1.Analysis of the current situation of self-paid medical examination projects for patients in a public medi-cal examination institution in Xinjiang

Objective By conducting a statistical analysis of self-paid health examination items among individuals at a public medical institution in Xinjiang in 2023,this study aims to understand the preferences of different demographic groups re-garding health examination items.Methods Data from self-paid health examinations in 2023 were collected and analyzed using statistical methods such as chi-square tests and binary logistic regression to examine the frequency of selection,differences,and influencing factors of health examination items among different demographic groups.Results ① The total number of research subjects was 11,228,including 5,205 males and 6,023 females,with ages ranging from 18 to 94 years old.② The frequency of choosing examination items for males ranked from highest to lowest in the top 5 were thyroid B-ultrasound,prostate and bladder B-ultrasound,color Doppler echocardiography,spiral CT of the lungs,and glycosylated hemoglobin determination.For females,the frequency of choosing examination items ranked from highest to lowest in the top 5 were thyroid B-ultrasound,breast B-ultra-sound,vaginal B-ultrasound,vaginal discharge routine test combination,and spiral CT of the lungs.③ Older individuals were more likely to choose color Doppler echocardiography,spiral CT of the lungs,and glycosylated hemoglobin determination.Youn-ger individuals were more likely to choose thyroid B-ultrasound,13-carbon urea breath test;those with lower educational attain-ment were more likely to choose thyroid B-ultrasound,color Doppler echocardiography,spiral CT of the lungs,glycosylated hemo-globin,and 13-carbon urea breath test;those with fewer physical examinations were more likely to choose thyroid B-ultrasound,color Doppler echocardiography,spiral CT of the lungs,and 13-carbon urea breath test.Conclusion By analyzing the prefer-ences and influencing factors of health examination items among different demographic groups,targeted plans can be proposed to assist medical institutions in designing more appropriate health examination packages tailored to diverse populations.
2.Construction and application assessment of a dispatching system based on UWB positioning technique for emergency equipment

Objective:To construct a dispatching system based on ultra wide band(UWB)positioning technique,so as to enhance dynamic management capabilities for equipment and clinical response efficiency.Methods:The dispatching system based on UWB positioning technique for emergency equipment included internet of things(IoTs)perception layer of network,network layer,data service layer,and business application layer of intelligently medical equipment,which integrated the positioning technique with UWB centimeter level and the deep Q-network(DQN)algorithm,and it can realize real-time localization and intelligent dispatching decisions for equipment.The used thirty-five emergency equipment in the intensive care unit(ICU)ward of Eastern Branch of Ningbo Medical Center Li Huili Hospital from January 2023 to December 2024 were selected.Based on different management modes,25 equipment using conventional manual dispatching during January and June 2023 were divided into control group,while 25 equipment using the UWB dispatching technique-based dispatching system during July and December 2023(included 15 equipment of the original control group and 10 new equipment at later stage)were used as observation group.The dispatching efficiency,utilization rate,positioning precision,and maintenance efficiency of equipment between two groups were compared.Results:The dispatching time for equipment in the observation group was(7.2±0.8)min,which significantly shorter than(12.2±1.5)min in the control group,and the difference was significant(t=18.34,P<0.05).The distance of movement,and response time for fault in the observation group were significantly lower than those in the control group,and the utilization rate of equipment was significantly higher than that in the control group,and the differences were significant(t=15.67,14.28,6.92,P<0.05).The repair rate for fault in the observation group was significantly higher than that in the control group,and the difference was significant(x2=7.80,P<0.05).The positioning error of the system is(35±10)cm.Conclusion:The UWB-based dispatching system can significantly optimize the dispatching process for resource of emergency equipment,which can improve utilization and management efficiency for equipment.It can provide reliably technical support for rapid response in clinical emergency.
3.Analysis of mental health policy for medical staff from the perspective of policy tools

Objective To take text analysis of the mental health promotion policies for Chinese medical staff,and provide guidance for the optimization and improvement of subsequent policies.Methods Two-dimensional analysis framework composed of policy tools and two-factor theory were established,and external attribute analysis and content analysis of 19 mental health promotion policies issued by the central level were conducted.Among the policy text types,the most notifications,followed by opinions and the least work plan.A total of 368 policy codes were selected in the included policy texts.Among the 180 codes in the dimension of policy tool,authority,incentive,capacity building,guidance and organizational change accounted for 19.44%,15.56%,28.89%,31.11%and 5.00%respectively.Among the 188 codes in the two-factor theoretical dimension,health care factors and incentives accounted for 63.83%and 36.17%,respectively.There are rich types of policy tools for mental health promotion of Chinese staff workers,but they show an unbalanced state and pay less attention to the motivation of medical staff.
4.Effects and Mechanism of Quercetin on Osteogenic Differentiation of BMSCs

