Objective: To investigate the effect of neurite outgrowth inhibitor extracellular peptide residues 1-40 (NEP1-40) combined with poly (lactic-co-glycolic acid) (PLGA) and gelatin electrospun fiber membrane on facial nerve repair in rats. Methods: According to the principle of random grouping, 108 male SD rats were divided into four groups (n = 27 in each group, approved by the ethics committee), namely, the sham group, control group, PLGA group, and NEP1-40 + PLGA group. A facial nerve fracture model was established for all of the groups except for the sham group. The control group received no further treatment, the PLGA group and the NEP1-40+PLGA group were supported by PLGA membrane, and the NEP1-40+PLGA group received one immediate local injection of NEP1-40 (5 μg/μL) at a dose of 10 μL. Facial nerve function analysis, electrophysiological examination, transmission electron microscope observation, HE staining, and immunohistochemical staining of myelin marker S100β and axonal marker β3-tubulin were used to evaluate the recovery of injured facial nerves of rats at 2, 4 and 8 weeks. Results : At 8 weeks, the facial nerve function score of the NEP1-40+PLGA group was better than that of the control group and PLGA group (P < 0.001), and facial nerve function was significantly restored. Electrophysiological examination of nerve action potentials at the injured facial nerve showed that the amplitude in the NEP1-40+PLGA group was higher than that of the control group and PLGA group (P < 0.001), but there was no significant difference in latency and conduction velocity results between the groups (P > 0.05). At 2, 4, and 8 weeks, transmission electron microscopy showed that the number of myelinated nerve fibers and myelin sheath thickness in the cross-section of the injured facial nerve in the NEP1-40+PLGA group were greater than those in the other groups (P < 0.05). At 8 weeks, HE staining showed that the facial nerves in the control group had partially recovered, but the overall cell distribution was uneven and the boundary with surrounding tissues was slightly blurred. In contrast, the NEP1-40+PLGA group had a relatively uniform cell distribution and a clearer boundary with surrounding tissues. At 2, 4, and 8 weeks, the immunohistochemical results showed that in the cross-section of the injuried facial nerve, NEP1-40 increased the expression of neural markers S100 β and β3-tubulin, especially β3-tubulin, which was close to normal levels (P > 0.05) Conclusion NEP1-40 is beneficial for the generation of new myelin sheaths and axons at the site of injury, and it can promote the repair and regeneration of injured facial nerves to a certain extent, thus accelerating the recovery of injured nerve function.

Objective To explore the effects of transurethral blue laser vaporization of prostate for benign prostatic hyperplasia(BPH)on the sexual function and lower urinary tract symptoms.Methods A retrospective analysis was conducted on 91 BPH patients admitted to our department during Mar.and Aug.2023.The patients were divided into two groups,46 in the blue laser group,and 45 in the transurethral plasmakinetic resection of prostate(PKRP)group.The operation time,bladder irrigation time,catheter indwelling time and postoperative hospitalization time of the two groups were compared.Postvoid residual(PVR),maximum urinary flow rate(Qmax),international prostatic symptoms score(I PSS),quality of life score(QoL),international index of erectile function-5(IIEF-5)score,the Chinese index of premature ejaculation(CIPE)score and incidence of retrograde ejaculation were compared before and 6 months after operation.Results The operation time[(24.66±5.38)min vs.(62.50±3.85)min],bladder irrigation time[(20.40±2.78)h vs.(51.93±5.02)h],catheter indwelling time(1.65±0.89)d vs.(6.73±2.98)d]and postoperative hospitalization time[(4.26±1.57)d vs.(8.45±2.28)d]were significantly shorter in the blue laser group than in the PKRP group(P＜0.05).There were no significant differences in Qmax,PVR,QoL and IPSS between the two groups before and 6 months after operation(P＞0.05).Six months after surgery,the IIEF-5 score and CIPE score of the blue laser group were higher than those of the PKRP group[(20.02±2.63)vs.(17.64±1.75);(30.88±3.45)vs.(26.25±3.51)],but the incidence of retrograde ejaculation was lower(4.35％vs.35.60％),with statistical significance(P＜0.05).Conclusion Blue laser vaporization of the prostate can improve lower urinary tract symptoms and protect sexual function.

