1.Research Progress in Nucleus Pulposus Tissue Engineering in Lumbar Intervertebral Disc

Lumbar intervertebral disc degeneration is a common pathological process in the spine,with the main clinical symptoms of low back pain,numbness of lower limbs,and defecation dysfunction.The occur-rence and development of lumbar intervertebral disc degeneration are determined by multiple factors,and the pathophysiological and cellular mechanisms remain to be fully understood.Nucleus pulposus tissue engineering is a new biotherapy that combines biological histology with material science to treat diseases including lumbar inter-vertebral disc degeneration.Clinicians should fully learn the complex relationship between nucleus pulposus tissue engineering and lumbar intervertebral disc degeneration,which will facilitate the clinical treatment of lumbar in-tervertebral disc degeneration,the rehabilitation of lumbar intervertebral disc after treatment,and the prevention of this disease in the population.
2.Cloning and Expression Analysis of Squalene Epoxidase Gene in Poria cocos

ObjectiveTo clone squalene epoxidase (SE), a potential key rate-limiting enzyme involved in the synthesis pathway of Poria cocos triterpenes, from P. cocos and analyze for bioinformatics and expression. MethodThe total RNA was extracted by the kit and reverse-transcribed to cDNA. Specific primers were designed, and the cDNA was used as a template for cloning the SE gene, which was analyzed for bioinformatics. The expression of P. cocos qualene epoxidase(PcSE) was examined by Real-time polymerase chain reaction(Real-time PCR) in P. coco Shenzhou No. 10, Xiangjing 28, and 5.78 strains. ResultThe full length of PcSE is 1 571 bp, containing four exons and three introns. The obtained CDS sequence is 1 413 bp, encoding 470 amino acids. This protein is a hydrophobic protein with no signal peptide structure and has two transmembrane structural domains with a FAD/NAD (P) binding domain and SE structural domain localized to the mitochondrial membrane and the plasma membrane. The homologous sequence alignment with fungi of the Poriferae family is 80.92%, and the phylogenetic tree shows that PcSE protein is most closely related to P. cocos from the US. The results of Real-time PCR showed that the PcSE was expressed in all three strains, with the highest expression in 5.78 strain, and there was no significant difference in PcSE expression among the three strains. ConclusionFor the first time, the PcSE gene was cloned and analyzed from P. cocos, providing a basis for further research on the function of PcSE and the analysis of P. cocos triterpene biosynthesis pathway.
3.Study on Protective Mechanism of Dexmedetomidine on Cerebral Injury in Sepsis Model Rats Based on SIRT 1/ Akt/GSK3β/β-catenin Signaling Pathway

OBJECTIVE:To in vestigate the effects of dexmedetomidine (Dex)on SIRT 1/Akt/GSK3β/β-catenin signaling pathway in cerebral injury of sepsis model rats ,and explore the mechanism of its protecitve effect on cerebral injury. METHODS : A total of 80 male SD rats were randomly divided into sham operation group (Sham group ),sepsis group (CLP group ),CLP+Dex group(10 μg/kg Dex),CLP+Dex+Sirtinol group (10 μg/kg Dex+2 μL/100 g SIRT 1 inhibitor sirtinol ),with 20 mice in each group. Two hours before modeling ,CLP+Dex+Sirtinol group was injected with sirtinol via lateral ventricle. Sepsis model was induced by cecal ligation and perforation in each group (in sham group ,only operation was performed but no ligation was performed). At 0,3,6 h after modeling ,CLP+Dex group and CLP+Dex+Sirtinol group were given Dex (10 μg/kg) intraperitoneally,Sham group and CLP group were given constant volume of normal saline intraperitoneally. Cerebral tissue water content,Evans blue (EB)content,apoptosis in cerebral cortex ,the levels of IL- 1β and TNF-α in cerebral tissue as well as the protein expression of SIRT 1,p-Akt,p-GSK3β and β-catenin in hippocampus were detected 24 h after last medication. RESULTS : Compared with Sham group ,cerebral tissue water content ,EB content ,the number of apoptotic cells in cerebral cortex as well as the levels of IL- 1β and TNF-α in cerebral tissue were increased significantly(P<0.05),while the protein expression of SIRT 1, p-Akt,p-GSK3β and β-catenin in hippocampus were decreased significantly (P<0.05). Compared with CLP group ,cerebral tissue water content ,EB content ,the number of apoptotic cells in cerebral cortex as well as the levels of IL- 1β and TNF-α in cerebral tissue were decreased significantly in CLP+Dex group (P<0.05),while the protein expression of SIRT 1,p-Akt,p-GSK3β and β-catenin in hippocampus were increased significantly (P<0.05). Sirtinol could significantly reverse the above-mentioned cerebral protection and factor regulation effects of Dex (P<0.05). CONCLUSIONS :Dex can protect the cerebral tissue of sepsis model rats,which may play an anti-inflammatory and anti-apoptotic role by activating SIRT 1/Akt/GSK3β/β-catenin signaling pathway ,so as to reduce cerebral edema ,protect blood-brain barrier and reduce cerebral injury.
4.Clinicopathological features of primary central nervous system T-cell lymphomas

