1.The distribution pattern of traditional Chinese medicine syndromes and influencing factors for primary liver cancer: An analysis of 415 cases

ObjectiveTo investigate the influencing factors for traditional Chinese medicine (TCM) syndromes of primary liver cancer, and to provide a theoretical basis for the TCM syndrome differentiation and standardized treatment of liver cancer. MethodsTCM syndrome differentiation was performed for 415 patients who were admitted to Shanxi Institute of Traditional Chinese Medicine and were diagnosed with primary liver cancer based on pathological or clinical examinations from January 2019 to December 2023. The chi-square test was used for comparison of categorical data between groups, and the unordered polytomous logistic regression model was used to investigate the influencing factors for TCM syndromes of liver cancer. ResultsThe common initial symptoms of the 415 patients with primary liver cancer included pain in the liver area (31.81%), abdominal distension (25.30%), abdominal pain (15.18%), and weakness (13.98%), and the main clinical symptoms included poor appetite (70.84%), fatigue (69.16%), pain in the liver area (67.47%), poor sleep (59.04%), abdominal distension (53.01%), and constipation (52.53%). There were significant differences in TCM syndromes between patients with different sexes, courses of the disease, clinical stages, Child-Pugh classes, presence or absence of intrahepatic and extrahepatic metastasis, and presence or absence of transcatheter arterial chemoembolization (TACE) and radiofrequency ablation (all P<0.05). The logistic regression analysis showed that male sex was a risk factor for damp-heat accumulation (odds ratio [OR]=2.036, P=0.048) and the syndrome of spleen-kidney Yang deficiency (OR=5.240, P<0.001); a course of disease of<1 year was a risk factor for damp-heat accumulation (OR=2.837, P=0.004) and syndrome of Qi stagnation and blood stasis (OR=2.317, P=0.021), but it was a protective factor against syndrome of spleen-kidney Yang deficiency (OR=0.385, P=0.005); Child-Pugh class A/B was a protective factor against liver-kidney Yin deficiency (OR=0.079, P<0.001); intrahepatic metastasis was a risk factor for liver-kidney Yin deficiency (OR=5.117, P=0.003) and syndrome of spleen-kidney Yang deficiency (OR=3.303, P=0.010); TACE was a protective factor against liver-kidney Yin deficiency (OR=0.171, P<0.001) and syndrome of spleen-kidney Yang deficiency (OR=0.138, P<0.001); radiofrequency ablation was a risk factor for damp-heat accumulation (OR=4.408, P<0.001) and liver-kidney Yin deficiency (OR=32.036, P<0.001). ConclusionSex, course of disease, Child-Pugh class, intrahepatic metastasis, TACE, and radiofrequency ablation are the main influencing factors for TCM syndromes of liver cancer.

Mycophenolic acid (MPA), the active moiety of both mycophenolate mofetil (MMF) and enteric-coated mycophenolate sodium (EC-MPS), serves as a primary immunosuppressant for maintaining solid organ transplants. Therapeutic drug monitoring (TDM) enhances treatment outcomes through tailored approaches. This study aimed to develop an evidence-based guideline for MPA TDM, facilitating its rational application in clinical settings. The guideline plan was drawn from the Institute of Medicine and World Health Organization (WHO) guidelines. Using the Delphi method, clinical questions and outcome indicators were generated. Systematic reviews, Grading of Recommendations Assessment, Development, and Evaluation (GRADE) evidence quality evaluations, expert opinions, and patient values guided evidence-based suggestions for the guideline. External reviews further refined the recommendations. The guideline for the TDM of MPA (IPGRP-2020CN099) consists of four sections and 16 recommendations encompassing target populations, monitoring strategies, dosage regimens, and influencing factors. High-risk populations, timing of TDM, area under the curve (AUC) versus trough concentration (C0), target concentration ranges, monitoring frequency, and analytical methods are addressed. Formulation-specific recommendations, initial dosage regimens, populations with unique considerations, pharmacokinetic-informed dosing, body weight factors, pharmacogenetics, and drug-drug interactions are covered. The evidence-based guideline offers a comprehensive recommendation for solid organ transplant recipients undergoing MPA therapy, promoting standardization of MPA TDM, and enhancing treatment efficacy and safety.

