1.Efficacy of laparoscopic radical cystectomy with indocyanine green fluorescence imaging versus standard lymph node dissection: a randomized comparative study

Objective: To investigate the efficacy,safety and feasibility of regional lymph node dissection in laparoscopic radical surgery for bladder cancer under the guidance of indocyanine green fluorescence imaging. Methods: A total of 30 patients with muscle invasive bladder cancer (T2/T3NxM0) who volunteered to enter the clinical trial were randomly divided into the indocyanine green imaging guided laparoscopic regional lymph node dissection group (n=15) and the standard pelvic lymph node dissection group (n=15).The number of positive lymph nodes,operation time,intraoperative bleeding volume,incidence of lymph leakage,and tumor recurrence and metastasis rate 2-year after surgery were collected. Results: The number of positive lymph nodes was (4.20±1.66) and (4.60±1.72) respectively in the indocyanine green and standard groups,with no statistically significant difference (P>0.05).There were no statistically significant difference in the tumor recurrence and metastasis rates 2-year after surgery between the two groups (P>0.05).However,the indocyanine green group had shorter operation time,less intraoperative bleeding volume,and lower incidence of lymphatic leakage than the standard group (P<0.05). Conclusion: Indocyanine green fluorescence imaging guided laparoscopic lymph node dissection has comparable clinical efficacy to standard lymph node dissection,but with fewer complications.
2.Engineered Escherichia coli Nissle 1917 targeted delivery of extracellular PD-L1-mFc fragment for treating inflammatory bowel disease.

Inflammatory bowel disease (IBD) is an autoimmune disorder involving complex immune regulation, where balancing localized and systemic immunosuppression is a key challenge. This study aimed to enhance the therapeutic efficacy by engineering the probiotic Escherichia coli Nissle 1917 (EcN). We removed endogenous plasmids pMUT1 and pMUT2 from wild-type EcN and expressed the mPD-L1 (19‒238 aa)-mFc fusion protein on the bacterial surface using a cytolysin A (ClyA) fragment. This modification stabilized mPD-L1 (19‒238 aa) protein expression and promoted its recruitment to outer membrane vesicles (OMVs). The engineered strain, EcNΔpMUT1/2-ClyA-mPD-L1-mFc (EcN-ePD-L1-mFc), features conditional ePD-L1-mFc expression under the araBAD promoter, enhancing gut-targeted release and reducing systemic side effects. This strain improved treatment targeting and efficiency by enabling direct ePD-L1-mFc interaction with immune cells at inflammation sites. OMVs from this strain induced Treg proliferation, inhibited effector T cell proliferation in vitro, and significantly improved intestinal inflammation and colonic epithelial barrier repair in vivo. Additionally, the bacterium restored intestinal microbiota balance, increasing Lactobacillaceae and reducing Bacteroides. This study highlights the engineered bacterium's potential for targeted intestinal immune modulation and offers a novel local IBD treatment approach with promising clinical prospects.
3.Application of indocyanine green fluorescence imaging in the precise dissection of lymph nodes during laparoscopic radical resection of bladder cancer

Objective To investigate the efficacy,safety and feasibility of laparoscopic lymph node dissection guided by indocyanine green(ICG)fluorescence imaging.Methods A total of 30 patients with muscle-invasive bladder cancer(MIBC,T2/T3NxM0)who were admitled to the Cangzhou People's Hospital during Mar.2018 and Jun.2022 were included.The lymph nodes were cleared with ICG fluorescence imaging first,and then the remaining lymph nodes were cleared according to the standard pelvic lymph node range.The lymph node positive rate of ICG fluorescence imaging guided laparoscopic lymph node precise dissection and standard pelvic lymph node dissection was analyzed by self-comparison.Results ICG fluorescence imaging guided laparoscopic lymph node precise dissection needed shorter operation time than standard pelvic lymph node dissection[(21.80±6.80)min vs.(47.70±10.73)min,P<0.05].There was no significant difference in the lymph node positive rate between the two approaches[(11.34±9.06)%vs.(12.36±9.43)%,P>0.05],but the former approach caused less damage on blood vessels and nerves.Conclusion Indocyanine green fluorescence imaging guided laparoscopic lymph node dissection has comparable efficacy to standard pelvic lymph node dissection,which can reduce ineffective lymph node dissection,shorten the operation time,and reduce the risk of complications.
4.Optimization of preparation method of rectovaginal fistula animal model based on principle of magnetic compression technique

