1.Teaching and practice exploration on the integration of multi-disciplines in teaching of operative dentistry

Objective:In order to effectively educate students on tooth defect diseases and restoration techniques, the teaching and research team at the School of Stomatology of Qingdao University developed an integrated course operative dentistry. This course is specifically designed to align with the undergraduate curriculum reform requirements of "Three Early, Two Multiple and One Integration".Methods:The teaching practice of operative dentistry was conducted with the stomatology undergraduates of grade 2020 and 2021. The course group redesigned the curriculum by reorganizing teaching content, rebuilding teaching resources, reconstructing teaching mode, and reshaping the evaluation system. Evaluation of the curriculum design included test scores, satisfaction questionnaires, and teacher self-evaluation. Statistical analysis was performed using SPSS 27.0 software, including independent sample t-tests for numerical variables and Fisher's exact test or Wilcoxon rank sum test for categorical variables. Results:Through the integrated course learning, 92.68% (38/41) of the students of grade 2020 and 97.44% (38/39) of the students of grade 2021 can basically or completely master various dental defect repair schemes and understand the importance of preserving natural tooth. 82.93% (34/41) of the students of grade 2020 and 100.00% (39/39) of the students of grade 2021 could basically or completely master the key points of various dental defect restorations.Conclusions:Through the practice of teaching, it has been confirmed that the integrated course of operative dentistry is beneficial to students' mastering the knowledge and skills of dental defect diseases and their restoration in a more comprehensive way, and is conducive to the cultivation of clinical thinking, hands-on ability and the patient-centered, comprehensive problem-solving thinking, which is in line with the guidance on the innovation and development of medical education.
2.Analysis on current status and outcomes of comprehensive control of cardiovascular disease risk factors based on community population cohort

Objectives:To describe the use of antihypertensive, antidiabetic and lipid-lowering drugs, and evaluate the effects on blood pressure, blood glucose and blood lipids controls required by Chinese Guideline on the Primary Prevention of Cardiovascular Diseases (the guideline) in a community-based cohort of individuals at high risk for cardiovascular disease. To analyze the association of the uses of antihypertensive, antidiabetic and lipid-lowering drugs, and the comprehensive control of blood pressure, blood glucose and blood lipids with cardiovascular disease. Methods:From the CHinese Electronic health Records Research in Yinzhou (CHERRY), those who were at high risk for cardiovascular disease and aged 40-75 years as of January 1, 2013 in in Yinzhou District of Ningbo, Zhejiang Province were selected as study subjects. The information about their antihypertensive, antidiabetic, and lipid-lowering drug uses between 2013 and 2015 was collected, and blood pressure, blood glucose, and blood lipid measurements were conducted during the follow-up. The study constructed two kinds of comprehensive scores: the comprehensive medication score based on the guideline requirement for the treatment of hypertension, diabetes and hyperlipidemia, dividing the study participants into the compliancy group and non-compliancy group; and the comprehensive control score based on the guideline requirement for blood pressure, blood glucose, and blood lipids control, dividing the study participants into better control group, moderate control group, and poor control group. Cox proportional hazards regression model was used to analyze the association of the comprehensive medication score and comprehensive control score with cardiovascular disease. The incidence data of cardiovascular disease were collected from January 1, 2015 (baseline time) to August 31, 2020 (follow up end time).Results:A total of 79 734 participants at high risk for cardiovascular disease were included in the study, in whom 68.4%, 27.4%, and 4.2% had 1, 2, or 3 cardiometabolic conditions (hypertension, diabetes, or hyperlipidemia), respectively. In the participants with hypertension, diabetes, and hyperlipidemia from 2013 to 2015, the proportions of those who had two years of medication compliancy records were 66.0%, 67.4%, and 13.9%, respectively. In the hypertension patients, 59.2% had better blood pressure control, in the diabetes patients, 28.7% had better blood glucose control, and in the patients with hyperlipidemia, 27.4% had better blood lipid control. After a median follow-up of 5.66 years, 4 088 cardiovascular disease cases or deaths occurred. After multivariate adjustment, compared with the non-compliancy group, the compliancy group had lower risk for cardiovascular disease ( HR=0.91, 95% CI: 0.85-0.96). Compared with the better control group, the poor control group had an increased risk for cardiovascular disease ( HR=1.67, 95% CI: 1.53-1.81). In the moderate control group, the risk increased significantly in the diabetes patients ( HR=1.29, 95% CI: 1.07-1.56), while no additional risk for cardiovascular disease was observed in non-diabetes patients ( HR=1.06, 95% CI: 0.97-1.16). Conclusions:Compliancy to the medication required by the guideline is associated with lower risk for cardiovascular disease. However, it is still necessary to improve the medication compliancy in people at high risk in primary prevention, especially in the patients with hyperlipidemia, due to their low taking rate of lipid-lowering drugs. Additionally, as the requirement of the guideline becomes more stringent, the management of disease has met more challenges. Notably, diabetes patients who have not met the guideline requirement are at high risk for cardiovascular disease, to whom the disease management should be strengthened.
3.Prognostic analysis of laparoscopic simultaneous radical cystectomy and nephroureterectomy.

