Lung cancer has the highest morbidity and mortality rate among all cancers. Because of the complex pathogenesis, there are limitations in the common Western medicine treatment methods. Clinical and experimental studies have proved that traditional Chinese medicine (TCM) can not only effectively treat lung cancer and alleviate the clinical symptoms of cancer patients but also reduce the adverse reactions and complications caused by surgery, chemotherapy, and radiotherapy to improve the quality of life of the patients. The biological behaviors of lung cancer cells, such as proliferation, invasion, and metastasis, are closely related to their metabolic reprogramming. Metabolic reprogramming in lung cancer involves a series of metabolic changes such as increased glucose uptake and consumption, enhanced glycolysis, increased amino acid uptake and catabolism, and enhanced lipid and protein synthesis. Studies have reported that TCM active components, extracts, and compound prescriptions can effectively inhibit the biological behaviors of lung cancer by regulating metabolic reprogramming. Therefore, this paper reviews the pharmacological mechanisms of TCM active components, extracts, and compound prescriptions in regulating metabolic reprogramming of lung cancer, with the aim of providing a new way of thinking for the treatment of lung cancer by TCM regulation of metabolic reprogramming of lung cancer cells. The available studies suggest that TCM mainly inhibits the extracellular signal-regulated protein kinase (ERK)/c-Myc, phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR), and hypoxia-inducible factor-α (HIF-1α) pathways. Furthermore, the expression of monocarboxylate transporter 4 (MCT4), glucose transporter 1 (GLUT1), pyruvate dehydrogenase (PDH), phosphofructokinase 1 (PFK1), pyruvate dehydrogenase kinase 1 (PDK1), pyruvate kinase M2 (PKM2), hexokinase (HK), lactate dehydrogenase (LDH), and lactate dehydrogenase A (LDHA) are inhibited. In this way, TCM inhibits the glucose uptake by lung cancer cells and glycolysis in lung cancer cells to reduce the energy metabolism of tumor cells, ultimately achieving the therapeutic effect on lung cancer.

Objective:To investigate the clinical value of 18F-NaF PET/CT coronary plague imaging in evaluating the long-term prognosis of patients with coronary artery disease (CAD). Methods:A retrospective cohort study was conducted among 54 patients (37 males and 17 females, aged (57.2±9.8) years) diagnosed with CAD from a multicenter study between September 2015 and October 2022. All patients underwent 18F-NaF PET/CT and coronary angiography (CAG) within 1 week, and the PET/CT imaging was performed at the First Hospital of Shanxi Medical University. Major adverse cardiovascular events (MACE) were followed up. ROC curves were established to obtain the optimal thresholds of SUV max and accumulated SUV max of all lesions of main coronary artery branches (S-SUV max) for predicting MACE. Cox proportional risk model and Kaplan-Meier method (log-rank test) were used to analyze the predictive value of PET parameters for MACE. Differences in metabolic parameters between 2 groups were compared by Mann-Whitney U test. Results:The median follow-up time of the 54 patients was 6.0(1.8, 6.6) years, and 13(24.1%) patients developed MACE, including 7 deaths, 5 myocardial infarction and 1 severe arrhythmia. S-SUV max in MACE group was significantly higher than that in the non-MACE group (2.64(2.08, 4.49) vs 1.83(0.95, 2.90); Z=-2.04, P=0.041). ROC curve showed that the optimal threshold of S-SUV max for MACE prediction was 2.05 (AUC=0.690). Multivariate Cox analysis showed that S-SUV max was a strong predictor of MACE (hazard ratio ( HR)=2.434(95% CI: 1.547-3.828), P<0.001). ROC curve showed that the optimal threshold of SUV max to predict MACE was 0.55 (AUC=0.659), and univariate Cox analysis showed that SUV max was a factor to predict MACE ( HR=10.192 (95% CI: 2.667-38.953), P=0.001). In 25 patients with incomplete revascularization (ICR), Kaplan-Meier analysis showed that the incidence of MACE in patients with positive 18F-NaF uptake (single medium stenosis (40%-70%) with SUV max≥0.55) was significantly higher than that in patients with negative 18F-NaF uptake (5/14 vs 0/11; χ2=6.07, P=0.014). Conclusions:18F-NaF PET/CT can be used as an independent predictor of MACE in patients with CAD and can quantitatively assess the long-term progression of moderate coronary artery stenosis. In the future, it is expected to be a new non-invasive way to guide the revascularization treatment decision of multi-vessel CAD.

