1.Imaging signs of osteoid osteoma in short tubular bone and differences from long tubular bone

Objective To investigate the imaging signs of osteoid osteoma(OO)of short tubular bone.Methods A retrospective analysis was conducted on clinical and imaging data from 19 cases pathologically confirmed of OO in short tubular bones,and a com-parision was made with the imaging findings of 67 cases of long tubular bones.Results Among the 19 cases of OO in short tubular bones,73.70%of cases were under 30 years old.There were 16 cases involved the hand's short tubular bones,with 14 cases in the phalanges(1 in the thumb,4 in the index finger,5 in the middle finger,2 in the ring finger,2 in the little finger)and 2 cases in the metacarpals.Three cases involved the short tubular bones of the foot,all in the metatarsals.Five cases reported increased pain at night,three cases experienced relief with nonsteroidal anti-inflammatory drugs,and two cases did not take any medication.All lesions were osteolytic,with mineralisation within the nidus and varying degrees of surrounding bone sclerosis,bone marrow edema,and soft tissue edema.Compared to OO in long tubular bones,OO in short tubular bones more frequently exhibited expansile and discontinu-ous cortical bone features,but the degree and extent of bone sclerosis were less pronounced.Conclusion Short tubular bones are a rare location for OO,often lacking typical clinical symptoms,which can easily lead to misdiagnosis.Therefore,close attention should be paid to its imaging characteristics.
2.Approaches of Foshan Traumatology and Bone-Setting School to the Prevention and Treatment of Splint-Associated Pressure Ulcers in Fracture Patients

Small splint external fixation,due to its advantages of low cost,good breathability,easy adjustability,and sufficient flexibility,has been widely used in clinical practice for fracture patients after manual reduction.Compared to plaster external fixation,small splint fixation is associated with fewer cases of pressure ulcers,but the ulcers still occur clinically.This article analyzes the causes of splint-associated pressure ulcers in fracture patients,and introduced the preventive measures and practice experience of Foshan traumatology and bone-setting practitioners in managing such complications.Their preventive strategies include the gentle and bone-following manipulations of pushing skin and squeezing bone,proper shaping of splints,use of soft cotton padding,appropriate tightness in bandage wrapping,and elevation of the affected limb to promote venous return.For patients suffering pressure ulcers,the external application of traumatic medicated yellow oil gauze combined with medicated yellow water gauze for wet compress has shown efficacy.These approaches provide valuable clinical references for reducing the incidence of splint-related pressure ulcers and for guiding the management of pressure ulcers.
3.Construction and evaluation of automatic measurement model of panoramic ultrasound biomicroscopy images based on deep learning

Objective:To develop and evaluate a deep learning-based automatic measurement model for panoramic ultrasound biomicroscopy (UBM) images.Methods:A diagnostic test study was conducted.Preoperative UBM examination results of 372 patients who underwent implantable collamer lens (ICL) implantation were collected at the Eye Center of Renmin Hospital of Wuhan University between February 2021 and March 2023.A total of 1 368 panoramic UBM images were obtained to establish an image database.The dataset was divided into a training set (760 images), a validation set (86 images) and an internal test set (522 images).An expert panel consisting of three ophthalmologists annotated the images.The UNet+ + network was used to automatically segment anterior segment tissues, such as the cornea, lens and iris.In addition, image processing techniques and geometric localization algorithms were developed to automatically identify the anatomical landmarks of pupil diameter (PD), anterior chamber depth (ACD), angle-to-angle distance (ATA) and sulcus-to-sulcus distance (STS) to complete the measurement of these parameters.Additionally, 480 panoramic UBM images of 135 patients (240 eyes) from Aier Eye Hospital of Wuhan University were used as an external test set to further evaluate the performance of the model in different centers.The consistency between the measurements from the model and expert panel, the Pentacam system was assessed.Finally, 150 images were randomly selected from the external test set for a human-machine comparison to further evaluate the model's performance.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Renmin Hospital of Wuhan University (No.WDRY-2022-K109) and Aier eye Hospital of Wuhan University (No.2023IRBKY120903).Written informed consent was obtained from each subject.Results:In the internal test dataset and external test dataset, with manual labeling as the reference standard, the model achieved a mean Dice coefficient of not less than 0.882.At least 95.65% of the anatomical landmark localization results had Euclidean distance differences within 250 μm.The intraclass correlation coefficients (ICCs) for the measurements of PD, ACD, angle-to-angle ATA, and STS were at least 0.958, with mean relative errors not exceeding 2.407%.With the Pentacam measurements as the reference standard, the ICCs for PD in the internal and external test sets were 0.540 and 0.466, respectively, while the ICCs for ACD were 0.946 and 0.908, respectively.In the human-machine comparison, the ICCs between the model's measurements and those of senior experts were all not lower than 0.969.Conclusions:The deep learning-based model can automatically measure anterior segment parameters from preoperative panoramic UBM images of patients undergoing ICL surgery.The model demonstrates a consistency comparable to that of senior experts, while providing higher efficiency.In terms of ACD measurement, the model shows good agreement between the measurements obtained from the model and Pentacam system.
4.Incidence, risk factors, and outcomes of falls among elderly in Beijing communities

