1.Effects of Yishen paidu formula on renal fibrosis in rats with chronic renal failure by regulating the ROS/TXNIP/NLRP3 pathway

OBJECTIVE To investigate the effects and mechanism of the Yishen paidu formula on renal fibrosis in rats with chronic renal failure (CRF) through the reactive oxygen species (ROS)/thioredoxin-interacting protein (TXNIP)/NOD-like receptor thermal protein domain associated protein 3 (NLRP3) pathway. METHODS Rats were randomly divided into control group, model group, Yishen paidu formula low-dose (Yishen paidu formula-L) group, Yishen paidu formula high-dose (Yishen paidu formula- H) group, Yishen paidu formula-H+pcDNA-NC group, and Yishen paidu formula-H+ pcDNA-TXNIP group, with 10 rats in each group. Except for control group, all other rats were fed a diet containing 0.5% adenine to establish a CRF model; the rats were then administered corresponding drugs or normal saline intragastrically or via tail vein, once daily, for 8 consecutive weeks. After the last administration, the levels of serum creatinine (Scr), blood urea nitrogen (BUN), ROS, superoxide dismutase (SOD), malondialdehyde (MDA), tumor necrosis factor-α (TNF-α), interleukin (IL)-6, and IL-1β were measured in each group. Pathological changes in renal tissue were observed, and the protein expression levels of Collagen Ⅲ, α-smooth muscle actin (α-SMA), transforming growth factor-β1 (TGF-β1), TXNIP and NLRP3 in renal tissue were detected. RESULTS Compared with model group, the renal histopathological damage and fibrosis of rats in Yishen paidu formula-L group and Yishen paidu formula-H group were significantly alleviated. The levels of Scr, BUN, ROS, MDA, TNF- α, IL-6 and IL-1β, and the protein expressions of Collagen Ⅲ, α-SMA, TGF-β1, TXNIP and NLRP3 were significantly decreased, while SOD levels were significantly increased (P<0.05). Moreover, the changes were more pronounced in the Yishen paidu formula-H group (P<0.05). Compared with Yishen paidu formula-H+pcDNA-NC group, above indexes of rats in Yishen paidu formula-H+pcDNA-TXNIP group were reversed significantly (P<0.05). CONCLUSIONS Yishen paidu formula can inhibit renal fibrosis in CRF rats by suppressing the ROS/TXNIP/NLRP3 pathway.

Oral squamous cell carcinoma (OSCC) is a common head and neck malignancy. Approximately 50% to 60% of patients with OSCC are diagnosed at a locally advanced stage (clinical staging III-IVa). Even with comprehensive and sequential treatment primarily based on surgery, the 5-year overall survival rate remains below 50%, and patients often suffer from postoperative functional impairments such as difficulties with speaking and swallowing. Programmed death receptor-1 (PD-1) inhibitors are increasingly used in the neoadjuvant treatment of locally advanced OSCC and have shown encouraging efficacy. However, clinical practice still faces key challenges, including the definition of indications, optimization of combination regimens, and standards for efficacy evaluation. Based on the latest research advances worldwide and the clinical experience of the expert group, this expert consensus systematically evaluates the application of PD-1 inhibitors in the neoadjuvant treatment of locally advanced OSCC, covering combination strategies, treatment cycles and surgical timing, efficacy assessment, use of biomarkers, management of special populations and immune related adverse events, principles for immunotherapy rechallenge, and function preservation strategies. After multiple rounds of panel discussion and through anonymous voting using the Delphi method, the following consensus statements have been formulated: 1) Neoadjuvant therapy with PD-1 inhibitors can be used preoperatively in patients with locally advanced OSCC. The preferred regimen is a PD-1 inhibitor combined with platinum based chemotherapy, administered for 2-3 cycles. 2) During the efficacy evaluation of neoadjuvant therapy, radiographic assessment should follow the dual criteria of Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 and immune RECIST (iRECIST). After surgery, systematic pathological evaluation of both the primary lesion and regional lymph nodes is required. For combination chemotherapy regimens, PD-L1 expression and combined positive score need not be used as mandatory inclusion or exclusion criteria. 3) For special populations such as the elderly (≥ 70 years), individuals with stable HIV viral load, and carriers of chronic HBV/HCV, PD-1 inhibitors may be used cautiously under the guidance of a multidisciplinary team (MDT), with close monitoring for adverse events. 4) For patients with a poor response to neoadjuvant therapy, continuation of the original treatment regimen is not recommended; the subsequent treatment plan should be adjusted promptly after MDT assessment. Organ transplant recipients and patients with active autoimmune diseases are not recommended to receive neoadjuvant PD-1 inhibitor therapy due to the high risk of immune related activation. Rechallenge is generally not advised for patients who have experienced high risk immune related adverse events such as immune mediated myocarditis, neurotoxicity, or pneumonitis. 5) For patients with a good pathological response, individualized de escalation surgery and function preservation strategies can be explored. This consensus aims to promote the standardized, safe, and precise application of neoadjuvant PD-1 inhibitor strategies in the management of locally advanced OSCC patients.
3.Impact of portal vein tumor thrombus classification on rebleeding in hepatocellular carcinoma patients with esophagogastric variceal bleeding