Objective:To investigate the clinicopathological features, diagnosis and differential diagnosis of primary central nervous system T-cell lymphomas (TPCNSL), and to analyze its biological behavior and prognosis.Methods:Three cases of TPCNSL were collected from September 2014 to September 2019 in the First Affiliated Hospital of Nanjing Medical University. They were evaluated by HE, immunohistochemistry (IHC) and molecular genetics, and the relevant literature was reviewed.Results:Among the 6 816 brain tumors, 97 were primary central nervous system lymphomas (PCNSL), including 3 TPCNSL. There were two male and one female patients, aged 60, 67, and 82 years. Clinically, they were presented with varying degrees of limb numbness and unstable gait. Microscopically, the tumor cells were distributed diffusely or around blood vessels. They showed significant atypia and brisk mitotic activity. By IHC, they were positive for LCA, CD3, CD43, TIA-1, and perforin. Two of three cases were positive for CD5 and granzyme B. T-cell receptor gene rearrangement was clonal. EBER in situ hybridization was negative. The patients were followed for 1 to 6 months; one patient received chemotherapy and died of recurrence 3 months after surgery. One patient died of recurrence 5 months after operation alone. One patient remained recurrence and metastasis free more than 4 months post surgery.Conclusions:PCNSL is uncommon, and most are B-cell lymphomas, while T-cell lymphomas are even rarer. As the latter may show atypical clinical manifestations, diverse histologic morphology and poor prognosis, early diagnosis and timely treatment are particularly important for patients to improve survival.
5.Clinical and genetic diagnosis in a hereditary hemorrhagic telangiectasia family

Objective:To investigate the clinical features and feasibility genetic diagnosis in a hereditary hemorrhagic telangiectasia (HHT) family,and to explore the application of gene mutation testing in HHT diagnosis.Methods:Medical histories and clinical features of a family were analyzed to diagnose HHT patients and suspected individuals according to the clinical diagnostic criteria.Sequence analysis of endoglin (ENG) and activin A receptor like type 1 (ACVRL1) gene in the proband was performed with PCR and Sanger sequencing technology.After the possible pathogenic mutation was identified in the proband,the specific mutation was detected in the suspected individuals and part of other family members.Then the genetic diagnoses were concluded.Results:There were 5 family members in 4 generations manifested with epistaxis.According to the clinical diagnosis criteria,the proband with epistaxis,mucocutaneous telangiectases,visceral arteriovenous malformation and family history was diagnosed as HHT;while 2 survival family members with epistaxis and family history were suspected individuals.A substitution mutation in the 5'-untranslated region(5'-UTR) of ENG c.1-127 C>T was detected in the proband and the 2 suspected individuals,which did not exist in other family members.Based on the clinical and genetic findings,the 2 clinically suspected individuals were diagnosed as HHT.Conclusion:There is great variability of the clinical manifestations among HHT patients.ENG c.1127 C>T mutation is the possible pathogenic variant of the HHT family.A combination of clinical and genetic diagnosis could improve the diagnosis and treatment of HHT.
6.Acute hypervolemic hemodilution improves cognitive function and reduces plasma S100βin elderly lung cancer patients following radical surgery

