1.Mechanism of Modified Si Junzitang and Shashen Maidong Tang in Improving Sensitivity of Cisplatin in EGFR-TKI Resistant Lung Adenocarcinoma Cells Based on Aerobic Glycolysis

ObjectiveTo investigate the mechanism of modified Si Junzitang and Shashen Maidong Tang [Yiqi Yangyin Jiedu prescription (YQYYJD)] in enhancing the sensitivity of cisplatin in epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI)-resistant lung adenocarcinoma cells based on aerobic glycolysis. MethodsThe effects of different concentrations of YQYYJD (0, 2, 3, 4, 5, 6, 7, 8 g·L-1) and cisplatin (0, 3, 6, 9, 12, 15, 18, 21, 24, 27 mg·L-1) on the proliferation and activity of PC9/GR cells were detected by the cell counting kit-8 (CCK-8) assay after 24 hours of intervention. The half-maximal inhibitory concentration (IC50) for PC9/GR cells was calculated to determine the concentrations used in subsequent experiments. PC9/GR cells were divided into blank group (complete medium), YQYYJD group (5 g·L-1), cisplatin group (12 mg·L-1), and combined group (YQYYJD 5 g·L-1 + cisplatin 12 mg·L-1). After 24 hours of intervention, cell viability was measured using CCK-8 assay. Cell proliferation was assessed by colony formation assay, and cell migration was evaluated by scratch and Transwell assays. Glucose consumption, lactate production, and adenosine triphosphate (ATP) levels were measured by colorimetric assays. The expression levels of glycolysis-related proteins, including hexokinase 2 (HK2), phosphofructokinase P (PFKP), pyruvate kinase M2 (PKM2), lactate dehydrogenase A (LDHA), glucose transporter 1 (GLUT1), and monocarboxylate transporter 4 (MCT4), were determined by Western blot. ResultsBoth YQYYJD and cisplatin inhibited the viability of PC9/GR cells in a concentration-dependent manner. The IC50 of PC9/GR cells for YQYYJD and cisplatin were 5.15 g·L-1 and 12.91 mg·L-1, respectively. In terms of cell proliferation, compared with the blank group, the cell survival rate and the number of colonies formed in the YQYYJD group, cisplatin group, and combined group were significantly decreased (P<0.01). Compared with the YQYYJD and cisplatin groups, the combined group showed a further significant reduction in cell survival rate and colony formation (P<0.01). In terms of cell migration, compared with the blank group, the cell migration rate and the number of cells passing through the Transwell membrane in the YQYYJD group, cisplatin group, and combined group were significantly decreased (P<0.01). Compared with the YQYYJD and cisplatin groups, the combined group exhibited a further significant reduction in cell migration rate and the number of cells passing through the Transwell membrane (P<0.01). In terms of glycolysis, compared with the blank group, glucose consumption, lactate production, and ATP levels in the YQYYJD group, cisplatin group, and combined group were significantly decreased (P<0.01). Compared with the YQYYJD and cisplatin groups, the combined group showed a further significant reduction in glucose consumption, lactate production, and ATP levels (P<0.05). Compared with the blank group, the protein expression levels of HK2, PFKP, PKM2, and LDHA in the YQYYJD, cisplatin, and combined groups were significantly decreased (P<0.01). The combined group showed a further significant reduction in the expression levels of these proteins compared with the YQYYJD and cisplatin groups (P<0.01). No significant differences were observed in the protein expression levels of GLUT1 and MCT4 among the groups. ConclusionYQYYJD can synergistically inhibit the proliferation and migration of PC9/GR cells and enhance their sensitivity to cisplatin. The mechanism may be related to the downregulation of the expression of glycolysis-related rate-limiting enzymes, including HK2, PFKP, PKM2, and LDHA, thereby inhibiting glycolysis.
2.Interpretation of European clinical practice guidelines for oligometastatic esophagogastric cancer

In recent years, with the clinical application of minimally invasive surgical techniques and comprehensive preoperative treatment, the survival rate of locally advanced esophageal cancer has significantly improved. However, the treatment of metastatic esophagogastric cancer still relies mainly on systemic therapy, and immunotherapy combined with chemotherapy has become a first-line treatment option, prolonging the survival of patients with metastatic esophageal cancer. Oligometastatic esophageal cancer is expected to bring survival benefits through systemic therapy combined with local treatment. The 2024 European clinical practice guidelines for oligometastatic esophagogastric cancer have been officially released, which standardize the definition, diagnosis, and treatment of oligometastatic esophageal cancer for further prospective studies. The authors interpret this guideline, especially by reviewing the clinical evidence of oligometastatic esophageal squamous cell carcinoma, to provide reference for the diagnosis and treatment of oligometastatic esophageal cancer in China.
3.Improvement effects and mechanism of Bazheng powder on chronic urinary tract infection in rats

