1.Application of fluorescence-guided laparoscopy in radical resection of hepatocellular carcinoma

Objective To investigate the clinical application value of fluorescence laparoscopy in radical resection of hepatocellular carcinoma (HCC).Methods Data of totally 113 patients with HCC in Henan Provincial People's Hospital between June 2016 to June 2018 were retrospectively analyzed.Among the 113 patients,46 patients underwent laparoscopic hepatectomy (LLR),and 67 patients underwent fluorescence guided laparoscopic hepatectomy (FLLR).Results No significant differences were observed between LLR group and FLLR group in terms of age,male proportion,liver function classification,surgical resection methods,and operation time (P>0.05).The positive ratio of specimen surgical margin in LLR group was significantly higher than that in FLLR group,13.0% vs.3.0%,and the difference was statistically significant (P<0.05).In the FLLR group,22 patients received fluorescence guided anatomic hepatectomy with indocyanine green (ICG),10 with positive staining and 12 with negative staining,and fluorescence imaging was observed 2 minutes after ICG injection.There was no significant difference in operation time,hospitalization cost and length of stay between positive and negative staining (P> 0.05).Conclusion Fluorescence laparoscopy has certain advantages in hepatectomy,and can display the boundary of hepatocellular carcinoma in real time to ensure the safe margin of tumor resection.
2.Analysis of endoscopic features for colorectal sessile serrated adenoma/polyp

Objective To evaluate the endoscopic features of colorectal sessile serrated adenoma/polyp(SSA/P). Methods The data of 109 cases of SSA/P and 218 cases of polyps randomly selected in Xijing Hospital from January 2014 to December 2016 were collected. The endoscopic features of SSA/P and polyps were compared, and the risk factors of occurrence and cancerization of SSA/P were analyzed. Results The mean age of patients in the SSA/P group was older than that of polyps group(P=0.011).The distribution of lesions was no significant difference between the two groups(P=0.092). The gross type of SSA/P was mainly type Ⅰ and Ⅱ, while polyps were more in type Ⅰ(P=0.036). According to the pit pattern of Kudo,type Ⅱ was more in SSA/P,but types Ⅰ and Ⅱ were more in polyps(P=0.004). For capillary pattern comparison,type Ⅱ was more in SSA/P,but type Ⅰ was more in polyps(P≤0.000 1). For surface morphological features comparison,the SSA/P group was more likely to be observed the mucous cap(P=0.002)and blood vessel thickening(P=0. 004). On pathologic diagnosis, the SSA/P group was more susceptible to atypia and carcinogenesis(P = 0.001). Higher microvascular morphological classification,being mucous cap,and blood vessel thickening were risk factors of atypia and carcinogenesis of SSA/P. Conclusion There were significant differences between SSA/P and polyps on lesion location,pit pattern,capillary pattern,surface structure characteristics,and risk factors of atypia and carcinogenesis.
3.Comparison of endoscopic submucosal tunnel dissection with endoscopic submucosal dissection for esophageal superficial neoplasms: a retrospective study

Objective To compare the safety and efficacy of endoscopic submucosal tunnel dissection (ESTD) and endoscopic submucosal dissection ( ESD) for treatment of esophageal superficial neoplasms. Methods A retrospective study was performed on data of patients with esophageal superficial neoplasms who were treated by conventional ESD or ESTD at Xijing Hospital of Digestive Diseases between January 2014 and December 2016. The procedure time, pathology, rate of en bloc resection and curative resection, and adverse events were compared between the two groups. Results A total of 113 consecutive patients were collected, including 49 undergoing ESTD and 64 undergoing ESD. ESTD had a shorter procedure time than ESD method [38. 0 min(21. 4-71. 0 min) VS 46. 5 min(32. 5-117. 5 min), P=0. 008],and the dissection speed of ESTD was faster than that of ESD[0. 42 cm2/min(0. 22-0. 59 cm2/min) VS 0. 34 cm2/min(0. 20-0. 42 cm2/min), P=0. 000]. There were no statistical differences in the en bloc resection rate ( 100. 0% VS 100. 0%, P=1. 000) or the curative resection rate (98. 0% VS 93. 8%, P=0. 386).There were no statistical differences on adverse event rates including post-procedure bleeding, perforation, fever, and thoracalgia.ESTD group showed a lower rate of muscular injury (20. 4% VS 39. 1%, P=0. 041) and intra-procedure bleeding (18. 4% VS 37. 5%, P=0. 036). A multivariate regression analysis for procedure time showed that ESTD method ( OR= 2. 801, 95%CI: 1. 116-7. 031, P=0. 028) and lesion area <9 cm2(OR=5. 049, 95%CI: 2. 088-12. 208, P=0. 000) were associated with a shorter procedure time.Conclusion ESTD is safe and effective for treatment of esophageal superficial neoplasms. It can shorten operative time, improve dissection speed, and reduce intra-procedure muscular layer injury and bleeding.
4.Risk factors influencing operating efficiency of endoscopic submucosal dissection for gastric mucosal lesions