Objective Radical surgery is extensively used in the treatment of lung cancer in elderly patients .This study aimed to investigate the effects of acute hypervolemic hemodilution (AHVHD) on the cognitive function and plasma S100βof elderly people following radical surgery for lung cancer . Methods A total of 112 lung cancer patients treated by radical surgery were equally randomized to an AHVHD and a non-AHVHD control group .Using the Mini-Mental State Examination scale ( MMSE) , we evaluated the cognitive function of the patients .We made comparisons between the two groups in the cerebral oxygen metabolism indexes and plasma S100βlevels at T0(immediately after anesthesia induction), T1(15 min after hemodilution), T2(60 min after hemodilution), T3(60 min before the end of surgery ) and T4 ( at the end of surgery ) as well as MMSE scores before and after operation . Results At T1 , T2, T3, and T4, the values of jugular venous oxygen saturation in the AHVHD group were (75.1 ±4.3), (78.5 ±3.1), (79.1 ± 4.4), and (79.6 ±2.8) %, respectively, with time-dependent decreases in Da-jvO2 and cerebral oxygen extraction , and significantly higher than that at T0 (P<0.05) and those in the control group ([63.4 ±2.0], [63.9 ±1.8], [64.2 ±3.0], and [64.7 ±3.2]%) (P<0.05).The levels of plasma S100βat T1, T2, T3, and T4 were remarkably lower in the AHVHD group ([0.53 ±0.04], [0.52 ±0.07], [0.51 ±0.06], and [0.50 ±0.08] μg/L) than in the control ([0.64 ±0.06], [0.64 ±0.04], [0.65 ±0.05], and [0.67 ±0.06] μg/L), with statistically significant differences between the two groups at the same time point (P<0.05).At 1, 3, 5, and 7 days after surgery, the MMSE scores were markedly higher in the AHVHD group (24.3 ±1.7, 26.5 ±1.9, 27.8 ±1.3, and 28.1 ±1.0) than in the control (21.1 ±1.6, 24.0 ±2.1, 25.9 ±1.8, and 26.4 ±1.7) (P<0.05). Conclusion AHVHD can significantly improve cognitive function and reduce plasma S 100βin elderly patients following radical surgery for lung cancer , which may be related to cerebral oxygen metabolism .
7.Effects of C-pseudonucleosides bearing thiazolidin-4-one as immunostimulants on differentiations of human lymphocytes

Objective To explore the effects of C-pseudonucleosides bearing thiazolidin-4-one as immunostimulants on differentiation and activation of human lymphocytes. Methods Peripheral blood mononuclear cells (PBMC) were isolated from healthy adults,and then incubated with immunostimulants (CH1a,CH2a,CH1b,CH2b and pidotimod).After 48 h,we collected the supernatants and then detected the concentrations of IL-2,IL-4 and IFN-γ using ELISA.After 72 h,the proliferation was detected using MTT method.PBMC incubated with immunostimulants (CH1a,CH2a,CH1b,CH2b and pidotimod),after 72 h,the cultural cells were collected and CD expressions of lymphocytes were analyzed by flow cytometry.Results All samples could stimulate proliferation of T cells.Immunostimulants CH1a,CH2a and pidotimod could elevate the expressions of CD3,CD4,CD19 and CD16CD56,and stimulate the secretions of IL-2 and IFN-γ. Immunostimulants CH1b and CH2b could elevate the expressions of CD3,CD4,CD19 and CD16CD56,and stimulate the secretions of IL-2 and IL-4. Conclusion Immunostimulants CH1a and CH2a could differentiate Th0 into Th1 and promote the proliferation of B cells as well as NK cells.However,immunostimulants CH1b and CH2b could differentiate Th0 into Th2 and promote the proliferation of B cells and NK cells.
8.Pneumatic Circulation Treatment on Lower Extremity Swelling and Blood Flow Velocity after Hip Replacement

