1.Comprehensive reconstruction of gradeⅠ-Ⅱdigital defects with hallux osteo-onychocutaneous flap of great toe: a report of 9 cases

Objective:To observe the clinical effect of comprehensive reconstruction of grades Ⅰ-Ⅱ thumb and finger defects with hallux osteo-onychocutaneous flaps of great toe.Methods:This is a retrospective study. From June 2020 to December 2023, comprehensive reconstruction surgery for Grade Ⅰ-Ⅱ digital defect were performed with hallux osteo-onychocutaneous flaps of great toe for 3 thumbs and 7 fingers in 9 patients in the Department of Hand and Microsurgery of Baoji Third Hospital. Causes of digital injury were: 4 of machine crush, 3 of electric saw cut, 1 of door crush, and 1 of electrical burn. There were 6 grade I digital defects (beyond the nail root) and 4 grade Ⅱ defects (last segment of digit). The defects of the digits were reconstructed by taking references of the shape and structure of the contralateral normal thumbs and fingers. Then the hallux osteo-onychocutaneous flaps of great toe were designed and harvested accordingly from the left and right great toes. Donor sites were covered by skin grafting or local dressing change. One patient was treated in emergency surgery, 6 in sub-emergency surgery and 2 in elective surgery. Integrated perioperative patient management was provided to all of the patients. Postoperative follow-ups were conducted through the visit of outpatient clinic, telephone calls or WeChat interviews. Flap survival, appearance and sensation recovery were evaluated according to the Evaluation Standard of Upper Limb Functional of Hand Surgery of Chinese Medical Association.Results:Vascular insufficiency of 1 digit occurred in surgery, and relieved by local treatment with lidocaine and warm saline. All 10 digits successfully survived, and all donor sites healed spontaneously. The postoperative follow-up period was 10 to 30 months, with an average of 18 months. One transferred nail was found in poor appearance (not flat), the rest of the reconstructed digits were good in appearance and function. The nail, finger print and fine sensation (TPD 5~8 mm), as well as digital flexion, extension, grasping and opposition of the reconstructed digits were all good. According to the Evaluation Standard of Upper Limb Functional of Hand Surgery of Chinese Medical Association, at the last follow-up visit, 5 digits were in excellent, 4 in good and 1 in fair.Conclusion:The comprehensive reconstruction of grades Ⅰ-Ⅱ digital defects with hallux osteo-onychocutaneous flap of great toe is an ideal surgical technique that can reconstruct the nail, finger print and sensation of a digit, with good postoperative function as well as an aesthetic and realistic appearance.

As a potential vectored vaccine,Newcastle disease virus(NDV)has been subject to various studies for vaccine development,while relatively little research has outlined the immunomodulatory effect of the virus in antigen presentation.To elucidate the key inhibitory factor in regulating the interaction of infected dendritic cells(DCs)and T cells,DCs were pretreated with the NDV vaccine strain LaSota as an inhibitor and stimulated with lipopolysaccharide(LPS)for further detection by enzyme-linked immunosorbent assay(ELISA),flow cytometry,immunoblotting,and quantitative real-time polymerase chain reaction(qRT-PCR).The results revealed that NDV infection resulted in the inhibition of interleukin(IL)-12p40 in DCs through a p38 mitogen-activated protein kinase(MAPK)-dependent manner,thus inhibiting the synthesis of IL-12p70,leading to the reduction in T cell proliferation and the secretion of interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),and IL-6 induced by DCs.Consequently,downregulated cytokines accelerated the infection and viral transmission from DCs to T cells.Furthermore,several other strains of NDV also exhibited inhibitory activity.The current study reveals that NDV can modulate the intensity of the innate?adaptive immune cell crosstalk critically toward viral invasion improvement,highlighting a novel mechanism of virus-induced immunosuppression and providing new perspectives on the improvement of NDV-vectored vaccine.
4.Clinical effect of Kinesio Tape for treating lower limb function in children with spastic cerebral palsy accompanied by knee hyperextension

Objective To study the clinical effect of Kinesio Tape for treating the lower limb function in children with spastic cerebral palsy accompanied by knee hyperextension.Methods Sixty children with spastic cerebral palsy treated by rehabilitation therapy in this hospital from August 2017 to December 2018 were se-lected as the study subjects and divided into the experiment group and control group,30 cases in each group.The control group adopted the conventional rehabilitation therapy,while the experiment group was combined with Kinesio Tape on the basis of conventional rehabilitation therapy.The treatment course lasted for 3 months.Before and after treatment,the children patients conducted the Gross Motor Function Measure-88(GMFM-88)scoring,Modified Ashworth Scale(MAS)grading on the triceps surae muscle in the affected side,surface electromyography,measurement of dorsiflexion angle of foot in knee extension position and meas-urement of maximum knee extension angle of knee joint.Results The GMFM-88 score,triceps surae muscle MAS grade in the affected side,foot dorsiflexion angle in Knee extension position,maximum knee extension angle in the erect position in the two groups were improved compared with those before treatment(P<0.05).Furthermore,the above indicators in the experimental group were superior to those in the control group(P<0.05).The surface electromyographic value had no statistical difference between the two groups(P>0.05).No obvious adverse reactions occurred during the treatment process with good compliance.The chil-dren's parents in the experiment group filled in Kinesio Tape satisfaction questionnaire,and had 100%satis-faction.Conclusion Kinesio Tape combined with routine rehabilitation therapy could effectively improve the muscular tension,joint activity,knee excessive extension degree and exercise function in children with spastic cerebral palsy accompanied by knee hyperextension.

