1.Predictive value of triglyceride-high-density lipoprotein cholesterol-glucose-body mass index andtriglyceride-glucose-body mass index in early pregnancy for gestational diabetes mellitus and pregnancy outcomes

Objective:To investigate the predictive value of the triglyceride-high-density lipoprotein cholesterol-glucose-body mass index (TyHGB) and triglyceride-glucose-body mass index (TyG-BMI) in early pregnancy for gestational diabetes mellitus (GDM) and pregnancy outcomes.Methods:A retrospective analysis was conducted, a total of 600 pregnant women who underwent prenatal examinations at North China Medical and Health Group Xingtai General Hospital from August 2020 to August 2023 were collected. Participants were divided into GDM group (76 cases) and non-GDM group (524 cases) based on GDM occurrence, and further categorized into adverse pregnancy group (101 cases) and favorable pregnancy group (499 cases) based on outcomes. Multivariate Logistic regression was used to identify risk factors for GDM and adverse pregnancy. The predictive performance of TyHGB and TyG-BMI was evaluated using receiver operating characteristic (ROC) curves.Results:After adjusting for confounders, TyHGB and TyG-BMI were independent risk factors for GDM ( OR = 1.569, 95% CI 1.259 -1.955; OR = 1.109, 95% CI 1.029 - 1.038) and adverse pregnancy ( OR = 1.438, 95% CI 1.193 - 1.734; OR = 1.021, 95% CI 1.014 - 1.029). The area under the curve (AUC) for TyHGB and TyG-BMI alone in predicting GDM was 0.677 and 0.731, respectively. Combined prediction of GDM yielded an AUC of 0.793, with sensitivity of 72.37% and specificity of 65.81%. For adverse pregnancy, the AUC of TyHGB and TyG-BMI alone were 0.666 and 0.692, respectively, while their combination achieved AUC of 0.746, with specificity of 69.34% and sensitivity of 65.35%. Conclusions:TyHGB and TyG-BMI in early pregnancy are risk factors for GDM and adverse pregnancy outcomes. Their combination can enhance predictive efficacy for both conditions.
2.Mechanism of resveratrol in treatment of sleep disorders in mice based on network pharmacology and experimental verification

