1.A case of hemopneumothorax caused by ruptured pulmonary sequestration during pregnancy

This article reported a pregnant woman admitted to the hospital due to "25 +2 weeks of amenorrhea and a 1-day history of shortened cervical canal accompanied by vaginal bleeding". The patient with pregestational diabetes mellitus and suboptimal glycemic control required prolonged hospitalization for tocolytic therapy due to shortened cervical length. She developed a cough at 31 weeks and 4 days of gestation, followed by right-sided intercostal pain and hypotension after coughing at 31 weeks and 6 days of gestation. Bedside chest ultrasound showed a small anechoic fluid collection (approximately 1.1 cm in width) in the right pleural cavity. The emergency cesarean section was performed at 31 weeks and 4 days of gestation. However, the intraoperative bleeding and other conditions were inconsistent with the obstetric clinical presentations of blood loss. Subsequent repeated ultrasound and CT examinations confirmed the diagnosis of pulmonary sequestration and right-sided progressive hemopneumothorax. On the same day, an emergency right lower lobectomy was performed, achieving stable postoperative recovery. Both mother and infant had favorable outcomes. Hemopneumothorax complicated by pulmonary sequestration is uncommon, and its occurrence during pregnancy is exceedingly rare. Multidisciplinary consultations, aggressive, rapid, and accurate diagnosis, and combined treatment are critical to ensuring maternal-fetal survival. Hemopneumothorax caused by the rupture of pulmonary sequestration during pregnancy represents a life-threatening condition. Emergency thoracotomy can timely clarify the cause, arrest bleeding, relieve compression, and resect the lesion, thereby reducing mortality and the complications risk.
2.Effect of High-Altitude Hypoxia on the Pharmacokinetics and Brain Distribution of Phenytoin Sodium in Epileptic Rats

Objective To investigate the effects of high-altitude hypoxic environment on the pharmacokinetic characteristics and brain tissue distribution of phenytoin sodium in epileptic rats.Methods A total of 70 male SPF-grade Wistar rats aged 2 months and weighing(200±20)g were used in the study.An epilepsy model was induced in the rats using the lithium chloride-pilocarpine method.The successfully modeled rats were randomly assigned to a normoxic treatment group and a high-altitude hypoxic treatment group.Phenytoin sodium was administered via intragastric gavage at a dose of 50 mg/kg in both groups.Blood samples were collected from the orbital venous plexus before treatment and 0.5,1,2,3,4,6,8,10,and 24 h post treatment.The animals were euthanized after the final blood collection,and samples of the liver and the whole brain tissue were collected.In the brain tissue distribution experiment,brain tissue samples were collected at 0.5,1,2,and 4 h after drug administration.The concentration of phenytoin sodium in rat plasma and brain tissue was determined by liquid chromatography-tandem mass spectrometry(LC-MS/MS),and the pharmacokinetic parameters were calculated using WinNolin 8.1 software.The expression levels of CYP2C9 in liver tissue and those of P-gp in brain tissue of epileptic rats were determined by Western blot.Results Compared with those in the normoxia group,the peak concentration,peak time,and half-life of phenytoin sodium in the high-altitude hypoxia group were significantly decreased by 46.0%,42.3%,and 55.5%,respectively(all P<0.05);the clearance rate was significantly increased by 162.0%(P<0.05);and the area under the curve of plasma concentration-time curve was decreased by 45.6%(P<0.01).At 0.5,1,and 2 hours after administration,compared with that in the normoxia treatment group,the concentration of phenytoin sodium in the brain tissue of the high-altitude hypoxia treatment group was significantly decreased by 78.1%,63.5%,and 32.5%,respectively(all P<0.05).Western blot results showed that the expression levels of CYP2C9 in the liver tissue and P-gp in the brain tissue of rats in the high-altitude hypoxia group were approximately 1.78 and 1.65 times higher than those in the normoxia group,respectively(both P<0.05).Conclusion The hypoxic environment at high altitudes can promote the metabolism of phenytoin sodium,reduce its absorption efficiency,and change the characteristics its distribution in the brain,which may be related to the up-regulation of the expression of CYP2C9 in the liver and that of P-gp in the brain.
3.Evaluation of Right Ventricular Function in Cardiac Amyloidosis by Two-Dimensional Speckle Tracking Imaging

Purpose To investigate the value of two-dimensional speckle tracking in evaluating right ventricular function in cardiac amyloidosis(CA).Materials and Methods A total of 25 patients with CA,30 patients with hypertrophic cardiomyopathy(HCM)and 30 healthy controls were retrospectively selected from July 2021 to October 2023 in Wuhan Asia Heart Hospital.The regional difference was represented by the formula apical ratio.The parameters of conventional echocardiography,two-dimensional strain and apical ratio of the three groups were analyzed and compared.Results Only segmental strain was different in right ventricular structure and function between CA group and HCM group,and the differences were statistically significant(all P<0.05).In the CA group,the strain gradient of the right ventricle from the apex of the heart to the base was the same as that of the left ventricle,that was,the"apex preservation"pattern,which was not seen in the HCM group and the control group.Comparison of the right ventricular free wall longitudinal strain-apical ratio,right ventricle four-chamber longitudinal strain-apical ratio,left ventride global longitudinal strain-apical ratio among the three groups,it was found that the levels of CA group were higher than HCM group and control group,and the differences were statistically significant(H=40.40,43.18,55.43,all P<0.05).The right ventricular free wall longitudinal strain-apical ratio had the highest CA recognition value,the area under the curve was 0.935,sensitivity was 96%and specificity was 83%.Conclusion Two-dimensional speckle tracking can accurately evaluate the right ventricular function impairment in CA patients and provide a new choice for clinical evaluation and identification of CA patients.
4.Neuromodulatory mechanisms of sodium-glucose cotransporter 2 inhibi-tors in treatment of heart failure