Objective:To investigate the clinical effects of free posterior interosseous artery perforator flap on reconstruction of the soft tissue defects of multiple fingers in one hand.Methods:Clinical data of 9 patients with soft tissue defect of multiple fingers in one hand admitted to the Department of Hand Surgery, Yongkang Orthopedic Hospital between January 2021 and August 2023 were retrospectively studied. The patients were 7 males and 2 females, aged between 19 and 55 years old. The soft tissue defects of 2 patients with 3 fingers injury and 7 patients with 2 fingers injury were reconstructed with free posterior interosseous artery perforator flaps. The size of defects in single finger was 1.5 cm×2.5 cm-3.0 cm×4.5 cm. The size of flap was 2.5 cm×7.0 cm-3.5 cm×13.0 cm. The posterior cutaneous nerve of the forearm were sutured to the intrinsic nerve of palmar side of the 14 fingers. Donor sites in forearm were directly sutured. After discharge, regular follow-up at outpatient clinic and through WeChat interviews were conducted to observe the appearance and texture of the flaps, finger fullness, donor site function and appearance, as well as patient satisfaction. For the flaps with anastomosis of cutaneous nerve, the recovery of TPD of the flap was tested. Sensory recovery was assessed according to the British Medical Research Council (BMRC) sensory function assessment criteria. Hand functions were evaluated using the Total Active Movement (TAM) of fingers of the Hand Surgery of the Chinese Medical Association.Results:All of the 20 flaps survived and were evaluated through follow-up visits, which ranged from 6 to 25 months with an average of 13 months. The flaps were soft, unbloated and without obvious pigmentation. The protective sensations were recovered with an average static TPD of 14 flaps anastomosed with cutaneous nerve was of 9.7 (7-13) mm. Among them, S 3+ 10 fingers, S 3 4 fingers; 6 fingers of flap without sutured cutaneous nerve, S 2 5 fingers, S 1 1 finger. According to the TAM of Hand Surgery of Chinese Medical Association, the results were excellent for 15 fingers and good for 5 fingers. There was no scar contracture in the forearm donor sites and motor function was not affected. Conclusion:Posterior interosseous artery perforator flap has the advantages of thin flap, similar texture to the hand, good recovery in sensory, and minimal damage to the donor site. Therefore, it is a good option in reconstruction of soft tissue defects of multiple fingers in one hand.

Objective To analyze and compare the differences between 3D-printed patient-specific instrumentation(PSI)-assisted medial open-wedge high tibial osteotomy(OWHTO)and conventional medial OWHTO in terms of the postoperative mechanical stability,accuracy of weight-bearing alignment adjustment,and clinical outcomes.Methods Data from patients diagnosed with knee osteoarthritis(KOA)and undergoing OWHTO from Jan.2019 to Jan.2022 were collected.The patients were divided into the conventional method group(23 individuals)and 3D-printed PSI-assisted group(18 individuals)based on the surgical methods.The accuracy of correction between the two methods was evaluated by comparing the preoperatively planned target correction of the hip-knee-ankle(HKA)angle with the postoperative HKA angle difference.The preoperative posterior tibial slope(PTS)and postoperative PTS angle differences were also assessed.The clinical efficacy of the two methods was assessed by collecting and analyzing the Lysholm score,visual analog scale(VAS),and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)of the patients in both the groups prior to surgery and at the 1st,6th,12th,and 24th month postoperatively.The occurrence of postoperative complications in both the groups was analyzed to evaluate the safety of PSI-assisted OWHTO.Results There were no differences in demographic characteristics,preoperative imaging observations,and clinical symptoms between the two groups(P>0.05).With regard to the results of correction accuracy,the postoperative HKA angle difference was 2.7°±1.8° in the conventional OWHTO group and 0.8°±1.1° in the 3D-printed PSI-assisted OWHTO group(P<0.001).The postoperative PTS angle difference was 2.8°±2.2° for conventional OWHTO and 1.7°±1.9° for PSI-assisted OWHTO(P=0.003).In terms of clinical efficacy,the surgical time of the PSI-assisted group was 59.2±14.8 min.This was significantly shorter than that of the conventional method group(87.6±21.4 min)(P=0.019).The Lysholm,VAS,and WOMAC scores of the PSI-assisted group were superior to those of the conventional method group at each postoperative follow-up visit.With regard to postoperative complications,there were four cases(17.3%)in the conventional method group and three(16.7%)in the PSI-assisted group.The statistical difference between the two groups is not significant.Conclusions Compared with the conventional method,3D-printed PSI-assisted OWHTO demonstrated superior accuracy in correcting lower limb alignment,in conjunction with favorable clinical efficacy and safety.This study has provided an effective reference for clinicians in selecting surgical treatment plans.