Objective:To explore the mechanism of resveratrol in treatment of sleep disorders in mice based on network pharmacology and experimental verification.Methods:Resveratrol targets were obtained through traditional Chinese medicine systems pharmacology database and analysis platform, Swiss Target Prediction and SuperPred databases. Targets related to sleep disorders were collected from the gene expression omnibus, GeneCards, DrugBank, OMIM, CTD and MalaCards databases. Cytoscape 3.8.0 software was used to identify the core targets of resveratrol in treatment of sleep disorders via protein-protein interaction analysis. Gene ontology (GO) and Kyoto encyclopedia of genes and genomes (KEGG) pathway enrichment analyses were performed, and AutoDockvina software was used to conduct molecular docking between the top five core targets and resveratrol. Thirty ICR female mice were selected and divided into a control group, a model group, and low-, medium- and high-dose (10, 20, and 30 mg/kg) resveratrol groups according to the random number table method, with 6 mice in each group. Resveratrol was dissolved in 0.9% sodium chloride aqueous solution and 3% dimethyl sulfoxide. For mice in the low-, medium-, and high-dose resveratrol groups, 10, 20, and 30 mg/kg of resveratrol solution were intraperitoneally injected, respectively. The mice in the control group and the model group were intraperitoneally injected with 10 ml/kg of 0.9 % sodium chloride aqueous solution and 3 % dimethyl sulfoxide. Each group was injected once a day for 7 consecutive days. After 7 days of administration, a mouse sleep deprivation model was constructed using an improved multi platform water environment method. The spatial recognition and memory abilities of mice were tested using the Y-maze experiment, and the mRNA and protein relative expression levels of estrogen receptor 1 (ESR1) and B-cell lymphoma-2 (Bcl-2) were detected using real-time reverse transcription-PCR and Western blotting, respectively. Comparisons were made using t-test. Results:There were 47 common targets between resveratrol and sleep disorders, and the top five core targets were ESR1, Bcl-2, cytochrome P450, family 1, subfamily A, polypeptide 1 (CYP1A1), cytochrome P450, family 2, subfamily A, polypeptide 4 (CYP3A4) and cytochrome P450, family 1, subfamily B, polypeptide 1 (CYP1B1). GO enrichment analysis revealed that resveratrol in treatment of sleep disorders was mainly related to the regulation of unsaturated fatty acid metabolic processes, secondary metabolic processes, and responses to exogenous stimuli. KEGG pathway analysis showed that resveratrol acted mainly through pathways involving chemical carcinogenic-DNA adducts, cytochrome P450 metabolism of exogenous substances and estrogen signaling pathways. Molecular docking results indicated that resveratrol had strong binding affinity with ESR1, Bcl-2, CYP1A1, CYP3A4 and CYP1B1, with binding energies of ?31.92, ?27.72, ?34.44, ?34.02 and ?31.92 kJ/mol, respectively. In vivo experiments results demonstrated that the novel arm residence time in the low-, medium- and high-dose resveratrol groups [(52±5, 55±7, and 61±9) s] was higher than that in the model group [(45±4) s] ( P<0.05 or 0.01), and the percentage of spontaneous alternation [(53±4)%, (57±5)%, and (65±7)%] was higher than that in the model group [(47±3)%] ( P<0.05 or 0.01). The relative expression levels of ESR1 mRNA (0.42±0.10, 0.49±0.11, and 0.58±0.10) were higher than those in the model group (0.29±0.06) ( P<0.05 or 0.01). The relative expression levels of Bcl-2 mRNA (0.56±0.07, 0.65±0.10, and 0.77±0.11) were higher than those in the model group (0.44±0.08) ( P<0.05 or 0.01). Similarly, the relative expression levels of ESR1 protein (0.32±0.02, 0.50±0.02, and 0.62±0.02) were higher than those in the model group (0.24±0.01) ( P<0.05 or 0.01). The relative expression levels of Bcl-2 protein (0.45±0.08, 0.69±0.06, and 0.72±0.06) were higher than those in the model group (0.17±0.04) ( P<0.05 or 0.01). Conclusions:Resveratrol exerts therapeutic effects on sleep disorders by acting on ESR1 and Bcl-2.
3.Effect of lymphatic circulation reconstruction under supermicrosurgery in secondary lymphedema of lower limbs

Objective To analyze the efficacy of vascularized lymph node transplantation(VLNT)combined with lymphatic venous anastomosis(LVA)in the treatment of secondary lower extremity lymphedema assisted by super microsurgical techniques.Methods A retrospective analysis was performed for 15 patients with secondary lower limb lymphedema who underwent VLNT+LVA surgery in the Department of Reconstructive and Reconstructive Microsurgery of Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine from July 2021 to July 2023,and compared the circumference and volume of each segment of the lower limb between the preoperative and postoperative 90 days.LVA according to the results of ICG examination,3-5 parts of the lower limb were selected for"Z"shaped surgery,and the lymphatic vessels and venules were anastomosed under the microscope in the subcutaneous fat layer.VLNT confirmed and labeled the saphenous branch of the descending knee artery and its accompanying veins in the popliteal fossa,and the latissimus dorsi lymph node flap was incised,and the flap vessels were anastomosed with the saphenous branch of the descending knee artery and its accompanying veins.Postoperative observation of flap vascularization.The circumference and volume of the affected limb were measured before surgery and 90 days after surgery.Results A total of 15 patients with lower extremity lymphedema were included without serious complications.Statistical analysis showed that the circumference of all levels of the affected limb and the volume of the affected limb were improved 90 days after operation compared with those before surgery.Among them,the limb circumference and volume at each level from the highest point of the dorsum of the foot to 52 cm above the ankle improved 90 days after the operation compared with those before the operation.Conclusion LNT+LVA treatment for secondary lower extremity lymphedema can effectively control edema and improve the function of the affected limb.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
7.Clinical Characteristics and Medium to Long-term Follow-ups of Surgery for 23 Cases of Clear Cell Papillary Renal Cell Tumor