From August to December 2023, a cross-sectional study was conducted to analyze the core managers of allergy nursing-related work in eighty-eight hospitals in Guangdong Province supported by the Allergy Nursing Branch of the Guangdong Nurses Association and the Nursing Group of the Allergy Branch of the Guangdong Medical Association. An online survey was conducted using the Delphi method to compile a questionnaire on the current status and demand for specialized development of allergy nursing in Guangdong Province. Chi-square tests and Fisher′s exact probability method were used to analyze the differences in the current status of specialty technology applications and the development and demand of specialized nursing between hospitals with or without allergy specialty departments. The results showed that among the eighty-eight hospitals in the nineteen prefecture-level cities surveyed in Guangdong Province, 17 hospitals (19%) had established allergy specialty departments, all of which were top-tier tertiary hospitals. The establishment duration [ M ( Q1, Q3)] was 3 (1, 10) years. The implementation rates of skin prick test (94%, 16/17), in-vitro examination (82%, 14/17), nasal endoscopy (82%, 14/17), subcutaneous specific immunotherapy (82%, 14/17), multidisciplinary consultation (65%, 11/17), fractional exhaled nitric oxide detection (47%, 8/17), skin patch test (41%, 7/17), and autologous serum skin test (35%, 6/17) in hospitals with allergy department were higher than those in hospitals without allergy department [59%, 42/71 ( χ 2= 7.462, P=0.006)], [54%, 38/71 ( χ 2= 4.717, P=0.030)], [51%, 36/71 ( χ 2= 5.599, P=0.018)], [34%, 24/71 ( χ 2= 13.177, P<0.001)], [24%, 17/71 ( χ 2= 10.505, P=0.001)], [24%, 17/71 ( χ 2= 5.540, P=0.019)], [11%, 8/71 (Fisher′s exact test, P=0.008)], [11%, 8/71 (Fisher′s exact test, P=0.025)]. In the development plan of allergy nursing in the next two years, hospitals that had set up allergy departments had a higher proportion in adding nurses (82%, 14/17, χ 2= 13.177, P<0.001), formulating specialized nurse training plans (71%, 12/17, χ 2= 3.980, P=0.046), formulating group standards or clinical guidelines (41%, 7/17, Fisher′s exact test, P=0.046), and formulating assessment standards (53%, 9/17, Fisher′s exact test, P=0.002). The demand for all aspects of allergy specialty nursing knowledge and skills accounted for more than 50%, and it was not related to whether the hospital set up an allergy department. In conclusion, the specialized development of allergy nursing in Guangdong province is still in its early stages, and there is a great demand for improving knowledge and skills in allergy. Further standardization of specialty construction is needed, along with the training of allergic nurses, to promote the comprehensive development of allergic reaction nursing specialties from point to area.

