Objective To analyze the efficacy of the"sandwich"technique for treating varicocele(VC).Methods A total of 310 patients with VC(365 affected veins)were selected and divided into interventional treatment group and non-interventional treatment group.The baseline data,hospitalization data,and 6-month follow-up data of the two groups were analyzed.Results The age of patients in the interventional treatment group was significantly lower than that in the non-interventional treatment group(P＜0.05).The surgical time and hospital stay in the interventional treatment group were significantly lower than those in the non-interventional treatment group(P＜0.05).In the non-interventional treatment group,two patients experienced surgical site infections,and one patient opted for interventional treatment due to recurrence after non-interventional treatment.After surgery,the diameter of the spermatic vein significantly decreased in both the interventional and non-interventional treatment(P＜0.05).Conclusion The"sandwich"technique(embolization coil combined with foam sclerotherapy)is an effective treatment for VC.

Objective To analyze the etiological characteristics of pediatric bacterial meningitis(BM)and change of biochemical indicators in cerebrospinal fluid,and establish a diagnostic model for pathogen differentiation based on biochemical indicators in cerebrospinal fluid.Methods Eighty children with BM in the Children's Hospital Affiliated to Zhengzhou University from May 2021 to May 2023 were selected as BM group,and their pathogen distribution as well as cytological and biochemi-cal indicators in cerebrospinal fluid were analyzed.Forty non-central nervous system infections chil-dren with cytological and biochemical testing in cerebrospinal fluid in the same period were included as control group.The relationships between indicators in cerebrospinal fluid and pathogen types in BM children were analyzed,and a diagnostic model was constructed.Results A total of 85 pathogens were detected in 80 BM children,including 48 Gram-negative bacteria(56.47％)and 37 Gram-pos-itive bacteria(43.53％).The BM group showed significantly higher white blood cell count and lym-phocyte percentages,lower neutrophil and monocyte percentages,lower glucose and chloride levels in cerebrospinal fluid,and higher protein levels in cerebrospinal fluid when compared to the control group(P＜0.05).Among BM children,those infected with Gram-negative bacteria had significantly higher white blood cell count and lymphocyte percentages,as well as lower glucose and chloride levels in cerebrospinal fluid when compared to those with Gram-positive bacterial infections(P＜0.05).Multivariate Logistic analysis revealed that white blood cell count,lymphocyte percentage,glucose,and chloride levels in cerebrospinal fluid were significantly associated with pathogen types(P＜0.05).A diagnostic model based on these factors demonstrated an area under the receiver operating characteristic(ROC)curve of 0.925 for distinguishing Gram-positive and Gram-negative infections,with a cutoff value of 0.754,asensitivity of 76.25％,and aspecificity of 98.33％.Conclusion The primary pathogens of pediatric BM include Acinetobacter baumannii,Escherichia coli,Klebsiella pneumoniae,coagulase-negative Staphylococcus,Staphylococcus aureus,and Streptococcus pneumoni-a,accompanied by elevated white blood cell count and lymphocyte percentages as well as reduced glucose and chloride levels in cerebrospinal fluid,which is helpful in differentiating Gram-positive from Gram-negative infections and guiding antibiotic therapy.

Objective The purpose of this study was to explore the application effect of BOPPPS combined with case simulation teaching method in nursing risk education of nursing students.By comparing the traditional teaching methods,the in-fluence of BOPPPS combined with case simulation teaching method on nursing students' occupational risk cognition ability,pa-tient safety perception ability and teaching satisfaction was evaluated.Methods 100 cases of nursing students in geriatric oncol-ogy department were divided into observation group and control group,50 cases in each group.The control group used traditional teaching methods;the observation group adopted the teaching mode of BOPPPS combined with case teaching method.The occu-pational risk cognition ability,patient safety perception ability and satisfaction with teaching mode were compared between the two groups.Results The occupational risk cognitive ability,patient safety perception ability and teaching satisfaction of the observa-tion group were better than those of the control group(P＜0.05).Conclusion The application of BOPPPS combined with case simulation teaching method in nursing risk teaching of nursing students can improve their occupational risk cognition ability,pa-tient safety perception ability and teaching satisfaction.

