1.Association of urinary cadmium levels with peripheral leukocyte classification counts among middle-aged and older adults aged 40-89 in selected areas of China

Objective:To investigate the association of urinary cadmium levels with peripheral leukocyte classification counts among middle-aged and older adults aged 40 to 89 years in selected areas of China.Methods:The research was based on the survey of the impact of soil quality of agricultural land on human health in typical areas conducted in 2019-2020. A total of 5 600 middle-aged and older adults aged 40 to 89 years were included by using a multi-stage stratified random sampling method. Baseline characteristics of the subjects were collected and physical examinations were performed. Random midstream urine was collected to measure urinary cadmium and urinary creatinine and fasting venous blood was collected to measure the leukocyte count, neutrophil count, lymphocyte count, monocyte count and eosinophil count. The linear mixed effect model was used to analyse the association of urinary cadmium levels with leukocyte classification counts, and the dose-response relationship between them was analyzed by using the restricted cubic spline (RCS) function.Results:The age of the subjects was (63.17±12.02) years; 2 851 (50.91%) were males; and the M ( Q 1, Q 3) of urinary creatinine-corrected urinary cadmium levels was 2.69 (1.52, 4.69) μg/g·creatinine. After adjusting for confounding factors, the results of linear mixed effects model analysis showed that for each 1-unit increase in urinary creatinine-corrected urinary cadmium level, the percentage change [% (95% CI)] of leukocyte count and lymphocyte count was -1.70% (-2.61%, -0.79%) and -1.57% (-2.86%, -0.26%), respectively. RCS function showed a negative linear relationship between urinary creatinine-corrected urinary cadmium levels and leukocyte counts and lymphocyte counts, respectively (all Pnon-linear>0.05). Conclusion:Urinary cadmium levels are negatively associated with leukocyte count and lymphocyte count among middle-aged and older adults aged 40 to 89 years in selected areas of China.
2.The application and reflection of emerging molecular detection technologies in Salmonella detection

Salmonella is an important foodborne pathogen and one of the main causes of diarrhea. Every year, about 550 million people suffer from diarrhea due to Salmonella infection, of which about 230 000 die. It has become a major global public safety issue. The application fields of Salmonella detection involve food safety, water quality monitoring, animal husbandry, public health monitoring, and medical diagnosis. The detection requirements mainly come from three aspects: pathogen identification, serotype identification, drug resistance and virulence identification. In recent years, the detection technology for Salmonella has made rapid progress, especially the emergence and development of emerging molecular detection technologies, providing new perspectives for Salmonella detection in different scenarios. However, due to the diversity of Salmonella serotypes and the complexity of detection scenarios, existing detection technologies still have some pain points (such as long detection time, cumbersome operation steps, low scene adaptability, etc.). This article will elaborate on the application of several emerging molecular detection technologies with distinct characteristics, such as CRISPR Cas technology, digital PCR technology, sequencing technology, and microfluidic technology, in Salmonella detection. It aims to provide a reference for the development and improvement of Salmonella detection technology and the establishment of infection warning and control systems.
3.Analysis of current status of internet-based patient education materials on labor analgesia in China

Objective:To analyze the current status of internet-based patient education materials related to labor analgesia in China.Methods:Labor analgesia-related materials were retrieved and screened according to the search habits of Chinese search engine users. The coverage and accuracy of the content were evaluated by 3 anesthesiologists. The Chinese version of the Patient Education Materials Assessment Tool for Print Materials was used to subjectively assess comprehensibility and operability from a medical text perspective. The consistency of the evaluation results of the three anesthesiologists was analyzed using the intraclass correlation coefficient method. A machine learning method combined with ChatGPT-4.0 was employed to establish a Chinese readability classification model to objectively evaluate the readability difficulty of the included materials from a Chinese text perspective.Results:A total of 97 web pages were retrieved, with 21 valid materials included in the study. The coverage rate of contraindications for labor analgesia was only 62% (13/21), and the accuracy rate of materials exceeding 90% was 71% (15/21). Internet-based materials that were easy to understand accounted for 81% (17/21), while the constituent ratio of internet-based materials with instructional significance were only 5% (1/21). The intraclass correlation coefficient values of consistency evaluation for coverage rate, accuracy rate, and comprehensibility and operability were 0.975, 0.833, 0.758, and 0.773, respectively ( P<0.001). Internet-based materials suitable for compulsory education level were only 5% (1/21), while those suitable for high school and above education level accounted for 43% (9/21). Conclusions:There are numerous internet-based patient education materials related to labor analgesia in China, but the quality needs improvement. In the future, a collaborative model of " anesthesiology+ linguistics" should be developed to provide patients with more comprehensive, accurate, and pregnant-friendly patient education materials.
4.Correlation analysis between personality traits and non suicidal self injury behavior in adolescent depres-sion patients

