OBJECTIVE To analyze the status of targeted monitoring of the health care-associated infections in in-tensive care unit(ICU)of a three-A hospital of northwest China in recent 7 years so as to provide bases for formu-lating effective prevention and control measures for the health care-associated infections.METHODS The related data were successively collected from the ICU hospitalized patients of the Second Affiliated Hospital of Air Force Medical University who were under the targeted monitoring by nosocomial infection real-time monitoring system from Jan.2017 to Dec.2023.The data included the incidence of infections,infection sites,use of catheters,inci-dence of catheter-related infections,and distribution and prevalence trend of pathogens.RESULTS A total of 49,137 hospitalized patients from five ICU wards of respiratory medicine department,neurosurgery department,neurology department,thoracic surgery department and critical care medicine department were under the monito-ring,1446(1681 case-times)of whom had health care-associated infections,with the infection rate 2.94％,the case-time infection rate 3.42％.The respiratory system was the major infection site of the patients with the health care-associated infections.Among the patients with instrument-associated infections,20.70％had ventilator-asso-ciated pneumonia(VAP),5.71％had urinary catheter-associated urinary tract infection(CAUTI),and 1.96％had catheter-related bloodstream infection(CRBSI).Totally 405 strains of non-repetitive pathogens were isolated from 477 patients with instruments-associated infections,78.77％of which were gram-negative bacteria.The iso-lation rate of multidrug-resistant organisms(MDROs)was 44.12％,and Acinetobacter baumannii was the pre-dominant species of pathogen isolated from the patients with VAP.CONCLUSIONS The targeted monitoring of health care-associated infections may facilitate the awareness of dynamic changes of the infections in a accurate and timely manner so as to provide bases for developing effective prevention and control measures for the health care-associated infection.It is necessary to strengthen the prevention and control of the infections in critical care medi-cine department,neurosurgery department as well as the lower respiratory tract infections and prevent the MDROs infection in the meantime.

Children， as a special group， frequently experience of off-label drug use worldwide. Common reasons for off-label drug use in children include the lack of data on pediatric patients during the clinical trial stage of drug development， delayed updates to drug instructions， and the non-standard professional behavior of some doctors. Off-label drug use in children is a double-edged sword. It could save lives and provide a way to explore additional functions of drugs， while it may also lead to the phenomenon of hyper-indication abuse， increasing the risk of adverse drug events. Regulating off-label drug use in children can safeguard the best treatment rights and interests of children. It is recommended to encourage pharmaceutical enterprises to conduct research and development of pediatric new drugs， simplify the approval process for drug instructions amendments， accumulate evidence-based medical evidence for off-label drug use in children， standardize the process of off-label drug use in children in medical institutions， continuously improve the standardized diagnosis and treatment capabilities of pediatricians， and actively cooperate with the families of pediatric patients in diagnosis and treatment， so as to comprehensively safeguard the rights and interests of both doctors and patients.

Rheumatoid arthritis(RA)is a chronic erosive joint inflammation as the main clinical manifestation of autoimmune disease.Currently,commonly used laboratory diagnostic indicators include rheumatoid factor(RF),anti-citrullinated protein antibody(ACPA),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),etc.Among them,ACPA can be found in patients'serum at an early stage and has a higher diagnostic value.However,nearly one-third of RA patients are still ACPA-negative,and there is a lack of effective diagnostic markers.Therefore,it is of great significance to explore new biomarkers for the diagnosis of serum ACPA-negative RA patients and their clinical application value for early diagnosis and treatment.In recent years,there has been an increasing number of studies on diagnostic markers for ACPA-negative RA,mainly in the areas of proteomics,immune mechanisms,genetic inheritance and autoantibody production,and a number of new autoantibodies and proteins with relative specificity have been identified.This article reviews the value of the clinical application of these new autoantibodies and proteins in the diagnosis of ACPA-negative RA and their outlook,thus providing some reference value for the early diagnosis of the disease.

To address the challenges of extracting nonlinear features from motor imagery electroencephalogram(EEG)signals and effectively capturing functional connectivity between EEG channels,a classification and recognition method for motor imagery EEG signals is proposed based on mutual information and adaptive graph convolutional network.The proposed method extracts frequency domain information by sub-frequency banding on the original motor imagery EEG signals,uncovers the nonlinear relationships within EEG signals by an adjacency matrix constructed with mutual information neural estimation method,and finally achieve null-frequency feature extraction by capturing the dynamic correlation strength between channels with an adaptive graph convolutional network incorporating convolutional block attention module.On the BCI Competition Ⅳ 2a and BCI Competition Ⅲ 3a datasets,the proposed method has average accuracies of 83.14％and 88.19％,respectively,demonstrating that it can effectively reveal functional connectivity between EEG channels,providing a new approach for decoding motor imagery EEG signals.

