1.Effects of triptolide exposure during pregnancy/lactation on the reproductive system of male offspring in rats

OBJECTIVE To investigate the effects of triptolide (TP) exposure during pregnancy and lactation on the reproductive system development and function in male offspring of rats, providing a reference for medication safety during pregnancy and lactation. METHODS Pregnant rats were randomly divided into control group (12 rats, normal saline) and T1-T4 groups [12, 13, 14, 17 rats that received TP at 200, 400, 600, and 800 μg/(kg·d) respectively]. They were given relevant medicine/normal saline intragastrically, once a day, until the offspring were born and naturally weaned, the intragastric administration volume of each rat was consistently 2 mL. After 60 days of feeding, reproductive organ weights and coefficients were measured in male offspring, testicular and epididymal histology and sperm morphology were observed. Sperm motility, sperm count, and serum levels of gonadotropin-releasing hormone (GnRH), follicle stimulating hormone (FSH), luteinizing hormone (LH), and testosterone (T) in the epididymides were analyzed. Protein expressions of glycogen synthase kinase 3α (GSK3α), phosphorylated GSK3α (p-GSK3α), and phosphatase 1γ2 (PP1γ2) in sperm were also determined. RESULTS Compared with the control group, the testicular and epididymal weights, serum levels of GnRH and T, the relative protein expression of PP1γ2 were significantly decreased in T1-T4 groups. Additionally, in the T2 to T4 groups, there were significant reductions in the weight and coefficient of the seminal vesicle, total number of sperm, sperm concentration, sperm motility as well as relative protein expressions of GSKα, p-GSK3α in the offspring rats. Furthermore, the epididymal coefficient in the T3 and T4 groups, the testicular coefficient, mean sperm track velocity and sperm curvature velocity in the T4 group were significantly decreased (P< 0.05); the number of abnormal sperm, rate of sperm abnormality, and levels of FSH and LH in the offspring rats of the T1 to T4 groups were all significantly increased (P<0.05); in the offspring rats of the T1 to T4 groups, there was a decrease in the number of epithelial cells in the seminiferous tubules of the testes. Within the epididymal tissue, degenerative and necrotic changes in the epithelial cells were visible, accompanied by mild infiltration of inflammatory cells in the stroma. CONCLUSIONS TP exposure during pregnancy and lactation disrupts reproductive organ development, impairs spermatogenesis and sperm motility, as well as suppresses androgen synthesis in male offspring,thereby having a negative impact on the development of the reproductive system. These effects may be mechanistically linked to regulation of GSK3α, p-GSK3α and PP1γ2 protein expressions.

Background and Objectives:
The Fractional Flow Reserve and Intravascular UltrasoundGuided Intervention Strategy for Clinical Outcomes in Patients with Intermediate Stenosis (FLAVOUR) trial demonstrated non-inferiority of fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) compared with intravascular ultrasound (IVUS)-guided PCI. We sought to investigate the cost-effectiveness of FFR-guided PCI compared to IVUS-guided PCI in Korea.
Methods:
A 2-part cost-effectiveness model, composed of a short-term decision tree model and a long-term Markov model, was developed for patients who underwent PCI to treat intermediate stenosis (40% to 70% stenosis by visual estimation on coronary angiography).The lifetime healthcare costs and quality-adjusted life-years (QALYs) were estimated from the healthcare system perspective. Transition probabilities were mainly referred from the FLAVOUR trial, and healthcare costs were mainly obtained through analysis of Korean National Health Insurance claims data. Health utilities were mainly obtained from the Seattle Angina Questionnaire responses of FLAVOUR trial participants mapped to EQ-5D.
Results:
From the Korean healthcare system perspective, the base-case analysis showed that FFR-guided PCI was 2,451 U.S. dollar lower in lifetime healthcare costs and 0.178 higher in QALYs compared to IVUS-guided PCI. FFR-guided PCI remained more likely to be cost-effective over a wide range of willingness-to-pay thresholds in the probabilistic sensitivity analysis.
Conclusions
Based on the results from the FLAVOUR trial, FFR-guided PCI is projected to decrease lifetime healthcare costs and increase QALYs compared with IVUS-guided PCI in intermediate coronary lesion, and it is a dominant strategy in Korea.
3.Anti-vascular dementia effect of Yifei xuanfei jiangzhuo formula by inhibiting mitochondrial fission

