1.Clinical Application and Pharmacological Effects of Guizhi Fulingwan in Treatment of Uterine Fibroids: A Review

Uterine fibroids are a common benign tumor of the female reproductive system, characterized by increased menstrual flow, lower abdominal pain, and prolonged menstrual periods. Modern medicine believes that the onset of this disease is related to genetic factors, environmental factors, hormone levels, et al, while the specific mechanism remains unclear, and the prevention and treatment of uterine fibroids has become a hot topic of concern for many experts and scholars in the medical field. At present, the treatment of uterine fibroids in clinical practice is mainly based on hormone drugs and uterine artery embolization, and severe cases require hysterectomy. However, the use of hormone drugs for treatment has serious side effects and is prone to recurrence after surgery. Since hysterectomy can cause severe harm to women, it is necessary to explore safer and more effective treatment methods. Traditional Chinese medicine (TCM) has rich clinical experience in the treatment of uterine fibroids, advocating for syndrome differentiation and treatment. The TCM methods that regulate Qi and blood and balance Yin and Yang have been commonly adopted, with significant efficacy and minimal side effects, being more conducive to the recovery. Guizhi Fulingwan are first recorded in the Synopsis of the Golden Chamber written by the famous medical expert ZHANG Zhongjing during the Eastern Han dynasty. This prescription has the effects of activating blood, resolving stasis, and eliminating mass, and it is thus mainly used for treating abdominal mass in women. In recent years, Guizhi Fulingwan has also been applied in the clinical treatment of tumors and has demonstrated definite efficacy in the treatment of uterine fibroids. Studies have shown that the therapeutic mechanisms of Guizhi Fulingwan for uterine fibroids involve regulating cell proliferation and apoptosis, improving immune function, reducing inflammation, improving hemorheological indicators, inhibiting tumor angiogenesis, and regulating sex hormone levels. This article mainly reviews the treatment of uterine fibroids with Guizhi Fulingwan from three aspects: theoretical basis, clinical application, and pharmacological mechanism. It is expected to provide scientific research ideas and guidance for exploring the clinical treatment of uterine fibroids.

Traditional Chinese medicine (TCM) injections, a hallmark achievement in the modernization of TCM, hold significant importance in advancing the development of TCM and enhancing clinical therapeutic effects. However, with the expanded clinical application of TCM injections, safety concerns have garnered widespread attention, particularly allergic reactions. Numerous researchers have delved into the safety issues of TCM injections. This paper summarized the research related to allergic reactions of TCM injections, including the types of allergic reactions, the allergenic components, their evaluation models and detection methods, as well as the challenges encountered in current research, aiming at providing references and basis for in-depth research and safe application of allergic reactions of TCM injections.
3.Surveillance and early warning index system for schistosomiasis in the middle and lower reaches of the Yangtze River basin

Under the current situation of "low prevalence and low infection" of schistosomiasis in China, and to provide a basis for achieving the goal of eliminating schistosomiasis by 2030 proposed by the Healthy China Action (2019 - 2030) as scheduled, the Hunan Provincial Corps Hospital of the Chinese People's Armed Police Force established a schistosomiasis monitoring and early warning index system based on the previous studies on schistosomiasis early warning index system and the recent literature analysis, combined with the current potential risk factors affecting the transmission and prevalence of schistosomiasis, and organized two rounds of expert consultation and carried out project promotion meetings. The experts reached a consensus on the comprehensiveness and practicability of the index system, aiming to lay a solid foundation for construction of China's schistosomiasis prevention and control early warning system.
4.Impact of pathological aging of the endometrium on embryo implantation

Decreased endometrial receptivity is a critical factor contributing to the decline in fertility among women of advanced age. With the increase of age, the endometrium exhibits diminished hormonal responsiveness, a phenomenon known as inflammaging, and an imbalance in immune cell populations. These changes impede the processes of endometrial epithelial-mesenchymal transition and decidualization, thereby negatively impacting embryo implantation and reducing overall fertility potential. Furthermore, adverse lifestyle choices, exposure to environmental factors, inflammatory conditions, and repeated interventions within the uterine cavity can result in pathological aging of the endometrium that does not align with its chronological age. This misalignment may be associated with the emergence of various reproductive disorders, even in younger women. This paper seeks to investigate the relationship between pathological aging of the endometrium and reproductive disorders, particularly in the context of embryo implantation, with the aim of providing new insights into the pathogenesis, clinical diagnosis, and treatment of reproductive disorders.
5.Impact of Wenyang Huazhuo Formula on spontaneous pregnancy outcomes in advanced-age infertile patients with spleen-kidney yang deficiency syndrome