Objective To explore risk factors of influencing operating efficiency of endoscopic submucosal dissection (ESD) for gastric mucosal lesions. Methods The data of 304 cases with gastric mucosal lesion undergoing ESD in Xijing Hospital of Digestive Disease from April 2009 to February 2017 were retrospectively analyzed. The procedure time and complete resection rate ( R0 resection rate ) were regarded as indicators of ESD efficiency. Risk factors influencing procedure time and R0 resection rate were analyzed using Chi-square test and logistic regression. Results Using median procedure time of 45 min as the cutoff value, Chi-square test showed that specimen size ( P=0. 000) , lesion location ( P=0. 001) , and pathological type ( P=0. 003) affected the operation time. Further logistic regression analysis indicated that specimen size (≥40 mm/<40 mm, P<0. 001, OR=3. 748, 95%CI: 2. 247-6. 254) and lesion location (upper or middle 1/3 of stomach/lower 1/3 of stomach, P=0. 001, OR=2. 180, 95%CI: 1. 318-3. 606) were independent risk factors of procedure time. Using R0 resection as outcome measure, neither single nor multiple parameter analysis was statistically significant. Conclusion Specimen size and lesion location are independent risk factors influencing efficiency of gastric mucosal ESD, and could be possibly used to estimate the procedure time of ESD.
5.Surgical treatment of hepatic hemangiomas: a study of 908 patients

Objective To study the surgical treatment of hepatic hemangiomas and the timing of surgery.Methods A retrospective study was conducted on 908 patients with giant hemangiomas who underwent surgery between December 1997 and December 2017.The clinical data,surgical indications,surgical outcomes,lesion size,and the effect of TAE were compared.Results The diameter (mean + /-S.D.)of the resected hepatic cavernous hemangiomas was (11.1 ± 6.2) cm (the longest diameter was 60 cm).585 patients (64.4%) underwent enucleation of hepatic hemangiomas and 323 patients (35.6%) underwent anatomical hepatectomy.Six patients died perioperatively (mortality rate 0.7%).The incidence of severe complication (Clavien-Dindo grade 3 ~ 5) was 3.8%.The incidence of severe postoperative complication for enucleation (2.7%) was significantly less than anatomic liver resection (5.6%,P <0.05).When the lesion was more than 20 cm,the complication and mortality rates were significantly higher than those less than 20 cm (P < 0.05).The complication and mortality rates in patients who underwent TAE before surgery were significantly higher than those without TAE (P < 0.05).Conclusions Surgical enucleation of hemangiomas was superior to anatomical hepatectomy.With increase in tumor size,the risk of surgery increased.Surgical treatment was safe and effective for giant hepatic hemangiomas.For giant hepatic hemangiomas with significant increase in size,prompt surgical treatment is recommended.

Professor WEI Pin-kang has engaged in prevention and treatment of gastrointestinal tumors through intrgrated traditional Chinese and Western medicine for more than forty years. According to the characteristics of the gastric cancer development and using the philosophy thinking of TCM comparative states, combined with the ancient theory of phlegm syndrome in TCM, Professor WEI Pin-kang put foward his original theory that is the concept of phlegm differentiation of gastric cancer theory, and built the theory of the stomach sputum pollution, which can renovate the body's environment, eradicating the cause and improving the quality of life for cancer patients.
7.The cognitive function of patients with early Parkinson's disease: a five-year follow-up

