Objective: To characterize the sequence polymorphism of HLA-A, -B, and -C loci in the northern Han Chinese population and to evaluate their selection signals and phylogenetic patterns based on long-read sequencing data. Methods: A total of 408 unrelated healthy blood donors were enrolled. Long-read sequencing of HLA-A, -B, and -C loci was performed using the PacBio Sequel II platform. The Ewens-Watterson neutrality test, sliding-window nucleotide diversity (π) and Tajima′s D analyses were conducted, together with genetic distance heatmaps, principal coordinates analysis (PCoA), and maximum likelihood phylogenetic analysis to assess polymorphism distribution, selection signals, and phylogenetic structure across the three loci. Results: The analyzed sequence lengths for the HLA-A, -B, and -C loci were 3 257 bp, 3 794 bp, and 3 831 bp, respectively; the numbers of locus-specific sequence types were 44, 82, and 56, with corresponding locus-specific sequence type diversities of 0.905 2, 0.964 9, and 0.939 8, respectively. The Ewens-Watterson test showed no significant deviation from neutrality (P>0.05). Sliding-window analysis revealed elevated π and Tajima′s D values in exons 2 and 3 as well as in certain intronic regions of HLA-A and HLA-B, whereas HLA-C exhibited relatively lower overall diversity. Genetic distance heatmaps, PCoA, and phylogenetic analysis consistently showed clear clustering in HLA-A and HLA-C, while HLA-B displayed the highest phylogenetic heterogeneity. Conclusion: HLA-A, -B, and -C gene sequences in the northern Chinese Han population exhibit high levels of polymorphism and marked regional heterogeneity. HLA-B shows greater diversity, stronger signals of selection, and more complex phylogenetic structure compared with HLA-A and HLA-C.

Purpose: This Phase II trial was objected to evaluate the efficacy and safety of adding fulvestrant to neoadjuvant chemotherapy in patients with estrogen receptor (ER)+/human epidermal growth factor receptor 2 (HER2)– locally advanced breast cancer (LABC). Additionally, the study aimed to investigate the association of 16α-18F-fluoro-17β-fluoroestradiol (18F-FES) positron emission tomography (PET)–computed tomography (CT) and metabolites with efficacy. Materials and Methods: Fulvestrant and EC-T regimen were given to ER+/HER2– LABC patients before surgery. At baseline, patients received 18F-FES PET-CT scan, and plasma samples were taken for liquid chromatography–mass spectrometry analysis. The primary endpoint was objective response rate (ORR). Secondary endpoints included total pathologic complete response (tpCR) and safety. Results: Among the 36 patients enrolled, the ORR was 86.1%, the tpCR rate was 8.3%. The incidence of grade ≥ 3 treatment-emergent adverse events was 22%. The decrease in ER value in sensitive patients was larger than that in non-sensitive patients, as was Ki-67 (p < 0.05). The maximum standardized uptake value, mean standardized uptake values, total lesion ER expression of 18F-FES PET-CT in sensitive patients were significantly higher than those in non-sensitive patients (p < 0.05). Moreover, these parameters were significantly correlated with Miller and Payne grade and the change in ER expression before and after treatment (p < 0.05). Thirteen differential expressed metabolites were identified, which were markedly enriched in 19 metabolic pathways. Conclusion This regimen demonstrated acceptable toxicity and encouraging antitumor efficacy. 18F-FES PET-CT might serve as a tool to predict the effectiveness of this therapy. Altered metabolites or metabolic pathways might be associated with treatment response.

Objective:To evaluate the effectiveness of nursing interventions based on self-efficacy theory in patients with primary glaucoma undergoing scleral flap suture trabeculectomy.Methods:A convenience sampling method was used to select 350 patients with primary glaucoma undergoing scleral flap suture trabeculectomy admitted to the Department of Ophthalmology at the First Affiliated Hospital of Zhengzhou University from October 2021 to December 2023. Participants were randomly divided into a control group ( n=175) and an intervention group ( n=175) using a random number table. The control group received routine nursing care, while the intervention group received nursing interventions based on self-efficacy theory for a duration of three months. Visual status (visual acuity and intraocular pressure), self-management behaviors, and vision-related quality of life were compared between the two groups before and after the intervention. Results:After the intervention, the intervention group had significantly better visual acuity and lower intraocular pressure compared to the control group (both P<0.05). Scores across all dimensions of the Glaucoma Self-Management Questionnaire were significantly higher in the intervention group than in the control group, while scores on the 25-item National Eye Institute Visual Function Questionnaire were significantly lower (both P<0.05) . Conclusions:Nursing interventions based on self-efficacy theory are effective in improving visual acuity and reducing intraocular pressure, enhancing self-management behaviors, and improving vision-related quality of life in patients with primary glaucoma undergoing scleral flap suture trabeculectomy.