Objective To investigate the effects of electroacupuncture on the intestinal NIMA-related kinase 7(NEK7)/NOD-like receptor thermal protein domain associated protein 3(NLRP3)signaling pathway in mice with Parkinson's disease.Methods According to the randomized number table method,36 C57BL/6 mice were randomly divided into the control group,the model group,and the electroacupuncture group,with 12 mice per group.The Parkinson's disease mouse model was established by gavage of rotenone solution(10 mg/kg)for 28 d.After molding,the electroacupuncture group was stimulated with"Fengfu"(DU17),"Taichong"(LR3),and"Zusanli"(ST36)for 14 d,while the control group and the model group were only treated with immobilization.The motor ability of mice was detected by pole climbing test and hindlimb rating score,the positive expressions of nigral tyrosine hydroxylase(TH)and colon occludin were detected by immunohistochemistry,the histological morphology of colon was observed by hematoxylin-eosin staining,and Western blotting was used to detect the protein expressions of NEK7,NLRP3,Caspase-1,and interleukin-1β(IL-1β).Results Compared with the control group,mice in the model group had lower score on the pole climbing test and a higher hindlimb rating score(P＜0.01);the average optical densities of nigral TH and colon occludin were decreased(P＜0.01);significant inflammatory infiltration was observed in the colonic tissue,and the muscularis propria was thinned;and the protein expressions of NEK7,NLRP3,Caspase-1,and IL-1β in the colonic tissue were elevated(P＜0.01).Compared with the model group,mice in the electroacupuncture group had higher score on the pole climbing test and a lower hindlimb rating score(P＜0.01);the average optical densities of nigral TH and colon occludin were increased(P＜0.01);the degree of inflammatory infiltration of colonic tissues decreased,and the muscularis propria was thickened;and the protein expressions of NEK7,NLRP3,Caspase-1,and IL-1β of colonic tissues were decreased(P＜0.01).Conclusion Electroacupuncture at"Fengfu"(DU17),"Taichong"(LR3),and"Zusanli"(ST36)can improve motor functional impairments in mice with Parkinson's disease,and the mechanism may be through the inhibition of intestinal NEK7/NLRP3 pathway,improving the intestinal barrier damage,relieving the intestinal inflammation,and improving the dopaminergic neuron injury.

Objective N-methyl-D-aspartate(NMDA)receptor subunit 2B(NR2B)and its phosphorylation are involved in cerebral ischemia/hypoxic neural injury.Hypoxic preconditioning(HPC)can serve as an endogenous protective intervention to protect the brain from ischemic/hypoxic injury.This study intended to explore the effect of HPC on NR2B and the phosphorylation of its two tyrosine sites(1252 and 1336)in hippocampal cells through in vivo and in vitro experiments and thus determine the role of NR2B in HPC neuroprotection.Methods 6～8 weeks-old male SPF-grade ICR mice and the mouse hippocampal neuron cell line HT22 were repeatedly exposed to hypoxia to replicate HPC animal and cell models.Western blot and immunofluorescence were applied to detect the levels of NR2B and the phosphorylation levels of its tyrosine 1336(pY1336NR2B)and 1252(pY1252NR2B)residues in the hippocampus of mice and HT22 cells.The distributions of NR2B,pY1336NR2B,and pY1252NR2B in the synaptic site(TxP)and extrasynaptic site(TxS)were analyzed by Western blot.The levels of cleaved caspase-3 and α-spectrin,which indicate cell apoptosis,were also detected.Results HPC downregulated the levels of NR2B and pY1336NR2B in the mouse hippocampus and HT22 cells.Changes in NR2B and pY1336NR2B levels in the TxS of the mouse hippocampus were similar to those in hippocampus and HT22 cells,whereas changes in the TxP showed the opposite trend.Conclusions Downregulation of NR2B and pY1336NR2B may be involved in HPC-induced neuroprotection,and their localization at synapses and extrasynapses may play different roles in neuroprotection.