Background:
SM-like (LSM) genes a family of RNA-binding proteins, are involved in mRNA regulation and can function as oncogenes by altering mRNA stability. However, their roles in B-cell progression and tumorigenesis remain poorly understood.
Methods:
We analyzed gene expression profiles and overall survival data of 123 patients with mantle cell lymphoma (MCL). The LSM index was developed to assess its potential as a prognostic marker of MCL survival.
Results:
Five of the eight LSM genes were identified as potential prognostic markers for survival in MCL, with particular emphasis on the LSM.index. The expression levels of these LSM genes demonstrated their potential utility as classifiers of MCL. The LSM.index-high group exhibited both poorer survival rates and lower RNA levels than did the overall transcript profile. Notably, LSM1 and LSM8 were overexpressed in the LSM.index-high group, with LSM1 showing 2.5-fold increase (p < 0.001) and LSM8 depicting 1.8-fold increase (p < 0.01) than those in the LSM.index-low group.Furthermore, elevated LSM gene expression was associated with increased cell division and RNA splicing pathway activity.
Conclusions
The LSM.index demonstrates potential as a prognostic marker for survival in patients with MCL. Elevated expression of LSM genes, particularly LSM1 and LSM8, may be linked to poor survival outcomes through their involvement in cell division and RNA splicing pathways. These findings suggest that LSM genes may contribute to the aggressive behavior of MCL and represent potential targets for therapeutic interventions.

8.Effects of left bundle branch pacing versus right ventricular septal pacing on plasma N-terminal pro-brain natriuretic peptide levels and ventricular remodeling in patients with third-degree atrioventricular block

Objective:To compare the effects of left bundle branch pacing and right ventricular septal pacing on plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) levels and ventricular remodeling in patients with third-degree atrioventricular block.Methods:A retrospective analysis was conducted on 83 patients with third-degree atrioventricular block who received permanent dual-chamber pacemakers at Xi'an International Medical Center Hospital from January 2021 to December 2023. Participants were allocated into two groups based on the positioning of the ventricular electrode. The control group ( n = 40) underwent right ventricular septal pacing, while the observation group ( n = 40) underwent left bundle branch pacing. Pacing parameters were recorded for each group at 7 days and 6 months post-surgery. Comparisons were made between preoperative and postoperative electrocardiogram QRS waves, plasma NT-proBNP levels, and echocardiographic data, including left atrial diameter, left ventricular end-diastolic diameter, left ventricular ejection fraction (LVEF), left atrial volume index (LAVI), tricuspid annular plane systolic excursion (TAPSE), peak velocity of tricuspid regurgitation (TRPV), the ratio of early diastolic peak velocity to late diastolic peak velocity at the mitral annulus, and the ratio of early diastolic transmitral velocity to early diastolic mitral annular velocity (E/Ea). Additionally, the occurrence of complications within 6 months post-surgery was calculated for each group. Results:At 6 months post-surgery, the duration of the QRS waves and NT-proBNP levels in the control group were significantly greater than those in the observation group [(131.45 ± 18.40) ms vs. (115.32 ± 17.30) ms, (348.55 ± 48.80) ng/L vs. (318.12 ± 47.72) ng/L, t = 4.12, 2.87, both P < 0.05]. At 6 months post-surgery, the control group exhibited increased LAVI and E/Ea and decreased LVEF and TAPSE, and the LAVI and E/Ea in the control group were significantly higher than those in the observation group ( t = 2.01, 6.23, both P < 0.05). At 6 months post-surgery, LVEF and TAPSE in the control group were significantly lower than those in the control group ( t = -2.88, -3.72, both P < 0.05). During the 6-month follow-up, neither group experienced any surgery-related complications. Conclusion:Left bundle branch pacing outperforms right ventricular septal pacing in maintaining electrophysiological and mechanical synchronization, providing better preservation of cardiac structure and function, and demonstrating favorable physiological pacing characteristics.

10.The norm of the Physical Activity afterschool Questionnaire for Preschooler (P-PAQ) in urban areas of China

Objective:
To establish the norm of the Physical Activity afterschool Questionnaire for Preschooler(P-PAQ) in urban areas of China, so as to provide a basis for graded guidance from the family perspective and to improve children s physical activity levels.
Methods:
From October 2020 to January 2021, 6 267 children aged 3-6 years old were recruited from 40 kindergartens in eight cities across six major administrative regions by stratified cluster sampling, and the P-PAQ initially developed by the researchers of this study were completed by the primary caregivers. The questionnaire was administered to collect data relating to the amount of physical activity undertaken by the preschoolers, and the norm was determined by quartiles. Data relating to parental concepts of sports and parental behavior were assessed by calculating mean scores in order to establish the norm.
Results:
Among preschoolers in urban areas, the M(P 25 ,P 75 ) of total physical activity time (min/day), moderate-to-vigorous physical activity time (min/day), outdoor time (min/day) and screen time (min/day) on school days outside kindergarten and on weekends were 84 (54,120), 22 (8,40), 12 (0,24) and 18 (6,30), and 170 (115,240), 60 (30,95), 90 (35,120) and 30 (20,60), respectively. When the score of parents sports concept and behavior (total score of 40) were≥34, 29-<34, 24-<29, <24, it was defined as four levels about above medium, medium, lower medium and lower, respectively. And for two dimensions,when the score of parental sports concept were ≥19, 17-<19, 15-<17, <15,and the score of parental behaviors were ≥16, 12-<16, 8-<12, <8, it was defined as four levels about upper medium, medium, lower medium and lower, respectively.
Conclusion
The norm of extracurricular activities among preschool children in Chinese cities has good representativeness and appropriate threshold values, which could provide a valuable reference for early assessment, as well as guidance in relation to out-of-school physical activity behaviors among children aged 3-6 years old.
