1.Mechanism of astragaloside IV promoting bone marrow EPCs mobilization in diabetic ulcer rats

Objective:To investigate the effect of astragaloside IV (AS-IV) regulating the signal axis of stromal cell-derived factor-1α (SDF-1α)/CXC chemokine receptor 4 (CXCR4) on the mobilization of bone marrow endothelial progenitor cells (EPCs) to peripheral blood in diabetes skin ulcer (DSU) rats.Methods:Twenty four SPF grade male Sprague Dawley (SD) rats were selected to make the model of type 2 diabetes rats by intraperitoneal injection of 30 mg/kg 1% (plastid ratio) streptozotocin, and then round full-thickness skin with a diameter of 2 cm was cut on both sides of the waist and back to make the skin ulcer model of diabetes rats. After that, they were randomly divided into AS-IV group (50 mg/kg AS-IV), blocker group (50 mg/kg AS-IV+ 5 mg/kg AMD3100) and model group. At the same time, a blank group ( n=8) was set up, The drug was administered via intraperitoneal injection, and the model group and blank group were treated with 0.9% NaCl of equal volume. On the 10th day, peripheral blood, femoral bone marrow, and wound neovascularization tissues of rats were collected. The number of EPCs in peripheral blood of each group of rats was measured by flow cytometry, and the protein expression of SDF-1α and CXCR4 in peripheral blood, femoral bone marrow, and wound neovascularization tissues of rats was detected by enzyme-linked immunosorbent assay (ELISA); At the same time, the wound healing rates of each group were tested. Results:On the 10th and 21st day after modeling, the wound healing rate of each group of rats was compared. The blank group healed the fastest, while the model group healed the slowest. The AS-IV group had better healing than the model group and the blocker group, and the difference was statistically significant (all P<0.05). On the 10th day after modeling, the positive rates of peripheral blood EPCs in the white group, AS-IV group, and blocker group were significantly higher than those in the model group (all P<0.05), while the positive rates of peripheral blood EPCs in the blocker group were significantly lower than those in the AS-IV group (all P<0.05). On the 10th day after modeling, the protein expression of SDF-1α and CXCR4 in the wound, serum, and bone marrow of the model group was the lowest, while the protein expression in the blank group was the highest (all P<0.05). The protein expression of SDF-1α and CXCR4 in the wound, serum, bone marrow of the AS-IV group was significantly higher than that of the blocker group and model group, and the difference was statistically significant (all P<0.05). Conclusions:Astragaloside IV can promote the mobilization and migration of endothelial progenitor cells from bone marrow to peripheral blood in diabetes ulcer rats by regulating SDF-1α/CXCR4 signal axis, and can participate in angiogenesis of diabetes ulcer wounds as seed cells to promote the healing of diabetes skin ulcers.
2.Results of Lung Cancer Screening with Low-dose Computed Tomography and Exploration of Risk Factors in Guangzhou

Background and objective Both of lung cancer incidence and mortality rank first among all cancers in China.Previous lung cancer screening trials were mostly selective screening for high-risk groups such as smokers.Non-smoking women accounted for a considerable proportion of lung cancer cases in Asia.This study aimed to evaluate the outcome of community-based mass screening in Guangzhou and identify the high-risk factors for lung cancer.Methods Residents aged 40-74 years in Guangzhou were screened with low-dose computed tomography(LDCT)for lung cancer and the pulmonary nodules were classified and managed according to China National Lung Cancer Screening Guideline with Low-dose Computed Tomography(2018 version).The detection rate of positive nodules was calculated.Before the LDCT examination,residents were required to complete a"lung cancer risk factors questionnaire".The risk factors of the questionnaire were analyzed by least absolute shrinkage and selection operator(LASSO)penalized Logistic regression analysis.Results A total of 6256 residents were included in this study.1228 positive nodules(19.63%)and 117 lung cancers were confirmed,including 6 cases of Tis,103 cases of stage Ⅰ(accounting for 88.03%of lung cancer).The results of LASSO penalized Logistic regression analysis indicated that age ≥50 yr(OR=1.07,95%CI:1.06-1.07),history of cancer(OR=3.29,95%CI:3.22-3.37),textile industry(OR=1.10,95%CI:1.08-1.13),use coal for cooking in childhood(OR=1.14,95%CI:1.13-1.16)and food al-lergy(OR=1.10,95%CI:1.07-1.13)were risk factors of lung cancer for female in this district.Conclusion This study highlighted that numerous early stages of lung cancer cases were detected by LDCT,which could be applied to screen-ing of lung cancer in women.Besides,age ≥50 yr,personal history of cancer,textile industry and use coal for cooking in childhood are risk factors for women in this district,which suggested that it's high time to raise the awareness of early lung cancer screening in this group.
3.Methylprednisolone combined with mycophenolate mofetil for the treatment of oral mucosal pemphigoid:a case report and literature review