Sodium glucose cotransporter(SGLT)2 inhibitors,initially developed as antihyperglycemic agents,have demonstrated remarkable cardioprotective effects in numerous clinical trials and are now utilized in the treat-ment of heart failure across a range of ejection fractions.SGLT2 is predominantly expressed in the renal proximal tubules,brain,and epicardial adipose tissue within the human body.Research indicates that the protective effects of SGLT2 inhibi-tors in heart failure are linked to their direct modulation of SGLT2 receptors in both the kidneys and epicardial adipose tis-sue.Furthermore,SGLT is also present in the brain,and recent studies have revealed that SGLT2 inhibitors can enhance sympathetic activity in patients with metabolic syndrome,suggesting a potential neuroregulatory role.Given that excessive activation of the sympathetic nervous system can contribute to the onset and progression of heart failure,this paper reviews the neural regulatory mechanisms of SGLT2 inhibitors to provide insights for a deeper understanding of heart failure and to optimize their clinical application.
5.Application of PDCA cycle method in internal error control management of intravenous drug dispen-sing center

Objective This study aims to explore the application effectiveness of the PDCA cycle method in preventing and reducing internal errors in the intravenous drug dispensing center(PIVAS).Methods Internal error data from our hospital's PIVAS in 2020 and 2021 were collected.The data from 2020 represented the pre-implementation of the PDCA cycle,while the data from 2021 represented the post-implementation period.The changes in internal errors and error rates before and after imple-mentation were compared.Results After implementing the PDCA cycle management measures,the annual error rate decreased from 0.887%o in 2020 to 0.681‰ in 2021,a decrease of 23.22%.Conclusion The PDCA cycle method can effectively reduce the occurrence of internal errors,improve work accuracy,and ensure the safety of clinical drug use in PIVAS.
6.Quantitative Evaluation of Mitral Valve Structure and Function in Mitral Valve Prolapse by Three-Dimensional Transesophageal Echocardiography

Purpose To evaluate the static structural changes of mitral annulus and leaflets in mitral valve prolapse via three-dimensional transesophageal echocardiography(3D-TEE),and their continuous dynamic changes during the cardiac cycle.Materials and Methods Twenty-six patients with mitral valve prolapse diagnosed by transesophageal echocardiography in Wuhan Asia Heart Hospital from June 2022 to June 2023 were retrospectively included,and 30 patients who were examined by transesophageal echocardiography in our hospital during the same period due to monitoring of patent foramen ovale or non-cardiac surgery were selected as the control group.The related parameters of the valve annulus were measured.In addition,the meaningful parameters were measured at late diastole(LD),early systole(ES),middle systole(MS)and late systole(LS),and the change rate was calculated.The static structure of mitral valve device was compared between the two groups,and the differences of various parameters at different times were analyzed.Results Compared with the control group,the annulus height(AH)and the ratio of annulus height and combined width(AHCWR)in the case group decreased,and the other parameters increased(t=-7.447-12.472,all P<0.05),but there was no statistical difference in the maximum displacement distance of the annulus between the two groups(P>0.05).Comparing the related parameters of LD,ES,MS and LS at four different time points,compared with the control group,all the parameters in the case group increased except AH and AHCWR(t=-16.224-15.683,all P<0.05).Comparing the continuous changes at four different times,the continuous changes of all parameters in the control group were statistically significant(F=7.632-323.837,all P<0.05),and there were significant differences in the parameters of the control group from LD to ES(all P<0.05).Except anterolateral to posteromedial diameter,intertrigonal diameter and annulus area,the continuous changes of other parameters in the case group were statistically significant(F=3.041-71.082,all P<0.05),and only non-planar angle,AH and AHCWR in the case group had statistical differences in LD to ES(all P<0.05).Compared with the control group,the change rate of parameters in the case group decreased,and the difference was statistically significant(t/U=-7.069-780.000,all P<0.05),and the change rate of C-ring was greater than that of intertrigonal diameter.Conclusion 3D-TEE can quantitatively analyze the dynamic changes of mitral annulus and leaflets,and analyze the continuous changes of mitral valve in cardiac cycle in detail,which can provide more accurate preoperative evaluation for patients and guide treatment.
7.Application of the 5A management model based on personalized health education records in community patients with type 2 diabetes

Objective:To investigate the effectiveness of the 5A (Ask, Assess, Advise, Assist, Arrange follow-up) management model based on personalized health education records in community patients with type 2 diabetes mellitus.Methods:A convenience sampling method was used to select patients with type 2 diabetes mellitus who were enrolled with family doctors in Yuxin Community, Haidian District, Beijing, from January 2022 to May 2023. Using a random number table method, the patients were divided into an observation group ( n=68) and a control group ( n=68). The control group received routine community management, while the observation group received management based on the 5A model and personalized health education records. Differences in metabolic indicators, including glycated hemoglobin (HbA1c), fasting blood glucose, 2-hour postprandial blood glucose, body mass index (BMI), and waist circumference, as well as self-management behaviors were compared between the two groups at 3, 6, and 12 months of intervention. Results:During the entire intervention period, four cases were lost in the observation group and eight cases were lost in the control group. Finally, data from 124 patients were included in the analysis, including 64 cases in the observation group and 60 cases in the control group. Repeated measures analysis of variance showed interaction effects between time points and groups for HbA1c, BMI, waist circumference, and self-management behavior scores among patients with diabetes, with statistically significant differences ( P<0.05) . Conclusions:The 5A management model based on personalized health education records has a positive impact on improving metabolic indicators and self-management behaviors in community patients with type 2 diabetes mellitus.
