As an extension of the central nervous system, the retina is the only tissue in the body that allows direct in vivo observation of both neural and vascular structures, offering a unique non-invasive window for the assessment of cerebral neurovascular diseases. Recent studies have shown that multimodal imaging techniques—such as fundus photography, optical coherence tomography(OCT), and OCT angiography(OCTA)—enable quantitative analysis of retinal indicators including vascular caliber, fractal dimension, retinal nerve fiber layer thickness, and microvascular density. These metrics can, to some extent, reflect the severity and progression risk of cerebrovascular and neurodegenerative diseases such as stroke, Alzheimer's disease, and Parkinson's disease. Consequently, retinal imaging holds promise as a powerful tool for early screening, risk stratification, and treatment monitoring. This article systematically reviews the application of retinal imaging technologies in the diagnosis and management of cerebral neurovascular disorders, with a particular focus on the important role of artificial intelligence(AI)-driven image analysis in enhancing screening efficiency, improving risk assessment accuracy, and promoting multidisciplinary collaborative care, thereby providing insights for clinical translation.

Objective:This study explores the diagnostic efficiency of a modified semi-structured clinical interview (MSCI) based on the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition (DSM-5) for bipolar disorder (BD) by comparing MSCI with a traditionally structured interview and a self-reported diagnostic scale. Methods:MSCI, Structured Clinical Interview for DSM Axis Ⅰ Disorders-Patient Edition (SCID-Ⅰ/P), and 32-item hypomania checklist (HCL-32) were simultaneously administered in 445 outpatients with major depressive disorder ( n=100) or BD ( n=345). All patients were followed up for at least half a year and at least three times to confirm the diagnosis based on the clinical data, follow-up evaluation, and treatment efficacy. The sensitivity and specificity of the three diagnostic strategies were compared using the chi-square test. Results:The sensitivity of MSCI was significantly higher than that of SCID-Ⅰ/P (0.67 vs. 0.32, χ 2=82.10, P<0.001) but was similar to that of HCL-32 (0.67 vs. 0.70, χ 2=2.22, P=0.136). MSCI presented significantly lower specificity than SCID-Ⅰ/P (0.96 vs.1.0, χ 2=4.08, P=0.043) but significantly higher specificity than HCL-32 (0.96 vs.0.67, χ 2=27.89, P<0.001). For the diagnosis of bipolar Ⅰ disorder, no significant difference was found between the sensitivity of MSCI (1.0) and SCID-Ⅰ/P (0.73) (χ 2=3.47, P=0.062) and between the sensitivity of MSCI and HCL-32 (0.73) (χ 2=3.47, P=0.062). For bipolar Ⅱ disorder, the sensitivity of MSCI (0.90) was significantly higher than that of SCID-Ⅰ/P (0.41) (χ 2=92.49, P<0.001) and that of HCL-32 (0.66) (χ 2=22.51, P<0.001). For BD with mixed features, the sensitivity of MSCI (0.37) was significantly higher than that of SCID-Ⅰ/P (0.19) (χ 2=10.85, P<0.001) but lower than that of HCL-32 (0.80) (χ 2=49.40, P<0.001). For other specified bipolar and related disorders, the sensitivity of MSCI (0.41) did not significantly differ from that of SCID-Ⅰ/P (0.18) (χ 2=2.73, P=0.100) but was significantly lower than that of HCL-32 (0.70) (χ 2=4.53, P=0.033). Conclusion:Compared to SCID-Ⅰ/P and HCL-32, MSCI has better diagnostic efficiency for BD.

Chinese Critical Care Medicine 2022;34(8):873-874
At present, the extension tube of the ventilator, a long corrugated tube used in clinical practice, needs to be disconnected from the endotracheal tube when performing open sputum suction. Under positive pressure, the condensed water containing pathogenic microorganisms in the ventilator will be splashed. Some will form aerosol suspended in the air, pollute the air, increase the risk of cross-infection and operator pollution, while the ventilator will emit a corresponding alarm sound, resulting in noise pollution. Based on the above factors, a ventilator pipeline extension tube was designed by the medical staff of department of critical care medicine of the Eighth Medical Center of People's Liberation Army General Hospital and department of critical care medicine of Beijing Friendship Hospital of Capital Medical University, and was obtained the National Utility Model Patent of China (ZL 2020 2 1476518.X). The device consists of the main pipe, first pipe sleeve, second pipe sleeve, valve body, joystick, axial sleeve, etc., which can not only effectively avoid air pollution and reduce the risk of cross-infection, but also reduce noise pollution and the negative impact of loud noise on patients and medical staff.