Objective To investigate the feasibility of the optimally designed"I"shaped structure mag-net based on the principle of magnetic compression technique for the preparation of rectovaginal fistula animal model.Methods Using 10 New Zealand female rabbits as the model animals,two self-designed magnets were inserted through the vagina and anus respectively after anesthesia,and the two magnets were adjusted to the appropriate position and made them attraction each other to form a magnet-rectovaginal partition-magnet structure.When the compression tissue between the magnets became ischemic necrosis and fell off,the two magnets formed the"1"shape structure and were located in the stoma of rectovaginal fistula to prevent the stoma from becoming smaller or even closing itself.The operation time and rectovaginal fistula formation time were recorded.The experimental rabbits were killed in postoperative 2 weeks,and the rectovaginal fistula specimens were obtained.The formation of fistula orifice was observed and the size of fistula orifice was meas-ured.Results The animal model of rectovaginal fistula was successfully prepared in all 10 experimental rab-bits.The procedure of intraoperative magnet placement was smooth and the operation time was(1.55±0.65)min.The experimental animals were generally in good condition after surgery,and the fistula orifice was formed on postoperative(4.80±0.75)d.After taking the gross specimen of rectovaginal septum in postopera-tive 2 weeks,the magnet was removed.The fistula orifice of rectovaginal fistula was visible with the diameter of(5.86±0.38)mm.Conclusion The"I"shaped structure magnet designed based on the principle of mag-netic compression technique could be used in the preparation of the rectovaginal fistula animal model.Its oper-ation is simple with high success rate of model preparation and good uniformity in fistula orifice.
5.The effect of ultrasound-guided thoracic paravertebral nerve block on anesthesia and stress response in middle-aged and elderly patients undergoing radical surgery for lung cancer

Objective:To investigate the effect of ultrasound-guided thoracic paravertebral nerve block (TPNB) on anesthesia efficacy and stress response in middle-aged and elderly patients undergoing radical surgery for lung cancer.Methods:A total of 104 lung cancer patients who underwent thoracoscopic radical resection for lung cancer at Qingdao Traditional Chinese Medicine Hospital from November 2020 to May 2023 were selected and randomly divided into an observation group (52 cases) and a control group (52 cases) using a random number table method. The observation group received general anesthesia combined with ultrasound-guided TPNB anesthesia during surgery, while the control group received general anesthesia. Two groups of patients were compared in terms of surgical time, intraoperative blood loss, remifentanil and propofol dosage, fluctuation trends of mean arterial pressure (MAP) and heart rate (HR), pain levels in resting and coughing states after surgery, differences in serum stress response indicators [fasting plasma glucose (FPG), cortisol (Cor), norepinephrine (NE), epinephrine (E)] before and after surgery, and adverse reactions.Results:There was no statistically significant difference in surgical time, intraoperative blood loss, and single lung ventilation time between the observation group and the control group (all P>0.05). The usage of remifentanil and propofol in the observation group was lower than that in the control group, and the difference was statistically significant (all P<0.05). The MAP fluctuation changes at four time points before anesthesia induction, 30 minutes during surgery, 90 minutes during surgery, and after surgery in the observation group were smaller than those in the control group. Among them, the MAP at 30 minutes during surgery and 90 minutes during surgery was higher than that in the control group (all P<0.05); There was no statistically significant difference in HR fluctuations between the observation group and the control group at the four time points (all P>0.05). The Visual Analog Scale (VAS) scores of the observation group patients in resting and coughing states at 12 and 24 hours after surgery were lower than those of the control group at the same time point (all P<0.05). The serum Cor and E levels in the observation group were lower than those in the control group at 12 hours after surgery, and the serum NE levels were lower than those in the control group at 12 and 24 hours after surgery (all P<0.05). The incidence of adverse reactions caused by anesthesia in the observation group was lower than that in the control group ( P<0.05). Conclusions:The use of ultrasound-guided TPNB combined with general anesthesia in patients undergoing radical surgery for lung cancer is beneficial in reducing intraoperative blood pressure fluctuations, alleviating postoperative pain and stress response, and reducing adverse reactions caused by anesthesia.