4.Inhibitory effect of Fuzheng Huaji Decoction against non-small cell lung cancer cells in vitro and the possible molecular mechanism.

OBJECTIVES:
To investigate the inhibitory effect of Fuzheng Huaji Decoction against non-small cell lung cancer (NSCLC) cells in vitro and explore the underlying mechanism.
METHODS:
The active ingredients and targets of Fuzheng Huaji Decoction were identified using TCMSP and SwissTargetPrediction databases. NSCLC-related targets from GeneCards and PharmGKB were intersected with the targets of the Decoction, and a protein-protein interaction (PPI) network was constructed to identify the core targets, which were analyzed with GO and KEGG pathway enrichment analysis. Cultured A549 cells were treated with different concentrations of Fuzheng Huaji Decoction-medicated serum, and the changes in cell proliferation, apoptosis, and protein expressions were examined using CCK-8 assay, annexin V-FITC/PI staining and Western blotting.
RESULTS:
Fuzheng Huaji Decoction contained 140 active ingredients, and 707 drug-disease intersecting targets were identified. Among these targets, TP53, AKT1, HIF1A, GAPDH, ALB, EGFR, CTNNB1, and TNF were identified as the core targets which were involved in the biological processes related to kinases and receptors and the PI3K-AKT, Ras, calcium, and MAPK pathways. Molecular docking studies indicated strong binding affinity of the active ingredients with TP53, AKT1, and HIF1A. In cultured A549 cells, treatment with 2.5%, 5%, and 10% Fuzheng Huaji Decoction-medicated serum significantly inhibited cell proliferation, promoted cell apoptosis, and downregulated the expression levels of HIF1A, p-AKT (Thr308), and TP53 proteins.
CONCLUSIONS
Fuzheng Huaji Decoction inhibits proliferation of NSCLC cells possibly by downregulating the expressions of HIF1A, p-AKT (Thr308), and TP53.

5.Research progress of dermatomyositis-related autoantibodies in interstitial lung disease

Myositis antibodies are divided into myositis-specific autoantibodies(MSA)and myositis-associated autoantibodies(MAA).There are heterogeneity in the mechanism,clinical features and prognosis of interstitial lung disease(ILD)in the different myositis antibodies.In MSA,anti-melanoma differentiation-related gene 5(MDA5)antibody and anti-aminoacyl synthetase(ARS)antibody are highly correlated with the occurrence of ILD.Patients with MDA5+DM-ILD usually have a rapidly progressive and poor prognosis.The progress of ILD in ARS+DM patients was slow,and the response to treatment is good,but it is easy to relapse.In MAA,anti-Ro52 antibodies often coexist with MSA antibodies,and clinical manifestation is closely related to coexisting antibodies.This review has summarized roles of myositis antibodies in ILD.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.
7.Effects of continuing nursing model based on "Internet+" in premature infants

Objective:To explore the effects of the continuing nursing model based on "Internet +" in premature infants and their families.Methods:Premature infants and their families admitted to the Department of Neonatology, Wenzhou Central Hospital, from July 2021 to October 2022 were selected by convenience sampling. They were randomly divided into control group and observation group, each consisting of 108 pairs. Infants in the control group received routine nursing intervention, while those in the observation group received "Internet +" based continuing nursing based on the care provided to the control group. Physical growth indicators (weight, length, head circumference), Neonatal Behavioral Neurological Assessment (NBNA) scores, family caregiving knowledge and ability scores were measured before intervention (at birth) and after intervention (at 6 months of age) in both groups. The occurrence of delayed physical development and unplanned rehospitalization of premature infants at 6 months of age was also recorded.Results:After intervention, the physical growth indicators, NBNA scores, and family caregiving knowledge and ability scores of both groups improved, with the observation group higher than the control group, and the differences were statistically significant (all P<0.05). At 6 months of age, the observation group had lower rates of delayed physical development and unplanned rehospitalization compared to the control group, with statistically significant differences (all P<0.05) . Conclusions:The continuing nursing model based on "Internet +" can effectively improve the caregiving knowledge and ability of premature infants' families, promote the growth and development of premature infants, and reduce the risk of delayed physical development and rehospitalization.
8.Content determination of sodium pyrosulfite and sodium sulphate in phentolamine mesylate injection by ion chromatography

