Irritable bowel syndrome (IBS) is a functional bowel disorder characterized primarily by abdominal pain and altered defecation habits. In recent years, traditional Chinese medicine (TCM) has made progress in multiple aspects of IBS research and treatment, including syndrome distribution, development of TCM formulas, clinical efficacy evaluation, external therapies, and psychosocial regulation. However, it still faces challenges such as over-reliance on symptomatic manifestations rather than biomarkers for diagnostic criteria, and the lack of high-quality evidence-based data supporting the efficacy of TCM formulas in treating IBS. This paper proposed that TCM diagnosis and treatment of IBS should adhere to the strategy of integrating the holistic concept with syndrome differentiation and treatment, combining TCM external therapies such as acupuncture, moxibustion and acupoint application), and emphasizing individualized diagnosis and treatment for psychosomatic abnormalities. Future research should integrate multi-omics technologies, artificial intelligence and other methods to deepen the understanding of the pathogenesis of IBS and the mechanisms of TCM formulas, so as to promote the standardization and internationalization of TCM in the diagnosis and treatment of IBS.

Chronic gastritis is a chronic inflammation of the gastric mucosa caused by various etiologies and can be categorized into chronic non-atrophic gastritis and chronic atrophic gastritis.Chronic atrophic gastritis is a common disorder of the digestive system characterized by gastric mucosal gland atrophy, mucosal thinning, and basal layer thickening. The development of intestinal metaplasia and atypical hyperplasia on this basis is recognized as a precancerous lesion of gastric cancer and represents a key stage in the"inflammation-cancer transformation"of chronic gastritis. However, universally recognized and effective treatment strategies for this"inflammation-cancer transformation"process are lacking in clinical practice. This study integrates Correa′s cascade reaction with clinical practice, summarizing the pathogenesis of the"inflammation-cancer transformation"of chronic gastritis as weakness of the middle jiao and blood stasis. It suggests that the"inflammation-cancer transformation"process involves the pathological development of spleen and stomach deficiency, transportation and transformation dysfunction, turbid phlegm, blood stasis, and the gradual formation of cancerous toxins, with spleen and stomach weakness as the core mechanism and phlegm and blood stasis as the crucial pathological link.Based on an in-depth exploration of the deficiency of the middle jiao and blood stasis, supported by pharmacological research and clinical experience, this paper proposes the therapeutic approach of strengthening the spleen and replenishing qi,expelling phlegm and activating blood. It discusses the related prescriptions in preventing and treating the"inflammation-cancer transformation"of chronic gastritis. This study aims to provide new perspectives and insights for the prevention and treatment of chronic gastritis with traditional Chinese medicine, offering a novel framework for clinical treatment.

AIM:To evaluate the clinical efficacy of 0.05% cyclosporine eye drops(Ⅱ)combined with sodium hyaluronate eye drops in treating patients with type 2 diabetes mellitus(T2DM)and moderate-to-severe dry eye.METHODS:A total of 120 T2DM patients(120 eyes)with moderate-to-severe dry eye, treated at the endocrinology and ophthalmology departments at the Affiliated Hospital of Xuzhou Medical University from January 2024 to September 2024, were enrolled in the study. The patients were randomly divided into two groups: combination group [0.05% cyclosporine eye drops(Ⅱ)+ sodium hyaluronate eye drops] and control group(sodium hyaluronate eye drops alone), with 60 cases(60 eyes)in each group. Assessments were conducted at baseline and at 1, 2, and 3 mo post-treatment, including the ocular surface disease index(OSDI), non-contact tear meniscus height(NITMH), first non-invasive tear breakup time(NIBUTf), meibomian gland loss score, lipid layer thickness grade, conjunctival hyperemia grade, and corneal fluorescein staining(FL)score. At 3 mo after treatment, changes in tear inflammatory factors were observed, and corneal subbasal nerve plexus(SBN)morphology/density were analyzed using in vivo confocal microscopy(IVCM).RESULTS:At 1, 2, and 3 mo post-treatment, both groups showed statistically significant increases in NITMH and NIBUTf compared to baseline(all P<0.05), with greater improvement observed in the combination group(both P<0.05). OSDI and FL scores significantly decreased from baseline(all P<0.05), with more pronounced reductions in the combination group(both P<0.05). Meibomian gland loss scores showed no significant improvement in either group(all P>0.05). At 3 mo after treatment, tear levels of interleukin 6(IL-6)and matrix metalloproteinase-9(MMP-9)significantly decreased in both groups(all P<0.001), with a greater reduction noted in the combination group(both P<0.001). The combination group displayed increased corneal nerve branch density and nerve fiber density, along with decreased nerve tortuosity and dendritic cell(DC)density compared to baseline(all P<0.001), while the control group did not show significant changes(all P>0.05).CONCLUSION: The combination of 0.05% cyclosporine eye drops(Ⅱ)and sodium hyaluronate eye drops significantly improves clinical outcomes in T2DM patients with moderate-to-severe dry eye. This treatment effectively alleviates ocular surface inflammation, restores corneal nerve morphology and density, and demonstrates a favorable safety profile.

