanted using super Flow-through technique with transfer of a free anterolateral thigh perforator flap (ALTPF). The little finger was abandoned. The donor site of the flaps were sutured directly. In stage II surgery, bilateral metatarsal tendon transfer and right sural nerve transfer were performed to rebuild the motor and sensory functions of the replanted digits. At 2 years of postoperative follow-up, the texture of the replanted 1st-4th digits and transferred flap in right hand was soft without rupture, and the sensation was restored to S 2+. Some digital flexion and extension were restored, which enable the patient to fulfil the most essential requirement in daily life. According to the Evaluation Standard of Upper Limb Functional of Hand Surgery of Chinese Medical Association, the functional recovery was fear. Only a linear scar remained at the donor site of thigh.
10.Analysis of dynamic functional connectivity abnormalities in obsessive-compulsive disorder patients with different gray matter structural subtypes

Objective:To analyze the patterns of dynamic functional connectivity(dFC) at rest in different obsessive-compulsive disorder(OCD) subtypes based on gray matter volume, and explore the relationship between abnormal gray matter volume and dFC patterns and clinical symptoms in different OCD subtypes.Methods:Clinical data were prospectively collected from a total of 48 patients with OCD from inpatient and outpatient departments of the Fourth Affiliated Hospital of Qiqihar Medical College(20 cases) and Qiqihar Mental Health Center(28 cases) from September 2013 to March 2020. There were 33 males and 15 females, aged 18 to 42(27.1±6.7) years. During the same period, 39 age-matched healthy controls were also recruited, including 31 males and 8 females, aged 18 to 43(28.5±8.6) years. Structural and resting-state functional magnetic resonance imaging scans were performed on 48 OCD patients(OCD group) who met the ICD-10 diagnostic criteria and 39 healthy controls(control group). Using voxel-based morphometry(VBM) and semi-supervised machine learning methods, the patient group was characterized by gray matter volume to perform subtype analysis, and brain regions with abnormal gray matter volume were selected as regions of interest for whole-brain voxel-wise dFC analysis. Correlations between abnormal gray matter volume and dFC and clinical symptoms in OCD patients were analyzed.Results:Based on gray matter volume, OCD patients were computed into two subtypes, of which subtype 1 increased gray matter volume in the right central posterior gyrus(809 voxels; t=4.31; GRF corrected voxel P<0.001, cluster P<0.05 two-tailed), and subtype 2 showed reduced gray matter volume in the left superior temporal gyrus(1 118 voxels; t=-4.37; GRF corrected voxel P<0.001, cluster P<0.05 two-tailed). In subtype 1, the dFC between the right central posterior gyrus and left medial superior frontal gyrus was significantly reduced(187 voxels; t=-4.42; GRF corrected voxel P<0.001, cluster P<0.05 two-tailed); in subtype 2, dFC between the left superior temporal gyrus and left hippocampus, and left central opercular cortex was significantly reduced(272 voxels、99 voxels; t=-4.69, -5.01; GRF corrected voxel P<0.001, cluster P<0.05 two-tailed). In subtype 1, dFC between the right central posterior gyrus and left medial superior frontal gyrus was negatively correlated with illness duration( r=-0.579; P=0.002, uncorrected). In subtype 2, dFC between the left superior temporal gyrus and left hippocampus was positively correlated with illness duration( r=0.578; P=0.003, uncorrected). Conclusions:In the resting state, patients with different subtypes of OCD may have different patterns of dFC abnormalities based on gray matter volume.
