1.Evaluation of traditional Chinese medicine apprenticeship education in Chinese herbal curriculum of western medical institutions

Objective To explore the application and effectiveness of the apprenticeship education model in Chinese herbal medicine teaching at Western medical college.Methods By comparing classic lecture-based teaching with a combined approach integrating apprenticeship education,the study assesses the impact on student learning outcomes.Participants included students from the 2018 cohort of the eight-year clinical medicine program and the 2022 cohort of the"4+4"pilot program at Peking Union Medical College,who received classic teaching methods and apprenticeship case-based teaching methods,respectively.Upon course completion,students completed a 14-item multiple-choice questionnaire covering essential theory of Chinese medicine,as well as specific categories such as qi-regulating,blood-activating herbs,among others.Results The overall accuracy rate in the apprenticeship case-based teaching group was significantly higher than that in the classic teaching group(P<0.01).Conclusions The apprenticeship education model of Traditional Chinese Medicine has a positive effect on teaching of Chinese herbal medicine at West-ern medical college and warrants further promotion and application.
2.The Pathogenic Evolution and Therapeutic Approaches of Rheumatoid Arthritis Complicated by Sarcopenia Based on the Theory of 'Bi (痹) of both Body and Viscera'

Based on the theory of 'bi (痹) of both body and viscera', this paper systematically discussed the pathogenic evolution of rheumatoid arthritis (RA) complicated by sarcopenia from the perspective of traditional Chinese medicine (TCM). It is proposed that a transformation pathway is muscle bi to spleen bi and then atrophy bi, and the core pathogenesis is diaharmony of ying (营) and wei (卫). Combining modern molecular biology research, it is suggested that imbalances in skeletal muscle homeostasis mediated by the tumor necrosis factor-like weak inducer of apoptosis-ibroblast growth factor-inducible 14 (TWEAK-Fn14) axis may provide the material basis for the evolution of this disease. Furthermore, this authors proposed TCM treatment strategies, including "regulating and tonifying ying and wei, warming yang and moving bi, nourishing the viscera by supporting the body", and "promoting the spleen and resolving turbidity, clearing heat and promoting circulation, regulating the organs and calming the body". These strategies emphasize the simultaneous treatment of body and viscera, integrated prevention and treatment, so as to provide TCM theoretical foundation and clinical approaches for preventing and treating RA complicated by sarcopenia.
3.Genetic analysis of fetus with DMD gene mutation accidentally found in prenatal diagnosis with chromosomal microarray analysis

Objective:To explore the clinical significance of chromosome microarray analysis (CMA) in the prenatal detection of DMD gene variants in fetuses without family history during prenatal diagnosis.Methods:A retrospective analysis was conducted on amniotic fluid samples from 12 629 pregnant women who underwent CMA prenatal diagnosis due to high-risk factors at Women and Children′s Hospital Affiliated to Qingdao University between January 2019 and December 2024. Samples with CMA-indicated DMD gene variants were further verified by multiplex ligation-dependent probe amplification (MLPA).Results:(1) Among 12 629 amniotic fluid samples, CMA detected 11 samples with DMD gene deletions or duplications (6 male and 5 female fetuses), which were confirmed by MLPA. (2) All 11 samples with DMD gene variants had no family history of genetic diseases, including 5 deletions and 6 duplications. All of the 5 DMD gene deletions occurred in male fetuses and were all pathogenic, and the pregnant women chose to terminate the pregnancies. Among the 6 DMD gene duplications cases, 1 male fetus was diagnosed as pathogenic and had pregnancy termination; the other 5 duplication cases were female fetuses, in which 1 were pathogenic and 4 were likely pathogenic. They continued pregnancy until delivery, and follow-up showed no DMD-related symptoms. (3) Pedigree analysis revealed that among the 11 samples with DMD gene variants, 3 were de novo mutations, 7 were inherited from mothers, and 1 had an unknown origin.Conclusions:For fetuses without pseudohypertrophic muscalar dystrophy family history but requiring invasive prenatal diagnosis for other reasons, CMA helps to increase the detection of DMD gene variants in fetuses. Testing pregnant women for DMD pathogenic gene carriers could effectively prevent the birth of pseudohypertrophic muscalar dystrophy children.

The qi-flavor compatibility theory originated from The Yellow Emperor's Inner Classic.This theory is based on yunqi theory,with qi as the core,and recognizes diseases from the imbalance of qi transformation in the internal and external environment of the human body;it combines four-qi and five-flavor theories,and jointly regulates the qi transformation of nature and the human body from the aspects of cold and heat and prosperity and decline;it emphasizes"carefully observing the qi rules and not missing the patho-genesis",and according to the prosperity and decline of yunqi and the cold and warm of the climate,the deficiency and excess of the viscus qi and the cold and heat of the disease,the flavor of medicine and food is used to resist the invasion of abnormal climate on the human body,and adjust the qi transformation of the five internal viscera,so that the human body can be restored to a state of qi activity harmony and yin and yang balance.The imbalance of yin and yang is the root cause of disease,with cold and heat as its manifesta-tions,which are balanced by the four qi;prosperity and decline are the root causes,which are nourished and purged by the five flavors.The combination of four qi and the five flavors is essentially a multi-faceted adjustment of the imbalance of yin and yang in the human body.The mutual compatibility of multiple flavors can expand the scope of action of the drug,restrict the bias of the drug's fla-vor,and achieve both expelling evil and strengthening the body.