Objective:To evaluate the role of hippocampal β4-Subunit-Containing Nicotinic Acetylcholine Receptors (Chrnb4) in postoperative delirium in aged mice.Methods:Forty-eight SPF male C57BL/6J mice, aged 18 months, weighing 29-35 g, were assigned into 6 groups using a random number table method: tibial fracture group (TF group, n=6), sham operation group (Sham group, n=6), tibial fracture + adipenine group (TA group, n=9), tibial fracture + control vehicle group (TV group, n=9), sham operation + adipenine group (SA group, n=9), and sham operation + control vehicle group (SV group, n=9). The postoperative delirium model was prepared by tibial fracture under sevoflurane anesthesia. Tibial fracture was simulated by implanting a steel pin into the tibia and then clamping it, while sham group only received a longitudinal incision and suture after anesthesia. A microinjection cannula was implanted into the mouse skull at 5 days before developing the model in TA group, TV group, SA group and SV group. Three mice from each group were randomly selected for microelectrode implantation in the hippocampal CA1 area. Starting from 30 min after surgery, adipenine (62.5 nmol/μl) 2 μl was infused into the cerebral ventricle for 7 consecutive days in TA and SA groups, and vehicle (2 μl) was administered instead at a 24-h interval for 7 consecutive days in TV and SV groups. The expression of Chrnb4 mRNA in the hippocampal tissues was detected by real-time quantitative polymerase chain reaction at 24 h after surgery. On the 7th and 8th days after surgery, the open-field test and O-maze experiment were conducted to assess the impulsive-like behavior in TA, TV, SA and SV groups. After the behavioral test, the expression of glial fibrillary acidic protein (GFAP) and vesicular glutamate transporter 1 (Vglut1) in the hippocampal CA1 region was detected by immunofluorescence staining. The local field potentials in the hippocampal CA1 region were recorded during the open field test. Results:The expression of Chrnb4 mRNA in the hippocampal tissues was significantly up-regulated in TF group compared to Sham group ( P<0.05). Compared with TV group, the percentage of central path distance in the open field test and percentage of time spent in the open arms of the O-maze were significantly decreased, the power of β-waves in the CA1 field potentials was decreased, the expression of GFAP and Vglut1 in the hippocampal CA1 region was down-regulated, and the co-staining area of GFAP + and Vglut1 + was decreased in TA group and SV group ( P<0.05). There was no significant difference in each parameter between SA group and SV group ( P>0.05). Conclusions:Hippocampal Chrnb4 may be involved in the mechanism of postoperative delirium in aged mice, and this process may be related to inhibition of neuron excitotoxicity.

