1.Causal relationship between mental disorders and hypercholesterolemia: a Mendelian randomization analysis

BackgroundMetabolic syndrome (MetS) is highly prevalent in patients with mental disorders, including elevated diastolic or systolic blood pressure, elevated fasting glucose, hypercholesterolemia, abdominal obesity and so on. As an important component of MetS, the relationship between hypercholesterolemia and mental disorder has been extensively reported, whereas few genome-wide association studies (GWAS) have been conducted to identify the causal role of mental disorders in hypercholesterolemia. ObjectiveTo explore the potential causal relationship between mental disorders and hypercholesterolemia by two-sample Mendelian randomization (MR) method. MethodsSummary data from GWAS were analyzed. Single nucleotide polymorphisms (SNPs) strongly associated with mental disorders were chosen as instrumental variables, and hypercholesterolemia was used as outcome variable. MR analysis utilized inverse-variance weighted (IVW), MR-Egger regression and weighted median estimation (WME) as the primary analytical tool, and supplemented by simple mode (SM) and weighted mode (WM). The causal relationship between mental disorders and the risk of hypercholesterolemia was illustrated in terms of odds ratio (OR). ResultsA total of 36 SNPs associated with mental disorders were identified as instrumental variables. The primary findings from IVW revealed existence of a causal relationship between mental disorders and hypercholesterolemia (IVW: OR=1.067, 95% CI: 1.026~1.109, P=0.001). Findings from the additional methods (MR-Egger regression, WME, SM, WM) were basically consistent with those reported in IVW method. Further verification indicated that the causal relationship between mental disorders and the risk of hypercholesterolemia was not affected by genetic polymorphism (P>0.05). The absence of heterogeneity was confirmed through Cochran's Q test and MR-Egger regression (P>0.05). Furthermore, no causal association in the reverse direction was found (P>0.05). ConclusionThere is a causal relationship between mental disorders and hypercholesterolemia, and patients with mental disorders may have an increased probability of suffering from hypercholesterolemia.
2.Application of a wearable visual field meter based on extended reality glasses in macular disease

Objective:To evaluate the application effect of a wearable visual field meter based on extended reality (XR) glasses for patients with macular disease.Methods:A self-controlled study was conducted.A total of 41 consecutive patients (41 eyes) with macular disease were recruited at Renmin Hospital of Wuhan University from October 2022 to October 2024.All patients underwent 10-2 center visual field test using a self-developed wearable visual field meter (XRVF), and the results were compared with those obtained using a traditional Humphrey field analyzer (HFA).The comparison parameters included mean retinal sensitivity (MS), false positive rate (FPR), false negative rate (FNR), and testing duration.A subject satisfaction questionnaire was administered.This study followed the Declaration of Helsinki.The study protocol was approved by the Medical Ethics Committee of Renmin Hospital of Wuhan University (No.WDRY2024-K263), and all subjects signed the informed consent form.Results:The retinal sensitivity of patients measured by HFA was (23.24±3.71)dB, which was higher than (22.01±3.45)dB by XRVF, showing a statistically significant difference ( t=4.924, P=0.036).The FPR measured by HFA and XRVF were (2.39±2.51)% and (2.59±3.29)%, respectively, and the FNR were (3.49±6.05)% and (3.74±5.38)%, respectively, showing no statistically significant difference ( t=-3.624, P=0.948; t=-1.241, P=0.519).The median test duration for HFA and XRVF was 6.15 (5.78, 6.65) and 5.98 (5.71, 6.69)minutes, respectively, without statistically significant difference ( Z=-1.987, P=0.953).92.6% of the subjects thought the device was comfortable, simple and practical. Conclusions:The XRVF has good consistency with the HFA, can effectively and reliably evaluate the visual field function of patients with macular disease, and is easily accepted by patients.
3.Clinical study of treating atlanto-axial joint disorder with against-lateral correction Tuina

Objective:To observe the clinical efficacy of against-lateral correction Tuina(Chinese therapeutic massage)in treating atlanto-axial joint disorder(AAJD)and imaging changes.Methods:A total of 142 patients with AAJD were recruited.They were randomly allocated to a trial group and a control group using the random number table method,with 71 participants in each group.The trial group was treated with against-lateral correction Tuina 3 times weekly.The control group was offered conventional physical traction therapy once daily.The interventions lasted 2 weeks in both groups.The two groups of participants were observed before and after treatment for their changes in the global pain scale(GPS)score,visual analog scale(VAS)score for dizziness assessment,cervical range of motion(ROM)in rotation,and the extent of atlanto-dental displacement.Results:The GPS and VAS scores dropped after treatment in both groups(P<0.05)and were lower in the trial group than in the control group after treatment and at the follow-up(P<0.05).Participants in the trial group achieved a significant increase in the cervical ROM in rotation after treatment and at the follow-up compared to the pre-treatment value(P<0.05)and surpassed the control group(P<0.05);the control group only showed an increase in the left-side rotation(P<0.05).After the intervention,neither the intra-group nor the between-group comparison revealed significant differences in the extent of atlanto-dental displacement(P>0.05),though the trial group presented an improving tendency.Conclusion:Compared to physical traction,the against-lateral correction Tuina method works more significantly in improving pain,dizziness,and ROM in AAJD patients.