7.Significance and role of apprenticeship education in Traditional Chinese Medicine curriculum of western medical institutions

The apprenticeship education of Traditional Chinese medicine(TCM)is an important pathway for the cultivation of talents in TCM education.The combination of institutional education and apprenticeship education is considered to be the most suitable educational model that aligns with the inherent characteristics of TCM education.The current status of TCM education in western medical institutions and the main challenges include the difficulty in transitioning between western and Chinese medical reasoning and limited clinical internship hours for TCM.The strengths and features of TCM apprenticeship education lie in cultural heritage,classical teachings,mentorship,practice orientation and personalized education.Therefore,integration of TCM apprenticeship education and clinical internships for western medical students represents a new educational model for medical undergraduates.

9.Feasibility of evaluating coronary artery calcium score on virtual non-contrast scan in dual-layer spectral detector CT

Objective:
To evaluate the accuracy and feasibility of coronary artery calcium score ( CACS) on virtual non-contrast scan ( VNC) images obtained from coronary artery CT angiography ( CCTA) scan with dual -layer spectral detector CT (SDCT) .
Methods :
The data of 197 patients who underwent CCTA scan in hospital were analyzed retrospectively,and 88 patients with CACS >0 were further analyzed. Linear regression analysis of CACS and coronary artery calcium volume ( CACV) of true non-contrast (TNC) images and VNC images ( CACS-TNC, CACS-VNC,CACV-TNC,CACV-VNC) was performed to obtain linear regression equation and correction coefficients λ 1AVG and λ2AVG .CACS-VNC and CACV-VNC were corrected by the corresponding regression equation and recorded as CCACS-VNC and CCACV-VNC,respectively.Spearman correlation coefficient was used for correlation analysis and Bland-Altman plot was used for consistency test.Mann-Whitney U test was used to compare the difference between the two groups.
Results :
For the total coronary artery,there was a strong correlation between CACS- TNC and CACS-VNC (rs = 0. 952,P <0. 001 ,λ 1AVG = 2. 19 ) ,CACV-TNC and CACV-VNC ( rs = 0. 954,P < 0. 001,λ2AVG = 1. 93) .The results of Mann-Whitney U test showed that there was no significant difference between CACS-TNC and CCACS-VNC or between CACV-TNC and CCACV-VNC,and the Bland-Altman plot showed good consistency between CACS-TNC and CCACS-VNC ,CACV-TNC and CCACV-VNC.
Conclusion
VNC images based on SDCT can accurately measure CACS and be used for cardiovascular risk classification,which is expected to replace TNC scan and reduce the radiation dose of patients.
10.Evaluation of correlation between left ventricular changes and left atrial function in hypertrophic cardiomyopathy based on quantitative cardiac MR

Objective:To explore whether left ventricular interstitial fibrosis is associated with left atrial enlargement and left atrial dysfunction in patients of hypertrophic cardiomyopathy(HCM) with preserved ejection fraction.Methods:From October 2018 to September 2021, 59 HCM including 30 with enlarged maximal left artrial volume index (LAVI max), 29 with normal LAVI max and 28 age-and gender-matched controls were retrospectively enrolled. Imaging protocol included cine sequence, late gadolinium enhancement and T 1 mapping.The relationships between left ventricular mass index (LVMI), quantitative myocardial fibrosis and left atrial-related indexes were analyzed. One-way analysis of variance with Bonferroni post hoc correction or Kruskal-Wallis was performed for continuous variables. Categorical variables were assessed using the Chi-square test or Fisher′s exact test. Pearson or Spearman analysis was used for linear or monotonic nonlinear correlations. Results:The left ventricular end-diastolic volume index, left ventricular end-systolic volume index, left ventricular cardiac output and LVMI of HCM with enlarged LAVI max group were higher than HCM with normal LAVI max group and control group( P<0.05).Correlation analysis showed that LVMI correlated positively with LAVI max( r=0.780, P<0.001) and minimal left artrial volume index (LAVI min) ( r=0.816, P<0.001), extracellular volume correlated positively with LAVI max( r=0.462, P<0.001) and LAVI min( r=0.483, P<0.001),%LGE was correlated positively with LAVI max( r=0.311, P<0.05) and LAVI min( r=0.327, P<0.05),left ventricular index interstitial volume was correlated negatively with left atrial ejection fraction of reservoir ( r=-0.669, P<0.001),left atrial ejection fraction of conduit ( r=-0.472, P<0.001),left atrial ejection fraction of pump ( r=-0.518, P<0.001)and left atrial expansion index( r=-0.626, P<0.001). Conclusion:There is association between LVMI and fibrosis and left atrial enlargement and phases dysfunction in HCM with preserved ejection fraction.