Objective To explore the correlation between personality traits and non suicidal self injury behavior in adolescent depression patients,in order to enhance understanding of the psychological health risks of depression patients.Methods This study was conducted at the Wuhan Mental Health Center from September 2021 to September 2023.A total of 138 patients with first-onset moderate to severe adolescent depression were included.The patients were divided into a group without self injury suicidal behavior(n=25),a non suicidal self injury(NSSI)group(n=78),and an attempted suicide(SA)group(n=35).Collect general information and data on neurotic personality scores,negative life events,emotional states,and social psychological factors using the Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD-24),and other psychological assessment tools.Using statistical methods for correlation and regression analysis to study the relationship between personality traits and non suicidal self injury behavior.Results There were significant differences in personality traits such as internal and external tendencies,psychoticism,neuroticism,and concealment among the non self injurious suicide behavior group,non suicidal self injurious behavior group,and suicide behavior group.The scores of the non suicidal self injurious behavior group and suicide behavior group were generally higher than those of the non self injurious self injurious behavior group(P<0.05);There were significant differences in self injury behavior scores among these three groups,especially in the suicide behavior group,which had the highest score,but there was no significant difference in life event scores;There were significant differences in scores for anxiety,depression,childhood trauma,stressful life events,and self-esteem among the three groups.Among them,the suicide behavior group had the highest scores for anxiety,depression,childhood trauma,and stressful life events,but the lowest score for self-esteem(P<0.05);Internal and external tendencies,psychoticism,neuroticism,and concealment are significantly correlated with non suicidal self injury behavior,and multiple regression analysis results show that these factors,as well as anxiety,depression,childhood trauma,stressful life events,and self-esteem,are inde-pendent influencing factors of non suicidal self injury behavior.The best predictive threshold for internal and exter-nal tendencies is-10.5%.Conclusion This study reveals the correlation between personality traits and non sui-cidal self injury behavior in adolescent depression patients.Understanding these relationships in clinical practice and intervention strategies can help better identify high-risk groups and provide personalized mental health support.
5.Impact of preoperative sarcopenia on clinical outcomes after radical surgery in gastric cancer patients and its relationship with postoperative cognitive dysfunction