OBJECTIVE To investigate the improvement effects and its mechanism of Bazheng powder on chronic urinary tract infection (CUTI) induced by Escherichia coli in rats. METHODS The rats were divided into normal control group, model group, levofloxacin group (45 mg/kg) and Bazheng powder group (4.95 g/kg), with 10 rats in each group. Except for the normal control group, other groups were administered an intravesical injection of Escherichia coli suspension (1×10⁸ cfu/mL) via the urethra to establish CUTI model; at the same time, rats in each group were administered the corresponding medicinal solution or water by gavage once a day for 4 consecutive weeks. After the last medication, blood routine tests (white blood cell count and lymphocyte percentage), the levels of serum inflammatory factors [interleukin-1β (IL-1β), IL-6, IL-8, tumor necrosis factor-α (TNF-α)], and immune indicators [CD4, CD8, secretory immunoglobulin A (SIgA)], renal function indicators [cystatin C (Cys-C), α1- microglobulin (α1-MG), urea and creatinine] were all determined; the pathological changes in renal and bladder tissues in rats were observed. The protein expressions of Toll-like receptor 4 (TLR4), nuclear factor-κB (NF-κB), and nucleotide-binding domain leucine-rich repeat and pyrin domain-containing receptor 3 (NLRP3) in rat bladder tissues were detected. RESULTS Compared with the normal control group, the levels of IL-1β, IL-6, IL-8, TNF-α, CD8, Cys-C, α1-MG, urea and creatinine in serum, as well as the protein expressions of TLR4, NF-κB and NLRP3 in bladder tissues, were significantly elevated in the model group (P<0.05). Conversely, the levels of CD4 and SIgA were significantly decreased (P<0.05). Pathological changes, such as extensive infiltration of inflammatory cells, were observed in both renal and bladder tissues. Compared with the model group, the above quantitative indicators in the Bazheng powder group were significantly improved (P<0.05), with no obvious inflammatory lesions observed in either renal or bladder tissues. CONCLUSIONS Bazheng powder can alleviate inflammatory reaction and improve the immune function of CUTI rats, and its mechanism may be related to the inhibition of TLR4/NF-κB/ NLRP3 signaling pathway.
4.The relationship between iodine nutrition during pregnancy and gestational metabolic diseases: a review

Iodine is an essential micronutrient for the synthesis of thyroid hormones and for growth and development in the human body. Recent studies have revealed that iodine can exert biological functions beyond the thyroid. By modulating oxidative stress and gut microbiota abundance, it influences systemic metabolic homeostasis. Imbalance in iodine nutrition during pregnancy is closely associated with the onset and progression of gestational metabolic diseases. Based on studies pertaining to the relationship between iodine nutrition during pregnancy and gestational metabolic diseases from 1980 to 2025, this review summarized the relationship of iodine nutrition during pregnancy with gestational metabolic diseases, including gestational diabetes mellitus, gestational hypertension and preeclampsia, gestational overweight or obesity, and gestational dyslipidemia, and described the underlying biological mechanisms, so as to provide the evidence for formulating prevention and control strategies for gestational metabolic diseases.

Background::Carotid intima-media thickness (IMT) and diameter, stiffness, and wave reflections, are independent and important clinical biomarkers and risk predictors for cardiovascular diseases. The purpose of the present study was to establish nationwide reference values of carotid properties for healthy Chinese adults and to explore potential clinical determinants.Methods::A total of 3053 healthy Han Chinese adults (1922 women) aged 18-79 years were enrolled at 28 collaborating tertiary centers throughout China between April 2021 and July 2022. The real-time tracking of common carotid artery walls was achieved by the radio frequency (RF) ultrasound system. The IMT, diameter, compliance coefficient, β stiffness, local pulse wave velocity (PWV), local systolic blood pressure, augmented pressure (AP), and augmentation index (AIx) were then automatically measured and reported. Data were stratified by age groups and sex. The relationships between age and carotid property parameters were analyzed by Jonckheere-Terpstra test and simple linear regressions. The major clinical determinants of carotid properties were identified by Pearson’s correlation, multiple linear regression, and analyses of covariance.Results::All the parameters of carotid properties demonstrated significantly age-related trajectories. Women showed thinner IMT, smaller carotid diameter, larger AP, and AIx than men. The β stiffness and PWV were significantly higher in men than women before forties, but the differences reversed after that. The increase rate of carotid IMT (5.5 μm/year in women and 5.8 μm/year in men) and diameter (0.03 mm/year in both men and women) were similar between men and women. For the stiffness and wave reflections, women showed significantly larger age-related variations than men as demonstrated by steeper regression slopes (all P for age by sex interaction <0.05). The blood pressures, body mass index (BMI), and triglyceride levels were identified as major clinical determinants of carotid properties with adjustment of age and sex. Conclusions::The age- and sex-specific reference values of carotid properties measured by RF ultrasound for healthy Chinese adults were established. The blood pressures, BMI, and triglyceride levels should be considered for clinical application of corresponding reference values.