Objective To investigate the application of mycophenolate mofetil(MMF)in oral mucosal pemphigoid and provide a clinical reference.Methods One case of glucocorticoids combined with MMF in the treatment of oral mucosal pemphigoid was reported,and the clinical application of MMF in oral mucosa-related bullous diseases was dis-cussed.Results One patient with a clinical diagnosis of"oral mucosal pemphigoid"was treated with methylpredniso-lone(36 mg,qd,morning dose)or combined hydroxychloroquine sulfate(0.1 g/time,bid)and thalidomide capsules(50 mg,qd,bedtime)and other drugs.The patient's disease was slowly controlled but prone to recurrence.The treat-ment regimen was immediately adjusted,i.e.,methylprednisolone(36 mg,qd,morning dose)was combined with MMF(0.5 g/time,bid)for 2 weeks,which resulted in ideal lesion healing control.After 8 weeks of methylprednisolone com-bined with MMF,the dose of methylprednisolone was gradually reduced to 12 mg,qd,and MMF was reduced to 0.5 g,qd,the patient's symptoms improved significantly,and no obvious lesions were found in the mouth.The dose was then reduced and maintained according to the principle of pemphigoid treatment.Methylprednisolone(8 mg,qd,morning dose)and MMF(0.5 g,qd)have been used for 6 months of maintenance treatment,and they are still being followed up.As yet,the patient's condition is stable without obvious lesions and new blisters,and no obvious side effects have been observed.A review of the literature shows that MMF is widely used in the field of dermatology to treat a variety of im-mune diseases,such as connective tissue diseases and autoimmune blistering diseases.According to the reports of ad-verse reactions to MMF,digestive system reactions are the most common adverse reactions;therefore,patients with ac-tive gastrointestinal diseases should be treated with caution,followed by bone marrow suppression,and it is recommend-ed to monitor liver function and blood routine in patients using MMF.The safety and efficacy of MMF for treating pem-phigoid involving the skin have been reported in the literature,but oral mucosal doctors still lack experience for treating mucous membrane pemphigoid.Conclusions As a new immunosuppressant,MMF has high safety and no obvious side effects and can be considered as a combination adjuvant drug for patients with severe clinical disease and refractory oral mucosal pemphigoid.
4.Study on the Predictive Value of Serum Hsa-miR-30c-5p Level Expression in Patients with Type 2 Diabetes Mellitus for Micro vascular Complications

Objective To explore the predictive value of serum homo sapiens microRNA(hsa-miR)-30c-5p expression in patients with type 2 diabetes mellitus(T2DM)for microvascular complications.Methods A total of 205 T2DM patients admitted to Bazhong Central Hospital from May 2021 to September 2022 were selected as the diabetes group,and the diabetes group was further divided into diabetes with combined group(n=124)and non combined group(n=81)according to the microvascular complications of the patients.In addition,205 healthy people who underwent physical examination during the same period were selected as the control group.The expression of hsa-miR-30c-5p in serum was detected by reverse transcription-polymerase chain reaction(RT-PCR)and compared.The factors affecting microvascular complications were analyzed using multivariate logistic regression analysis,and receiver operating characteristic(ROC)curves were plotted to predict the value of serum hsa-miR-30c-5p expression in predicting microvascular complications in T2DM patients.Results The expression of serum hsa-miR-30c-5p in the combined group(0.58±0.06)and the non-combined group(0.72±0.08)were lower than that in the control group(0.89±0.21),and the differences were significant(t=16.038,7.079,all P=0.001).The combined group was lower than the non-combined group,and the difference was significant(t=14.289,P=0.001).The course of diabetes[(OR(95%CI):3.873(2.976~4.770)],uric acid[(OR(95%CI):2.125(1.211~3.040)]and glycosylated hemoglobin[(OR(95%CI):2.680(1.745~3.616)]were independent risk factors for microvascular complications in T2DM patients(all P<0.05),while the time within the target range of glucose[(OR(95%CI):0.491(0.135~0.846)]and serum hsa-miR-30c-5p[(OR(95%CI):0.532(2.976~4.770)]were protective factors for microvascular complications in T2DM patients(all P<0.05).The sensitivity,specificity and area under the curve(95%CI)of serum hsa-mir-30c-5p expression in predicting microvascular complications in T2DM patients were 81.45%,85.19%and 0.802(0.741~0.854),respectively.Conclusion The expression of serum hsa-miR-30c-5p in patients with T2DM is abnormally reduced,and serum hsa-miR-30c-5p is a protective factor for microvascular complications in patients with T2DM.It may have a certain predictive value for microvascular complications in patients with T2DM.
5.Analysis of quality of life and influencing factors in patients with multidrug-resistant tuberculosis