Objective To explore the effect and mechanism of quercetin on osteogenic differentiation of bone marrow mesenchymal stem cells(BMSCs).Methods BMSCs were divided into a blank control group(Control)and quercetin(Quercetin)low-dose group(4.8 mL/kg),medium-dose group(9.6 mL/kg),and high-dose group(19.2 mL/kg)intervened with drug-containing serum,while the positive control group was treated with osteogenic differentiation medium,respectively.The cell cycle was analysed by flow cytometry,cell proliferation was detected by MTT assay,cell activity was determined by Alkaline phosphatase(ALP)assay kit,and calcified nodule formation was observed by alizarin red staining.The expression of β-catenin and the key factors of osteogenic differentiation,runt-related transcription factor 2(RUNX2)and osteocalcin(OCN),were detected by qPCR and Western blot,respectively.Results Compared with the control group,quercetin-containing serum significantly promoted the proliferation of BMSCs(P=0.000205,P=0.000063)and enhanced the formation of calcium nodules,and increased osteogenic and ALP activity after osteogenic differentiation.The results of qPCR and Western blot showed that the quercetin group significantly up-regulated the mRNA and protein expression of β-catenin(P<0.0001),RUNX2(P<0.0001)and OCN(P<0.0001)during osteogenic differentiation.Conclusion Quercetin can effectively promote the osteogenic differentiation of BMSCs,and its mechanism is achieved by activating the Wnt/β-catenin signaling pathway and up-regulating the expression of the osteogenesis-related transcription factor RUNX2.
5.Effect of Yishen Paidu Formula on inflammatory response in chronic renal failure rats by regulating Calcineurin/NFAT signal pathway

Objective:To explore effect of Yishen Paidu Formula on inflammatory response in chronic renal failure(CRF)rats and role of Calcineurin/nuclear factor of activated T cell(NFAT).Methods:CRF rat model was constructed,and randomly grouped into model group,low,medium and high doses Yishen Paidu Formula groups,with 10 rats in each group,another 10 rats were fed normally as a blank group;general situation of rats was recorded;urine protein quantification kit was applied to detect 24-hour urine protein level of CRF rats in each group;serum creatinine and urea nitrogen levels of CRF rats were detected;ELISA was applied to detect serum IL-1β,IL-6 and TNF-α levels in CRF rats;pathological changes of renal tissue were observed by HE staining;Western blot was applied to detect expressions of fibroblast growth factor 23(FGF23),Klotho protein,Calcineurin,NFAT protein in renal tissue of CRF rats in each group.Results:Compared with blank group,levels of creatinine,urea nitrogen in serum,24 h urine protein in urine,IL-1β,IL-6,TNF-α in serum,and protein levels of FGF23,Calcineurin,NFAT in kidney tissue were obviously increased in model group,level of Klotho protein was obviously decreased(P<0.05).Compared with model group,levels of creatinine,urea nitrogen in serum,24 h urine protein in urine,IL-1β,IL-6,TNF-α in serum,and protein levels of FGF23,Calcineurin,NFAT in kidney tissue were obviously decreased in low,medium and high doses Yishen Paidu Formula groups,level of Klotho protein was obviously increased,which were more significant with dosage increase(P<0.05).Conclusion:Yishen Paidu Formula may alleviate inflammatory response in CRF rats by inhibiting Calcineurin/NFAT signaling pathway.

8.Multidisciplinary management of a pregnant woman with PAX2 gene variant presenting solitary kidney and chronic kidney disease stage 4: a case report

Pregnancy with chronic kidney disease (CKD), particularly in stages 4-5, carries high risks of adverse outcomes including maternal renal failure, preeclampsia/eclampsia, fetal growth restriction, and preterm birth. This report described a 26-year-old woman with congenital solitary kidney, polycystic ovaries, and uterine septum due to PAX2 gene variant, complicated by CKD stage 4. Through multidisciplinary team precision management and individualized treatment strategies, including timely initiation of dialysis, the patient successfully maintained pregnancy until 34 +1 weeks and delivered a female infant via cesarean section. This case summarizes key management experiences for end-stage renal disease in pregnancy, highlighting early risk assessment, precise nutritional management, hemodialysis protocol optimization, and the crucial role of multidisciplinary collaboration, providing valuable references for managing CKD-complicated pregnancies.