"Toxicity Testing in the 21st Century—A Vision and Strategy"proposed by the National Research Council of US has brought innovative directives and objectives for toxicity evaluation and risk assessment,pushing forward the next generation of toxicity testing and risk assessment.In this initiative,the concept of adverse outcome pathways(AOPs)has emerged as a prominent methodology,capturing the attention of toxicologists and researchers due to its promising applications in recent years.The quantitative AOP(qAOP)is an extension of the adverse outcome pathway,which is built upon the foundational qualitative adverse outcome pathway model and leverages mathematical frame-works to depict dose-response and/or response-response relationships.This article reviews the princi-ples and advancement surrounding qAOP,introduceds two prevalent methodologies for constructing qAOP,Bayesian network models and regression models,and demonstrates diverse applications of qAOP.Actual cases are used to underscore the transformative role of qAOP in contemporary toxicology and risk assessment practices.
7.Research on the sources,structures and identification technology of glucosamine drugs

Objective To sort out and summarize the researches of source,structures and identification technologies of glucosamine drugs,and provide a reference for the development and research of this kind of drugs.Methods The sources of glucosamine drugs was identified by stable isotope ratio test,and the crystal structures of glucosamine drugs was identified by X-ray powder diffraction.Results When the carbon isotope ratio was between-11‰ and-13‰,the source of glucosamine was from microbial fermentation.When the carbon isotope ratio was between-17‰ and-24‰,the source of glucosamine was from microbial animals.The 2θ angles of the strongest diffraction peak of hydrochloric glucosamine were 16.525°,12.360° and 17.330°,the 2θ angles of the strongest diffraction peak of sodium sulfate were 32.124° and 19.035,the 2θ angle of the strongest diffraction peak of the glucosamine sulfate potassium chloride/sodium was 27.036°,and the 2θ angle of the strongest diffraction peak of the physical mixture of glucosamine hydrochloride and chloride/sodium sulfate(2∶1)was 12.391°.Through X-ray powder diffraction technology,the glucosamine sulfate potassium chloride/sodium eutectic complex salt and the physical mixture of glucosamine hydrochloride and chloride/sodium sulfate.can be distinguished.Conclusion The research can effectively identify the sources and structures of glucosamine drugs,which is simple,accurate and reliable,and provides technical support for the supervision and management of glucosamine drugs.
8.Effectiveness Analysis of Laparoscopic and Open Surgeries in the Treatment of Intrahepatic Cholangiocarcinoma:a Propensity Score Matching Study

Objective To preliminarily analyze the efficacy of laparoscopic surgery versus open surgery in patients with intrahepatic cholangiocarcinoma(ICC).Methods A retrospective analysis was conducted on 84 patients with ICC who underwent surgical resection from September 2010 to March 2023,including 45 cases in the laparoscopic surgery group and 39 in the open surgery group.By using propensity score matching,34 matched pairs were successfully identified.The study compared intraoperative blood loss,proportion of achieving≥6 lymph nodes dissection,R0 resection rate,complication,postoperative hospital stay,survival,and recurrence between the two groups.Results In the laparoscopic group,intraoperative blood loss was significantly less than that in the open surgery group(median:250 ml vs.750 ml,Z=-3.406,P=0.001).The proportion of achieving≥6 lymph node dissection in the former was lower than that in the latter,but the difference was not statistically significant(5.9%vs.17.6%,χ2=1.275,P=0.259).There was no statistical difference in R0 resection rate between the laparoscopic group and the open surgery group[88.2%(30/34)vs.82.4%(28/34),χ2=0.469,P=0.493].The complication rate in the laparoscopic group was 17.6%(6/34),significantly lower than 47.1%(16/34)in the open surgery group(χ2=6.719,P=0.010);however,the rate of severe complication was the same in both groups at 2.9%(χ2=0.000,P=1.000).Laparoscopic surgery significantly shortened the postoperative hospital stay(median:7 d vs.10 d,Z=-3.021,P=0.003).The median overall survival in the laparoscopic group was 60.6 months,significantly longer than 15.9 months in the open surgery group(log-rank χ2=5.788,P=0.016).The median recurrence-free survival was 26.4 months in the laparoscopic group,significantly longer than 7.6 months in the open surgery group(log-rank χ2=4.532,P=0.033).Conclusions Compared to open surgery,laparoscopic surgery for ICC offers advantages such as less blood loss,lower complication rate,shorter postoperative hospital stay and longer survival.It achieves comparable R0 resection rate and adequate lymph node dissection.
9.Expression of PD-L1/B7-H3 in intrahepatic cholangiocarcinoma and its correlations with clinicopathological features and prognosis