Objective To explore the effects of eugenol combined with traditional Chinese medicine Wuxing music(EUG+MUSIC)on skin inflammation,behavioral characteristics,hormone and neurotransmitter levels,and the intestinal microbiota in a mouse model of 2,4-dinitrofluorobenzene(DNFB)-induced atopic dermatitis(AD).Methods A mouse model of AD was established by induction with DNFB for 21 consecutive days;throughout the study,changes in dorsal skin lesions and scratching behavior were recorded in mice and changes in dorsal skin lesions and pruritus were recorded.Mice underwent open-field,elevated 0 maze,tail-suspension,and forced-swimming tests on days 19 and 20.The mice were then sacrificed on day 21 and the organ index was calculated.Serum levels of immunoglobulin E,interleukin(IL)-4,and IL-10 were determined by enzyme-linked immunosorbent assay.Pathological changes in back-skin sections were detected by hematoxylin-eosin(HE)and toluidine blue staining,and brain levels of corticosterone,dopamine,and serotonin were measured.The composition of the intestinal microbiota was analyzed by 16S rRNA gene sequencing.Results The skin lesion score and skin inflammatory cell infiltration were significantly reduced in the EUG+MUSIC group.Regarding the behavioral tests,mice in the EUG+MUSIC group showed more active locomotion,an increased desire to explore unfamiliar environments,and increased struggling time in stressful situations.Brain levels of serotonin,dopamine,and corticosterone were also increased in the EUG+MUSIC group.16S rRNA gene sequencing showed that DNFB affected the diversity of the intestinal microbiota in AD mice,while EUG+MUSIC treatment remodeled the gut microbial composition,including increasing the relative abundance of gut microbes such as Ligilactobacillus and Lachnospiraceae and decreasing Parabacteroides and Bacteroides spp.Spearman's analysis showed significant correlations among altered gut microbiota,AD-associated skin manifestations,and anxiety-and depressive-like behaviors arising from comorbidities.Conclusions Eugenol combined with traditional Chinese medicine five-elements music therapy may have a positive effect on comorbid anxiety-and depression-like behaviors by regulating the intestinal microbiota and improving the behavioral performance in mice with AD,thus providing new ideas and method for improving AD and its associated anxiety-and depressive behaviors.

OBJECTIVE To analyze the current status of hospital-acquired infections(HAI)in cancer patients in Henan Cancer Hospital,and explore the application of Case Mix Index(CMI)based on disease diagnosis-related groups(DRG)in the evaluation of HAI risks.METHOD Data on HAI of patients in cancer hospitals in Henan Province from Jun.2023 to Jun.2024 were statistically analyzed,including incidence rates,distribution of infec-tion sites and composition ratios of pathogens,high-risk departments were identified by CMI-adjusted 1 000-day incidence rates of HAI infections,and the current status of HAI in cancer patients were analyzed.RESULTS The incidence of HAI in cancer patients was 1.67％(4 315/259 087),and the incidence rate of HAI in 1 000 days was 2.46 per 1000.The sites of infection were mainly the respiratory tract,the abdominal cavity,the surgical sites and the bloodstream.A total of 2 381 strains of pathogens were cultured and isolated,including 1 570 strains of gram-negative bacteria,638 strains of gram-positive bacteria and 173 strain s of fungi,and the top five strains de-tected were Escherichia coli(24.49％),Klebsiella pneumoniae(15.41％),Pseudomonas aeruginosa(9.11％),Staphylococcus aureus(8.86％)and Staphylococcus epidermidis(4.66％).After CMI adjustment,the order of decreasing was dominated by surgical departments,while the order of increasing was dominated by non-surgical departments,and the top five departments with the highest adjusted incidence rates were Orthopedics and Soft Tissue department,Hematology department,Hepatobiliary Surgery department,General Surgery department and Urology department.CONCLUSIONS In the prevention and control of HAI in cancer patients,attention should be given to the gram-negative bacteria,and close monitoring of infections in the respiratory tract,abdominal cavity,and surgical sites is essential.This study uses the CMI to adjust the HAI incidence rate of cancer patients in order to identify high-risk departments that currently require focused management,it also analyzes the DRGs codes with a highest proportion of HAI in each department,with a particular focus on patients in these high-risk groups,and it aims to avoid and minimize the occurrence of HAI among cancer patients as much as possible through refined DRGs analysis and targeted prevention and control measures.
