1.Clinical Application and Pharmacological Effects of Guizhi Fulingwan in Treatment of Uterine Fibroids: A Review

Uterine fibroids are a common benign tumor of the female reproductive system, characterized by increased menstrual flow, lower abdominal pain, and prolonged menstrual periods. Modern medicine believes that the onset of this disease is related to genetic factors, environmental factors, hormone levels, et al, while the specific mechanism remains unclear, and the prevention and treatment of uterine fibroids has become a hot topic of concern for many experts and scholars in the medical field. At present, the treatment of uterine fibroids in clinical practice is mainly based on hormone drugs and uterine artery embolization, and severe cases require hysterectomy. However, the use of hormone drugs for treatment has serious side effects and is prone to recurrence after surgery. Since hysterectomy can cause severe harm to women, it is necessary to explore safer and more effective treatment methods. Traditional Chinese medicine (TCM) has rich clinical experience in the treatment of uterine fibroids, advocating for syndrome differentiation and treatment. The TCM methods that regulate Qi and blood and balance Yin and Yang have been commonly adopted, with significant efficacy and minimal side effects, being more conducive to the recovery. Guizhi Fulingwan are first recorded in the Synopsis of the Golden Chamber written by the famous medical expert ZHANG Zhongjing during the Eastern Han dynasty. This prescription has the effects of activating blood, resolving stasis, and eliminating mass, and it is thus mainly used for treating abdominal mass in women. In recent years, Guizhi Fulingwan has also been applied in the clinical treatment of tumors and has demonstrated definite efficacy in the treatment of uterine fibroids. Studies have shown that the therapeutic mechanisms of Guizhi Fulingwan for uterine fibroids involve regulating cell proliferation and apoptosis, improving immune function, reducing inflammation, improving hemorheological indicators, inhibiting tumor angiogenesis, and regulating sex hormone levels. This article mainly reviews the treatment of uterine fibroids with Guizhi Fulingwan from three aspects: theoretical basis, clinical application, and pharmacological mechanism. It is expected to provide scientific research ideas and guidance for exploring the clinical treatment of uterine fibroids.
2.Novel therapeutic advances for uveal melanoma

Uveal melanoma(UM)is the most common primary intraocular malignancy in adults, characterized by high invasiveness and unique metastatic biological features. Although local treatments(such as proton beam therapy and brachytherapy)can effectively control the primary lesion, approximately 50% of patients eventually develop distant metastasis, with the liver being the primary target organ(occurring in 90% of cases). This highlights a paradigm shift in treatment focus from mere local control to systemic prevention and management. For metastatic UM(mUM), current treatment strategies encompass biomarker-guided molecular targeted therapy, immunotherapy(including Tebentafusp, vaccines, and oncolytic virus therapy), and liver-directed therapy. Focusing on the synergy between local and systemic prevention and control, this article systematically elaborates on the precision local treatment for primary UM, the decision-making pathway for systemic treatment of metastatic UM based on molecular subtyping, the integration of local and systemic therapies for liver metastases, and the translational value of nanomedicine in addressing therapeutic bottlenecks. It provides insights for optimizing clinical management of mUM and developing novel therapeutic strategies.
3.Research progress on allergenic components in traditional Chinese medicine injections

Traditional Chinese medicine (TCM) injections, a hallmark achievement in the modernization of TCM, hold significant importance in advancing the development of TCM and enhancing clinical therapeutic effects. However, with the expanded clinical application of TCM injections, safety concerns have garnered widespread attention, particularly allergic reactions. Numerous researchers have delved into the safety issues of TCM injections. This paper summarized the research related to allergic reactions of TCM injections, including the types of allergic reactions, the allergenic components, their evaluation models and detection methods, as well as the challenges encountered in current research, aiming at providing references and basis for in-depth research and safe application of allergic reactions of TCM injections.
4.Characteristics of brain glymphatic system changes in patients with amnestic mild cognitive impairment based on perivascular spaces and DTI-ALPS index assessment