Objective:To explore the expression of PD-L1 and B7-H3 in intrahepatic cholangiocarcinoma (ICC) and their relationship with the prognosis after surgery.Methods:Clinicopathological data of 63 ICC patients undergoing surgery at the Department of General Surgery, Peking University Third Hospital, from Sep 2016 to Mar 2023 were retrospectively analyzed.Results:PD-L1 and B7-H3 are highly expressed in 41.3% and 60.3% of ICC patients, respectively, and their expression levels were positively correlated with each other( P=0.005). High PD-L1 expression is significantly associated with poor tumor differentiation ( P=0.014), while high B7-H3 expression is significantly related to lymph node metastasis ( P=0.035). In patients with high co-expression of PD-L1 and B7-H3, the overall survival ( P=0.031) and recurrence-free survival ( P=0.031) were significantly decreased. High co-expression of PD-L1 and B7-H3 is an independent risk factor for recurrence in ICC ( HR=3.998, 95% CI: 1.211-13.198; P=0.023). Conclusions:ICC patients with high expression of both PD-L1 and B7-H3 have a poor prognosis. Using both as the combined biomarker significantly improves the prediction of long-term survival after surgery, compared to using either one alone.
10.Effect of permissible hypercapnia combined with remote ischemic preconditioning on brain oxygen saturation and postoperative cognition in patients undergoing thoracoscopic lung cancer surgery

Objective To explore the effects of remote ischemic preconditioning combined with permissive hypercapnia on brain oxygen saturation and postoperative cognition in patients which undergoing thoracoscopic lung cancer surgery.Methods A collection of 64 patients elective requiring thoracoscopic lung cancer surgery who were divided into control group and combined group according to the randomized grouping method,with 32 cases in each group.The PaCO2 in the control group of patient was maintained at normal,and patients in the combination group were given permissive hypercapnia ventilation strategies and performed remote ischemic preconditioning,PaCO2 is maintained at 45-50mmHg(1mmHg=0.133kPa).Record the cerebral oxygen saturation(rSO2)at the five time points before operation(T0),10min after one lung ventilation(T1),30min after one lung ventilation(T2),10 min after lung recruitment(T3)and the end of surgery(T4),measured the internal jugular venous blood oxygen saturation(SjvO2)and calculated cerebral arteriovenous blood oxygen content difference(CaO2-CjvO2),brain oxygen uptake rate(CERO2).Monitored the average arterial pressure(MAP)and heart rate(HR)of the hemodynamic indicators at the above five time points.The scores of cognitive function were recorded 1 day before operation and 1 day and 3 days after operation;detected the levels of serum neuron-specific enolase(NSE),amyloid β(Aβ)and S100β protein(S100β)in 1 day before surgery,24hours after surgery and 48hours after surgery;Comparison of postoperative related indicators and adverse reactions between the patients of two groups.Results The rSO2 and SjvO2 of combined group were higher than control group in the T1-T4,but CaO2-CjvO2 and CERO2 were lower than those of control group.There was no significant difference in HR and MAP between two groups from T0-T4.The mini-mental state examination(MMSE)score of the combined group was significantly higher than that of the control group on the 1 day after operation.The level of serum NSE,Aβ and S100β in the combined group was lower than those of control group at 24hours and 48hours after operation.There was no significant difference in incidence of postoperative adverse reactions and postoperative related indexes between the two groups.Conclusion Permissive hypercapnia combined with remote ischemic preconditioning can increase cerebral oxygen saturation in patients undergoing thoracoscopic lung cancer surgery,improve cerebral oxygen metabolism and reduce the levels of serum neuron-specific enolase,β-amyloid protein and S100β protein,decrease the postoperative cognitive dysfunction.