Objective To explore the health-related quality of life(HRQOL)among patients with multidrug-resistant and rifampin-resistant pulmonary tuberculosis(MDR/RR-PTB)and analyze its influencing factors.Methods Data was collected from 50 MDR/RR-PTB patients registered for treatment management in the"Tuberculosis Management Information System"in Bazhong City from 2021 to 2024 as resistance group.Fifty pulmonary tuberculosis patients sensitive to anti-tuberculosis drug treatment registered in the system during the same period were selected as control group.A cross-sectional survey method was employed using the quality of life instruments for chronic disease pulmonary tuberculosis(QLICD-PT)to measure and compare the HRQOL between two groups.Multiple linear regression analysis was used to analyze factors affecting the HRQOL of MDR/RR-PTB patients.Results Except for physiological function,there were statistically significant difference between MDR/RR-PTB group and control group in terms of total quality of life score,psychological function,social function,and specific modules(P<0.05).Univariate analysis showed that there were statistically significant differences in the total quality of life scores among patients with different genders,medical insurance statuses,monthly family income situations,presence or absence of comorbidities,lymphocyte count,albumin,C-reactive protein,and body mass index(P<0.05).Multiple linear regression analysis revealed that(P<0.05)there was significant difference for the partial regression coefficient tests of patient's medical insurance and monthly family income situations(P<0.05).Conclusion The HRQOL of MDR/RR-PTB patients is lower than that of pulmonary tuberculosis patients sensitive to anti-tuberculosis drugs.The patient's health insurance and monthly family income are potential factors affecting the HRQOL of MDR/RR-PTB patients.
6.Fatigue development trajectory and its influencing factors in middle-aged and elderly patients with stroke

Objective To analyze the trajectory categories of fatigue development in middle-aged and elderly stroke patients and explore its influencing factors. Methods A convenient sampling method was used to select 230 middle-aged and elderly stroke patients in the Luzhou People's Hospital from January 2021 to January 2023 as the research subjects. A self-compiled general information-questionnaire, the Fatigue Severity Scale (FSS), the Pittsburgh Sleep Quality Index (PSQI), and the Beck Depression Inventory (BDI) were used to investigate the sociodemographic and clinical data, fatigue, sleep quality, and depression status of patients at 2 days after admission and 3 and 6 months after discharge. The latent growth curve model was used to fit the categories and trajectories of fatigue development, and the unordered multinomial Logistic regression model was used to analyze the influencing factors of trajectory categories of fatigue development. Results The fatigue development trajectory of middle-aged and elderly stroke patients showed three potential categories, namely the low-level fatigue stability group (33.04%), the medium-level fatigue rapid aggravation group (42.17%), and the high-level fatigue slow aggravation group (24.79%). 7.Research Progress of chitosan derivatives as materials for siRNA nano-delivery systems

Small interfering RNA(siRNA) has been the focus of attention in the field of drug development for its excellent properties such as efficiency, specificity and transient nature in treating diseases. However, the poor stability and low cellular uptake of naked siRNAs make it difficult for them to exert their gene silencing effects, a high-quality drug delivery system is needed to deliver them to target cells where they can function. Many nano delivery systems are no longer suitable for siRNA transport due to toxicity and drug loading problems, chitosan derivatives are beginning to receive widespread attention because of their high water solubility, safety and stability. This paper reviewed the research progress of chitosan derivatives as siRNA nano-delivery systems and provided a reference for researchers who are interested in the development of nano-delivery systems.
8.Influence of online and offline Satir family therapy on emotions of adolescents with depressive disorder and their parents

