1.SPOC model for training narrative ability of general practitioners and its effectiveness

The study was conducted among 120 general practitioners (GPs) in Shanghai Fengxian District from August 2022 to March 2024, the participants were randomly divided into two age-matched groups with 60 GPs in each group. The intervention group completed a 3-month SPOC (Small Private Online Course)program, including two weeks of narrative medicine theory (concepts, questioning skills, parallel medical record writing, and professionalism) followed by monthly writing practices (three parallel records per month) and group discussions. Both groups performed routine clinical tasks, while the control group received no narrative medicine training. Baseline data were collected, Jefferson Empathy Scale scores, and Doctor-Patient Communication Skills Evaluation Scale scores were evaluated pre- and post-intervention. There were no significant differences in empathy ( P>0.05) or communication skills ( P>0.05) between two groups before the intervention. Post-intervention, both groups showed improved empathy (both P<0.05) and communication skills (both P<0.05) after intervention, while the intervention group were outperforming the control group in both measures ( P<0.05). These findings suggest that SPOC-based narrative competence training effectively enhances the empathy and doctor-patient communication abilities of general practitioners.
2.Design and application of a ventilator circuit interface protective device for weaning.

With the continuous advancement and innovation in medical equipment technology, the transition between high-flow oxygen therapy, non-invasive ventilation, and invasive ventilation can be easily achieved by adjusting the ventilation mode of ventilators. During the weaning phase for tracheotomized patients, it is necessary to disconnect the ventilator circuit, change the ventilator mode, and gradually extend the weaning time to achieve complete ventilator liberation. During the weaning process, due to patients' excessive dependence on the ventilator, there may be situations where respiratory endpoints and Y-connectors of the ventilator are reconnected for invasive ventilation. However, during the weaning process, the Y-connector and expiratory end connectors are exposed to the air, which cannot ensure the tightness of the ventilator circuit, easily increasing the probability of ventilator circuit contamination and subsequently the risk of ventilator-associated pneumonia (VAP). To overcome these issues, the research team of department of critical care medicine of Zhongda Hospital Southeast University has designed a ventilator circuit interface protective device for weaning and has obtained a National Utility Model Patent of China (ZL 2023 2 1453385.8). The main body of the protective device is a Y-connector plug, consisting of multiple components, including a sealing piece, a protective cover, a sealing plug, an interface 1 (connects with the patient's tracheal tube), an interface 2 (connects with the respiratory branch of the ventilator), and an interface 3 (connects with the expiratory branch of the ventilator), featuring a unique design and easy operation. During the patient's weaning training process, the interface 1 and interface 2 is disconnected from the patient's tracheal tube and respiratory branch, respectively. The interface 1 is plugged with a stopper, and the interface 2 is covered with a protective cover to ensure the tightness of the expiratory branch and Y-connector of the ventilator. During the period when the patient is using the ventilator, the protective cover and plug are removed, and connecting them together ensures the tightness of the device itself, reducing the incidence of VAP caused by ventilator circuit contamination, avoiding nosocomial infections, and shortening the prolonged use of invasive ventilation, increased complication rate, extended hospital stay, and increased medical cost associated with weaning.

4.Construction and application of performance appraisal data monitoring and management system for tertiary public hospitals

The performance appraisal of public hospitals is the most official and authoritative assessment and evaluation of tertiary public hospitals in China,and it is an important measure to guide hospitals to improve their internal management level and achieve high-quality development.In this study,a data monitoring management system based on the performance appraisal indicators of national tertiary public hospitals was developed and constructed through intelligent collection and reporting,report in-tegration,visual analysis,data drilling,etc.,which realized the one-stop dynamic management of indicators,optimized the data filling process of national examination indicators,improved the data quality and credibility,and promoted the integration of na-tional assessment and hospital assessment.the intelligent management level of the hospital has been improved,which provides strong support for the hospital's refined operation management and scientific decision-making.
5.Surgical strategy and early efficacy of biportal endoscopic transforaminal lumbar interbody fusion for lum-bar spondylolisthesis with osteoporosis

