1.Changes of hemagglutinin gene characteristics of influenza virus A(H3N2) during the 2022-2024 influenza season in Beijing

Objective:To analyze the changes in the phylogenetic and antigenic characteristics of the hemagglutinin (HA) gene of influenza virus A(H3N2) [A(H3N2)] during the 2022-2024 influenza seasons in Beijing.Methods:The data of influenza-like cases and A(H3N2) strains from 17 network laboratories and their corresponding sentinel hospitals were collected during the 2022-2024 influenza seasons. The HA genes were amplified and sequenced after extracting nucleic acids of the chosen virus strains. BioEdit, the nucleotide and amino acid sequence identity were conducted, and the maximum likelihood method in MEGA 5.0 software was used to construct the phylogenetic tree of HA genes. Web Logo displayed the amino acid mutation, and the N-glycosylation sites of HA online were analyzed using the NetNGlyc1.0 Server online. The Datamonkey platform was utilized to analyze the positive selection pressure sites of the HA protein.Results:The 2022-2024 influenza season includes 2022-2023 and 2023-2024. During the influenza seasons of 2022-2024, the positive rates of A(H3N2) nucleic acid were 10.35% (2 127/20 543) and 10.47% (4 386/41 876), respectively. In the 2022-2023 influenza season, there were two peaks in the A(H3N2). The comparison of HA genes between all A(H3N2) strains studied with the 2022-2024 vaccine strain (A/Darwin/9/2021) revealed that all of the strains studied have the two amino acid mutations involving 186 and 225 receptor binding sites. There were 31 amino acid substitutions in the 2022-2023 influenza season, of which 18 variant sites involved antigenic determinants. There were 35 amino acid mutations during the 2023-2024 influenza season, of which 14 were related to antigenic determinants. There were changes in the genetic evolutionary subclades of A(H3N2) strains in two influenza seasons: from 2022 to 2023, three evolutionary subclades were co-prevalent together, with the 3C.2a1b.2a.2a.3a.1 accounting for 76.67% (23/30), the 3C.2a1b.2a.1a accounting for 20.00% (6/30), the 3C.2a1b.2a.2a.1 accounting for 3.33% (1/30); from 2023 to 2024, two subclades were prevalent, with 3C.2a1b.2a.2a.3a.1 accounting for 95.12% (39/41) and 3C.2a1b.2a.2a.1 accounting for 4.88% (2/41). The glycosylation site changes of the HA protein of A(H3N2) have been enhanced from 2023 to 2024. The 145 amino acid position of the HA protein of the A(H3N2) was the positive selection site for stress selection site analysis.Conclusions:The evolutionary subclades of the HA gene of A(H3N2) in Beijing showed changes from 2022 to 2024, and the glycosylation site polymorphism of the HA protein of A(H3N2) significantly increased from 2023 to 2024. Continuous monitoring of HA mutations in the A(H3N2) is crucial, providing a basis for developing influenza prevention and control strategies, as well as new strategic support for screening influenza vaccine components, vaccine design, and discovery of drug targets.
2.Public Database-based Study to Explore the Expression and Role of DDB1 in Lung Adenocarcinoma.

BACKGROUND:
Lung adenocarcinoma (LUAD) is the predominant subtype of non-small cell lung cancer (NSCLC). Damage-specific DNA binding protein 1 (DDB1), as a core protein of the CUL4-DDB1 ubiquitin ligase complex, is involved in the regulation of DNA damage repair, epigenetic modification, and cell cycle checkpoint activation. While the involvement of DDB1 in tumour progression through DNA repair and RNA transcriptional regulation has been reported, its expression and role in LUAD remain to be elucidated. This study aims to investigate the expression and role of DDB1 in LUAD.
METHODS:
The expression, clinicopathological features and prognosis of DDB1 in LUAD were analysed using databases such as UALCAN, Kaplan-Meier Plotter and GEPIA; The interaction network and enriched functional pathways were constructed by GeneMANIA and Metascape; the correlation between DDB1 and immune cells by combining with TISIDB infiltration was evaluated, and the clustering results of cell subtypes and the expression of DDB1 in different immune cell subpopulations were analysed by single-cell sequencing; finally, tissue microarrays were used to further verify the expression and prognostic value of DDB1 in LUAD.
RESULTS:
The mRNA and protein expression of DDB1 in LUAD tissues were significantly higher than those in normal tissues (P<0.01), and the high expression correlated with later clinical stage (P<0.001), lymph node metastasis (P<0.001) and poor prognosis (P<0.001). Functional enrichment showed that DDB1 was involved in DNA repair and RNA transcriptional regulation, and TISIDB evaluation revealed that DDB1 was negatively correlated with the expression level of immune cells, suggesting the potential regulation of the immune microenvironment. Single cell analysis showed that DDB1 was mainly expressed in T cells, alveolar macrophages and dendritic cells. Tissue microarrays confirmed that overall survival was shorter in the DDB1 high expression group (P<0.001), and Cox multifactorial analysis showed that DDB1 was an independent predictor of LUAD prognosis.
CONCLUSIONS
DDB1 is highly expressed in LUAD, which is associated with poor prognosis, and is closely related to tumor immune cell infiltration, and is involved in tumourigenesis and development through DNA repair and RNA transcriptional regulation. DDB1 can be used as a potential prognostic marker and therapeutic target for LUAD.