OBJECTIVE:
To investigate the safety and prognostic factors influencing the treatment of upper urinary tract urothelial carcinoma (UTUC) combined with bladder cancer (BCa) by laparoscopic simultaneous radical cystectomy and nephroureterectomy (RCNU).
METHODS:
The clinical data of patients admitted to Peking University Third Hospital for laparoscopic RCNU surgery from January 2009 to September 2023 were analyzed retrospectively. Based on the same gender, age (±5 years), history of uroepithelial tumors, underlying diseases, T-stage, N-stage, M-stage, American Society of Anesthesiologists (ASA) score, Charlson comorbidity index, and body mass index (BMI) (±5), 34 patients with RCNU were matched 1 ∶1 with patients with bladder cancer who underwent laparoscopic radical cystectomy (RC) alone. Kaplan-Meier survival analysis was used to calculate patient survival, and Cox proportional regression risk model was used to analyze clinical factors affecting prognosis.
RESULTS:
Of the 68 patients enrolled, the follow-up rate was 100% with a median follow-up time of 27.0 (11.7, 60.2) months. Comparison of intraoperative conditions (including operation time, estimated intraoperative bleeding, intra-operative blood transfusion, etc.) between the two groups of patients showed no significant difference (P>0.05). Comparison of preoperative creatinine and postoperative creatinine between the two groups of patients showed significant differences (P < 0.05). The perioperative Clavien grade Ⅲ-Ⅳ complication rates were 2.9% (1/34) in the RC group and 5.9% (2/34) in the RCNU group. There was no significant difference in terms of perioperative complications between the two groups. Overall survival was significantly lower in the patients receiving RCNU compared with the matched group receiving RC alone (P < 0.05). Cox regression analysis suggested that two factors, high N stage and high postoperative creatinine, were independent risk factors affecting the prognosis of patients in the 2 groups (P < 0.05).
CONCLUSION
The overall survival prognosis of patients undergoing RCNU surgery was worse compared with laparoscopic RC surgery alone during the same period. There was no clinically significant difference between the two groups in terms of operation time, intraoperative bleeding, and perioperative complications, and there were clinically significant differences in preoperative renal function and post-operative renal function.

4.Quantitative Analysis of the Impact of Various iCBCTs on the Image Quality of Lung Adaptive Radiotherapy.

OBJECTIVE:
To investigate the impact of different iterative cone beam CT (iCBCT) scanning beam currents from a ring-mounted linac on synthetic CT image quality for lung adaptive radiotherapy under lung scanning protocol.
METHODS:
The CIRS lung motion phantom was configured to simulate conventional respiratory motion pattern, followed by 4D-CT simulation. After transferring the radiotherapy plan to the ring-mounted Halcyon 3.0 linac, three groups of typical iCBCT scans with different beam currents [ I low (160 mA), I middle (282 mA), and I high (491 mA)] were performed and corresponding image reconstructions were completed. Synthetic CT (sCT) images were subsequently obtained based on the deformable registration algorithm.
RESULTS:
Compared to the corresponding CBCT images, the sCT images exhibited a significant reduction in artifacts. The fine structure of the planning CT (pCT) image was preserved for sCT images corresponding to different scanning beam currents, with Dice similarity coefficients exceeding 0.90 for all cases.
CONCLUSION
The image quality of sCT corresponding to different iCBCTs is comparable to that of pCT, and changes in iCBCT beam parameters have a negligible impact on sCT image quality. Taking into account both image quality and imaging dose factors associated with the beam currents, iCBCT with a lower beam current on the ring-mounted Halcyon linac offers greater clinical value in lung adaptive radiotherapy.

7.Correlation analysis of serum IL-6, OPN levels and bone metabolism markers in pregnant women with gestational diabetes mellitus

