1.Epigenetic mechanism of Diwu Yanggan Capsule in improving liver regeneration microenvironment in a rat model of liver cancer

ObjectiveTo investigate the epigenetic mechanism of Diwu Yanggan Capsule in improving liver regeneration microenvironment in a rat model of liver cancer by regulating DNA methylation, and to provide a basis for scientific clinical medication. MethodsA total of 48 specific pathogen-free Sprague-Dawley rats were divided into normal group, model group, and Diwu Yanggan Capsule group using a random number table, with 16 rats in each group. The Solt-Farber two-step method was used to establish a rat model of liver cancer. The rats in the Diwu Yanggan Capsule group were given Diwu Yanggan Capsule at a dose of 750 mg/kg/d by gavage, and those in the normal group and the model group were given an equal volume of normal saline by gavage. Liver tissue samples were collected from each group of rats after 16 weeks of continuous intervention; DNA methylation chips were used to analyze the change in DNA methylation in liver tissue, and gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses were used for data analysis. In addition, the MeDIP-PCR technique was used to detect the changes in candidate differentially methylated genes such as YWHAB, ADCK2, ERLIN2, SEMA3B, and TPH2 in the liver tissue of rats, and Western blot and RT-qPCR were used to verify the expression of key methylated genes. The independent-samples t test was used for comparison of continuous data between two groups, and a one-way analysis of variance was used for comparison between multiple groups, while the least significant difference t-test was used for further comparison between two groups. ResultsThe DNA methylation chip analysis showed that compared with the normal group, the model group had significant methylation changes in the promoter region of 2 422 genes in liver tissue of rats. The GO functional enrichment analysis and the KEGG pathway enrichment analysis showed that these differentially methylated genes were significantly enriched in metabolic pathways such as steroid hormone biosynthesis and drug metabolism-cytochrome P450. Compared with the model group, the Diwu Yanggan Capsule group had significant reversal of promoter methylation in 1 650 genes, and the KEGG enrichment analysis showed that these genes were mainly involved in the pathways closely associated with cell proliferation, apoptosis, and microenvironment regulation, such as the calcium ion signaling pathway, the cAMP signaling pathway, and the extracellular factor signaling pathway. Compared with the model group, the Diwu Yanggan Capsule group had a significant increase in the promoter methylation level of the ADCK2 gene (P<0.05) and significant reductions in the promoter methylation levels of the ERLIN2 and TPH2 genes (all P<0.05). Compared with the model group, the Diwu Yanggan Capsule group had significant reductions in the mRNA expression levels and the protein expression levels of the ADCK2 (all P<0.05). ConclusionAbnormal DNA methylation in liver tissue participates in the development and progression of liver cancer. The effect of Diwu Yanggan Capsule on DNA methylation level is an important epigenetic mechanism for its effect in the prevention and treatment of liver cancer.
2.Nutritional evaluation of yeast protein

Objective To evaluate the nutritional value of yeast protein, and to provide laboratory evidence for its application in the field of food and health food. Methods The amino acids content of yeast protein, soybean protein, whey protein and mixed protein was detected. Five nutritional evaluation methods, including amino acid score, ratio of total essential amino acid, ratio coefficient of amino acid, Pearson's correlation coefficient, and equally weighted gray correlation analysis were used to evaluate the protein quality. The protein efficacy test of the four proteins was performed in Sprague-Dawley rats to compare their nutritional value in vivo. Results The results showed that yeast protein was rich in essential amino acids, accounting for 47.58% of the total amino acids, and the ratio of essential amino acids to non-essential amino acids reached 0.91. Five nutritional evaluation methods all revealed that the yeast protein was an excellent protein in quality. The results of the protein efficacy test showed that yeast protein was able to promote the growth and development of the animals, and keep them in good nutritional condition, which was similar to whey protein, soy protein, and casein. The mixed protein had the highest corrected efficiency ratio among all the proteins tested in this study, possibly because the mixed ingredients made the amino acids complementary and thus had higher nutritional value. Conclusion Yeast protein can be used as a useful supplement to plant-derived and animal-derived protein products.
3.Influence of montmorillonite powder combined with racecadotril granules on serum IL-2,IL-6,IL-10 and hs-CRP levels in children with rotavirus enteritis