Objective:To evaluate the therapeutic efficacy of the Wenyang Huazhuo Formula on spontaneous pregnancy outcomes in advanced-age infertile patients with spleen-kidney yang deficiency syndrome.Methods:A retrospective cohort study design was employed to gather clinical data from 303 advanced-age infertile patients with spleen-kidney yang deficiency syndrome who sought treatment at the Department of Reproductive Medicine in the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine between June 2019 and June 2023. Patients were categorized into two groups based on their treatment regiments: a basic treatment group (non-exposed group, n=151) and a group receiving basic treatment in conjunction with the Wenyang Huazhuo Formula (exposed group, n=152). The study compared and analyzed pregnancy outcomes, including time to pregnancy (TTP) within one year, ongoing pregnancy rate, and clinical pregnancy rate, between the two groups over a one-year follow-up period. Additionally, stratified analyses were performed based on age and anti-Müllerian hormone (AMH) levels to evaluate the impact of Wenyang Huazhuo Formula on pregnancy outcomes across different patient subgroups. Results:The TTP in the exposed group was significantly reduced compared with the non-exposed group ( HR=1.70, 95% CI: 1.12-2.57, P=0.013), suggesting a significant shortening of their TTP, and the ongoing pregnancy rate [28.29% (43/152)] and the clinical pregnancy rate [33.55% (51/152)] were significantly higher than those in the non-exposed group [13.91% (21/151), P=0.020; 21.85% (33/151), P=0.023], and the early abortion rate [15.69% (8/51)] was significantly reduced compared with the non-exposed group [36.36% (12/33), P=0.031]. Stratified analyses by age revealed that for patients aged 35-37 years, the exposed group exhibited a significantly shorter TTP ( HR=1.67, 95% CI: 1.03-2.70, P=0.038), while no significant difference in TTP was observed in the 38-44-year old patients ( HR=1.78, 95% CI: 0.77-4.10, P=0.174). Regarding the ongoing pregnancy rate, the exposed group showed a significant higher rate [31.31% (31/99)] than the non-exposed group [13.91% (15/112), P=0.022] among patients aged 35-37 years, whereas no significant difference was observed in the 38-44 years old group ( P=0.386). Stratified analysis based on AMH levels showed that both low AMH levels (<1.68 μg/L) and high AMH levels (≥1.68 μg/L) patients in the exposed group experienced a significant reduction in TTP compared with the non-exposed group ( HR=8.07, 95% CI: 1.04-62.52, P=0.046; HR=1.57, 95% CI: 1.01-2.43, P=0.044, respectively), along with a notable improvement in the ongoing pregnancy rate [13.64% (6/44) vs. 0% (0/29), P=0.038; 34.26% (37/108) vs.17.21% (21/122), P=0.028, respectively]. Further stratified analysis indicated that, compared with patients with low AMH levels, both the exposed group and the non-exposed group of patients with high AMH levels exhibited a significantly shorter TTP ( HR=1.91, 95% CI: 1.02-3.69, P=0.043; HR=9.48, 95% CI: 1.30-69.22, P=0.002, respectively), along with a significantly higher ongoing pregnancy rate ( P=0.014; P=0.008). Conclusion:The Wenyang Huazhuo Formula significantly shortens TTP, enhances both the clinical pregnancy rate and the ongoing pregnancy rate and reduces the early abortion rate in advanced-age infertile women diagnosed with spleen-kidney yang deficiency syndrome.

Kinesiophobia is prevalent among school-aged children with asthma,and severe kinesiophobia can hinder their participation in physical activities and disease control,adversely affecting their physical and mental health.This paper reviews the concept,development,assessment tools,influencing factors,and intervention strategies of kinesiophobia in school-aged children with asthma,analyzes existing challenges,and proposes directions for future research.It aims to provide insights for developing scientific and personalized intervention plans to enhance children's exercise levels and improve their quality of life.
7.Effect and mechanism of glutamine enhanced enteral nutrition on Treg/Th17 balance in rats with severe acute pancreatitis

Objective:To explore the effect and mechanism of glutamine(Gln)enhanced enteral nutrition on regulatory T cells/T helper cell 17(Treg/Th17)balance in rats with severe acute pancreatitis(SAP).Methods:Eighty rats were randomly divided into Sham group,SAP group,enteral nutrition group(EN)and Gln+EN group,with 20 rats in each group.SAP model were constructed by retrograde injection of 5%sodium taurocholate into the pancreatic duct.After 4 hours of modeling,nasojejunostomy catheterization was used to nutritional treatment,EN group and Gln+EN group were given enteral nutrition solution and added Gln enteral nutrition solution,respectively;sham group and SAP group were given equal volume of 0.9%NaCl solution.After 48 hours,HE staining was used to detect pathological damages in pancreatic tissue;ELISA was used to detect amylase,lipase,IL-6,TNF-α,IL-10 and IL-17 levels in serum;flow cytometry was used to detect Th17 and Treg cells proportion in spleen and pancreas;RT-qPCR was used to detect Foxp3,CTLA-4,GITR,RORγt,IL-17A mRNA levels in spleen;immunohistochemical staining,immunofluorescence staining,Western blot were used to detect MPO,CD68,IL-6,TNF-α,IL-10,IL-17 levels in pancreas.Results:Pancreatic structure in Sham group was normal,without bleeding or necrosis;pancreatic structure in SAP group was disrupted with significant bleeding,necrosis and inflammatory infiltration;pancreatic structure in EN group and Gln+EN group were only a small amount of bleeding,necrosis and inflammatory infiltration.Compared with Sham group,histological score,amylase and lipase levels in serum in SAP group were increased,and MPO+neutrophils and CD68+macrophages in pancreas were increased,IL-6,TNF-α,IL-17 levels in serum and pancreas were increased,IL-10 level was decreased,Th17 cell percentage,RORγt,IL-17A mRNA levels were increased,while Treg percentage,Treg/Th17,Foxp3,CTLA-4 and GITR mRNA levels were decreased(all P<0.05);compared with SAP group,histological score,amylase and lipase levels in serum in EN group and Gln+EN group were decreased,and MPO+neutrophils and CD68+macrophages in pancreas were decreased,IL-6,TNF-α,IL-17 levels in serum and pancreas were decreased,IL-10 level was increased,Th17 cell percentage,RORγt,IL-17A mRNA levels were decreased,while Treg cell percentage,Treg/Th17,Foxp3,CTLA-4 and GITR mRNA levels were increased(all P<0.05);and Gln+EN has a better therapeutic effect on SAP than EN(all P<0.05).Conclusion:Gln enhanced enteral nutrition can regulate Treg/Th17 balance in SAP rats and reduce systemic inflammatory response.