3.Clinical-MRI radiomics combined model for differentiating borderline ovarian tumor from epithelial ovarian cancer

Objective To explore the value of clinical-MRI radiomics combined model for differentiating borderline ovarian tumor(BOT)from epithelial ovarian cancer(EOC).Methods Totally 139 patients with BOT(BOT group)and 307 patients with EOC(EOC group)confirmed by postoperative pathology and underwent preoperative pelvic MRI were retrospectively enrolled and randomly divided into training set(n=312)and test set(n=134)at a ratio of 7∶3.Multivariable logistic regression was used to identify independent clinical predictors for differentiating BOT and EOC,then a clinical model was constructed.Radiomics features were extracted from the volumes of interest(VOI)of lesions on T2WI,diffusion weighted imaging(DWI)and apparent diffusion coefficient(ADC)images,respectively,and single-sequence and multi-sequence MRI radiomics models were built using extreme gradient boosting(XGBoost)based on data in training set.The optimal MRI radiomics model was selected according to the highest area under the curve(AUC)in test set,and a clinical-MRI radiomics combined model was constructed combined the optimal radiomics model with independent clinical predictors.The performances of clinical model,the optimal MRI radiomics model and the combined model for differentiating BOT and EOC were compared in test set.SHapley Additive exPlanations(SHAP)analysis was applied to interpret key predictive features in the best model.Results Patients' age,carbohydrate antigen 153(CA153)and carbohydrate antigen 125(CA125)were all independent predictors for differentiating BOT and EOC(all P<0.05).Multi-sequence MRI radiomics model was the optimal MRI radiomics model.The combined model showed superior performance(AUC=0.929)for discriminating BOT and EOC compared with clinical model(AUC=0.881)and multi-sequence MRI radiomics model(AUC=0.871)(both P<0.05).SHAP beeswarm plot revealed that the top 10 important features of combined model included age,CA153 and CA125,as well as entropy,kurtosis and gray level non-uniformity from ADC and DWI sequences.Conclusion Clinical-MRI radiomics combined model based on multi-sequence MRI radiomics features and clinical features could be used to effectively differentiate BOT from EOC.
4.Farrerol relaxes isolated pulmonary arteries in C57BL/6J mice by activating Kv channel

Aim To study the diastolic effect and mechanism of farrerol on isolated pulmonary arteries of C57BL/6J mice.Methods After anesthesia,mouse lung tissue was quickly removed and placed into the 4 ℃ K-H buffer,pulmonary arteries were isolated under the microscope and cut into 2 mm long vascular rings for spare use.(1)The effect of farrerol on the resting tension of isolated mouse pulmonary arteries:in the resting state,the active mouse pul-monary artery rings were treated with different concentrations of farrerol(10-6,3×10-6,10-5,3×10-5 and 10-4 mol/L).(2)Farrerol relaxed mouse pulmonary artery experiment:pulmonary arteries were contracted using phenylephrine(PE,1 μmol/L)or KCl(60 mmol/L),and when the contraction reached the platform,different concentrations of farrerol(10-6,3×10-6,10-5,3×10-5 and 10-4 mol/L)was added.(3)Farrerol inhibited pulmonary artery contraction experi-ment:under conditions with or without the addition of farrerol,pulmonary arteries were contracted using different concen-trations of PE(10-9,3×10-9,10-8,3 × 10-8,10-7,3×10-7 and 10-6 mol/L)or KCl(20,30,40,60,80 and 120 mmol/L),and the pulmonary artery muscle tension was recorded.(4)Calcium free and recalcification experiments:under conditions with or without the addition of farrerol,the changes of isolated mouse pulmonary artery tension were meas-ured in the state of calcium free or recalcification { 2.5 mmol/L[Ca2+]ex }.(5)The relationship between farrerol in-duced relaxation of isolated mouse pulmonary arteries and potassium ion channels:firstly,60 mmol/L KCl solution was used to contract the mouse pulmonary arteries until the platform.Then,3 mmol/L aminopyridine(4-AP),2 mmol/L tet-raethylammonium(TEA),30 μmol/L BaCl2,and 10 μmol/L glibenclamide(Gli)were added and treated for 15 min.Subsequently,the pulmonary arteries were relaxed using a concentration gradient of farrerol.Results Farrerol had no significant effect on the mouse pulmonary arteries in the resting state,but had a concentration-dependent relaxing effect on the mouse pulmonary arteries pre-contracted with PE and KCl.While the pretreatment of 3×10-5 mol/L farrerol could sig-nificantly reduce the maximum contraction of mouse pulmonary arteries induced by PE and KCl(P<0.01),as well as sig-nificantly reduce the contraction of mouse pulmonary arteries induced by KCl under calcium free or recalcification conditions(P<0.01).Addition of the voltage-dependent potassium ion channel blocker 4-AP significantly reduced the maximum diastolic rate of mouse pulmonary arteries induced by farrerol(P<0.01),while addition of the high conductivity calcium activated potassium ion channel blocker TEA,inward rectifying potassium ion channel blocker BaCl2,or ATP sensitive po-tassium ion channel blocker Gli had no significant effect on the vasodilation effect of farrerol(P>0.05).Conclusion Farrerol has a relaxing effect on isolated mouse pulmonary arteries,and its mechanism may be related to open voltage-de-pendent potassium ion channels.