Objective:To analyze the correlation between serum interleukin-6 (IL-6) and osteopontin (OPN) levels and bone metabolism markers in pregnant women with gestational diabetes mellitus.Methods:A total of 89 pregnant women with GDM who were admitted to Shanxi Children’s Hospital from Aug. 2020 to May. 2022 were selected as the observation group, and 85 normal pregnant women who underwent regular health examination were selected as the control group. Clinical data of the two groups were compared and GDM related factors were analyzed. Serum IL-6, OPN, bone metabolic markers β collagen special sequence (β-CTX), type I procollagen N-terminal propeptide (PINP), n-terminal mid-section osteocalcin (N-MID), fasting blood glucose (FBG) and fasting insulin (FINS) levels of the two groups were detected. Insulin resistance index (HOMA-IR) was calculated, and the relationship between serum IL-6, OPN and β-CTX, PINP and N-MID was analyzed.Results:The age, gestational times and pre-pregnancy BMI of the observation group were higher than those of the control group, and the family history of diabetes was more than that of the control group, with statistical significance between the two groups ( t=14.31, 31.46, 9.47, 10.33, P<0.05). Logistic regression analysis showed that the older the age and the more pregnancies, the higher the pre-pregnancy BMI. Independent risk factors for developing GDM were associated with a large family history of diabetes mellitus [ ( OR (95% CI) =1.569 (1.370-1798), 8.533 (5.067-14.367), 1.460 (1.301-1.639), 3.677 (1.611-8.396). P<0.05) ]; Serum levels of IL-6, OPN, FBG, FINS and HOMA-IR in observation group were higher than those in control group ( t=16.64, 24.76, 10.97, 11.01, 55.70, all P<0.05). The β-CTX of observation group was higher than that of control group, and the PINP and N-MID of observation group were lower than that of control group ( t=3.64, 12.19, 12.52, P<0.05). Spearman correlation analysis showed that serum IL-6 and OPN were positively correlated with β-CTX ( r=0.781, 0.746, P<0.05). Serum IL-6 and OPN were negatively correlated with PINP and N-MID ( r=-0.264, -0.305; -0.208, -305, P<0.05) . Conclusions:In pregnant women with GDM, serum IL-6 and OPN are positively correlated with β-CTX, and serum IL-6 and OPN are negatively correlated with PINP and N-MID, respectively. These indicators can be used as indicators of disease monitoring, and play an important role in protecting the health of pregnant women with GDM.

9.Transcatheter Aortic Valve Replacement in Patients With Pure Native Aortic Regurgitation: Results From a Multicenter Registry Study

Objective::Patients with untreated severe aortic regurgitation (AR) have a high risk of mortality. Transfemoral transcatheter aortic valve replacement (TF-TAVR) is a treatment option for AR; however, the safety and efficacy of this technique have not been sufficiently established. This study aimed to evaluate the clinical and anatomical variables correlating with device success of TF-TAVR using a self-expanding valve system for pure AR.Methods::Patients with pure native severe AR who underwent TF-TAVR using a self-expanding valve system were registered at 5 Chinese centers. The primary endpoint was device success at 1 month after TAVR. The secondary endpoint was the composite of major adverse cardiovascular events (MACE) at 6 months, including all-cause death, ischemic stroke, emergency conversion to cardiac surgery, and permanent pacemaker implantation. Echocardiography was used to analyze the left ventricular function before the TAVR procedure and during follow-up. Multivariable logistic regression and Cox regression analyses were performed to find relevant independent risk factors.Results::Between September 2019 and February 2022, 79 patients with AR were enrolled in the study. At 1 month, device success was achieved in 60 (75.9%) patients. By 6 months, 29 (36.7%) patients had MACE. Echocardiography revealed improved left ventricular function after TAVR. Multivariate regression analysis demonstrated that the Society of Thoracic Surgeons risk score (odds ratio 0.760, 95% confidence interval (CI): 0.584-0.989; P = 0.041) and annulus perimeter (odds ratio 0.888, 95% CI: 0.796-0.992; P = 0.035) were 2 predictors of device success. Moreover, annulus perimeter (<80.2 mm), but not Society of Thoracic Surgeons risk score, was associated with a significant reduction in MACE at 6 months (hazard ratio 2.223, 95% CI: 1.060-4.659; P = 0.028). Conclusions::TF-TAVR using a self-expanding valve system appears to be a safe and feasible treatment for patients with pure native severe AR, particularly those with a less enlarged annulus.

At 1 month, device success was achieved in 60 (75.9%) patients. By 6 months, 29 (36.7%) patients had MACE. Echocardiography revealed improved left ventricular function after TAVR. Multivariate regression analysis demonstrated that the Society of Thoracic Surgeons risk score (odds ratio 0.760, 95% confidence interval (CI): 0.584-0.989; P = 0.041) and annulus perimeter (odds ratio 0.888, 95% CI: 0.796-0.992; P = 0.035) were 2 predictors of device success. Moreover, annulus perimeter (<80.2 mm), but not Society of Thoracic Surgeons risk score, was associated with a significant reduction in MACE at 6 months (hazard ratio 2.223, 95% CI: 1.060-4.659; P = 0.028). Conclusions::TF-TAVR using a self-expanding valve system appears to be a safe and feasible treatment for patients with pure native severe AR, particularly those with a less enlarged annulus.