Objective To investigate the effect of montmorillonite powder combined with racecadotril granules on serum levels of interleukin-2 (IL-2),interleukin-6 (IL-6),interleukin-10 (IL-10) and high-sensitivity C-reactive protein (hs-CRP) in children with rotavirus enteritis.Methods From June 2014 to July 2016,76 children with rotavirus enteritis in the People's Hospital of Pujiang County were selected and randomly divided into control group and study group according to the digital table,with 38 cases in each group.The two groups were given basic treatment after admission,on the basis,the control group was treated with montmorillonite powder,and the study group was treated with montmorillonite powder combined with racecadotril granules.The two groups were treated for 3 days.The clinical efficacy,clinical symptoms (stool,stool frequency,dehydration,vomiting,fever) recovery time,serum IL-2,IL-6,IL-10 and hs-CRP levels at admission and after treatment,and the incidence of adverse reactions were statistically compared between the two groups.Results The clinical efficacy of the study group (mild constipation,rash,nausea) was better than the control group[19 cases(50.00%) vs.11 cases(28.95%),16 cases(42.11%)vs.15 cases(39.47%),3 cases(7.89%) vs.12 cases(31.58%)],the difference was statistically significant(Z =2.358,P < 0.05).The recovery time of stool character[(45.58 ± 10.11) h],stool frequency [(44.96 ± 10.03) h],dehydration[(33.15 ± 10.27)h],vomit[(18.16 ±6.46)h],and fever[(25.51 ± 10.32)h]of the study group were less than those of the control group(t =9.618,8.344,8.557,3.921,9.487,all P < 0.05).The serum levels of IL-2,IL-6,IL-10 and hs-CRP before treatment between the two groups had no statistically significant differences (all P > 0.05).After treatment,the serum levels of IL-2 and IL-10 in the study group were higher than those in the control group,the serum levels of IL-6 and hs-CRP were lower than those in the control group,the differences were statistically significant(t =5.545,7.705,7.090,10.352,all P < 0.05).The incidence rate of adverse reactions between the study group and the control group had no statistically significant difference(10.52% vs.13.15%,x2 =0.000,P > 0.05).Conclusion Montmorillonite powder combined with racecadotril granules in the treatment of rotavirus enteritis can effectively relieve the clinical symptoms,improve the serum inflammatory factors level,reduce the inflammatory reaction in vivo,improve the therapeutic effect and has high safety.
4.Microvascular decompression and percutaneous balloon compression for treatment of primary trigeminal neuralgia-a clinical prospective cohort study

Objective To compare clinical outcome of microvascular decompression (MVD) and percutaneous balloon compression (PBC) by using a prospective cohort study in order to provide a reliable evidence for the clinical decision-making. Methods Patients with trigeminal neuralgia hospitalized at Hangzhou First People′s Hospital in 2010 were chosen as database for cohort study. The patients were divided into MVD group (30 cases) and PBC group (30 cases). The clinical efficacy was followed by independent observers for 36 months after surgery. Chi-square test for hierarchical data, t test for quantitative data, and Kaplan-Meier plot for clinical outcomes were applied in the research. The endpoint was follow-up accomplishment or severe occurrence. Results Sixty patients were included in the research till the endpoint. The general records before surgery were almost the same with the literature records. By comparing painless period, mild and severe relapse, MVD group was superior to PBC group (P < 0.05). As for the painless survival period, MVD group was 96.7% of pain free after 1 year, 93.3% after 3 years, while PBC group was 90.0% after 1 year and 83.3% after 3 years. Regarding 3 years of follow-up, the relapse seemed occurred after 1 year in both groups. Conclusions As a curative and nondestructive procedure , MVD is more effective and has longer lasting pain free period , which should be considered as the first choice of treatment for trigeminal neuralgia in healthy people.
5.Treatment of refractory trigeminal neuralgia by micro-balloon compression trigeminal ganglion