ObjectiveTo investigate the effect of pneumatic circulation treatment on lower extremity swelling and blood flow velocity after hip replacement.Methods48 patients after hip replacement were divided into two groups: treatment group (n=24) and control group (n=24). The treatment group received pneumatic circulation treatment and continuous passive motion, while the control group only received continuous passive motion 2 d after operation, all patients wearing elastic stockings. Circumference of the affected leg was measured and Visual Analogue Scale (VAS) was assessed, and Doppler ultrasonography was to check the blood flow velocity of femoral and deep veins in lower limb before, 3 d and 7 d after rehabilitation.ResultsThere was no difference in circumference, VAS, and the lower limb blood flow velocity between the two groups before operation (P>0.05). There was significant difference in circumference and VAS 3 d after treatment (P<0.01), and a significant difference in circumference and blood flow velocity (P<0.01) but no difference in VAS (P>0.05) 7 d after treatment between the two groups.ConclusionPneumatic circulation treatment may facilitate to eliminate lower extremity swelling and prevent deep vein thrombosis after total hip replacement.
9.Comparative study of ultrasonography-guided optical imaging system and full-field digital mammography in breast neoplasms

Objective To compare the diagnostic values of US-guided optical imaging system (OPTIMUS) and full-field digital mammography (FFDM) in breast neoplasms. Methods Sixty-four patients suspected of breast neoplasms underwent OPTIMUS and FFDM before surgery. All patients were proved histopathologically. The accuracy of diagnosis was compared with chi-square test. Results Sixty-five masses were delineated by OPTIMUS and FFDM. The diagnostic accuracy, sensitivity and specificity of OPTIMUS were 86. 2% ( 56/65 ), 87.0% ( 20/23 ) and 85.7% ( 36/42 ), respectively. The positive likelihood ratio was 6. 09 and the negative likelihood ratio was 0. 175. The diagnostic accuracy, sensitivity and specificity of FFDM were 73.8% (48/65), 82. 6% ( 19/23 ) and 71.4% (30/42), respectively. The positive likelihood ratio was 2. 892 and the negative likelihood ratio was 0. 243. There were no significant differences between OPTIMUS and FFDM ( x2 = 3. 077 ,P > 0. 05). Conclusion OPTIMUS is similar and supplementary to FFDM for the diagnosis of breast neoplasms.
10.Changes in androgen receptor protein and mRNA expressions in intracardiac ganglia of orchidectomized rats

BACKGROUND:Most foreign and domestic studies on the effect of androgen receptor(AR) on cardiovascular system are limited to myocardium.However,cardiac nervous system plays an important role in the cardiac functional regulation.OBJECFIVE:To observe the AR protein and mRNA expressions in intracardiac ganglia of male orchidectomized rats,to investigate whether androgen affects the expressions,and to make a comparison with normal rats and testosterone-substituted rats after orchiectomy.DESIGN:A randomized controlled experiment.SETTING:Department of Anatomy,Medical College,Wuhan University of Science and Technology;Department of Anatomy,Tongji Medical College,Huazhong University of Science and Technology.MATERIALS:This study was carried out in the Department of Anatomy,Medical College,Wuhan University of Science and Technology between January and May 2003.Sixty healthy adult male Wistar rats,with body mass of 150 to 250g,were provided by the Animal Experimental Center,Tongji Medical College,Huazhong University of Science and Technology.METHODS:Sixty rats were randomized into 3 groups:①Control group,in which,testicles of rats were untouched.②Orchiectomy group,in which,bilateral testicles of rats were dissected.③Testosterone group,in which,the orchidectomized rats were injected with testosterone,0.2 mg once,once every 3 days,3 weeks totally.AR protein and its mRNA expressions in intracardiac ganglia of male orchidectomized rats were detected by immunohistochemical method and in situ hybridization method.MAIN OUTCOME MEASURES:The positive rate of AR-positive cells in intracardiac ganglia (the number of positive cells/the number of total cells×100%)and its mean absorbance.RESULTS:All the 60 rats were involved in the final analysis.AR-positive nerve cells were found in all the intracardiac ganglia of atrial posterior wall in the 3 groups;The positive rate of AR protein and mRNA positive nerve cells in intracardiac ganglia and its mean absorbance were significantly lower in the orchiectomy group than in the control group and in the testosterone group (P<0.05).But no obvious differences in the positive rate and mean absorbance existed between testosterone group and control group(P>0.05).CONCLUSION:AR exists in the intracardiac ganglia of atrial posterior wall,and it is regulated by androgen.