ObjectiveTo investigate the impact of portal vein tumor thrombus (PVTT) classification on rebleeding in hepatocellular carcinoma (HCC) patients with different PVTT subtypes and esophagogastric variceal bleeding (EGVB), and to provide a reference for formulating rational treatment regimens for such patients. MethodsA retrospective study was performed for 130 patients with HCC and PVTT who were treated due to EGVB in Beijing Ditan Hospital, Capital Medical University, from July 2020 to January 2025, and according to whether endoscopic treatment was performed, the patients were divided into endoscopic treatment group with 97 patients and conservative treatment group with 33 patients. Demographic and clinical data were collected from all patients, and the two groups were compared in terms of hemostasis success rate and rebleeding rate. The independent-samples t test was used for comparison of normally distributed continuous data between groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups. The Kaplan-Meier method was used to estimate the cumulative incidence rate of rebleeding in patients with different subtypes of PVTT. Propensity score matching (PSM) was performed for the endoscopic treatment group and the conservative treatment group to balance the baseline data of the two groups. The Cox proportional hazards model was used to perform univariate and multivariate analyses and identify independent risk factors for rebleeding. ResultsThe endoscopic treatment group had a significantly lower cumulative rebleeding rate within 6 months than the conservative treatment group (35.1% vs 57.6%, hazard ratio [HR]=0.480, 95% confidence interval [CI]: 0.272—0.851, P=0.019). The PVTT Ⅲ—Ⅳ group had a significantly higher cumulative rebleeding rate within 6 months than the PVTT Ⅱ group (52.2% vs 37.5%, HR=1.744, 95%CI: 1.008 — 3.018, P=0.022). After PSM, there was no significant difference in rebleeding rate between the endoscopic treatment group and the conservative treatment group (38.1% vs 14.3%,HR=1.500,95%CI:0.125 — 2.002, P=0.588), while the PVTT Ⅲ—Ⅳ group had a significantly higher cumulative rebleeding rate than the PVTT Ⅱ group (58.8% vs 12.5%,HR=1.561,95%CI:1.195 — 12.499,P=0.033). The multivariate Cox regression analysis showed that PVTT subtype (HR=1.412, 95%CI: 0.998 — 1.997, P=0.049), platelet count (HR=1.006, 95%CI: 1.001 — 1.010, P=0.021), C-reactive protein (HR=1.011, 95%CI: 1.001 — 1.021, P=0.026), and ascites (HR=1.803, 95%CI: 1.059 — 3.068, P=0.030) were independent risk factors for rebleeding. ConclusionFor HCC patients with PVTT and EGVB, endoscopic treatment can successfully achieve hemostasis, while it fails to significantly reduce rebleeding rates. PVTT classification can affect the risk of rebleeding, and patients with PVTT types Ⅲ—Ⅳ have a relatively high rebleeding rate.