Objective:To explore the influence of online and offline family therapy based on the Satir model on emotions of adolescents with depressive disorder and their parents in remote areas.Methods:A total of 98 cases adolescents with depressive disorder treated in the psychosomatic medicine of the Second Affiliated Hospital of Nanchang University from January 2021 to June 2021 and their parents were selected as the objects. The adolescents with depressive disorder and their parents were randomly divided into the control group (49 parents and 49 adolescents) and the observation group (49 parents and 49 adolescents). The control group received the medical treatment (sertraline 100 mg/d) and the routine health education, while the observation group received the online and offline Satir family therapy on the basis of the intervention of the control group. Generalized anxiety disorder-7 (GAD-7) and patient health questionnaire-9 (PHQ-9) were used to investigate the negative emotions of the parents of the two groups before and 12 weeks after the intervention. The screen for child anxiety related emotional disorders (SCARED) and depression self-rating scale for childhood (DSRS) were used to investigate the negative emotions of the adolescents before and 12 weeks after the intervention.The SPSS 20.0 software was used for statistical analysis. t test was used to compare the SCARED scale score and DSRS score changes of the adolescents in the two groups, and χ 2 test was used to compare the proportional changes of parents' anxiety and depression. Results:The scores of SCARED (51.55±12.69 vs 36.82±7.69, t=15.839) and DSRS (25.08±4.81 vs 16.88±2.16, t=13.047) of adolescents in the control group were significantly different before and after the intervention (both P<0.05). The scores of SCARED (51.16±15.84 vs 31.31±7.72, t=14.385) and DSRS (24.12±4.81 vs 14.08±2.03, t=14.723) of adolescents in the observation group were significantly different before and after the intervention (both P<0.05). After the intervention, the scores of SCARED and DSRS in the observation group were lower than those in the control group ( t=3.540, 6.609, both P<0.05). Before intervention, there was no significant difference in the proportion of anxiety and depression between the parents of the two groups (χ 2=1.837, 3.547, both P>0.05). After 12 weeks of intervention, there was a statistically significant difference in the proportion of anxiety and depression between the two groups, which were lower in the observation group than those in the control group (χ 2=5.995, 4.009, both P<0.05). Conclusion:Online + offline family therapy based on the Satir model can not only effectively reduce anxiety and depression of adolescents, but also effectively reduce anxiety and depression of their parents.It is especially suitable for outpatient management of children with depressive disorder in remote areas.
9.Analysis of clinical diagnosis and treatment in chronic hepatitis B combined with autoimmune hepatitis

Objective:To summarize the clinical diagnosis and therapeutic method in chronic hepatitis B (CHB) combined with autoimmune hepatitis (AIH).Methods:Clinical manifestations, laboratory examination, imaging, histopathological characteristics, treatment and prognosis of 19 cases diagnosed with CHB combined with AIH followed at the outpatient Department of Gastroenterology of Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine during December 2013 to June 2018 were retrospectively analyzed. Paired sample t-test was used before and after treatment for the measurement of normal distribution data. Measurement data of non-normal distribution were expressed as a median (quartile spacing) and Wilcoxon matched-pairs signed rank test was used before and after treatment.Results:Among the 19 cases, 5 were male and 14 were female. The age of onset was 35 to 63 years, and the average age was 47.10 ± 8.76 years. There were 12 cases diagnosed with CHB before AIH, 5 cases diagnosed with AIH before CHB, and 2 cases diagnosed with AIH and CHB at the same time. After the definite diagnosis of CHB combined with AIH, nucleoside (acid) analogues (antiviral against hepatitis B virus) combined glucocorticoid therapy were given, and azathioprine or mycophenolate mofetil (immunosuppressant) was added according to the intrahepatic inflammation (inflammation graded at G3 and above) and leukocyte conditions. The duration of treatment varied between 2 weeks to 16 (median treatment duration of 6 weeks), except for one case who was just diagnosed and followed up. Biochemical indicators and immunoglobulin of the remaining 18 cases before and after treatment was significantly decreased, and the differences were statistically significant ( P < 0.05), with HBV DNA < 20 copies/ml. Conclusion:CHB combined with AIH diagnosis can be easily missed. Therefore, it requires comprehensive diagnosis combined with clinical characteristics, autoantibodies, and immunoglobulin levels with special emphasis on pathological characteristics of liver tissue. Anti-HBc-positive patients using immunosuppressant should be carefully monitored for HBV DNA and anti-HBV treatment should be given if necessary.
10. Research progress and diagnostic value of exosome microRNA in hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is the most common type of primary liver cancer, and its mortality ranks fourth in global malignant tumors. Early diagnosis of HCC has an important impact on the survival rate of patients. Exosomal microRNA (miRNA) is closely related to the occurrence and development of HCC. It can be secreted and transferred to recipient cells through the corresponding target genes, and play a role of regulating cancer progression. Exosomal miRNAs have great differences in the expression of HCC patients and in vitro cell lines, and it has the advantages of high content, specificity and stability relative to circulating miRNAs. It not only can better distinguish between HCC and healthy patients, but also can be further differentiated from hepatitis or cirrhosis, and it also has certain value in the diagnosis of recurrent HCC.