Objective To develop an expert consensus on subcutaneous injection for allergen-specific immunotherapy.Methods Relevant domestic and intemational literature was reviewed,and nursing experts who had experiences in subcutaneous injection of allergen-specific immunotherapy were interviewed to form the initial draft of the consensus.A total of 85 experts from 42 hospitals nationwide were invited to participate in discussions.2 rounds of expert consultations,adjustments,revisions,and improvements were made to the initial draft,and an online meeting was held to form the final version of the consensus.The content approved by more than 75%of the expert group is adopted,or it will be discussed or deleted.Results The expert consensus includes operational standards for subcutaneous injection of allergen-specific immunotherapy,identification and management of adverse reactions,and health education.Conclusion The consensus demonstrates strong scientific rigor and practicality,providing guidance for nursing practices in the field of clinical allergology.
7.Experiences and implications of promoting healthy eating in school aged children by World Health Organization

Abstract
In order to promote the high quality development of school meals, the article systematically analyzes the Policy Report on Promoting Healthy Meals in Schools issued by World Health Organization (WHO), introduces ways of changing the physical environment of meals and adjusting the provision of dietary information, clarifies the five steps of basic research-goals setting-action plans-implementation-impact evaluation, and points out that the development of school meals in China is facing the realistic difficulties such as unreasonable school nutrition structure, unbalanced nutrition supply, low nutritional awareness of students and serious double malnutrition burden. The paper puts forward the enlightenment of reasonable use of nudge to help students develop scientific eating habits, strengthen school health education, and reduce students double burden of malnutrition.
8.Prevention and treatment of high-risk human papilloma virus infection from the perspective of "prevention treatment of disease" based on the theory of "circular flow of Qi"

The occurrence of cervical cancer in women is closely related to high-risk HPV infection, and timely and effective interruption of high-risk HPV infection is of great significance to prevent the occurrence of cervical cancer. Huang Yuanyou's theory of "circular flow of Qi" emphasizes on the harmonization of the overall Qi flow, which can explain the physiopathology of women. The occurrence of high-risk HPV infection is related to the loss of spleen and earth transportation in the middle Jiao, poor circulation of Qi, and the low resistance of the body to evil, resulting in the malfunctioning of clear and turbid. Based on the theory of "circular flow of Qi" combined with the idea of "prevention treatment of disease", the author proposes to "prevent the disease before it occurs, regulate the middle earth to preserve the correct Qi" and "prevent the disease before it occurs". The principles of prevention and treatment of high-risk HPV infection are "prevent before the disease, regulate the middle earth to preserve the righteousness", "promote and descend to dispel the poisonous evil", and "prevent recurrence after the disease, balance yin and yang and harmonize Qi and blood", in order to provide reference for clinical treatment.
9.Nursing cooperation for single-port laparoscopic cholecystectomy based on magnetic anchor technique