Objective To investigate the efficacy of biportal endoscopic transforaminal lumbar interbody fusion(BE-LIF)in the treatment of lumbar spondylolisthesis complicated with osteoporosis.Methods From June 2021 to June 2022,36 cases of lumbar spondylolisthesis complicated with osteoporosis were treated with BE-LIF,including 9 males and 27 females,aged(65.94±6.83)years(range:51~76 years).All were single segment,and the responsible segment was L3/4 in 4 cases,L4/5 in 26 cases,and L5/S1 in 6 cases.Imagingexamination showed 1 and 2 degree of spondylolisthesis in 28 and 8 cases,and degenerative and ischemic spondylolisthesis in 24 and 12 cases respectively.Preoperative average bone mineral density T value of the lumbar spine was(-3.19±0.82)SD,and T value of the femur(-2.26±0.86)SD.Oswestry disability index(ODI)was used to evaluate lumbar spine function;visual analogue scale(VAS)to evaluate low back pain and leg pain.ODI,VAS of low back and leg pain were recorded respectively before the operation,at discharge and at the last follow-up.MacNab criteria were used to evaluate the surgical efficacy at the last follow-up.The anterior and posterior disc height,segmental lordotic angle and percentage of slip were measured on the lateral lumbar X-ray films before and after the operation.All patients received standard anti-osteoporosis treatment after surgery.Results All patients completed the opera-tion in one stage.The average operation time was(160.97±35.01)min(range:105~245 min),and the average intraoperative blood loss was(72.50±47.53)ml(range:20~150 mL).The mean follow-up time was(15.17±6.53)months(range:6~26 months).There were statistically significant differences in both VAS score of back and leg and ODI score at discharge and last follow-up when compared with those before the surgery(F=107.48,103.66,55.52,P<0.001).Macnab criteria of the last follow-up showed excellent in 21 cases,good in 12 cases,fair in 3 cases,and the excellent and good rate was 91.7%.Radiographic results showed that the height of the ante-rior and posterior disc height,segmental lordotic angle and percentage of slip were all improved immediately after surgery and at the last follow-up when compared with those before operation,but the anterior and posterior disc height,segmental lordotic angle at the last follow-up were decreased comparing with those immediate after the operation;the difference was statistically significant(P<0.001).The fusion rate at the last follow-up was 91.7%(33/36).The complication rate was 13.8%(5/36),including 1 case of intraoperative endplate injury,1 case of nerve root cuff tear,1 case of delayed postoperative wound infection,and 2 cases of posterior cage migration.Conclusion BE-LIF for the treatment of lumbar spondylolisthesis complicated with osteoporosis can achieve better immediate reduction effect,good clinical outcomes and high fusion rate in short-term follow-up,but the anterior and posterior disc height,segmental lordotic angle are partially lost at the last follow-up.There are still complications,and beginners should be cautious when performing the operation.Postoperative standardized anti-osteoporosis treatment is still an important measure to ensure the curative effect of surgery.
6.Serum myostatin and follistatin as biomarkers of sarcopenia in elderly women

Objective:This study aimed to explore the clinical value of myostatin(MST) and follistatin(FST) as biological biomarkers in evaluating sarcopenia in elderly women.Methods:This was a retrospective cross-sectional study that enrolled 350 females aged 20-89 years who underwent physical examinations in Shanghai Huadong Hospital in 2021. Demographic characteristics, muscle mass, fat mass, bone mineral density, hand grip strength, gait speed, and serum indices of MST and FST were collected.Results:The serum levels of MST did not change significantly with age. However, the serum levels of FST increased with age. In women aged≥60 years, MST was positively correlated with total lean mass and appendicular skeletal muscle index(ASMI; r=0.236, P=0.041; r=0.289, P=0.014), while FST was negatively correlated with ASMI( r=-0.265, P=0.030). In multivariate stepwise regression analysis, after adjusting for age, body mass index, hip bone mineral density, and total fat mass, only FST was independently correlated with ASMI( β=-0.238, P=0.006), while MST was not correlated with ASMI. The receiver operating characteristic curve was plotted using muscle mass reduction as the state variable and serum FST level as the test variable. The area under the curve was 0.753. And when the FST cutoff value was 17.49 ng/mL, the maximum Jordan index was 0.46, with a sensitivity of 77.3% and a specificity of 68.7%. Women aged ≥60 years were divided into three groups based on serum FST levels. Compared to the upper third of the serum FST level group, the low third of the FST level group had a significantly reduced risk of suffering from sarcopenia( OR=0.098, P =0.036). Conclusions:Serum FST lever has a better correlation with muscle mass among elderly women, making it a promising biomarker for evaluating muscle mass.
7.A survey on the intention to stay and its influencing factors among primary health care workers during COVID-19 epidemic