Objective To establish a method for the determination of contents of sodium metabisulfite and sodium sulfate in phentolamine mesylate injection.Methods The ion chromatography method with a DionexIonPac AS11-HC(250 mm×4.0 mm,5 pm)column was used,with potassium hydroxide solution as eluent,gradient elution,flow rate of 1.0 mL/min.The column temperature was 30 ℃ and the injection volume was 25 μL.Results Sodium metabisulfite showed good linearity(r=0.999 3)in the range of 10.596-211.920 μg/mL,and its average recovery rate was 100.00%,with an RSD of 1.4%(n=9).Sodium sulfate showed good linearity(r=0.999 8)in the range of 1.027-20.540 μg/mL,and its average recovery rate was 99.96%,with an RSD of 1.8%(n=9).Conclusion The established method is simple,accurate,sensitive and suitable for the determination of sodium metabisulfite and sodium sulfate contents in phentolamine methylate injection.
9.IgM Subclass Antibody 3A6 Inhibits Atherosclerotic Plaque Formation in ApoE-/-Mice

Objective To investigate the effect of the anti-oxidized low density lipoprotein(ox-LDL)IgM subclass antibody 3A6 on the atherosclerotic plaque formation in ApoE-/-mice.Methods The ApoE-/-mice were randomly divided into three groups:control group(intraperitoneal injection of normal saline),5G8group(intraperitoneal injection of nature anti-LDL IgM subclass antibody)and 3A6group(intraperitoneal injection of natural anti-ox-LDL IgM subclass antibody),with 6mice in each group.The body weight of mice and the level of serum lipid in each group were measured after 12 weeks of high-fat and high-cholesterol diet.hematoxylin-eosin staining was used to analyze the pathological changes and plaque area of the aorta in mice.Results There was no significant difference in body weight,serum lipid level and ox-LDL level among the three groups(P=0.087).Compared with the control group,the area of atherosclerotic plaque was significantly decreased in the 3A6group(P=0.023).Conclusion The natural IgM subclass antibody 3A6 can effectively inhibit the formation of atherosclerotic plaque in ApoE-/-mice.
10.Epidemiological and clinical characteristics of 140 children with acute brucellosis

Objective:To analyze the epidemiological and clinical characteristics of children with acute brucellosis, providing reference for early diagnosis and standardized treatment of brucellosis in children.Methods:The data of 140 children with acute brucellosis at Xi'an Children's Hospital from April 2012 to October 2021 were collected. Their general information, epidemiological characteristics, clinical manifestations, laboratory and imaging examinations, treatment and prognosis were analyzed retrospectively.Results:Among 140 children with acute brucellosis, there were 78 males and 62 females, with a median age of onset and interquartile range of 3.4 (1.8, 5.8) years; 88.6% (124/140) of these children came from rural areas. The peak season for the onset of brucellosis was summer (47.1%, 66/140); 92.9% (130/140) had a confirmed epidemiological history; 7.9% (11/140) of the affected children were latent carriers. Among the 129 children with obvious clinical manifestations, fever accounted for 99.2% (128/129), hyperhidrosis accounted for 52.7% (68/129), and involvement of the reticuloendothelial system, joints, and nervous system accounted for 46.5% (60/129), 44.2% (57/129), and 6.2% (8/129), respectively, joint effusion accounted for 64.9% (37/57). All patients tested positive for tiger red plate agglutination test; 97.9% (137/140) of the patients had a serum tube agglutination test titer ≥1 ∶ 100. Positive blood culture accounted for 85.7% (120/140). The positive detection rate of serum tube agglutination test was higher than that of blood culture (χ 2 = 13.69, P < 0.001). Brucella was cultured from the cerebrospinal fluid of 8 children with concomitant encephalitis. The initial treatment plan for all children was oral compound sulfamethoxazole/doxycycline + rifampicin. In cases of arthritis, osteomyelitis or encephalitis, third-generation cephalosporins or meropenem were used intravenously at the same time. Dexamethasone was used for patients suffered from bilateral abductor nerve damage. All patients were cured without recurrence. Two cases had sequelae, namely introverted vision in the right eye and knee joint movement disorders. Conclusions:Most children with acute brucellosis have a confirmed epidemiological history and are mainly distributed in rural areas. The main clinical manifestation is fever, and joint involvement is also common. Standardized treatment can cure them.