OBJECTIVE To investigate the intervention effect and its potential mechanism of Yifei xuanfei jiangzhuo formula by inhibiting mitochondrial fission in a vascular dementia (VaD) model rats. METHODS VaD rat model was established by bilateral common carotid artery ligation. The experimental animals were randomly divided into sham operation group (SHAM), model group (MOD),Yifei xuanfei jiangzhuo formula low-dose group (YFXF-L), Yifei xuanfei jiangzhuo formula high-dose group (YFXF-H), and Donepezil hydrochloride group (positive control), with 9 animals in each group. After 30 days of intervention, the spatial learning memory ability was assessed by Morris water maze experiment; HE staining was used to observe histopathological changes in CA1 area of hippocampus; ELISA was used to detect the levels of serum inflammatory factors [interleukin-1β (IL-1β) and IL-4]; Western blot was used to detect the expressions of heat shock protein 90 (HSP90)/mixed lineage kinase domain-like protein (MLKL)/dynamin-related protein 1 (Drp1) pathway-related proteins, mitochondrial fusion proteins (MFN1, MFN2), and adenosine triphosphate synthase 5A (ATP5A) in hippocampal tissues. The immunohistochemistry was used to detect the level of phosphorylated MLKL (p-MLKL); real-time fluorescence quantitative PCR was adopted to detect mRNA expressions ofHSP90, MFN1, MFN2 and ATP5A. RESULTS Compared with SHAM group, the escape latency of rats in the MOD group was significantly prolonged, the number of crossing the platform was significantly reduced, and the hippocampal tissues showed typical neuronal damage characteristics, the positive expression level of p-MLKL and the serum level of IL-1β significantly increased, while the serum level of IL-4 significantly decreased, the protein and mRNA expression of HSP90, as well as the protein expressions of p-MLKL/MLKL and p-Drp1(Ser616)/Drp1 were all significantly increased in hippocampal tissue, the protein and mRNA expressions of MFN1, MFN2 and ATP5A, and protein expression of p-Drp1(Ser637)/Drp1 were all significantly decreased (P<0.05). After the intervention of Yifei xuanfei jiangzhuo formula, above indicators in each treatment group were all significantly reversed (P<0.05). CONCLUSIONS Yifei xuanfei jiangzhuo formula may alleviate neuronal damage and neuroinflammatory responses in VaD rats by regulating the HSP90/MLKL/Drp1 signaling pathway, inhibiting mitochondrial fission, thereby maintaining mitochondrial dynamic balance and improving mitochondrial function.
4.Association between unhealthy lifestyle and risk of heart disease and diabetes in the elderly in Xi'an

Objective To investigate the association between lifestyle and risk of heart disease and diabetes in the elderly population in Xi'an City. Methods From January 2021 to January 2024, a staged cluster sampling method was used to investigate the lifestyle and the occurrence of heart disease and diabetes in elderly population aged 60 years and above in the communities of Xi'an. Multivariate logistic regression was used to analyze the relationship between lifestyle and the risk of heart disease and diabetes. Results A total of 413 elderly people were investigated, of which 31.96% had heart disease, 27.12% had diabetes, and 10.90% had diabetes with heart disease. Multivariate logistic regression analysis revealed that age, BMI, family history, sweet food preference, smoking, and sitting and lying for a long time were risk factors for diabetes in the elderly population (P<0.05). Age, BMI, family history, history of diabetes, preference for salted products, smoking, drinking, and sitting and lying for a long time were risk factors for heart disease in the elderly population (P<0.05). Conclusion The incidence rates of heart disease and diabetes are high in the elderly population in Xi'an City. The risk of diabetes is related to unhealthy lifestyles such as sweet food preference, smoking, and sitting and lying for a long time, while heart disease is related to unhealthy lifestyles such as preference for salted products, smoking, drinking, and sitting and lying for a long time.
5.Effectiveness of respiratory rehabilitation intervention in patients undergoing thoracoscopic radical resection for lung cancer