Objective:To investigate the intention to stay on among primary health care workers (PHWs) during the COVID-19 epidemics and its influencing factors.Methods:An online questionnaire survey was conducted among PHWs from 62 primary health institutions in 31 provinces, autonomous regions and municipalities across China selected by multi-stage whole cluster random sampling method between May and October 2022. According to the job position, the PHWs were divided into 5 categories: general practitioners (GPs), nurses, public health doctors, managers and support staff. Intention to stay was measured using the Chinese version of the Intention to Stay Questionnaire. Multiple linear regression model was used to analyze the influencing factors of the intention to stay in PHWs of different occupational categories, including personal factors, work factors, factors related to the COVID-19 and psychological reactions.Results:A total of 3 769 PHWs from 44 community health service centers, 18 township hospitals of 27 provinces/autonomous regions and 4 municipalities participated in this survey. The mean age of participants was (37.4±9.2) years, including 2 971(78.8%) women. The mean score of intention to stay of participants was 21.7±4.1. Compared with GPs, managers had lower intention to stay ( P=0.004). Age, female, in marriage, monthly income, years of primary care service, self-evaluation of unit support function, MSQ-SF score, and PA score were positively correlated with the score of intention to stay (all P<0.05), while PHQ-9 score, EE score, and DP score were negatively correlated with the score of intention to stay (all P<0.05). The MSQ-SF scores of all occupational categories were positively correlated with the scores of intention to stay (all P<0.01). In addition, among GPs, monthly income, years of service in primary care, self-evaluation of unit support function, and PA scores were positively correlated with the score of intention to stay (all P<0.05), while PHQ-9 scores and EE scores were negatively correlated with the score of intention to stay (all P<0.05). Among nurses, age, female gender, monthly income, self-evaluation of unit support function, and PA score were positively correlated with the score of intention to stay (all P<0.05), while EE scores and retention will score were negatively associated with ( P<0.001). Among public health doctors, in marriage was positively correlated with the score of intention to stay ( P=0.018). Among managers, DP score was negatively correlated with the score of intention to stay ( P=0.001). Among support staff, female gender, monthly income, years of primary care service, self-evaluation of unit support function, and PA scores were positively correlated with the score of intention to stay (all P<0.05), while EE score and DP score were negatively correlated with intention to stay (all P<0.05). Conclusions:The intention to stay of PHWs in China during the COVID-19 was at an intermediate level, which was affected by many factors. Among them, job satisfaction was positively correlated with the intention to stay of all occupational categories, and the influencing factors of different occupational categories have some variations.

Prone position ventilation(PPV)is a lung protective ventilation strategy widely used in patients with acute respiratory distress syndrome(ARDS)to improve ventilation and oxygenation of the lungs by changing the patient's position.During PPV treatment,patients may face a variety of complications and risks,such as pressure injury,brachial plexus injury,corneal injury and unplanned extubation,which aggravate the patient's pain,increase medical costs and affect the prognosis of the disease.To this end,medical staff at Zhongda Hospital Southeast University jointly designed a PPV patient care kit pillow and obtained a National Utility Model Patent of China(patent number:ZL 202221128961.7).The PPV patient care kit pillow is composed of head pad,chest pad,hip pad and lower limb pad,with gel on its surface,which can reduce friction force and shear force;grooves are set on the middle of head pad,two sides above chest pad,middle of chest pad,middle of hip pad and lower limb pad,which can effectively avoid compression of corresponding parts and reduce the risk of pressure injury;and air bags are set on both sides below head pad,hip pad and lower limb pad,which can be inflated unilaterally or bilaterally at the same time and used for deflecting or elevating the body;in addition,a tube limiting groove is set on the outside of head pad,which can effectively fix the tracheal tube and reduce the risk of tube displacement and unplanned extubation.The overall structure of the pillow of this PPV patient care kit conforms to the angle of human mechanics and has strong versatility,which can improve the comfort,safety and effectiveness of PPV therapy for patients and reduce the work burden of medical staff,and has good application value.

Objective:To investigate the effects of breast milk to total milk intake ratio during hospitalization on the duration of antibiotic therapy in preterm infants less than 34 weeks of gestation.Methods:Clinical data of preterm infants ( n=1 792) less than 34 gestational weeks were retrospectively collected in 16 hospitals of Jiangsu Province Neonatal-Perinatal Cooperation Network from January 1, 2019, to December 31, 2021.