Objective To evaluate the clinical effects of percutaneous micro-balloon compression (PBC) trigeminal ganglion for the treatment refractory trigeminal neuralgia. Methods Surgical results of 452 patients with trigeminal neuralgia treated by PBC from October 2009 to May 2013 were analyzed retrospectively. 125 cases aged over 80 years old and 70 cases′ pain belongs to the first branch neuralgia. Such procedures as Meckle′s cave cannulated with No.4 Forgarty catheter and the balloon inflated and compressed the gasserian ganglion monitored by X-ray were observed by PBC. Follow-up interview and curative effects were recorded. Results The average hospitalization was 6.1 days. Among them, 432(95.6%) cases had immediate relief from pain. The overall pain relief rate was 97.8% in our group without serious surgical complications. Postoperative complications include hemifacial numbness in 385 patients (85.2%), mild masseter muscle weakness in 248 patients (54.9%), diplopia in 2 patients. All symptoms relived or disappeared within 1 ~ 6 months. The average follow-up intervier period is 23.5 months. The recurrence rate is 10.2% (46 cases). Conclusion PBC is a safe and effective method with high pain relief rate in the treatment of refractory trigeminal neuralgia , especially for the treatment of the high risk patients , patients with recurrent symptoms or the patients suffered from the first branch neuralgia.
6.Application of discriminant analysis in the screening of cytomegalovirus pneumonia

Objective To look for a reliable and convenient judgement criteria for the screening of cytomegalovirus pneumonia in order to reduce misdiagnosis and resulted mistherapy.Methods Process collected data on fifty-six cytomegalovirus pneumonia and forty-two common viruses induced asthmatic bronchitis cases by use of discriminant analysis to construct prediction model of diagnosis result.Results Only three indexes including age,lymph count and platelet count were selected into the model via sift.The performance of the established screening model showed as follows:sensitivity was 80.36%,specificity was 80.95%,misdiagnosis rate was 19.05%,false negative rate was 19.64%,diagnostic accordance rate was 80.61%.Conclusion Being concise and of strong maneuverability and high accuracy in prediction,cytomegalovirus pneumonia diagnosis model constructed through discriminant analysis can provide powerful screening means for medical staff.
7.Effect of Transcranial Magnetic Stimulation on the Expression of c-Fos and Brain-derived Neurotrophic Factor of the Cerebral Cortex in Rats with Cerebral Infarct

The effect of transcranial magnetic stimulation (TMS) on the neurological functional recovery and expression of c-Fos and brain-derived neurotrophic factor (BDNF) of the cerebral cortex in rats with cerebral infarction was investigated. Cerebral infarction models were established by using left middle cerebral artery occlusion (MCAO) and were randomly divided into a model group (n=40) and a TMS group (n=40). TMS treatment (2 times per day, 30 pulses per time) with a frequency of 0.5 Hz and magnetic field intensity of 1.33 Tesla was carried out in TMS group after MCAO. Modified neurological severity score (NSS) were recorded before and 1, 7, 14, 21, and 28 day(s) after MCAO. The expression of c-Fos and BDNF was immunohistochemically detected 1, 7,14, 21, and 28 day(s) after infarction respectively. Our results showed that a significant recovery of NSS (P<0.05) was found in animals treated by TMS on day 7, 14, 21, and 28 as compared with the animals in the model group. The positive expression of c-Fos and BDNF was detected in the cortex surrounding the infarction areas, while the expression of c-Fos and BDNF increased significantly in TMS treatment group in comparison with those in model group 7, 14, 21, and 28 days (P<0.05) and 7,14, 21 days (P<0.01) after infarction, respectively. It is concluded that TMS has therapeutic effect on cerebral infarction and this may have something to do with TMS's ability to promote the expression of c-Fos and BDNF of the cerebral cortex in rats with cerebral infarction.