Objective:To investigate the incidence, risk factors, and outcomes of falls among the elderly community population in Beijing.Methods:A cross-sectional survey was conducted using stratified multistage random sampling to select urban and rural residents aged 65 years and older in Beijing. Mortality data was collected after the baseline survey for 5 years. The incidence of falls was weighted based on the composition ratios of age and gender from the 2010 Nation-wide Population Census of Beijing. A logistic regression model was used to analyze the impacts of demographic sociology of common chronic diseases on fall occurrence. The Cox proportional hazards regression model was used to analyze the fall and 5-year survival association.Results:A total of 2 968 participants completed the questionnaire, at cross-sectional survey, with an average age of (73.2±6.0) years, and 1 581 (53.8%) participants were female. Three hundred and sixty-one individuals experienced a fall within the past year. Among those who fell, 64 (17.7%) fell twice, and 95 (26.6%) fell three or more times. Of them, 14.4% (52) had post-fall fractures, with the wrist, knee, and hip being the most common fracture sites, accounting for 25.0%, 17.3%, and 15.4%, respectively. The weighted fall incidence was 12.4% (95% CI: 11.2%-13.5%). Aging, being female, and living in rural areas were more likely to fall. Logistic regression analysis showed that after adjusting for age, gender, and urban-rural status, the risk of falls for those living alone ( OR=1.48, 95% CI: 1.08-2.04) or living with children/grandchildren ( OR=1.51, 95% CI: 1.15-1.97) were significantly higher than those living with their spouse. In addition, the risk of falls was elevated significantly among the elderly with hypertension, diabetes, stroke, dementia, depression status, urinary incontinence, arthritis, insomnia, vision, and hearing loss, dependence on activities of daily living (ADL), general and poor self-rated health (SRH). The Cox proportional hazard regression model revealed that the 5-year risk of death increased by 65% ( HR=1.65, 95% CI: 1.29-2.11) for those who experienced a fall, which increased with fall frequency. This elevated risk persisted after adjusting for chronic conditions, ADL, and SRH. Conclusions:Ageing, female, living in rural regions, having common chronic diseases, dependence on ADL, general and poor SRH, living alone or living with children/grandchildren were associated with the elevated fall risk. The occurrence of fall was seasonal. The most common short-term adverse consequence after a fall was fractures, while the long-term effect was an increased risk of death.
5.Analysis of imaging signs of chondromyxoid fibroma