Objective To explore the characteristics and correlation between perivascular spaces (PVS) and diffusion tensor image analysis along perivascular spaces (DTI-ALPS) index in patients with amnestic mild cognitive impairment (aMCI). Methods A retrospective study was conducted on a total of 118 participants, including cognitive normal (CN) healthy controls and aMCI patients, recruited from the Department of Neurology, Zhongshan Hospital, Fudan University from September 2020 to September 2022. All participants underwent structural magnetic resonance imaging (MRI) and diffusion tensor imaging (DTI). An automatic segmentation algorithm was used to quantify PVS metrics in the brain, and DTI-ALPS index was calculated based on DTI. Differences in DTI-ALPS index and PVS metrics between the CN and aMCI groups were compared. Multivariate logistic regression was used to identify independent factors influencing aMCI. Correlation analysis was performed to assess relationships among DTI-ALPS index, PVS metrics, and cognitive scores. Results A total of 80 CN healthy controls and 38 aMCI patients were included. The DTI-ALPS index was significantly lower in the aMCI group compared with the CN group (1.28±0.18 vs 1.37±0.21, P=0.018), while differences in PVS metrics between groups were not statistically significant. Multivariate logistic regression analysis indicated that DTI-ALPS index was an independent factor affecting aMCI (OR=0.097, 95%CI 0.011–0.833, P=0.033). Correlation analysis revealed a positive relationship between DTI-ALPS index and MMSE score (r=0.210, P=0.023) as well as PVS length in the centrum semiovale (r=0.216, P=0.019). Conclusions The DTI-ALPS index may serve as an imaging biomarker for identifying early cognitive impairment, and patients with aMCI exhibit abnormal DTI-ALPS indices, suggesting that brain glymphatic system dysfunction may occur prior to morphological changes.
5.Research progress on the relationship between ovarian microenvironment and polycystic ovary syndrome

Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder in women that is a leading cause of infertility. Follicular development disorder, as the main cause of oligoovulation, is the key factor affecting the fertility of PCOS patients. Among them, the disorder of the ovarian microenvironment is an important inducement for follicular development disorder. Emerging research highlights that the ovarian microenvironment plays a sophisticated role in follicular development and maturation through various mechanisms, including the cellular constituents within the ovary, the extracellular matrix's structural support, adequate vascular supply, and the regulation of hormonal and growth factor signaling. These factors collectively ensure the normal growth and ovulation of follicles, thereby safeguarding female reproductive health. In women with PCOS, however, the ovarian microenvironment is compromised, leading to excessive extracellular matrix deposition and fibrosis, abnormalities in vascular architecture and function, immune-inflammatory reactions, and metabolic disturbances. These anomalies collectively hinder follicular growth and development, resulting in ovulation disorders. This comprehensive review will delve into the intricacies of these disruptions and their implications for fertility in PCOS patients.
6.Regulatory role of DNA demethylation mediated by TET protein in mammalian embryonic development and pregnancy outcome

DNA methylation is an important epigenetic modification in mammals, playing a crucial role in various physiological processes, including cell differentiation and the gene expression regulation. The ten-eleven translocation (TET) protein family of DNA demethylases is integral to the regulation of DNA methylation, as it catalyzes the oxidation of 5-methylcytosine to form 5-hydroxymethylcytosine. During early embryonic development, the genome undergoes extensive DNA demethylation, and any aberration in this reprogramming process can result in abnormal embryonic development and physiological defects in offspring. The TET proteins, due to their unique dynamics and multifaceted roles, facilitate DNA demethylation and are involved in development and maturation of germ cells, the establishment of pluripotency, cell lineage differentiation, and transcriptional processes throughout mammalian embryogenesis. Furthermore, these proteins are closely associated with the maintenance of pregnancy and susceptibility of progeny to disease. Factors such as genetic mutations, maternal health conditions, and exposure to adverse environmental influences can impact TET protein activity, resulting in abnormal patterns of DNA demethylation. A comprehensive investigation of the related mechanisms of TET proteins is essential for enhancing our understanding of epigenetic regulation during early life, diagnosing and treating related diseases such as early fetal development retardation, and informing strategies for the prevention and management of pregnancy.This article reviews the regulatory role of DNA demethylation mediated by TET protein in mammalian embryonic development and pregnancy outcomes.
7.Intraoperative hypothermia risk prediction models for patients undergoing cancer surgery: a scoping review