Objective:To develop a financing model for the urban and rural residents′ basic medical insurance (URBMI) based on medical service costs, providing a scientific foundation for achieving a balance between fund revenue and expenditure.Methods:Drawing on operational experience from regional hospitals, 10 key factors influencing URBMI fund expenditures were distilled: the number of hospital beds per thousand population, annual hospitalization rates per hundred population, average length of stay, bed occupancy rate, county-level hospitalization rate, regional age-specific medical expenditure index, proportion of hospital personnel costs in medical revenue, number of hospital staff per bed, average annual personnel costs per employee, and the average reimbursement rate for insured individuals. Based on these 10 key factors, a URBMI financing model for a sample region was developed, and the reasonable per-capita medical expenditure and the sustainability threshold of per-capita financing were preliminarily estimated.Results:Model calculations revealed that the predicted reasonable per-capita medical expenditure of the sample county was 4 140.9 yuan. Assuming an average reimbursement rate of 50% for URBMI enrollees, the per-capita financing requirement was at least 2 070.5 yuan. Sensitivity analysis showed that the sustainability threshold of per-capita financing was more sensitive to changes in average length of stay and the proportion of hospital personnel costs in medical revenue.Conclusions:This study constructed a URBMI financing model based on 10 key factors affecting fund expenditures. By adjusting regional characteristic variables, the model can reflect how regional economic conditions and health-care needs influence URBMI financing requirements.

Objective:To explore the association between the variability of triglyceride glucose index (TyG) and the risk of type 2 diabetes mellitus(T2DM).Methods:This study was a retrospective cohort study. A total of 22 929 community-dwelling elderly (aged≥60 years) who received annual health check-ups in Kunshan city of Suzhou Municipality during 2014 to 2021 were enrolled in the study. Fasting triglycerides and blood glucose were measured during annual physical check-ups and the TyG was calculated, the standard deviation of TyG measurements in three consecutive physical check-ups was used as the indicatior of TyG long-term variability. According to the quartile of TyG long-term variability, the study subjects were divided into four groups, namely Q 1 (0-0.14), Q 2 (>0.14-0.22), Q 3 (>0.22-0.33), Q 4 (>0.33-1.90). The outcome variable was the occurrence of T2DM. The relationship between TyG variability and T2DM incidence was analyzed by multivariate Cox regression. Results:In the study cohort 11 518 (50.2%) were females and the mean age was (67.42±5.35) years. By the end of follow-up, 2 934 cases of new T2DM were diagnosed, with an oveall incidence rate of 12.8%. After adjusting for multiple confounders and average TyG, long-term variability of TyG was significantly associated with T2DM risk ( HR=1.83, 95% CI: 1.51-2.20). The risk of T2DM in Q 4 group was significantly higher than that in Q 1 group ( HR=1.33, 95% CI: 1.19-1.47). Kaplan-Meier survival curve showed that long-term variability of TyG was significantly correlated with the cumulative risk of T2DM incidence ( P<0.001). Conclusions:TyG variability is an independent risk factor for T2DM, suggesting that attention should be paid not only to specific time-point TyG levels but also to TyG fluctuation for early identification of T2DM risk.

Ferroptosis is a form of regulated cell death that depends on iron and reactive oxygen spe-cies.Different from apoptosis,necrosis,and autophagy,ferroptosis is characterized by the accumulation of lipid peroxides in cells.Studies have discovered that ferroptosis is closely associated with the occurrence and develop-ment of tumors and inducing ferroptosis in tumor cells can enhance the therapeutic effects of drugs on tumors.This article summarizes the latest research progress in ferroptosis regarding its mechanisms and associations with tumors,aiming to provide a reference for further understanding the interaction mechanisms between ferroptosis and tumors and offering new insights and targets for the treatment of tumors.