Background and Aims:Preoperative sarcopenia is a syndrome characterized by reduced skeletal muscle mass and strength,and it is associated with various adverse postoperative outcomes.Gastric cancer patients may experience transient or persistent postoperative cognitive dysfunction(POCD),which significantly impacts their quality of life and prognosis.However,it remains unclear whether this complication is linked to sarcopenia.Therefore,this study was conducted to investigate further the impact of preoperative sarcopenia on postoperative complications and long-term outcomes in patients undergoing radical gastric cancer surgery,with a particular focus on the relationship between sarcopenia and POCD,in order to provide insights for preoperative assessment and postoperative management of gastric cancer patients. Methods:The clinical data of gastric cancer patients who underwent radical surgery in the Third Department of Surgery at the Fourth Hospital of Hebei Medical University between January 2014 and January 2015 were retrospectively collected.Patients were divided into the sarcopenia and non-sarcopenia groups based on preoperative L3 skeletal muscle index,handgrip strength,and gait speed measurements.The clinicopathologic characteristics of sarcopenic patients,as well as the impact of sarcopenia on short-term clinical outcomes and long-term prognosis,were analyzed.Additionally,factors influencing the development of POCD were determined. Results:A total of 320 gastric cancer patients were included,of whom 59(18.44%)were diagnosed with sarcopenia.Compared with the non-sarcopenia group,sarcopenic patients had significantly lower bady mass index,serum total protein,serum albumin,and hemoglobin levels,with a higher proportion of patients aged ≥60 years,NRS 2002 score ≥3,comorbid pulmonary disease,and those undergoing open surgery(all P<0.05).After balancing the baseline characteristics of the two groups using propensity score matching(PSM),each group included 59 patients.The analysis revealed that the overall incidence of postoperative complications was higher in the sarcopenia group than in the non-sarcopenia group(54.24%vs.32.20%,P=0.016).The sarcopenia group also had a significantly higher incidence of Clavien-Dindo grade Ⅱ-Ⅳ complications and postoperative infectious complications(27.12%vs.5.08%,P=0.001;33.90%vs.15.25%,P=0.019).The average hospital stay was significantly longer for sarcopenic patients(12.54±4.7 d vs.7.68±3.8 d,P=0.005).Additionally,the 5-year overall survival(OS)and disease-free survival(DFS)rates were lower in the sarcopenia group compared to the non-sarcopenia group(both P<0.05).Cox multivariate analysis showed that sarcopenia,tumor pT stage,and tumor pN stage were independent risk factors for 5-year OS and DFS.At the same time,adjuvant chemotherapy was a protective factor for prognosis(all P<0.05).Among the 118 patients after PSM,34(28.81%)were diagnosed with POCD.Logistic multivariate regression analysis indicated that preoperative sarcopenia,the number of preoperative comorbidities,and anesthesia duration of ≥2 h were independent risk factors for POCD,while intraoperative use of dexmedetomidine was a protective factor(all P<0.05). Conclusion:Preoperative sarcopenia is closely associated with unfavorable postoperative outcomes and the development of POCD in patients undergoing radical gastric cancer surgery.Clinicians should emphasize the detection of sarcopenia during preoperative evaluation and implement proactive interventions and postoperative management strategies to improve clinical outcomes and long-term survival rates.

8.Clinical value of serum PKN1,TNFRSF4 and DDIT4 in predicting lymph node metastasis and prognosis of patients with endometrial cancer

Objective To explore the clinical value of serum protein kinase N1(PKN1),tumor necrosis factor receptor superfamily member 4(TNFRSF4),and DNA damage-inducible transcription factor 4(DDIT4)in predicting lymph node metastasis and prognosis of patients with endometrial cancer.Methods A total of 180 cases of endometrial cancer patients treated in Tangshan Maternal and Child Health Hospital from October 2019 to October 2021 were selected as endometrial cancer group.In addition,180 patients with benign uterine diseases treated in this hospital during the same period were selected as the benign disease group,and 180 healthy patients who underwent physical examination in this hospital were selected as the healthy group.The endometrial cancer group was divided into 42 cases with lymph node metastasis and 138 cases without lymph node metastasis according to whether lymph node metastasis occurred.The patients were divided into poor prognosis group and good prognosis group according to the follow-up results.The levels of serum PKN1,TNFRSF4 and DD1T4 in each group were compared,the influencing factors of patient prognosis were analyzed by Logistic model,and the predictive value of serum PKN1,TNFRSF4 and DDIT4 on prognosis of patients was analyzed by drawing the receiver operating characteristic curve.Results Compared with the healthy group,the levels of serum PKN1 and DDIT4 in endometrial cancer group and benign disease group were in-creased,and the levels of serum TNFRSF4 were decreased,with statistical significance(P<0.05).The serum levels of PKN1 and DDIT4 in lymph node metastasis group were higher than those in no lymph node metasta-sis group,and the levels of TNFRSF4 were lower than those in no lymph node metastasis group,with statisti-cal significance(P<0.05).The proportion of poor prognosis group with low differentiation degree,positive lymphovascular space invasion and myometrial infiltration≥1/2 was higher than that of good prognosis group,and the difference was statistically significant(P<0.05).The levels of serum PKN1 and DDIT4 in the poor prognosis group were higher than those in the good prognosis group,and the levels of TNFRSF4 were lower than those in the good prognosis group,with statistical significance(P<0.05).Serum PKN1,DDIT4,LVSI and myometrial infiltration were risk factors for the prognosis of endometrial cancer,and serum TN-FRSF4 was protective factor for the prognosis of endometrial cancer(P<0.05).Serum PKN1,TNFRSF4 and DDIT4 combined predicted the prognosis of patients with endometrial cancer more effectively than each serum index alone(P<0.05).Conclusion Serum PKN1,TNFRSF4 and DDIT4 are related to lymph node metasta-sis and prognosis of patients with endometrial cancer,and have high predictive efficacy for patient prognosis.
9.Nephrotoxicity caused by polymyxin in the treatment of drug-resistant Gram-negative bacterial infections:a meta-analysis