5.Study on the Expression and Clinical Pathological Characteristics of Serum NOP53 mRNA and FNDC1 mRNA in Patients with Locally Advanced Rectal Cancer and Their Value in Evaluating the Efficacy of Neoadjuvant Radiotherapy and Chemotherapy

Objective To investigate the value of serum ribosomal biogenesis factor nucleolar protein53(NOP53)mRNA and fibronectin type Ⅲ domain containing 1(FNDC1)mRNA in evaluating the efficacy of neoadjuvant chemoradiotherapy(NACRT)in patients with locally advanced rectal cancer.Methods A total of 140 patients with locally advanced rectal cancer who received NACRT treatment in Yulin Hospital the First Affiliated Hospital of Xi'an Jiaotong University from January 2020 to January 2023 were selected as the study group.According to the efficacy of NACRT treatment,they were divided into good response group(99 cases)and poor response group(41 cases).At the same time,70 healthy people were selected as control group.Real-time fluorescence quantitative PCR was used to detect serum NOP53 mRNA and FNDC1 mRNA levels in the two groups.Logistic regression analysis was used to analyze the factors affecting the efficacy of NACRT.The value of serum NOP53 mRNA and FNDC1 mRNA in evaluating the efficacy of NACRT for rectal cancer was analyzed by receiver operating characteristic analysis.Results The expression of serum NOP53 mRNA(3.21±0.36)and FNDC1 mRNA(2.73±0.34)in the study group was higher than that in the control group(0.61±0.17,0.72±0.18),and the differences were statistically significant(t=57.267,46.287,all P<0.001).The expression of serum NOP53 mRNA(4.08±0.43,4.10±0.40)and FNDC1 mRNA(3.62±0.39,3.40±0.39)in patients with T stage T4 and N stage N1+N2 rectal cancer was higher than that in patients with T stage T3(2.52±0.30,2.02±0.29)and N stage N0(2.21±0.31,1.02±0.30),and the differences were statistically significant(t=25.241~40.106,all P<0.001).The proportion of T stage T4(73.17%),N stage N1+N2(75.61%),serum NOP53 mRNA(5.56±0.39)and FNDC1 mRNA(4.42±0.38)in the poor response group were higher than those in the good response group(32.32%,43.43%,2.24±0.31,2.03±0.29),and the differences were statistically significant(t=12.045~53.337,all P<0.001).T stage T4,N stage N1+N2,high serum NOP53 mRNA,high serum FNDC1 mRNA were risk factors affecting the efficacy of NACRT for rectal cancer(Wald χ2=9.463~15.589,all P<0.001).The AUC of serum NOP53 mRNA combined with FNDC1 mRNA in evaluating the efficacy of NACRT for rectal cancer higher than predicted by serum NOP53 mRNA and FNDC1 mRNA alone,and the differences were statistically significant(Z=4.645,4.321,all P<0.001).Conclusion The levels of serum NOP53 mRNA and FNDC1 mRNA in patients with locally advanced rectal cancer are increased,which are related to the poor clinicopathological features of patients.Combined with serum NOP53 mRNA,FNDC1 mRNA can effectively predict the clinical efficacy of NACRT in patients with rectal cancer.
6.Effects of two intermittent fasting strategies on postprandial lipid metabolism in adults

