1.Bioinformatics Analysis and Experimental Validation of the Mechanism of Leigongteng (Tripterygium wilfordii Hook. f.) in Treating Rheumatoid Arthritis

ObjectiveTo explore the potential mechanisms of Leigongteng (Tripterygium wilfordii Hook. f.) in treating rheumatoid arthritis (RA) using bioinformatics analysis and experimental validation. MethodsBioinformatics approaches, including the Gene Expression Omnibus (GEO), the traditional Chinese medicine Systems Pharmacology Database and Analysis Platform (TCMSP), Gene Ontology (GO) enrichment analysis, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment, protein-protein interaction (PPI) network analysis, molecular docking, receiver operating characteristic (ROC) analysis, and immune infiltration analysis, were used to predict the key active components of Leigongteng and its target genes for RA treatment. Experimental validation was conducted using human rheumatoid arthritis fibroblast-like synoviocytes (HFLS-RA) in vitro, with methotrexate as the positive control. A scratch assay was performed to assess cell migration after 24 hours of culture. Western blotting was used to detect protein expression levels, qPCR was used to measure target gene mRNA levels, and ELISA was conducted to evaluate inflammatory cytokine levels, including interleukin-1β (IL-1β), interleukin-6 (IL-6), interleukin-10 (IL-10), and tumor necrosis factor-α (TNF-α). ResultsA total of 117 target genes of Leigongteng were identified and intersected with RA-related genes, yielding 55 key genes. Further screening identified three core genes: PTGS2, CXCR4, and TIMP1. Based on the correspondence between potential drug targets and key components, triptolide and nobiletin were identified as the primary active compounds. Molecular docking results showed that both triptolide and nobiletin had binding energies lower than -5 kcal/mol with their respective target proteins, indicating strong interactions. In vitro experiments demonstrated that, compared with the blank control group, the triptolide, nobiletin, and positive control groups exhibited reduced cell migration rates after 24 hours of culture (P<0.01). The expression levels of PTGS2 and CXCR4 (both mRNA and protein) were significantly downregulated, while TIMP1 expression was upregulated. Levels of IL-1β, IL-6, and TNF-α decreased, whereas IL-10 levels increased (P<0.01). Compared with the positive control group, the triptolide and nobiletin groups showed increased cell migration rates, upregulated PTGS2 and CXCR4 expression (mRNA and protein), downregulated TIMP1 expression (mRNA and protein), increased IL-1β, IL-6, and TNF-α levels, and decreased IL-10 levels (P<0.05 or P<0.01). ConclusionThe key active components of Leigongteng, triptolide and nobiletin, may alleviate RA by inhibiting PTGS2 and CXCR4 while promoting TIMP1 expression, thereby suppressing inflammatory responses.
2.Evaluation of endometrial receptivity in infertile patients using multimodal ultrasound combined with sex hormone detection

Objective To explore the value of multimodal ultrasound combined with sex hormone detection in evaluating endometrial receptivity in infertile patients.Methods Ninety-five infertile patients who received in vitro fertilization and embryo transfer treatment in Hangzhou Women's Hospital from January 2020 to February 2023 were selected as study objects,and they were divided into pregnant group(55 cases)and non-pregnant group(40 cases)according to the pregnancy status of the first embryo transfer.Sex hormone levels and multimodal ultrasound scores were compared between two groups,and receiver operating characteristic(ROC)curve was drawn to evaluate the value of each indicator in predicting pregnancy of infertile patients individually and in combination.Results The endometrial thickness score,endometrial volume score,vascular blood flow score and multimodal ultrasound score of pregnant group were significantly higher than those of non-pregnant group(P<0.05).The levels of luteinizing hormone(LH),estradiol(E2)and progesterone(P)in pregnant group were significantly higher than those in non-pregnant group(P<0.05).ROC curve results showed that the area under the curve(AUC)of LH,E2 and multimodal ultrasound scores in predicting pregnancy in infertile patients were all>0.7,which had certain predictive value,and the three indicators combined with P had the highest predictive value,with an AUC of 0.889.Conclusion The combination of multimodal ultrasound and sex hormone detection can effectively evaluate the endometrial receptivity of infertile patients,and has certain reference value for predicting the pregnancy status of infertile patients.
3.Construction and application assessment of a dispatching system based on UWB positioning technique for emergency equipment