Objective:To observe pelvic floor muscle′s contraction in women with the second and full term natural parturition by intelligent ultrasonic examination.Methods:Ninety-five postpartum women who underwent natural parturition and post-partum check (6-9 weeks after delivery) in Henan Provincal People′s Hospital were recruited in this study from Jul to Dec 2018. Fifty-four puerpera were first parturition and 41 were second parturition. The morphology of urogenital hiatus was observed during both rest and contract maneuver by real-time 3D ultrasound. Several parameters were measured by intelligent ultrasound, such as hiatal area, anteroposterior diameter, transverse diameter, and the thickness of puborectalis muscles, and the D-values of all parameters were calculated in different maneuver. Meanwhile, the strength of pelvic muscle in two groups were measured and the ultrasonic diameters and muscle strength were compared. Results:The hiatal area, anteroposterior diameter, transverse diameter, and the thickness of bilateral puborectalis muscles between first natural parturition and second natural parturition during rest maneuver were (15.92±2.76)cm 2 vs (16.65±2.63)cm 2, (40.93±5.63)mm vs (40.27±6.21)mm, (55.54±6.05)mm vs (57.92±5.27)mm, (7.03±1.51)mm vs (7.44±1.23)mm, (7.49±1.44)mm vs (7.44±1.44)mm, there was no statistical difference( P>0.05). During contract maneuver, these data were (11.76±2.62)cm 2 vs (12.09±2.78)cm 2, (37.57±5.46)mm vs (37.18±4.71)mm, (42.21±5.69)mm vs (43.03±7.13)mm, (8.92±1.54)mm vs (8.87±1.23)mm, (8.90±1.60)mm vs (9.30±2.71)mm, there was no statistical difference( P>0.05). The D-values of all diameters were (4.16±2.38)cm 2 vs (4.43±2.70)cm 2, 3(1.00, 5.00)mm vs 1(1.00, 5.25)mm, (13.33±6.07)mm vs (14.41±6.54)mm, (1.90±1.78)mm vs (1.68±1.31)mm, 1(0.48, 2.40)mm vs 1.25(0.20, 2.13)mm, there was no statistically significant difference between two groups ( P>0.05). The non-eligibility rate of typeⅠmuscle fiber strength in two groups were 65% vs 68%(35 vs 28) and typeⅡmuscle fiber strength were 74% vs 79%(40 vs 32), there was no statistically difference between two groups ( P>0.05). Conclusions:In comparison to the primary and full term natural parturition women, there is no significant reduction of the pelvic floor muscle′s contraction by intelligent ultrasund in second and full term natural parturition women without abnormal pregancy history and complications. This could provide theoretical basis for the choice of the mode of second parturition and the rahabilitation of postpartum.

Chinese Journal of Rheumatology 2017;21(9):601-604
Objective To evaluate the clinical significance of serum thrombospondin-1 (TSP-1) in systemic lupus erythematosus (SLE), and explore its possible involvement in SLE pathogenesis. Methods One hundred and thirty-eight patients diagnosed with SLE, including 124 cases of females and 14 males, as well as 60 healthy controls were recruited into this study. Enzyme-linked immunosorbent assay (ELISA) was used to determine serum TSP-1 expression level between the two groups. Spearman correlation analysis method was used to analyze the correlation between TSP-1 level of complement 3, complement 4, anti-double-stranded DNA, Rheumatoid factor and systemic lupus erythematosus disease activity index (SLEDAI). Results TSP-1 level in healthy control group was much higher than that in SLE patients. TSP-1 serum levels in SLE patients was positively correlated with complement 3 (r=0.386, P<0.01), complement 4 (r=0.301, P<0.01), and negatively correlated with anti-double-stranded DNA (r=-0.221, P=0.009), RF (r=-0.186, P=0.029) and SLEDAI (r=-0.273, P=0.001). Conclusion TSP-1 may play an immune-regulatory role in the development of SLE by inhibiting inflammation and autoantibody production. It could become a potential therapeutic target for the treatment of SLE.

Journal of Central South University(Medical Sciences) 2011;36(9):859-864
OBJECTIVE:
To study the efficacy of allogeneic hemotopoietic stem cell transplantation (allo-HSCT) for hematological malignancy.
METHODS:
A total of 104 patients with hematological malignancy, who underwent allo-HSCT in Xiangya Hospital from December 1999 to January 2010, were retrospectively analyzed. Of the patients, the transplantation related mortality (TRM), relapse rate (RR), 5-year overall survival (OS) and disease free survival (DFS) were estimated by Kaplan-Meier analysis. The unfavorable prognostic factors were also statistically examined.
RESULTS:
Hematopoietic reconstitution was achieved in 101 patients. At the last data of follow-up, the incidences of severe acute graft versus host disease (aGVHD) and extensive chronic GVHD were 15.38% and 25.53%, and the TRM and RR were 15.66% and 21.76%, respectively. The estimated 5-year OS and DFS for all patients were (73.49±4.59)% and (63.10±5.32)%, respectively. Those for acute myeloid leukemia (AML) patients were (63.00±9.51)% and (49.30±9.96)%, and those for chronic myeloid leukemia (CML) patients were (83.87±5.06)% and (74.55±6.79)%, respectively. The survival analysis suggested the poor prognostic factors for allo-HSCT recipients including female sex, severe aGVHD and refractory hematological malignancy. Further multivariate analyses revealed that severe aGVHD and refractory hematological malignancy were the independent risk factors of poor prognosis for the recipients (P<0.05). The 5-year DFS of severe aGVHD and refractory hematological malignancy patients was (48.22±12.69)% and (42.09±12.31)%, respectively. The TRM of severe aGVHD, HLA-mismatched graft and unrelated donor transplant was significantly higher than that of the corresponding control groups (57.14% vs. 4.81%, 33.33% vs. 10.41%, 26.09% vs. 9.28%; P<0.05). The RR of refractory hematological malignancy was significantly higher than that of the control group (41.09% vs. 15.63%, P<0.05).
CONCLUSION
The treatment of allo-HSCT can improve the disease free survival of patients with hematological malignany and is an important therapeutic method for hematological malignancy. Severe aGVHD and refractory hematological malignancy are the independent risk factors of poor prognosis for the allo-HSCT recipients with hematological malignancy.