ObjectiveTo investigate the effects and potential mechanisms of morning and evening fasting on postprandial lipid responses, a post hoc analysis based on a crossover randomized controlled trial was conducted to assess the effects of different fasting strategies on postprandial lipid metabolism in community residents in Shanghai. MethodsA total of 23 participants took part in a randomized crossover trial involving two intervention days: morning fasting and evening fasting, with a washout period of 6 days between intervention days. Two-way analysis of variance was used to test the differences in total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), and the relative expression of circadian clock genes before and after the next meal under fasting. Wilcoxon rank sum tests were used to analyze the different metabolites between the two groups. Principal component analysis and Orthogonal partial least squares-discriminant analysis were conducted to evaluate the ability of metabolites to differentiate between morning fasting and evening fasting and identify the important differential metabolites. After adjusting for age, sex, and BMI, a partial correlation analysis was performed to identify metabolites associated with plasma lipids. In addition, important metabolites associated with plasma lipids were computed by pathway enrichment analysis. ResultsAfter evening fasting intervention, fasting TG level [(0.37±0.29) vs (0.27±0.18)] mmol·L-1, fasting and postprandial change values in TC [(2.74±0.47) vs (2.51±0.27)] mmol·L-1 and LDL-C [(1.32±0.38) vs (0.99±0.27)] mmol·L-1 were significantly lower than those after morning fasting (P<0.05). While, change values of fasting LDL-C [(0.89±0.37) vs (1.14±0.37)] mmol·L-1 and TG [(1.14±0.19) vs (1.28±0.17)] mmol·L-1 were significantly higher than those after morning fasting intervention (P<0.05). After fasting intervention, the relative expression of AMPK, CRY1, CLOCK, MTNR1B, AANAT, and ASMT was correlated with the amount of plasma lipid changes (P<0.05). Specifically, CLOCK and AANAT were upregulated following evening fasting and downregulated after morning fasting. Among the 217 important differential metabolites, 111 were correlated with plasma lipids, and which were primarily enriched in the cysteine and methionine metabolism pathways (P<0.05). ConclusionCompared to morning fasting, evening fasting was more effective in improving postprandial lipid responses, indicating that an evening fasting window during intermittent fasting could be conducive to cardiovascular disease prevention in adults. Meanwhile, it is suggested that morning and evening fasting may affect lipid responses through circadian rhythm oscillations and the cysteine and methionine metabolism pathways.
7.Analysis of the clinical effect of tirofiban in the treatment of early neurological deterioration in patients with acute ischemic stroke

OBJECTIVE To investigate the efficacy and safety of tirofiban for early neurological deterioration in patients with acute ischemic stroke. METHODS A total of 126 patients with early neurological deterioration of acute ischemic stroke who were admitted to the Department of Neurology, Heji Hospital Affiliated to Changzhi Medical College from January 2022 to December 2023 were selected and divided into observation group and control group according to random number table method, with 63 cases in each group. All patients received standardized treatment such as lipid-lowering and blood pressure-lowering therapy. Based on the standard treatment, patients in the control group additionally took Aspirin enteric-coated tablets 100 mg+Clopidogrel bisulfate tablets 75 mg orally (once a day, for 14 consecutive days). The patients in the observation group received Tirofiban hydrochloride and sodium chloride injection based on the standardized treatment [first intravenous infusion of 0.40 μg/(kg·min) for 30 min, and then continuous intravenous infusion of 0.10 μg/(kg·min) for 47.5 h]; subsequently, patients were given Aspirin enteric-coated tablets (100 mg) and Clopidogrel bisulfate tablets (75 mg) once a day for 14 consecutive days. The clinical efficacy, the National Institutes of Health Stroke Scale (NIHSS) score, modified Rankin Scale (mRS) score, and hemorheological indexes before and after treatment were compared between the two groups, and the adverse reactions were recorded. RESULTS The total effective rate (87.30%) of the observation group was significantly higher than that of the control group (71.43%) (P<0.05). NIHSS scores of the two groups at 1st, 7th and 14th day after treatment, the mRS score at 90th day after treatment, and the platelet aggregation rate, whole blood viscosity, plasma viscosity and fibrinogen at 14th day after treatment were significantly lower than those before treatment in the same group, and the observation group was significantly lower than the control group at the same period (P<0.05). The total incidences of adverse reactions such as nausea, headache, fever, gastrointestinal bleeding, oral and nasal mucosal bleeding and thrombocytopenia in both groups of patients were 28.57% respectively, with no statistically significant difference (P>0.05). CONCLUSIONS For patients with early neurological deterioration in acute ischemic stroke, the addition of tirofiban can accelerate the recovery of neurological function, improve blood hyperviscosity and platelet aggregation, and improve the prognosis of patients with good safety.
8.SMUG1 promoted the progression of pancreatic cancer via AKT signaling pathway through binding with FOXQ1.