Objective:To conduct a scoping review of risk prediction models for the development of intraoperative hypothermia in patients undergoing cancer surgery to inform clinical nursing practice and future research.Methods:Relevant literature on constructing or validating intraoperative hypothermia risk prediction models for cancer surgery patients in five foreign language databases (PubMed, Embase, Web of Science, Cochrane Library, CINAHL) and four Chinese language databases (China National Knowledge Infrastructure, Wanfang, VIP, Chinese Biomedical Database) were searched from the time of library construction to June 1, 2024, extracted information on the applicable target, incidence of intraoperative hypothermia, methodology of model construction, predictors and performance, etc. The Prediction model Risk Of Bias Assessment Tool was used to evaluate the risk of bias of the studies, and the included literature was analyzed and discussed.Results:A total of 15 pieces of literature involving 18 models were included, with the study population focussing on patients undergoing surgery for colorectal cancer. The rate of intraoperative hypothermia ranged from 15.14% to 61.5%. Model construction methods included 2 types of Logistic regression models and machine learning, and model presentation was based on column-line plots. There were 8 predictors that appeared with a frequency of ≥5, including age, body mass index, operation time, anaesthesia time, operating room temperature, intraoperative rehydration volume, intraoperative bleeding volume, and heat preservation method.Conclusions:The performance of the included model was good, but the risk of bias was high for the predictors and the analysis part, and nursing staff should pay close attention to the risk factors of intraoperative hypothermia in patients undergoing cancer surgery, construct a risk prediction model with low bias and high applicability, and validate and improve the existing risk prediction model.
8.Current status of cleaning and disinfection of medical bedding items in public medical institutions of Liangshan ethnic minority area

OBJECTIVE To investigate the current status of cleaning and disinfection of medical bedding items in public medical institutions of Liangshan ethnic minority area and analyze the risk of infections so as to provide ba-ses for optimizing the infection control strategies in the ethnic minority area.METHODS On the basis of question-naire survey,onsite sampling method and bacterial isolation and identification technique,a cross-sectional survey was conducted for the current status of cleaning and disinfection of medical bedding items in 15 different grades of public medical institutions of Liangshan ethnic minority area by stratified random sampling method from Sep.2021 to Sep.2023.RESULTS A total of 66 valid questionnaire were recycled,and 540 bedding item samples were col-lected.Totally 93.33%of the medical institutions relied on laundries for centralized cleaning of the medical bed-ding items.The implementation rates of management items such as policy formulation(100.00%)and pre-job training were remarkably higher in the tertiary hospitals than in the secondary and primary hospitals(allP<0.05).There were no significant differences in the qualified rates of surveillance of microbiological indexes for the medical bedding items among the medical institutions,the qualified rate of surveillance of microbiological inde-xes was highest(71.30%)in the general hospitals and was lowest(61.11%)in the infectious diseases specialized hospital.The qualified rate of surveillance of microbiological indexes was lowest(3.33%)in pediatrics department or internal medicine department,and it raised most remarkably(80.00%)after the disinfection,and there was significant difference(x2=36.274,P<0.001).CONCLUSIONS The current status of disinfection of the medical bedding items is not optimistic in the medical institutions of Liangshan ethnic minority area.There are the prob-lems such as imperfect system,insufficient equipment and absence of personnel training.It is necessary to com-plete the execution and supervision system for cleaning and disinfection standards,increase the cleaning facilities,promote the use of antibacterial textile and bring in smart management system so as to improve the cleaning and disinfection quality and reduce the risk of hospital-associated infections.
