1.Predictive value of reverse shock index multiplied by Glasgow coma scale score for mortality of trauma patients: a Meta analysis

Objective:To systematically evaluate the predictive value of the reverse shock index multiplied by the Glasgow coma scale score (rSIG) for mortality of trauma patients.Methods:A comprehensive literature search was conducted to identify studies on the predictive value of rSIG for mortality of trauma patients in the following databases from inception to April 2025, including CNKI, Wanfang Data, SinoMed, PubMed, Cochrane Library, Web of Science, and Embase. Two investigators independently screened the literature, extracted data, and assessed study quality according to predefined inclusion and exclusion criteria. The Quality assessment of diagnostic accuracy studies-2 (QUADAS-2) tool was used to evaluate the risk of bias in the included studies. Meta analysis was performed using Stata 17.0 software with a bivariate mixed-effects model. The following metrics were used to assess the predictive value of rSIG for mortality in trauma patients, including sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), and area under the summary receiver operating characteristic (SROC) curve (AUC). The influence of various factors on the predictive performance of rSIG was examined, including injury type, study design, region, sample size, cut-off value, rSIG measurement time, and outcome measures. Additionally, sensitivity analysis, Fagan′s nomogram, and Deeks′ funnel plot were employed to assess the robustness of the findings, clinical applicability, and publication bias.Results:A total of 15 studies involving 710 612 trauma patients were included, 26 105 of whom were deceased. Meta analysis results showed that rSIG had a pooled sensitivity of 0.78(95% CI 0.71, 0.84), a pooled specificity of 0.78(95% CI 0.68, 0.86), a pooled PLR of 3.60(95% CI 2.46, 5.27), a pooled NLR of 0.28(95% CI 0.22, 0.36), a pooled DOR of 12.70(95% CI 8.10, 19.91), and an AUC of 0.85(95% CI 0.81, 0.87) for predicting mortality of trauma patients. Subgroup analysis identified injury type as one of the major sources of heterogeneity, and the predictive specificity of rSIG was significantly higher in patients with multiple trauma (0.82) than in those with isolated traumatic brain injury (0.65) ( P<0.05). Sensitivity analysis indicated that the findings were robust and stable. Fagan′s nomogram showed that when the pre-test probability was 7%, the post-test probability of death increased to 21% in patients with low rSIG and decreased to 2% in those with high rSIG. Deeks′ funnel plots suggested no significant publication bias among the included studies ( P>0.05). Conclusion:Low rSIG has good predictive performance for mortality of trauma patients and can serve as an effective tool for early and rapid prognosis assessment with superior predictive performance in patients with multiple trauma compared to those with traumatic brain injury.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
3.Nursing care of a parturient with broad ligament hematoma complicated with hemorrhagic shock in vaginal delivery

To summarize the nursing experience of a case involving hemorrhagic shock caused by broad ligament hematoma in vaginal delivery after cesarean section.Nursing points include:recognition and first aid for occult postpartum haemorrhage;precise management of blood transfusion and volume resuscitation;objective-oriented temperature management strategy;postoperative observation and care for abdominal packing;provision of breastfeeding support in case of separation of mother and child;comprehensive psychological nursing support throughout all stages.Following active treatment and nursing interventions,the patient was discharged 10 d after surgery.
4.Mechanism of trazodone against endometrial carcinoma in vitro

Objective:To investigate the effects of trazodone on the growth, motility and ferroptosis of endometrial carcinoma cells, and to study its effect on phosphoinositide 3-kinase (PI3K)/protein kinase B (Akt)/ mechanistic target of rapamycin (mTOR) signaling pathway.Methods:Human endometrial carcinoma HEC-1A cells were divided into a control group and an experimental group. HEC-1A cells in the control group and in the experimental group were treated with 0 and 2 μmol/L trazodone dimethyl sulfoxide solution, respectively, for 24 h. Cell growth was evaluated by cell counting kit-8 and colony formation assay, cell motility was evaluated by scratch assay, Transwell assay and Western blotting, ferroptosis was evaluated by Western blotting and iron detection kit, and the effect of trazodone on PI3K/Akt/mTOR signaling pathway was evaluated by Western blotting. Analysis and comparisons were made using one-way analysis of variance and Tukey′s multiple comparisons.Results:The cell survival rate [(32.2±3.2%)] and the number of cell clones (18.0±4.0) in the experimental group were lower than those in the control group [(99.2±4.3)% and 35.0±5.0], and the differences were statistically significant (both P<0.01). The relative scratch width of the experimental group (0.57±0.07) was higher than that of the control group (0.24±0.05), and the difference was statistically significant ( P<0.01). The number of invasive cells in the experimental group (7.0±1.0) was lower than that in the control group (15.0±2.0), the difference was statistically significant ( P<0.01). The relative expression levels of matrix metalloproteinase-9 (0.50±0.05) and matrix metalloproteinase-2 in the experimental group (0.75±0.08) were lower than those in the control group (0.82±0.07 and 1.25±0.15), and the differences were statistically significant (both P<0.01). The iron level [(190.5±18.5)%] and the relative expression level of acyl-CoA synthetase long-chain family member 4 (0.63±0.06) in the experimental group were higher than those in the control group [(99.2±8.9)% and 0.38±0.05)], and the differences were statistically significant (both P<0.05). The relative expression level of glutathione peroxidase 4 in the experimental group (0.22±0.05) was lower than that in the control group (1.22±0.13) ( P<0.01). The levels of phosphorylated PI3K/PI3K, phosphorylated Akt/Akt and phosphorylated mTOR/mTOR in the experimental group (0.62±0.08, 0.35±0.05, and 1.46±0.18) were lower than those in the control group (1.47±0.16, 1.32±0.11, and 2.34±0.11), and the differences were statistically significant (all P<0.01). Conclusions:Trazodone may have an anti-tumor effect on endometrial carcinoma cells by inhibiting the growth and motility of tumor cells, promoting ferroptosis, and inhibiting the PI3K/Akt/mTOR signaling pathway.
5.Review of the Expanded Clinical Trial System in the United States, Japan and Canada and its Enlightenment for China