BACKGROUND:
Pancreatic cancer is a lethal malignancy prone to gemcitabine resistance. The single-strand selective monofunctional uracil DNA glycosylase (SMUG1), which is responsible for initiating base excision repair, has been reported to predict the outcomes of different cancer types. However, the function of SMUG1 in pancreatic cancer is still unclear.
METHODS:
Gene and protein expression of SMUG1 as well as survival outcomes were assessed by bioinformatic analysis and verified in a cohort from Fudan University Shanghai Cancer Center. Subsequently, the effect of SMUG1 on proliferation, cell cycle, and migration abilities of SMUG1 cells were detected in vitro . DNA damage repair, apoptosis, and gemcitabine resistance were also tested. RNA sequencing was performed to determine the differentially expressed genes and signaling pathways, followed by quantitative real-time polymerase chain reaction and Western blotting verification. The cancer-promoting effect of forkhead box Q1 (FOXQ1) and SMUG1 on the ubiquitylation of myelocytomatosis oncogene (c-Myc) was also evaluated. Finally, a xenograft model was established to verify the results.
RESULTS:
SMUG1 was highly expressed in pancreatic tumor tissues and cells, which also predicted a poor prognosis. Downregulation of SMUG1 inhibited the proliferation, G1 to S transition, migration, and DNA damage repair ability against gemcitabine in pancreatic cancer cells. SMUG1 exerted its function by binding with FOXQ1 to activate the Protein Kinase B (AKT)/p21 and p27 pathway. Moreover, SMUG1 also stabilized the c-Myc protein via AKT signaling in pancreatic cancer cells.
CONCLUSIONS
SMUG1 promotes proliferation, migration, gemcitabine resistance, and c-Myc protein stability in pancreatic cancer via protein kinase B signaling through binding with FOXQ1. Furthermore, SMUG1 may be a new potential prognostic and gemcitabine resistance predictor in pancreatic ductal adenocarcinoma.

9.Predictive value of a nomogram model based on clinical risk factors for ischemic stroke in patients with atrial fibrillation

Objective To explore the risk factors for ischemic stroke in patients with atrial fibrilla-tion(AF)and construct a prediction model.Methods A retrospective study was conducted on 253 AF patients treated at our hospital from January 2021 to December 2024.They were randomly divided into a modeling group(n=177)and a validation group(n=76)in a 7∶3 ratio.The model-ing group was further divided into a simple AF subgroup(96 cases)and a stroke subgroup(81 cases)based on the occurrence of ischemic stroke.Clinical data were collected,including low-density lipoprotein cholesterol,red cell distribution width(RDW),D-dimer,fibrinogen,N-terminal pro-B-type natriuretic peptide(NT-proBNP),high-sensitivity cardiac troponin I(hs-cTnI),etc.Univariate and multivariate logistic regression analyses were performed to identify risk factors for ischemic stroke,and a nomogram model was constructed.The predictive performance of the model was evaluated using ROC curve analysis,calibration curve analysis,and decision curve analysis(DCA).Results Multivariate logistic regression analysis indicated that left atrial diameter(OR=1.205,95%CI:1.046-1.388,P=0.010),RDW(OR=1.605,95%CI:1.138-2.263,P=0.007),D-dimer(OR=2.633,95%CI:1.229-5.640,P=0.013),fibrinogen(OR=3.053,95%CI:1.312-7.108,P=0.010),NT-proBNP(OR=1.005,95%CI:1.003-1.007,P=0.001),and hs-cTnI(OR=1.129,95%CI:1.046-1.218,P=0.002)were independent risk factors for ischemic stroke in AF patients.ROC curve analysis showed an AUC value of 0.926(95%CI:0.885-0.967)for the modeling group and 0.912(95%CI:0.847-0.978)for the validation group.H-L test showed Chi-square=8.005,P=0.433 for the modeling group and Chi-square=2.590,P=0.957 for the validation group.DC A demonstrated high practical value for the model.Conclusion The nomo-gram model constructed in the study has good predictive performance and clinical utility.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