Objective To analyze the imaging signs of chondromyxoid fibroma(CMF)and to improve the diagnostic accuracy of CMF.Methods The imaging data of 31 cases of CMF diagnosed by pathology were analyzed retrospectively and the characteristic imaging signs were analyzed.Results Among the 31 CMF patients,the male-to-female ratio was about 1.4∶1,and 75%of cases occurred before the age of 30;20 cases occurred in the long bones:15 cases in the tibia,4 cases in the femur,and 1 case in the fibula;15 cases occurred in the metaphyseal;7 cases occurred in the flat bones,and 4 cases occurred in the bones of the hands and feet.All lesions showed osteolysis with different degrees of expansion,lobulation,clear borders,and sclerotic margins on both X-ray plain film and CT.There was internal mineralization in 17 cases and internal bone ridge in 12 cases.All lesions showed T1 WI low-equal signal,T2WI medium-high signal in MRI,and enhanced scans were dominated by circumferential or nodular enhancement of the margins.Conclusion Multiple imaging examination methods can improve the diagnosis of CMF.

7.Synergistic treatment strategies of Chinese and Western medicine among elderly cancer patients

The incidence of tumors among the elderly is notably high, presenting significant challenges in terms of harm, complexity, and treatment.The processes of diagnosis and treatment often lack a precise clinical foundation and robust experimental evidence, resulting in numerous difficulties and dilemmas.A collaborative approach that integrates traditional Chinese and Western medicine, leveraging the strengths of both, can substantially alleviate these challenges and ensure that elderly cancer patients can undergo systematic, comprehensive, and intensive cancer treatments.Traditional Chinese medicine can play a vital role throughout the entire continuum of diagnosis and treatment for elderly cancer patients, whether by leading, assisting, or complementing other treatment modalities.This article employs the concepts of ′righteousness′and ′evil′from traditional Chinese medicine, where ′righteousness′signifies the ′state of a person′and ′evil ′denotes the ′state of a tumor′.We systematically explore collaborative diagnosis and treatment strategies that integrate traditional Chinese and Western medicine for tumor management in the elderly, categorizing the approaches into four conditions: strong righteousness with strong evil, strong righteousness with weak evil, weak righteousness with strong evil, and weak righteousness with weak evil.In cases where both righteousness and evil are strong, the treatment strategy is primarily dominated by Western medicine, with support from traditional Chinese medicine.Conversely, when righteousness is strong and evil is weak, the strategy entails a combination of both Chinese and Western medicine.In situations characterized by weak righteousness and strong evil, the treatment approach is largely guided by traditional Chinese medicine, while also considering equal attention to both modalities.Finally, when both righteousness and evil are weak, the coordinated diagnosis and treatment strategy is primarily based on Chinese medicine, with Western medicine adapting to the circumstances and intervening appropriately throughout the process.By accurately assessing the concepts of ′righteousness′and ′evil′and implementing collaborative diagnostic and treatment strategies that integrate both traditional Chinese and Western medicine, we can significantly enhance the physical condition of elderly cancer patients.This comprehensive approach not only boosts immunity and improves organ function but also increases tolerance to tumor treatments, alleviates complications, reduces adverse reactions, and ensures that elderly cancer patients can undergo systemic cancer treatment to its fullest extent.Ultimately, this strategy aims to improve prognosis, enhance quality of life, and extend the effective survival period.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
10.Results and Analysis of National Health Accounts in 2023 in China

Objectives:Accounting Total Health Expenditure(THE)in 2023,analyzing the changes of health financing in China since 2015,summarizing the main problems and challenges faced by health financing in China at present,and putting forward policy suggestions to improve health financing.Methods:Health expenditure accounting methods by financing source and by health provider were applied,and the change of total health expenditure in China is analyzed through the index analysis of time series.Results:THE in 2023 in China was 9 057.58 billion yuan,an increase of 6.77%over the previous year,accounting for 7.00%of GDP,and per capi-ta health expenditure was 6 425.32 yuan.The proportion of government health expenditure in THE decreased to 26.66%,the propor-tion of social health expenditure increased to 46.01%,and the proportion of household Out-of-Pocket(OOP)rebounded to 27.33%.Conclusions:The total scale of health expenditure in China continues to grow,but the growth rate slows down,and the stability and sustainability of health financing need to be strengthened.It is necessary to speed up the construction of a government investment mechanism based on health priority development,improve the social health insurance system,guide the rational resource allocation,and reduce the burden of medical treatment for residents.