The expanded clinical trial system is designed to provide clinical trial drugs for patients suffering from serious or life-threatening diseases for which there is no effective treatment, essentially seeking a balance between the accessibility of drugs to patients and the soundness of the development of the pharmaceutical industry, as well as between the risk of expanding the use of clinical trial drugs and the need to safeguard the rights and interests of patients’ lives and health. Article 23 of China's Drug Administration Law provides only a principled description of the applicable standards and operating procedures for this system, with no implementing regulations, making it difficult for the system to function effectively in practice. The United States, Japan, and Canada have made detailed provisions on the conditions of application, applicants, review subjects and contents, and safeguards of the system through laws, regulations, and guidelines. Based on China's legislative environment and regulatory reality, and considering that the system is still in the initial stage in China, it is recommended that appropriate reference be made to the beneficial experience of foreign countries and that regulations and normative documents be promulgated as soon as possible to clarify the types of expanded clinical trials, applicants, review subjects etc., and to determine the responsibilities of all the parties involved, to promote the standardization of China’s expanded clinical trial system, to promote the availability of medicines to patients, and to effectively safeguard the rights and interests of patients’ lives and health, and the soundness and safety of the development of the pharmaceutical industry.
6.Clinical characteristics and prognosis of Pseudomonas aeruginosa perito-neal dialysis-associated peritonitis

Objective To analyze the clinical characteristics and prognosis of Pseudomonas aeruginosa peritoneal dialysis-associated peritonitis(PaeP).Methods Peritoneal dialysis-associated peritonitis(PDAP)patients who were followed up in the nephrology outpatient department of a hospital from January 2019 to December 2020 were analyzed retrospectively.According to bacterial culture results,patients were divided into the PaeP group and non-PaeP group.Clinical characteristics of PaeP patients and antimicrobial susceptibility testing results of Pseudomonas aeruginosa were analyzed,clinical manifestations,laboratory test results,and prognosis of two groups of patients were compared.Results A total of 124 peritoneal dialysis patients were included in analysis,164 cases of peritoni-tis occurred,16 cases were in the PaeP group and 148 in the non-PaeP group.11 patients developed 16 episodes of Pseudomonas aeruginosa infection,accounting for 8.9%of PDAP patients.Among them,4 patients had peritoneal dialysis catheter exit-site infection,with 5 recurrence cases,1 case cured,1 case died,and 9 cases were extubated.Among the extubated patients,1 withdrew dialysis,3 were recovered to peritoneal dialysis after hemodialysis,5 changed to permanently hemodialysis,with a technical failure rate of 54.5%.Compared with the non-PaeP group,patients in the PaeP group had a shorter dialysis time(13.83±4.92 vs 38.53±35.77 months).During the infection period,C-reactive protein levels were higher(96.61±6.17 vs 45.87±44.65 mg/L),while albumin levels were lower(25.62±4.42 vs 29.46±8.25 g/L).At the onset of infection,the proportion of polymorphonuclear cells in perito-neal dialysis fluid was relatively higher.On the 5th day of treatment,the negative conversion rate of white blood cell count in peritoneal dialysis fluid was relatively low.Differences were all statistically significant(all P<0.05).The cure rate of patients in the PaeP group was lower than that in the non-PaeP group,the technical failure rate was higher than that in the non-PaeP group,both with statistically significant differences(both P<0.05).There was no statistically significant difference in the mortality between two groups of patients(P>0.05).Conclusion PaeP patients have severe clinical manifestations,poor clinical treatment prognosis,high recurrence and extubation rates.For patients with repeated episodes,resetting and replacing the tunnel after extubation is an effective means to re-duce technical failures.
7.The application of the CBL-PBL-EBM integrated teaching method in the standardized training of nephrology resident physicians

Objective To evaluate the efficacy of case-based learning(CBL)-problem-based learning(PBL)-evidence-based medicine(EBM)integrated teaching method in the standardized training of nephrology resident physicians.Methods Sixty internal medicine trainees who were on a 2-month rotation in the Department of Nephrology between February 1st,2023 and August 31st,2023,were recruited and randomized 1∶1 into the CBL-PBL-EBM group and the CBL group.The scores of the two groups in the theoretical assessment,case analysis,and general skills were compared at the end of the rotation.Results There was no significant difference between experimental group and control group(45.21±3.67 vs 44.02±4.90,P>0.05).The test scores of test groups were higher than those of control group(42.48±2.35 vs 38.45±3.24,P<0.05),and the comprehensive scores were higher than those of control group(92.24±1.69 vs 89.21±1.56,P<0.05).At the same time,the scores of the experimental group in independent learning,literature query ability,critical thinking,and induction and summary ability were al-so significantly better than those of the control group,with statistical significance(P<0.05).Conclusion The CBL-PBL-EBM teaching method is better than the CBL teaching method,which can effectively achieve the training goals and enhance the overall competence of nephrology trainees.