Objective:To construct a dispatching system based on ultra wide band(UWB)positioning technique,so as to enhance dynamic management capabilities for equipment and clinical response efficiency.Methods:The dispatching system based on UWB positioning technique for emergency equipment included internet of things(IoTs)perception layer of network,network layer,data service layer,and business application layer of intelligently medical equipment,which integrated the positioning technique with UWB centimeter level and the deep Q-network(DQN)algorithm,and it can realize real-time localization and intelligent dispatching decisions for equipment.The used thirty-five emergency equipment in the intensive care unit(ICU)ward of Eastern Branch of Ningbo Medical Center Li Huili Hospital from January 2023 to December 2024 were selected.Based on different management modes,25 equipment using conventional manual dispatching during January and June 2023 were divided into control group,while 25 equipment using the UWB dispatching technique-based dispatching system during July and December 2023(included 15 equipment of the original control group and 10 new equipment at later stage)were used as observation group.The dispatching efficiency,utilization rate,positioning precision,and maintenance efficiency of equipment between two groups were compared.Results:The dispatching time for equipment in the observation group was(7.2±0.8)min,which significantly shorter than(12.2±1.5)min in the control group,and the difference was significant(t=18.34,P<0.05).The distance of movement,and response time for fault in the observation group were significantly lower than those in the control group,and the utilization rate of equipment was significantly higher than that in the control group,and the differences were significant(t=15.67,14.28,6.92,P<0.05).The repair rate for fault in the observation group was significantly higher than that in the control group,and the difference was significant(x2=7.80,P<0.05).The positioning error of the system is(35±10)cm.Conclusion:The UWB-based dispatching system can significantly optimize the dispatching process for resource of emergency equipment,which can improve utilization and management efficiency for equipment.It can provide reliably technical support for rapid response in clinical emergency.
4.Effects and Mechanism of Quercetin on Osteogenic Differentiation of BMSCs

Objective To explore the effect and mechanism of quercetin on osteogenic differentiation of bone marrow mesenchymal stem cells(BMSCs).Methods BMSCs were divided into a blank control group(Control)and quercetin(Quercetin)low-dose group(4.8 mL/kg),medium-dose group(9.6 mL/kg),and high-dose group(19.2 mL/kg)intervened with drug-containing serum,while the positive control group was treated with osteogenic differentiation medium,respectively.The cell cycle was analysed by flow cytometry,cell proliferation was detected by MTT assay,cell activity was determined by Alkaline phosphatase(ALP)assay kit,and calcified nodule formation was observed by alizarin red staining.The expression of β-catenin and the key factors of osteogenic differentiation,runt-related transcription factor 2(RUNX2)and osteocalcin(OCN),were detected by qPCR and Western blot,respectively.Results Compared with the control group,quercetin-containing serum significantly promoted the proliferation of BMSCs(P=0.000205,P=0.000063)and enhanced the formation of calcium nodules,and increased osteogenic and ALP activity after osteogenic differentiation.The results of qPCR and Western blot showed that the quercetin group significantly up-regulated the mRNA and protein expression of β-catenin(P<0.0001),RUNX2(P<0.0001)and OCN(P<0.0001)during osteogenic differentiation.Conclusion Quercetin can effectively promote the osteogenic differentiation of BMSCs,and its mechanism is achieved by activating the Wnt/β-catenin signaling pathway and up-regulating the expression of the osteogenesis-related transcription factor RUNX2.
5.Multidisciplinary management of a pregnant woman with PAX2 gene variant presenting solitary kidney and chronic kidney disease stage 4: a case report