Chinese Journal of Experimental Traditional Medical Formulae 2023;29(18):1-7
ObjectiveTo explore the mechanism of Gegen Qinliantang (GQT) in improving ectopic lipid accumulation in the liver of db/db mice with type 2 diabetes mellitus (T2DM) by regulating the adenosine monophosphate-activated protein kinase (AMPK)-forkhead box O3a (FoxO3a) autophagy axis, to provide a scientific basis for clarifying the hypoglycemic mechanism of GQT and its clinical application. MethodSeventy-five spontaneous T2DM db/db mice and 15 normal db/m mice were selected and maintained on a regular diet for one week, followed by the measurement of blood glucose. They were then randomly divided into six groups, with 15 mice in each group, including normal group (0.2 g·kg-1 saline), metformin group (0.2 g·kg-1), high-, medium, and low-dose GQT group (31.9, 19.1, 6.9 g·kg-1), and model group (0.2 g·kg-1 saline). The mice were orally administered the corresponding drugs once daily for 12 weeks. Fasting blood glucose (FBG) and glycated hemoglobin (HbA1c) were detected. Fasting insulin (FINS) and free fatty acid (FFA) levels were measured by enzyme-linked immunosorbent assay (ELISA). Pathological changes in liver tissues were observed by hematoxylin-eosin (HE) staining. The protein expression levels of phosphorylated (p)-AMPK, p-FoxO3a, and autophagy-related proteins microtubule-associated protein 1 light chain 3 Ⅱ (LC3Ⅱ) and p62 were detected using Western blot. Immunofluorescence was used to detect the expression of hypoxia-inducible factor-1α (HIF-1α) in liver tissues. Real-time polymerase chain reaction (Real-time PCR) was performed to detect the mRNA expression of AMPK, FoxO3a, and LC3 in liver tissues. ResultCompared with the normal group, the model group showed pathological changes in liver tissues, increased FBG, HbA1c, FINS, and FFA levels (P<0.01), increased protein expression levels of p-AMPK, p62, and HIF-1α, decreased protein expression levels of p-FoxO3a and LC3Ⅱ in liver tissues (P<0.01), decreased mRNA expression of AMPK, and increased expression of FoxO3a (P<0.01). Compared with the model group, the treatment groups showed relieved liver tissue lesions and decreased FBG, HbA1c, FINS, and FFA levels (P<0.01). The expression of p-AMPK, p62, and HIF-1α increased, while the expression of p-FoxO3a showed a dose-dependent decrease in the high-dose GQT group. The expression of LC3Ⅱ increased in the metformin group and the high-dose GQT group (P<0.01). The mRNA expression of AMPK showed a dose-dependent increase, and the expression of FoxO3a showed a dose-dependent decrease in the treatment groups (P<0.01). ConclusionGQT can improve ectopic lipid accumulation in the liver of T2DM db/db mice, which may be related to the regulation of the AMPK-FoxO3a autophagy axis.

Objective:To compare the effect of systematic nursing in operating room and routine nursing in reducing intraoperative stress injury in patients with spinal fracture and spinal cord injury. Methods:A retrospective case-control study was conducted to analyze the clinical data of 285 patients with cervical or thoracolumbar fracture associated with spinal cord injury admitted to Honghui Hospital Affiliated to Xi'an Jiaotong University from January 2018 to December 2019, including 168 males and 127 females, with the age of 38-59 years [(47.8±8.5)years]. All patients underwent posterior decompression and fusion with internal fixation. Of all, 138 patients received systematic nursing in operating room including systematic evaluation and management before, during and after operation (observation group), and 147 patients received routine nursing including only intraoperative preventive care of pressure ulcer (control group). The incidence of pressure injury on the day after operation, degree of injury and location of injury at postoperative 3 days, and area of injury on the day after operation and at postoperative 3 days were compared between the two groups. The degree of injury was evaluated using the new stress injury staging assessment published by the American National Pressure Ulcer Advisory Panel (NPUAP).Results:The incidence of pressure injury in observation group [5.1% (7/138)] was lower than that in control group [12.2% (18/147)] on the day after operation ( P<0.05). The incidence of stage I, stage II, and stage III pressure injury in observation group [2.9% (4/138), 2.2% (3/138), 0.0%] was also lower than that in control group [8.2% (12/147), 3.4% (5/147), 0.6% (1/147)] at postoperative 3 days ( P<0.05). There was no significant difference in the incidence of pressure injury in the knee, chest, face and anterior superior iliac spine between the two groups ( P>0.05). The total proportion of pressure injury in the face and anterior superior iliac spine was 71% (5/7) in observation group, showing no significant difference from that in control group [83% (15/18)] ( P>0.05). The area of injury was (3.2±1.2)cm 2 and (3.2±1.1)cm 2 in observation group on the day after operation and at postoperative 3 days, lower than that in in control group [(5.1±1.5)cm 2 and (5.1±1.4)cm 2] ( P<0.01). Conclusion:Compared with the routine nursing, systematic nursing in operating room can significantly reduce the incidence, degree and area of intraoperative pressure injury in patients with spinal fracture accompanied by spinal cord injury, and deserves clinical promotion.