Objective:
To evaluate the effects of problem-oriented respiratory rehabilitation nursing on lung function, activity ability, length of hospital stay, and retention time of closed chest drainage tube in patients undergoing thoracoscopic radical resection for lung cancer, so as to provide a basis for improving the prognosis of patients with lung cancer surgery.
Methods:
A total of 119 patients with lung cancer who underwent thoracoscopic radical resection for lung cancer in the First Affiliated Hospital of Anhui Medical University from October 2023 to June 2024 were selected and randomly divided into the intervention group (n=59) and the control group (n=60). The control group received routine respiratory rehabilitation nursing of thoracic surgery. On the basis of the treatment and guidance received by the control group, the intervention group implemented problem-oriented respiratory rehabilitation nursing. First second expiratory volume (FEV1) and forced vital capacity (FVC) were used to evaluate pulmonary function at 1 day before surgery and 3 days after surgery, and the 6-minute walk distance (6MWD) was used to evaluate physical activity. The incidence of pulmonary complications, length of hospital stay and retention time of closed chest drainage tube were collected through the hospital's electronic medical records system. The intervention effects between the two groups were compared using variance analysis of repeated-measures analysis of variance and log-rank test.
Results:
The mean age of the control group was (60.77±9.31) years, with 28 males and 32 females. There were 4 cases of squamous cell carcinoma, 55 cases of adenocarcinoma, and 1 case of small cell carcinoma. The tumors were located in the right lobe in 41 cases. The mean age of the intervention group was (58.71±10.01) years, with 23 males and 36 females. There were 2 cases of squamous cell carcinoma, 56 cases of adenocarcinoma, and 1 case of small cell carcinoma. The tumors were located in the right lobe in 37 cases. There were no statistically significant differences in age, gender, education level, pathological type, and tumor location between the two groups (all P>0.05). Three days after surgery, there was an interaction effect between the group and the time in FEV1, FVC, and 6MWD between the two groups. The FEV1 and 6MWD in the intervention group were higher than those in the control group (both P<0.05). There was no statistically significant difference in FVC between the groups (P>0.05). There were 3 cases of postoperative complications in the control group, and no cases in the intervention group. Log-rank test showed that the length of hospital stay and retention time of closed chest drainage tube after surgery in the intervention group were shorter than those in the control group (both P<0.05).
Conclusion
The problem-oriented respiratory rehabilitation nursing can improve the lung function and activity ability of patients with lung cancer after surgery, shorten the length of hospital stay and retention time of closed chest drainage tube, and improve the quality of life.
6.Mechanism by which esketamine improves postoperative cognitive impairment in rats with hip fracture through AMPK/SIRT1/PGC-1α signaling pathway

OBJECTIVE To investigate the mechanism by which esketamine improves postoperative cognitive impairment in rats with hip fracture based on the AMP-activated protein kinase (AMPK)/silencing information regulatory factor 1 (SIRT1)/ peroxisome proliferator activated-receptor-γ coactivator-1α (PGC-1α) signaling pathway. METHODS Rats with hip fracture surgery were assigned into model group, esketamine group (10 mg/kg), inhibitor group (250 μg/mL AMPK inhibitor Compound C), and esketamine+inhibitor group (10 mg/kg esketamine + 250 μg/mL Compound C), and rats undergoing sham surgery were used as the control group, with 12 rats in each group. New object recognition and Barnes maze experiments were used to E-mail:448231@163.com evaluate cognitive function in rats. The levels of serum tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β),superoxide dismutase (SOD), malondialdehyde (MDA), gamma-aminobutyric acid (GABA), dopamine (DA) and glutamate (Glu), and the apoptosis of hippocampal neurons were detected. The pathological morphology of the hippocampal tissue and the ultrastructure of mitochondria were observed. The mRNA expression of B-cell lymphoma-2 (Bcl-2) and Bcl-2-associated X protein (Bax),the mRNA and protein expression of AMPK, SIRT1 and PGC-1α, as well as the expression of phosphorylated (p)-AMPK in hippocampal tissue, were detected. RESULTS Compared with the control group, the hippocampal neurons in the model group of rats were disordered, with more neurons necrotic and swollen mitochondria;the new object recognition index, the SOD, GABA, DA levels, Bcl-2, AMPK, SIRT1 and PGC-1α mRNA expression levels, and p-AMPK, SIRT1, PGC-1α protein expression levels were significantly reduced, while the latency and number of errors for locating unknown holes, the TNF-α, IL-1β, MDA and Glu levels, neuronal cell apoptosis rate, and Bax mRNA expression levels were significantly increased/prolonged (P<0.05). Compared with the model group, the esketamine group showed reduced pathological damage to the hippocampal tissue of rats, and the new object recognition index, the SOD, GABA and DA levels, the Bcl-2, AMPK, SIRT1 and PGC-1α mRNA expression levels, and p-AMPK, SIRT1, PGC-1α protein expression levels were significantly increased,while the latency and error frequency for locating unknown holes, TNF-α, IL-1β, MDA and Glu levels, neuronal cell apoptosis rate, and Bax mRNA expression levels were significantly decreased (P<0.05);the inhibitor group showed the opposite trend of changes in these indicators compared to the esketamine group (P<0.05).AMPK inhibitor could reverse the improvement effect of esketamine on the above indicators after hip fracture surgery in rats (P<0.05). CONCLUSIONS Esketamine may improve postoperative inflammatory response and oxidative stress levels in rats with hip fracture by activating the AMPK/SIRT1/PGC-1α signaling pathway, inhibiting neuronal cell apoptosis, improving mitochondrial structure, and promoting postoperative cognitive function recovery.