Pregnancy with chronic kidney disease (CKD), particularly in stages 4-5, carries high risks of adverse outcomes including maternal renal failure, preeclampsia/eclampsia, fetal growth restriction, and preterm birth. This report described a 26-year-old woman with congenital solitary kidney, polycystic ovaries, and uterine septum due to PAX2 gene variant, complicated by CKD stage 4. Through multidisciplinary team precision management and individualized treatment strategies, including timely initiation of dialysis, the patient successfully maintained pregnancy until 34 +1 weeks and delivered a female infant via cesarean section. This case summarizes key management experiences for end-stage renal disease in pregnancy, highlighting early risk assessment, precise nutritional management, hemodialysis protocol optimization, and the crucial role of multidisciplinary collaboration, providing valuable references for managing CKD-complicated pregnancies.
6.Study on the mechanism of Jiawei Jisheng Shenqi Decoction regulating HIF-1α/Notch pathway to improve hypoxia and antagonize liver cirrhosis

Objective To explore the mechanism of Jiawei Jisheng Shenqi Decoction in improving the hypoxic microenvironment and antagonizing liver cirrhosis.Methods In vivo experiments were conducted using a rat model of carbon tetrachloride(CCL4)-induced liver cirrhosis.Rats were divided into normal,model,colchicine,JWJSSQ low-dose,JWJSSQ medium-dose,and JWJSSQ high-dose group.Pathological changes in liver tissues in each group were examined by hematoxylin and eosin(HE)and Masson staining,changes in serum liver function were detected using test kits,levels of hyaluronic acid(HA),laminin(LN),procollagen Ⅲ(PC Ⅲ),and collagen typeⅣ(COL4)were detected by enzyme-linked immunosorbent assay(ELISA),and protein expression levels of hypoxia-inducible factor-1α(HIF-1α),Notch1,Jagged1,and α-smooth muscle actin(α-SMA)were detected by Western blot.In vitro experiments were conducted in HSC-T6 cells,and the optimal concentration of CoCl2(100 μ mol/L,200μmol/L,400 μmol/L,600 μmol/L and 800 μmol/L)in the cultured cells and the optimal concentration of drug-containing serum(5%,10%,15%,20%)were determined by Cell Counting Kit-8(CCK-8)assay.The migration ability of cells in each group was detected by scratch testing,and changes in the apoptosis rates were determined by flow cytometry.Protein expression levels of HIF-1α,Notch1,Jagged1,α-SMA,matrix metallopeptidase 9(MMP9),and tissue inhibitor of metalloproteinases 1(TIMP-1)were detected by Western blot.Results In the in vivo experiments,liver swelling,inflammatory cell infiltration,collagen deposition,and the appearance of pseudolobules were significantly increased in the model group compared with those in the normal group.Serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),HA,LN,PCⅢ,and COL4 were significantly increased and albumin(ALB)was significantly decreased in the model group,while liver levels of HIF-1α,Notch1,Jagged1,and α-SMA proteins were significantly increased(P<0.01).Liver swelling,inflammatory cell infiltration,and collagen deposition were significantly reduced in each treatment group compared with those in the model group,and the degree of fibrosis was reduced.Serum ALT,AST,HA,LN,PCⅢ,and COL4 were significantly decreased and ALB was significantly increased,while liver levels of HIF-1α,Notch1,Jagged1,and α-SMA proteins were also significantly decreased to varying degrees(P<0.05).In the in vitro experiments,hypoxia promoted HSC-T6 migration and reduced apoptosis,increased the protein expression levels of HIF-1α,Notch1,Jagged1,α-SMA,and TIMP-1,and reduced the expression levels of MMP9(P<0.01).Serum containing Jiawei Jisheng Shenqi Decoction inhibited HSC-T6 migration,promoted HSC-T6 apoptosis,lowered the expression of HIF-1α,Notch1,Jagged1,α-SMA,and TIMP-1 proteins,and enhanced the expression of MMP9 protein(P<0.01).The inhibitory effect of Jiawei Jisheng Shenqi on HSC-T6 cell activation was reversed by the HIF-1α agonist dimethyloxalylglycine.Conclusions Jiawei Jisheng Shenqi Decoction can improve the hypoxic microenvironment via the HIF-1α/Notch pathway,thereby exerting an anti-liver cirrhosis effect.