Objective:To investigate the curative effect of transfer of free anteriolateral thigh flap (ALTF) with partial iliotibial tract on reconstruction of composite tissue defects in dorsal wrist/hand.Methods:Ten patients who were admitted in the Department of Hand Surgery of Yongkang Orthopaedic Hospital from March 2018 to August 2022 were entered in the study due to extensive composite tissue defects in dorsal wrists and hands. The patients were 24-48 (37 in average) years old. All patients had tendon defects in 3 or more digits with 35 digits in total. The length of the tendon defects were 5.0-11.0 cm in various digits. The soft tissue defects ranged from 6.0 cm×8.0 cm to 11.0 cm×21.0 cm in size. Six patients had defects accompanied with metacarpal and phalangeal bone fracture and joint capsule. In emergency surgery, all patients received thorough debridement and VSD coverage after internal fixation for combined fractures. At 3-5 days after the primary surgery, free ALTF transfer with partial iliotibial tract was employed to reconstruct the defects on dorsal wrists and hands and the digit extension function. The size of flap was 6.0 cm×9.0 cm-9.0 cm×17.0 cm, and the rest of defect wound was covered by skin graft. All the donor sites were sutured directly, except 1 that received abdominal skin graft. Patients had the follow-ups in outpatient clinics, or via telephone or Wechat, to observe the sensation and appearance of the flap, and the healing of the donor site. The recovery of the extension function of the hand was evaluated according to the totle active motion (TAM) system.Results:All flaps survived except one that had vascular compromise after the surgery and it survived after two explorations and a vascular transfer. Postoperative follow-ups lasted for 6 to 24 months, with 13 months in average. At the last follow-up, a all the flaps were found soft in texture and slightly swollen in appearance. There was no obvious pigmentation or rupture of skin and extensor tendon. Sensory recovery achieved to S 2 in 7 patients and S 3 in 3 patients. The hand function was evaluated according to TAM system and rated as excellent for 3 digits, good for 18 digits, fair for 9 digits and poor for 5 digits. There were linear scars in the donor sites. Conclusion:Free ALTF transfer with partial iliotibial tract in the reconstruction of composite tissue defect of dorsal wrists and hands can not only reconstruct the wound surface, but also the extension of digits, which cuts short the course of treatment.

Peripheral nerve injury (PNI) is a common disease in the oral cavity that can easily lead to loss of function and abnormal appearance. The application of dental pulp stem cells (DPSCs) combined with tissue engineering in the repair of PNI is a research hotspot. DPSCs have the advantages of abundant sources, simple extraction, low immunogenicity and a high proliferation rate in vitro. They can differentiate into Schwann cells (SCs). SCs can induce autophagy and secrete key neurotrophic factors, such as nerve growth factor, brain-derived neurotrophic factor, ciliary neurotrophic factor and glial cell-derived neurotrophic factor. SCs are beneficial for the repair of nerve injury. DPSCs in different periods have differences in immune regulation, anti-inflammatory effects, expression of neural markers, angiogenesis and so on, which provide more diversified choices for nerve repair. At present, the introduction of tissue engineering provides a more controllable and improved microenvironment for DPSCs, which is conducive to the application and development of DPSCs in regenerative medicine and tissue engineering. However, there are still many problems to be solved, such as the selection of stem cells, functional link recovery, uncontrollable direction of axon regeneration, regulation of the peripheral nervous system and mechanism of repair.