6.Efficacy of brief cognitive behavioral therapy on improving quality of life improvement in patients with schizophrenia

Background The course of schizophrenia is prolonged,and patients have impaired social function and significantly reduced quality of life.Drug therapy combined with psychological therapy is particularly important for improving the quality of life of patients.Brief cognitive behavioral therapy(BCBT)has been widely applied in clinical practice,but current research on BCBT focuses more on improving patients' symptoms and lacks relevant reports on improving quality of life.Objective To evaluate the efficacy and influencing factors of BCBT combined with conventional treatment on improving the quality of life in patients with schizophrenia.Methods A total of 210 patients who met the diagnostic criteria for schizophrenia in the International Classification of Diseases(10th edition)(ICD-10)and were followed up at the outpatient department of Beijing Anding Hospital Capital Medical University from August 2011 to December 2016 were selected.Using a random number table method,patients were divided into study group and control group,with 105 cases in each group.Both groups received routine treatment,and the research group received a total of 8 BCBT sessions for 12 weeks on this basis.At the baseline period and 12 weeks of treatment,26 weeks of follow-up and 52 weeks of follow-up,Positive and Negative Syndrome Scale(PANSS),Personal and Social Performance Scale(PSP)and World Health Organization Quality of Life Brief(WHOQOL-BREF)were used for evaluation.Results The results of repeated measures analysis of variance showed that the time point effect and interaction effect of PANSS total score were statistically significant(F=118.783,8.083,P<0.01).The time point effect,inter group effect and interaction effect of PSP total score were statistically significant(F=94.358,4.048,5.490,P<0.05 or 0.01).The time point effect,inter group effect and interaction effect of the total score of WHOQOL-BREF were all statistically significant(F=12.330,4.168,4.142,P<0.05 or 0.01)Binary Logistic regression analysis showed that the study group(OR=1.861,95%CI:1.004~3.448)and young age(OR=1.044,95%CI:1.001~1.088)were protective factors for improving quality of life of patients,while high PANSS baseline score(OR=0.972,95%CI:0.945~0.999)was a risk factor for improving quality of life of patients.Conclusion The combination of BCBT and conventional treatment has an earlier onset of improvement in the quality of life of patients with schizophrenia,and long-term efficacy is superior to conventional treatment.
7.Clinical application of individualized lung rehabilitation training in rapid postoperative rehabilitation of lobectomy patients with concomitant high-risk factors

Objective To explore the effect of individualized lung rehabilitation training on rapid postoperative rehabilitation intervention in lobectomy patients with concomitant high-risk factors.Methods A total of 102 patients with high-risk factors undergoing pulmonary lobectomy admitted to our hospital from February 2021 to December 2022 were selected and divided into experimental group(51 cases)and control group(51 cases)by random number table method.The reference group received traditional standardized lung rehabilitation training,while the experimental group received individualized lung rehabilitation training.The clinical hospitalization time,chest tube removal time,lung function,motor function indicators,quality of life,and incidence of complications were compared between the two groups after intervention.Results After intervention,the hospitalization days[(10.34±2.06)d]and the chest tube removal time[(3.86±0.55)d]of the experimental group were significantly shorter than those of the control group[(13.23±2.21)d and(4.65±0.62)d](P<0.05)];the forced vital capacity[FVC,(2.52±0.42)L],maximum air volume[MVV,(66.82±7.32)L]and 1s forced expiratory volume[FEV1,(1.35±0.24)L]of experimental group were significantly higher than those of control group[(2.11±0.36)L,(62.02±6.59)L,(1.24±0.22)L](P<0.05);the FMA scores[(81.86±8.36)points]and 6 min walking distance[(221.66±25.24)m]were significantly higher than those of control group[(72.43±7.51)points and(183.23±20.75)m](P<0.05);the SF-36 score of the experimental group[(85.36±9.11)points]was significantly higher than that of the control group[(79.43±8.68)points](P<0.05);the incidence of complications in the experimental group was obviously lower than that in the reference group(5.88%vs 21.57%,P<0.05).Conclusion Individualized lung rehabilitation intervention training can effectively improve lung ventilation function and exercise tolerance in lobectomy patients with concomitant high-risk factors.

Objective This paper focuses on the field of clinical medicine,with the aim of verifying the feasibility of interdisciplinary literature evaluation model based on feature matching method,to provide support for further interdisciplinary scientific research evaluation.Methods Feature matching method,Delphi expert enquiry,and normalization method were adopted to establish an evaluation system in clinical medicine,which was further verified by case studies.Results The normalization coefficient reflects the difficulty of publishing articles with high impact factors.Through the process of feature matching and normalization,it was found that compared with the fields of Neurology,Psychiatry and Surgery,researchers in the fields of Oncology and Internal Medicine were more likely to publish articles with high impact factors.Conclusions Through case analysis,this study verifies the feasibility of interdisciplinary scientific paper evaluation system.The interdisciplinary paper evaluation system based on feature matching method and normalized evaluation method comprehensively considers the characteristics of various disciplines of clinical medicine and provides a new idea for the evaluation of clinical scientific research talents.

Venous thromboembolism (VTE) is one of common complications in hospitalized patients. Hospitalized patient tend to have high risk factors to develop VTE. As an important part of VTE prevention and treatment, it is of great significance for nurses to accurately identify risk factors, to conduct risk assessment of VTE in a timely manner, and to take appropriate preventive measures.
