ObjectiveTo explore the changes in color, odor and chemical components during wine-processing of Chuanxiong Rhizoma(CR), identify differential markers, and provide a basis for standardizing the process and establishing quality standards. MethodsFifteen batches of CR samples from 4 producing areas were collected. Colorimeter and electronic nose were used to detect the color changes and odor components of CR before and after wine-processing. Multivariate statistical methods including partial least squares-discriminant analysis(PLS-DA), principal component analysis(PCA), discriminant factor analysis(DFA) and Fisher discriminant analysis were applied to identify wine-processed CR at different processing stages and establish discriminant models, and differential components were screened out based on variable importance in the projection(VIP) value1. Then, high performance liquid chromatography(HPLC) was employed to detect the content changes of four components(ferulic acid, senkyunolide I, senkyunolide A and ligustilide) during the processing stages. ResultsThe differences of wine-processed CR at various stages were primarily reflected in color parameters L*(brightness value), a*(red-green value) and b*(yellow-blue value). Based on chromaticity differences, the color reference ranges were established for moderately processed CR, including L* of 46.75-48.24, a* of 5.37-6.07 and b* of 20.32-21.70. In odor analysis, DFA revealed significant differences among processing stages, and 11 odor markers were identified, with four differential markers(4-hydroxy-3-butylphthalide, isopropyl butyrate, L-limonene and 1-methoxyhexane) based on VIP values. HPLC results showed that there was no significant difference of the four components except for ligustilide in wine-processed CR at different stages. ConclusionThis study achieved rapid identification of wine-processed CR with different processing degrees by electronic sensory technology and differential component content detection, with discrimination accuracy rates of 92.4% and 93.272% for color and odor, respectively. This paper also established the reference ranges of main colorimetric parameters for wine-processed CR at different stages, and four differential components were screened out, providing a basis for standardizing the processing of wine-processed CR and establishing quality standards for this decoction pieces.

Objective:To explore the effects of miR-125b on the proliferation, invasion and migration of pancreatic cancer cells by targeting SMYD2 signaling pathway. Methods:The expression of miRNA-125b in Aspc-1 and BxPC-3 lines of pancreatic cancer cells were detected. miRDB, ENCORI and TargetScan databases were used to predict the potential target genes of miRNA-125b. The downstream target genes of miRNA-125b were identified by qPCR assay and double luciferase reporter gene assay. Western blot analysis was performed to detect SMYD2 protein expression after transfection with miRNA-125b inhibitor. EdU staining, Annexin V-FITC/PI assay and Annexin V-FITC/PI assay were used to detect the effects of miRNA-125b inhibitor transfection and simultaneous transfection of miRNA-125b and SMYD2 inhibitor on cell proliferation, clonogenesis and apoptosis. Results:The expression level of miRNA-125b in pancreatic cancer cell lines was higher than that in normal pancreatic duct cells ( P<0.05). The downstream target gene of miRNA-125b was identified as SMYD2 by qPCR assay and double luciferase reporter gene assay. The expression of SMYD2 protein in miR-125b inhibitor group was higher than that in NC group ( P<0.01). EdU cell proliferation assay showed that the number of miRNA-125b positive cells in inhibitor group was lower than that in NC group and Inhibitor NC group ( P<0.05). The number of clones in miR-125b inhibitor+si-SMYD2 group was more than that in miR-125b inhibitor group ( P<0.01). Annexin V-FITC/PI assay showed that the apoptosis number of cell cells in miR-125b inhibitor+si-SMYD2 group was lower than that in miR-125b inhibitor group ( P<0.01) . Conclusion:miRNA-125b is highly expressed in pancreatic cancer cells, and can directly affect the expression of SMYD2 gene, thereby promoting the proliferation and inhibiting the apoptosis of pancreatic cancer cells.