Objective:To summarize and analyze the clinical data of Chinese patients with colony-stimulating factor 1 receptor (CSF1R)-related leukoencephalopathy, and clarify the phenotypic and genetic characteristics of Chinese patients.Methods:Medical history of patients with CSF1R-related leukoencephalopathy diagnosed from April 1, 2018 to January 31, 2021 in the department of neurology of 22 hospitals in China was collected, and scores of Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment Scale (MoCA), magnetic resonance severity scale were evaluated. Group comparison was performed between male and female patients.Results:A total of 62 patients were included, and the male-female ratio was 1∶1.95. The age of onset was (40.35±8.42) years. Cognitive impairment (82.3%, 51/62) and motor symptoms (77.4%,48/62) were the most common symptoms. The MMSE and MoCA scores were 18.79±7.16 and 13.96±7.23, respectively, and the scores of two scales in male patients (22.06±5.31 and 18.08±5.60) were significantly higher than those in females (15.53±7.41 , t=2.954, P=0.006; 10.15±6.26, t=3.328 , P=0.003). The most common radiographic feature was bilateral asymmetric white matter changes (100.0%), and the magnetic resonance imaging severity scale score was 27.42±11.40, while the white matter lesion score of females (22.94±8.39) was significantly higher than that of males (17.62±8.74 , t=-2.221, P<0.05). A total of 36 CSF1R gene mutations were found in this study, among which c.2381T>C/p.I794T was the hotspot mutation that carried by 17.9% (10/56) of the probands. Conclusions:The core phenotypic characteristics of CSF1R-related leukoencephalopathy in China are progressive motor and cognitive impairment, with bilateral asymmetrical white matter changes. In addition, there exist gender differences clinically, with severer cognitive impairment and imaging changes in female patients. Thirty-six CSF1R gene mutations were found in this study, and c.2381T>C/p. I794T was the hotspot mutation.

Oral squamous cell carcinoma (OSCC) is a common cancer that develops from oral epithelial cells, it has a high incidence, mortality and teratogenic rate, which poses a serious threat to people′s life and health.The Hippo signaling pathway plays a key role in tumorigenesis, regulation of stem cell homeostasis, tissue regeneration, and organ size control. In OSCC, activation of Hippo signaling pathway can inhibit malignant biological behavior, epithelial mesenchymal transformation and distant metastasis of tumors, and improve the survival rate of patients. Considering the importance of the Hippo signaling pathway in the development of cancer, this paper summarized the composition and regulatory mechanism of Hippo pathway, elaborated the role of Hippo signaling pathway in the occurrence and development of OSCC.At the same time, make a simple generalization about the potential therapeutic approaches and strategies to reduce the risk of drug resistance for OSCC patients targeting this pathway.

Arsenic is a systemic poison ubiquitously distributed in the nature. Long-term arsenic exposure can lead to a variety of cancers. Oxidative stress is a well-recognized mechanism underlying arsenic carcinogenesis. Nuclear factor E2-related factor 2 (Nrf2) is a key transcription factor in cellular antioxidant defense. However, accumulating evidences suggest that environmental arsenic exposure may cause and promote arsenic-related cancers through Nrf2-mediated adaptive antioxidant response, which constitutively induces the expression of antioxidant enzymes and detoxification enzymes and confers survival advantage to the cell under stress. Therefore, elucidating the role of Nrf2 and its medicated antioxidant response in arsenic carcinogenesis is vital to prevention and treatment of cancers related to arsenic exposure.