Group RR comprised 70 patients(140 eyes)bilaterally implanted with ZXR00 IOLs(Tecnis ZXR00, where one target was -0.5 D to -0.75 D and the other was 0 to -0.25 D). Group RM comprised 60 patients(120 eyes)unilaterally implanted with both ZXR00 and ZMB00 IOLs(Tecnis ZMB00, 0 to -0.25 D). Postoperative outcomes were compared after 3 mo, including visual acuity, defocus curves, stereoacuity, modulation transfer functions(MTFs), higher-order aberrations, and Visual Function-14(VF-14)questionnaire responses.RESULTS: Group RR had superior bilateral intermediate vision, while the group RM had superior bilateral near vision(both P<0.05). Group RM also exhibited superior MTFs and reduced higher-order aberrations(both P<0.05). Stereoacuity and VF-14 questionnaire results showed no statistically significant difference between groups(P>0.05).CONCLUSION: The implantation of micromonovision has significantly improved near vision. IOLs and their collocation can be customized according to individual patient needs to achieve precise treatment and provide cataract patients with high-quality vision.

9.Clinical retrospective analysis of 41 cases of chronic recurrent parotitis

Objective:
To explore the clinical manifestations and treatment plans of chronic recurrent parotitis (CRP), and to provide a reference for the clinical diagnosis and treatment of CRP.
Methods:
Approval was obtained from the hospital’s Medical Ethics Committee, and a retrospective analysis and summary of the clinical features, imaging characteristics, diagnosis, and treatment of 41 CRP patients with complete data were performed.
Results:
Among the 41 patients with CRP, 14 were male and 27 were female, with a male-to-female ratio of approximately 1:2 (14/27). The age at first-time onset ranged from 3 to 23 years, with a median age of 6 years, and there were 38 patients (92.7%, 38/41) with the first onset age of under 18 years old. The age of the first visit to our hospital ranged from 4 to 72 years old, with an average age of (41.0 ± 17.3) years; the disease duration was 0.5 to 66 years, with an average of 35.0 ± 16.1 years. Twenty-five cases had bilateral parotid gland involvement (61.0%, 25/41). The clinical manifestations of CRP are repeated swelling of one or both parotid glands, along with discomfort, and this may be accompanied by mild edema or skin flushing and pus or jelly-like secretions at the duct openings. The typical manifestations of parotid angiography are: the dominant duct and branch ducts of the parotid gland do not have specific dilation or narrowing, and the peripheral ducts show characteristic “punctate, spherical, or cavitary” dilation and delayed emptying. Of the cases, 34 had abnormal enlargement of the main duct orifice (82.9%, 34/41), and 37 presented with abnormal anterior displacement of the accessory glands (90.2%, 37/41). The treatment plan of “antibiotic perfusion + aspiration and removal of obstruction (or aspiration after obstruction dissolution)+ postprandia massage along the direction of the parotid duct (from posterior to anterior) with multiple courses for consolidation”achieved favorable outcomes. The mean follow-up period of this group was(71.1+21.9)months, and no recurrence was observed during the follow-up period.
Conclusion
CRP is more prevalent in young females and frequently presents with bilateral involvement. Congenital anatomical defects, such as abnormal enlargement of the main duct orifice and abnormal anterior displacement of the accessory glands, are important predisposing factors. The multi-course comprehensive therapy centered on antibiotic infusion, removal and dissolution of obstructions, and post-prandial massage along the direction of the parotid duct has significant therapeutic effects and deserves clinical application.
10.Pathogenesis,Differentiation and Treatment of Pre-Metastatic Niche in Malignant Tumors Based on the Theory of "Toxin Accumulation Damaging Yin"

According to the theory of "toxin accumulation damaging yin", the accumulation of pathological products and the disruption of homeostasis in the pre-metastatic niche (PMN) of malignant tumors correspond to "toxin accumulation" and "yin damage" respectively. During the dynamic evolution of PMN, the main pathogenesis in the initial stage is healthy qi deficiency and phlegm congestion, obstruction in the ying (营) and wei (卫) level, for which the therapeutic approach is fortifying spleen and warming yang, reinforcing healthy qi, consolidating the root, and assissting in resolving phlegm. In the progression stage, the predominant mechanism is mutual binding of phlegm and stasis, with collateral damage and pathological transformation. Treatment should focus on resolving phlegm and eliminating stasis, using insect-derived medicinals to attack accumulation and block pathological transmission. In the terminal stage, the main pathogenesis involves phlegm-stasis transforming into fire, with depletion of qi and yin, for which it is suggested to replenish qi and nourish yin, combine clearing and tonifying methods to control fire-toxin. After the PMN has formed, pathogenic toxin may flow along the collaterals, tending to lodge in corresponding viscera with functional imbalance and deviation between deficiency and excess, eventually giving rise to malignant tumors. Understanding the pathogenesis of the PMN in the malignant tumors based on the "toxin accumulation damaging yin" theory may provide a valuable perspective for developing traditional Chinese medicine strategies for the prevention and treatment of tumor metastasis.