Objective To investigate the changes in cognitive function in patients with early Parkinson's disease (PD) in a 5-year follow-up.Methods A total of 181 PD and 173 normal participants were recruited between January 2009 and January 2012 at the Department of Neurology,the Second Hospital of Jiaxing City.Regression analysis was used to evaluate the risk factors of cognitive impairment,and changes in subdomains of the Montreal Cognitive Assessment (MoCA) were compared annually with baseline data.Results Baseline clinical data were similar between the two groups.The level of cognitive impairment was positively correlated to the age of onset and the Hamilton Anxiety Scale (H AMD) (t =3.326,P< 0.05;t =5.211,P<0.01),and negatively correlated to education level (t=-2.505,P<0.05).There were no statistical differences in the first (26.5 ± 2.6),second (26.3±3.2) and third year (25.9±2.9) in the total scores of MoCA,which significantly increased in the forth (24.4 ± 2.3,P<0.05) and fifth (24.1 ± 1.2,P<0.05) year compared with baseline levels (26.7±2.9).However,in the control group,differences between any two years in total scores were not statistically significant (all P>0.05).Seven subdomains of MoCA were attenuated,and four of them were significant different between the groups,including delayed recall,attention,abstraction task and orientation after the forth year of follow-up (all P < 0.05).Meanwhile,visuospatial execution capacity was attenuated before the third year,and then rose markedly (P<0.05).In the control group,the between-year differences of seven subdomains were not statistically significant (all P>0.05).Conclusions Cognitive function of PD patients decreases significantly in some domains,such as delayed recall,attention,abstraction task and orientation after a five-year followup.

Objective To explore the medical care and evacuation equipment at sea in China.Methods The present situation of the medical care and evacuation equipment at sea in China was discussed from the aspects of equipment system,medical service support,mechanism for utilization,management and maintenance as well as informatization.The problems were analyzed in equipment system,support ability,equipment integration and update as well as equipment performances.Results Some measures were put forward from the aspects of equipment system,support ability,equipment integration and update as well as equipment performances.Conclusion The development of medical care and evacuation equipment at sea has to take considerations on medical service requirements at sea,integrated civilian and military uses,personnel,innovation and etc.

Objective To explore the risk factors of pulmonary artery hypertension (PAH) and the its relationship with T cell subsets to provide a foundation for the prevention and treatment of PAH.Methods 154 maintained hemodialysis (MHD) patients in our dialysis center were recruited according to the criterion and divided into two groups subsequently:PAH group (pulmonary artery systolic pressure,PASP > 35 mmHg) and non-PAH group (PASP≤35 mmHg).The related clinical,biochemical and ultrasonic cardiogram data were collected and peripheral blood was acquired to detect the expressions of the surface antigen CD3,CD4,CD8 and CD69 with flow cytometry.Logistic regression analysis was used to find out the relationship between PAH and T cell subsets.Results There was no significant difference between 56 cases of PAH and 98 cases of non-PAH as regards gender,age,mean systolic and diastolic pressure,dialysis durations,morbidities of hypertension and diabetes,smoking rate,and left ventricular diameter.Compared with the non-PAH group,the PAH group demonstrated a lower percent of CD8 T cells and CD8 CD69 T cells,but a much higher left atrial diameter (LAD),Interventricular septum thickness,left ventricular posterior wall thickness,and NT-proBNP.The percentage of T cells,CD4 T cells and CD4 CD69 T cells showed no difference between the two groups.Multivariate analysis confirmed that PAH was negatively independently associated with the percentage of CD8 T cells and CD8CD69 T cells.Conclusions The decreased percentage of CD8 T cells and CD8CD69 T cells in the peripheral blood is a risk factor of PAH in maintained hemodialysis patients,and CD8 T cells may play an important role in the genesis of PAH.
10.Hardware Implementation of Numerical Simulation Function of Hodgkin-Huxley Model Neurons Action Potential Based on Field Programmable Gate Array.

Neuron is the basic unit of the biological neural system. The Hodgkin-Huxley (HH) model is one of the most realistic neuron models on the electrophysiological characteristic description of neuron. Hardware implementation of neuron could provide new research ideas to clinical treatment of spinal cord injury, bionics and artificial intelligence. Based on the HH model neuron and the DSP Builder technology, in the present study, a single HH model neuron hardware implementation was completed in Field Programmable Gate Array (FPGA). The neuron implemented in FPGA was stimulated by different types of current, the action potential response characteristics were analyzed, and the correlation coefficient between numerical simulation result and hardware implementation result were calculated. The results showed that neuronal action potential response of FPGA was highly consistent with numerical simulation result. This work lays the foundation for hardware implementation of neural network.