Objective To explore the key nursing cooperation points in single-port laparoscopic cholecystectomy based on magnetic anchor technique. Methods The general information of 24 patients with transumbilical single-port laparoscopic cholecystectomy based on magnetic anchor technique was analyzed. Combined with the surgical procedure, the key nursing cooperation points of this innovative surgery were analyzed from the perspective of operating room nurses. Results By learning the principles of magnetic anchor technique before surgery, understanding the usage and precautions of the magnetic anchor device, accurately passing the magnetic anchor device during surgery, and avoiding mutual interference between the magnetic anchor device and conventional surgical instruments, the operating room nurses successfully assisted the surgeons in completing 24 cases of transumbilical single-port laparoscopic cholecystectomy based on magnetic anchor technique. Conclusion Real-time understanding of the surgeon's operation progress during surgery, accurate delivery of instruments, and avoidance of interference between the magnetic anchor device and conventional surgical instruments are important factors in the nursing cooperation of this surgical procedure.
10.Aurantii Fructus Immaturus, Atractylodis Macrocephalae Rhizoma, and Their Combination Treat Colonic Motility Disorders in Rats with Slow Transit Constipation via PINK1/Parkin Signaling Pathway-mediated Mitophagy

ObjectiveTo observe the effects of Aurantii Fructus Immaturus, Atractylodis Macrocephalae Rhizoma, and their combination on slow transit constipation via PTEN-induced putative kinase 1 (PINK1)/Parkin pathway-mediated mitophagy. MethodFifty-six male SD rats were randomly assigned into normal group, model group, natural recovery group, Aurantii Fructus Immaturus group, Atractylodis Macrocephalae Rhizoma group, Aurantii Fructus Immaturus combined with Atractylodis Macrocephalae Rhizoma group, and mosapride group, with 8 rats in each group. Slow transit constipation model was established by gavage with loperamide (3 mg·kg-1·d-1) for 14 days in other groups except the normal group. After successful modeling, except that the model group was continuously induced by loperamide, the normal group and the natural recovery group were administrated with 0.9% normal saline by gavage, and the rats in the Aurantii Fructus Immaturus (1.35 g·kg-1·d-1) group, the Atractylodis Macrocephalae Rhizoma (2.7 g·kg-1·d-1) group, the Aurantii Fructus Immaturus combined with Atractylodis Macrocephalae Rhizoma (4.05 g·kg-1·d-1) group, and the mosapride (1.56 mg·kg-1·d-1) group were administrated with corresponding drugs by gavage for 7 days. The amount of feces, fecal water content, and intestinal propulsion rate of rats were determined. The pathological changes of the colon were evaluated by hematoxylin-eosin (HE) staining and Alcian blue-periodic acid-Schiff (AB-PAS) staining. The activity of respiratory chain complex and the ultrastructure of the colon tissue were determined by ultraviolet spectrophotometry and observed by transmission electron microscopy, respectively. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) was employed to determine the mRNA levels of PINK1, Parkin, and p62, and Western blot to determine the protein levels of microtubule-associated protein 1 light chain 3 (LC3), PINK1, and Parkin. ResultCompared with the normal group, the model group and the natural recovery group showed decreases in the amount of feces, fecal water content, intestinal propulsion rate (P<0.05,P<0.01), and activities of mitochondrial respiratory chain complexes Ⅱ, Ⅲ, and Ⅳ in the colon tissue (P<0.05,P<0.01). Further, the mRNA levels of PINK1 and Parkin and the protein levels of PINK1, Parkin, and LC3 were up-regulated (P<0.01) and the mRNA level of p62 was down-regulated in the model group (P<0.05) and the natural recovery group. Compared with the model group and the natural recovery group, the Aurantii Fructus Immaturus combined with Atractylodis Macrocephalae Rhizoma group showed increased amount of feces, fecal water content, intestinal propulsion rate, and activities of mitochondrial respiratory chain complexes Ⅱ, Ⅲ, and Ⅳ (P<0.05,P<0.01). Moreover, the combination meliorated the degree of mitochondrial swelling in the colon tissue, down-regulated the mRNA levels of PINK1 and Parkin and the protein levels of PINK1, Parkin, and LC3 (P<0.05,P<0.01), and up-regulated the mRNA level of p62 (P<0.05). ConclusionAurantii Fructus Immaturus and Atractylodis Macrocephalae Rhizoma, and their combination may remedy the colonic motility disorders in rats with slow transit constipation by blocking PINK1/Parkin signaling pathway to inhibit the excessive mitophagy in interstitial cells of Cajal in the colon tissue.