Objective To investigate the risk factors for mortality and bleeding complications in extracorporeal membrane oxygenation(ECMO)treated patients and to evaluate the impact of thrombocytopenia severity on the prognosis of ECMO therapy. Methods A total of 153 patients who received ECMO treatment at Peking University Third Hospital between January 2013 and September 2024 were en-rolled in this study. The patients were divided into death group(n=97)and recovery group(n=56)based on their final outcomes.Additionally, the patients were categorized into bleeding group(n=104)and non-bleeding group(n=49)based on the occurrence of bleeding complications during ECMO.Clinical baseline characteristics and extreme laboratory values during ECMO were compared be-tween groups.Logistic regression was used to analyze the risk factors for mortality and bleeding. The patients were further divided, based on the initial platelet(PLT)values on the day of catheter placement and the lowest platelet count during ECMO,into normal group(PLT≥ 100× 109/L),moderate reduction group[PLT=(50~99)× 109/L],and severe reduction group(PLT<50× 109/L).Kaplan-Meier analysis was used to compare survival rates among these groups. The patients in the moderate and severe reduction groups were further divided into a platelet transfusion group and a non-transfusion group, and the outcomes and complication rates were com-pared.Results The recovery group had a higher proportion of myocarditis, higher minimum values of PLT,Hb,and Fib,and higher initial PLT values, while the maximum values of lactic dehydrogenase(LDH),total bilirubin(T-Bil),prothrombin time(PT),and procalcitonin(PCT)were lower(all P<0.05)with significant differences.Logistic regression showed that age and maximum PCT were independent risk factors for mortality(OR=1.025 and 1.015 respectively, all P<0.05).The bleeding group had longer ECMO dura-tions,more plasma transfusions, lower minimum Hb values, and higher maximum values of WBC,neutrophils(Neu),and APTT(all P<0.05)with statistical differences. The minimum PLT value, maximum WBC value, and maximum APTT value were independent risk factors for bleeding complications(OR=0.986,1.062,and 1.004 respectively, all P<0.05).Kaplan-Meier analysis showed that the patients in the severe reduction group had lower survival rates, regardless of whether the grouping was based on initial or minimum platelet counts(all P<0.05).Platelet transfusion improved the mortality in the severe reduction group(P<0.05)but had no effect on the moderate reduction group. Conclusion Age and peak value of PCT are the risk factors for mortality in ECMO patients, while mini-mum PLT count, peak value of WBC and APTT are the risk factors for bleeding complications.Early intervention for infection and in-flammation during ECMO may improve the outcome of patients.Severe thrombocytopenia during ECMO therapy increased the risk of mortality, and targeted platelet transfusion may improve the survival of these patients.

Objective:To examine the relationships between perceived organizational support in job resources, self-efficacy in personal resources, and teaching commitment based on the job requirement-resource model, and to explore the path of influencing clinical teachers' teaching commitment. Methods:Using the convenience sampling method, 463 clinical teachers from 16 university-affilicated hospitals in Shandong Province, China were selected as the study subjects. Surveys were conducted using the Perceived Organizational Support Scale, Self-efficacy Scale, and Teaching Commitment Scale. A statistical analysis was performed using SPSS 26.0. A structural equation model was constructed using AMOS 26.0 to test the mediation effect.Results:①The level of teaching commitment of clinical teachers was relatively high, with a total score of (45.25±9.21) points. The total score of perceived organizational support was (38.22±9.75) points, and the total score of self-efficacy was (112.27±17.30) points. ②Perceived organizational support and self-efficacy were positively correlated with teaching commitment ( r=0.59, r=0.65, both P<0.01). ③ Perceived organizational support had significant and direct positive effects on teaching commitment and self-efficacy ( β=0.36, β=0.51, both P<0.01). Self-efficacy significantly and positively affected teaching commitment ( β=0.47, P<0.01). Self-efficacy played a significant and partial mediating role in the effect of perceived organizational support on teaching commitment (standardized mediating effect value of 0.239, P<0.01), and the mediating effect percentage was 39.83%. Conclusions:Perceived organizational support not only directly predicts clinical teachers' teaching commitment, but also indirectly affects teaching commitment through self-efficacy. University-affiliated hospital administrators should pay attention to clinical teachers' perceived organizational support to improve their self-efficacy and teaching commitment, thereby ensuring teaching quality.

Journal of Clinical Surgery 2025;33(9):948-952
Idiopathic Pulmonary Fibrosis(IPF)is a progressive disease characterized by declining respiratory function and high mortality.Macrophages play a pivotal role in its pathogenesis.Through polarization into pro-inflammatory M1 and pro-fibrotic M2 phenotypes, they contribute to a complex immunoregulatory network. In the early disease stages,M1 macrophage-mediated inflammation causes lung tissue injury, while M2 macrophages drive fibrogenesis by releasing pro-fibrotic factors such as TGF-β.Recent research has revealed that exosomes derived from macrophages serve as carriers for miRNAs,with specific miRNAs(e.g.,miR-328,miR-142-3p)demonstrating significant roles in pulmonary fibrosis.miR-328 promotes fibroblast proliferation and accelerates collagen deposition, whereas miR-142-3p attenuates fibrosis by modulating the TGF-β signaling pathway.Targeted intervention against these macrophage-associated miRNAs shows potential for clinical translation, potentially offering novel approaches for the early diagnosis and targeted therapy of IPF.However, translating these strategies into clinical practice requires overcoming challenges related to production and delivery systems. In conclusion,a deeper understanding of the mechanisms and translational applications of macrophage-derived miRNAs in IPF holds promise for ultimately improving patient prognosis and clinical outcomes.