Objective To investigate the clinicopathologic characteristics of clear cell papillary renal cell tumor and the medium to long-term effect of surgical treatment.Methods A total of 23 cases of clear cell papillary renal cell tumor treated in our department from October 2013 to January 2024 were retrospectively analyzed.For patients undergoing partial nephrectomy,the renal artery was dissociated and blocked through abdominal or retroperitoneal approach to remove the tumor,and then the wound was sutured.For patients undergoing radical nephrectomy,the renal artery,vein,and ureter were dissociated and clipped by Hem-o-lok for cutting off,and then the kidney was removed.Results The operations were accomplished in all the patients,including 18 laparoscopic operations and 5 robot-assisted laparoscopic operations.Among them,17 cases underwent partial nephrectomy,with an operating time of 73-229 min (median,149 min),blocking time of 9-35 min (median,21 min),blood loss of 10-100 ml ( median,20 ml),and postoperative hospitalization time of 3-28 d ( median,6 d).Six cases underwent radical nephrectomy,with an operating time of 110-232 min ( median,123 min),blood loss of 5-200 ml ( median,10 ml),and postoperative hospitalization time of 3-7 d ( median,4 d).Postoperative pathology showed clear cell papillary renal cell tumor with nuclear grade ( WHO/ISUP grade) of Ⅰ-Ⅱ.The 23 cases were followed up for 7-121 months (mean,53 months),of which 10 cases were followed up for more than 3 years and 9 cases for more than 5 years.One case was found to have contralateral renal lesions one year after operation and underwent laparoscopic partial nephrectomy,with a pathological result of clear cell papillary renal cell tumor.Subsequent re-examinations showed that there were cysts in both kidneys.At 7 years after the first operation,solid nodules in both kidneys were found,and a recurrence was considered.The patient was given active monitoring for 2 years ( CT re-examinations every 3-6 months) and was in stable condition.Another case was found gastric cardia tubular adenocarcinoma at 29 months after operation and was resected under gastroscopy.No recurrence was found during follow-ups for 121 months.The remaining 21 cases had no recurrence.Conclusions Preoperative diagnosis of clear cell papillary renal cell tumor is difficult,and surgery is an effective method for treating this disease.Medium to long-term follow-up shows a good prognosis,but there are still some cases of recurrence or combined with multiple primary cancer.Postoperative follow-ups should be noted.
8.Mechanisms of ionizing radiation-induced pyroptosis in human intestinal epithelial cells

Objective To investigate the mechanism underlying gasdermin E(GSDME)-mediated pyroptosis in radiation-induced intestinal injury and to find out whether gasdermin(GSDM)family members regulate pyroptosis through similar signaling pathways.Methods Human normal colon epithelial cells(NCM460)and human colon cancer cells(HT-29)were exposed to radiation of different doses and durations before pyroptosis indicators were evaluated by observing pyroptotic bubbles,cell survival,and the cleavage of pyroptosis execution proteins.HT-29 cells overexpressing GSDME were subjected to radiation,followed by enrichment analysis of pyroptosis-related differentially expressed genes using RNA-seq.Results Radiation induced substantial pyroptosis in NCM460 cells.Overexpression of GSDME in HT-29 cells resulted in substantial radiation-induced pyroptosis.The pyroptosis state of human intestinal cells was simulated in the HT-29 model cell line.Overexpressions of GSDME-N and GSDMD-N resulted in the expression of more than 50% of the differentially expressed genes in the pyroptosis state.Sequencing analysis showed that the genes in the pyroptosis state were mainly overrepresented in immune response,inflammatory response,and Rapl signaling pathway.Conclusion GSDME activation can mediate radiation-induced pyroptosis by producing GSDME-N fragments.GSDM family members participate in pyroptosis in a similar mode of regulation.Furthermore,radiation-induced activation of GSDME/D may regulate pyroptosis through immune response,inflammatory response,and Rap1 signaling pathway.
9.Research on community-based rehabilitation exercise and physical activity and its benefits for people with mild and moderate disability in Shenzhen, Guangdong, China