OBJECTIVE To evaluate the incidence of nephrotoxicity in patients with drug-resistant Gram-negative bacterial infections after the use of polymyxin, and to provide evidence-based reference for clinical rational drug use. METHODS PubMed, Embase, Web of Science, the Cochrane Library, Wanfang database, CNKI, VIP and SinoMed were searched to collect randomized controlled trials (RCTs) or cohort studies about the polymyxin (trial group) versus other antibiotics (control group) or polymyxin B (trial group) versus polymyxin E (control group). After literature screening, data extraction and quality evaluation, RevMan 5.4.1 software was used for meta-analysis. RESULTS A total of 37 studies were included, including 4 RCTs and 33 cohort studies, with a total of 5 871 patients. The meta-analysis results showed that in RCT [RR=2.64,95%CI (1.43,4.87),P=0.002] and in cohort studies [RR=1.59, 95%CI (1.27, 1.98), P<0.000 1], the incidence of nephrotoxicity in the trial group was significantly higher than control group. The results of the subgroup analysis of cohort studies showed that the incidence of nephrotoxicity in the trial group (receiving polymyxin) was significantly higher than control group (receiving new β-lactam and β-lactamase inhibitors and tigecycline); when Kidney Disease Improving Global Outcomes (KDIGO), renal replacement therapy or 0.5 times increase in serum creatinine were used as the standard of nephrotoxicity, the incidence of nephrotoxicity in the trial group was significantly higher than the control group (P<0.05). The incidence of nephrotoxicity in patients receiving polymyxin E was significantly higher than those using polymyxin B [RR=0.57, 95%CI (0.39,0.84), P=0.005]. CONCLUSIONS In the treatment of drug-resistant Gram-negative bacteria infections, the incidence of nephrotoxicity caused by polymyxin is relatively high. The TYU108F); incidence of nephrotoxicity caused by polymyxin E is higher than polymyxin B.
10.Analysis of treatment effect of 125I seed implantation in patients with radioactive iodine-refractory differentiated thyroid cancer

Objective:To investigate the efficacy and safety of 125I seed implantation in the treatment of radioactive iodine-refractory differentiated thyroid cancer (RAIR-DTC). Methods:Fourteen patients (8 males, 6 females, age: (62.0±10.7) years) with RAIR-DTC confirmed by pathology or imaging examination in the Central Hospital of Wuhan, Tongji Medical College between July 2017 and March 2021, were retrospectively analyzed. The patients were treated with 125I seed implantation guided by CT. Ultrasound, CT, 125I-SPECT/CT or MRI were performed at 1.5, 3, 6 and 12 months after the implantation to evaluate the changes of lesion volume, and serum thyroglobulin (Tg), as well as symptom relief were monitored and recorded. The paired t-test was used for data analysis. Results:The 125I seeds were successfully implanted (16 operations) in 14 patients with 25 lesions. Patients were followed up for 3-44 months (median: 6.5(4.5, 11.5) months). The total effective rate was 60.0%(15/25) and the total local control rate was 96.0%(24/25). The effective rate for metastatic lymph nodes was 10/17, and the local control rate was 16/17. The effective rate and the local control rate for local recurrence were 1/3 and 3/3 respectively, and those for bone metastasis were both 3/3, those for sinus metastasis were 0/1 and 1/1 respectively, and those for lung metastasis were both 1/1. In 8 patients with clinical symptoms, symtoms of 4 cases were completely relieved, those of 3 cases were partially relieved and 1 case had no remission. The Tg level and the tumor length were both decreased after operation ((245.99±44.85) μg/L vs (330.38±50.78) μg/L, t=2.92, P=0.010; (2.71±0.34) cm vs (3.78±0.27) cm, t=3.13, P=0.007). Conclusions:125I seed implantation, as a supplementary treatment for RAIR-DTC, is safe and effective. It has a good effect against metastatic and local recurrent lesions.