8.Application of digital scanning combined with reverse engineering technology in the teaching of full crown preparation

Objective:To investigate the application of digital scanning combined with reverse engineering technology in the teaching of full crown preparation.Methods:A total of 30 undergraduate students in the fifth grade of stomatology were selected from Shantou University Medical College and were divided into experimental group and control group using a random number table. Two resin teeth were distributed to each student and were placed on dental head simulators to perform full crown preparation. The students in the control group received teaching with analogies of experience, and those in the experimental group received teaching with digital scanning, i.e., full crown preparation for the second time after digital scanning for the first time of full crown preparation. The score was determined based on China Stomatological Association Standards: Guideline for the tooth preparation of dental ceramic crowns (T/CHSA 008—2023), with a total score of 100 points. SAS9.4 software was used for the two-independent-samples t test and the paired t-test, and the Kendall W concordance coefficient was used to investigate the consistency of evaluators. Results:There were significant changes after teaching in the preparation scores of the right maxillary central incisor (76.27 pre-demonstration vs. 84.70 post-demonstration, P<0.001) and the right maxillary first molar (72.10 pre-demonstration vs. 82.37 post-demonstration, P<0.001). Compared with the control group, the experimental group had a significant increase in the mean preparation score of the right maxillary first molar (14.00 vs. 6.53, t=2.64, P=0.014). In the experimental group, there were significant increases in the preparation scores of the right maxillary first molar for the occlusal surface (15.40 pre-demonstration vs. 19.33 post-demonstration, P<0.001), the buccolingual surface (18.13 pre-demonstration vs. 20.87 post-demonstration, P=0.016), and the proximal surface (12.40 pre-demonstration vs. 14.07 post-demonstration, P=0.004), as well as significant increases in the scores of the convergence angles of the buccolingual surface (2.80 pre-demonstration vs. 4.07 post-demonstration, P=0.004) and the proximal surface (3.47 pre-demonstration vs. 4.47 post-demonstration, P=0.008). Conclusions:Application of digital teaching for difficult crown preparation of posterior teeth can effectively improve the quality of crown preparation among students, standardize the teaching process of crown preparation, and increase the precision of crown preparation, thereby laying a foundation for promoting uniformity in talent cultivation for dental prosthodontics.
9.Impact of therapeutic plasma exchange intervention timing and liver injury periodization on the prognosis of pa-tients with exertional heat stroke

Objective To explore the prognostic impact and clinical application value of therapeutic plasma exchange(TPE)intervention timing and liver injury periodization in patients with exertional heat stroke(EHS).Methods Data of 127 EHS patients from the First Medical Center of the General Hospital of the People′s Liberation Army from January 2011 to December 2023 were collected,then divided into the death group and the survival group based on therapeutic outcomes and into 5 stages according to the dynamic changes of ALT,AST,TBIL and DBIL.According to propensity score matching analysis,11 patients in the survival group and 12 patients in the death group were included in the statistical analysis,and 20 of them were treated with TPE.The changes in indicators and clinical outcomes before and after TPE were observed,in order to evaluate the impact of intervention timing on prognosis.Results Among the 23 patients,14 had no liver injury or could progress to the repair phase,resulting in 3 deaths(with the mortality rate of 21.43%),while 9 patients failed to pro-gress to the repair phase,resulting in 9 deaths(with the mortality rate of 100%),with significant differences(P<0.05).The mortality rate of the first TPE intervention before the third stage of liver injury was 23.08%(3/13),while that of interven-tion after reaching or exceeding the third stage was 85.71%(6/7),and the difference was statistically significant(P<0.05).Conclusion TPE should be executed actively in EHS patients combined with liver injury before the third phase to lock its pathological and physiological processes,thereby improving prognosis and reducing mortality.

Objective To explore the effect of evidence-based nursing on medication compliance with multiple chronic disease patients in the community.Methods 60 patients with chronic diseases included in the center from January 2023 to June 2023 were selected,divided into control group and observation group before and after the application of evidence-based care,with 30 cases in each group.The control group adopted the usual care program,and the observation group adopted the evidence-based care program to compare the implementation rate of the review index before and after the evidence application,the medication compliance of the two groups,the drug holding rate and the drug knowledge level of community nurses for chronic diseases.Results After the application of evidence,the implementation rate of the review index was higher,and the medication compli-ance and drug holding rate of patients in the observation group were higher than that of the control group.After the application of evidence,the drug knowledge level of community nurses for chronic diseases was significantly improved,and the difference was statistically significant(P<0.05).Conclusion The evidence-based nursing practice can standardize the management of medi-cation compliance of patients with chronic diseases in the community,improve the medication compliance of patients with chroni diseases,and improve the knowledge level of chronic diseases in the community nurses.