ObjectiveTo investigate the effect of community-based rehabilitation exercise and physical activity on the physical activity levels, functional states, and quality of life for people with mild and moderate disabilities in community settings, to ascertain the health benefits of their engagement in such fitness sports activities, anchoring in the WHO "Global Action Plan on Physical Activity 2018-2030: More Active People for a Healthier World" and the guidelines tailored for people with disabilities, using the theoretical framework and methodology of the International Classification of Functioning, Disability and Health (ICF). MethodsAligning with the WHO guidelines for people with disabilities and the ICF, a community-based rehabilitation exercise and physical activity program was designed for individuals with mild to moderate disabilities, featuring activities like fitness training (aerobic and resistance exercises), skill exercises (such as balance and coordination), as well as sports and recreational games (ball and games, etc.). A total of 230 people with mild and moderate disabilities were recruited from 20 communities in Shenzhen, with types of disabilities including physical, speech, intellectual, and mental. Professional rehabilitation fitness instructors implemented and supervised the program. The activities were of low to moderate intensity, 30 to 50 minutes per session, five times a month for six months. The community-based rehabilitation exercise and physical activity progress of these individuals was surveyed using the International Physical Activity Questionnaire (IPAQ), and their overall functioning was evaluated with WHODAS 2.0. The health-related quality of life was measured with WHOQOL-BREF. The health benefits from participation in community-based rehabilitation exercise and physical activity were assessed in terms of functioning, activity involvement, and quality of life. ResultsAfter the fitness activities, participation levels significantly increased in IPAQ domains of work-related, transport-related, domestic and gardening activity, and leisure time (|t| > 3.391, P < 0.001). The scores significantly decreased in the domains of cognition, activity, self-care, getting along, life activities and participation; and overall scores also decreased in WHODAS 2.0 (t > 6.639, P < 0.001). The scores significantly increased in the four dimensions of WHOQOL-BREF (|t| > 7.486, P < 0.001). ConclusionAfter participating in a six-month community-based rehabilitation exercise and physical activity program of mild to moderate intensity, individuals with mild to moderate disabilities have improved in physical activity and engagement levels, and the overall functioning and quality of life.
10.Incidence and risk factors of deep vein thrombosis in patients with rheumatoid arthritis

Objective:To investigate the incidence and risk factors of deep vein thrombosis(DVT)in patients with rheumatoid arthritis(RA).Methods:The clinical data of RA patients who were hospi-talized in the Department of Rheumatology and Immunology of Aerospace Center Hospital from May 2015 to September 2021 was retrospectively analyzed,including demographic characteristics,concomitant diseases,laboratory examinations(blood routine,biochemistry,coagulation,inflammatory markers,rheumatoid factor,antiphospholipid antibodies and lupus anticoagulant,etc.)and treatment regimens.The patients were compared according to the presence or absence of DVT,and the t test,Mann-Whitney U test or Chi-square test were applied to screen for relevant factors for DVT,followed by Logistic regres-sion analysis to determine risk factors for DVT in patients with RA.Results:The incidence of DVT in the RA patients was 9.6%(31/322);the median age of RA in DVT group was significantly older than that in non-DVT group[64(54,71)years vs.50(25,75)years,P<0.001];the level of disease activity score using 28 joints(DAS28)-erythrocyte sedimentation rate(ESR)in DVT group was higher than that in non-DVT group[5.2(4.5,6.7)vs.4.5(4.5,5.0),P<0.001];the incidence of hypertension,chronic kidney disease,fracture or surgery history within 3 months,and varicose veins of the lower ex-tremities in DVT group was higher than that in non-DVT group(P<0.001).The levels of hemoglobin and albumin in DVT group were significantly lower than that in non-DVT group(P=0.009,P=0.004),while the D-dimer level and rheumatoid factor positive rate in DVT group were significantly higher than that in non-DVT group(P<0.001).The use rate of glucocorticoid in DVT group was higher than that in non-DVT group(P=0.009).Logistic regression analysis showed that the age(OR=1.093,P<0.001),chronic kidney disease(OR=7.955,P=0.005),fracture or surgery history with-in 3 months(OR=34.658,P=0.002),DAS28-ESR(OR=1.475,P=0.009),and the use of glu-cocorticoid(OR=5.916,P=0.003)were independent risk factors for DVT in RA patients.Conclu-sion:The incidence of DVT in hospitalized RA patients was significantly increased,in addition to tradi-tional factors,such as age and chronic kidney disease,increased DAS28-ESR level and the use of glu-cocorticoid were also independent risk factors for DVT.