Chinese Journal of Radiation Oncology 2024;33(10):893-901
Concurrent chemoradiotherapy (CCRT) refers to the simultaneous treatment of chemotherapy and radiotherapy, and the effect of radiotherapy is enhanced with low-dose chemotherapy, which can reduce tumor recurrence and metastasis and improve clinical prognosis of patients. At present, the main factors for the increase of radiosensitivity of concurrent chemotherapy is that concurrent chemotherapy prevents the repair of tumor cells, and chemotherapy and radiotherapy act on different cell cycles and have synergistic effects. However, even for patients with locally advanced cervical cancer (LACC) who have undergone CCRT, the 5-year survival rate is only 60%, which is still not ideal. In order to improve the efficacy, researchers have conducted a series of exploratory studies, which consist of the combination of targeted drugs and immunodrugs, and neoadjuvant regimens before CCRT, etc. Although targeted or immunologic drugs are effective treatment of LACC, in view of the lack of large-scale evidence-based medical evidence, multi-center prospective and randomized phase III clinical trials and high-level articles are needed to improve the level of evidence-based medicine. This consensus summarizes several key evidence-based medical studies published recently, especially the clinical research progress in targeted and immunological therapies, providing reference for domestic peers.

Objective To detect the expression profile of heme oxygenase-1(HO-1)during the process of wound repair in radiation-wound combined injury(R-W-CI),and evaluate its wound healing improving effects of R-W-CI by HO-1 knockdown with siRNA.Methods A total of 36 male C57BL/6J mice(8 weeks old)were randomly and equally divided into a simple skin wound group(W group)and a skin wound group combined with whole-body radiation(6 Gy)injury(R-W-CI group).During the wound healing process, the wounds were photographed and recorded, and the residual areas were quantified by Image J.Wound tissues were sampled and stained with HE staining for pathological and histological observation, and the damage to the hematopoietic system was assessed by dynamic examination of the peripheral blood. The expression and changes of HO-1 in wound tissues were detected by q-PCR and Western blotting.Then,26 male C57BL/6J mice(8 weeks old)were randomly and equally divided into siRNA knockdown HO-1 group(si-HO-1 group)and siRNA negative control group(si-NC group). After radiation combined injury was inflicted,60 μL of F127 gel loaded with si-HO-1(5 μm/L)was applied to each wound in the si-HO-1 group, and an equal amount of F127 gel loaded with negative control si-NC was applied to the wound in the si-NC group. The knockdown of HO-1 in wound tissues was detected by Western blotting, and the changes in wound area were observed. In the wound tissues harvested in 3 d after wounding, the expression of cytokines IL-1β,IL-6 and TNF-α was examined by q-PCR and the proliferation of granulation tissues was evaluated by Ki67 immunohistochemical staining.HE staining was performed on wound tissues on day 3 and day 9 post-injury to assess the improvement effect of knockdown of HO-1 on wound healing of radiation combined injuries. Results Compared with the W group,semi-quantitative analysis of the residual wound area showed that healing was significantly delayed in the R-W-CI group on days 7 and 10 post-injury(P<0.01).HE staining on day 7 showed that in the R-W-CI group, the re-epithelialization was delayed, and the growth of granulation tissues was poor;and at the same time, peripheral blood leukocytes and their classified counts showed a significant decrease in the early period after injury(P<0.05).Further tests indicated that the expression of HO-1 protein was slightly higher in the wound of the R-W-CI group than that of the W group in 3 and 7 d after injury, though no significant difference(P>0.05),whereas statistical difference was seen in 10 d(P<0.05),accompanied by the distribution of the full-length and truncated forms of HO-1 protein.Quantitative PCR obtained similar results in the mRNA expression of HO-1 in wounds in both 7 and 10 d after injury(P<0.05).siRNA intervention could effectively knock down the HO-1 protein level of the wounds(P<0.05),promote wound contraction(P<0.05),reduce the width of the wound(P<0.01),up-regulate the inflammatory cytokines IL-6 and TNF-α in 3 d,enhance the proliferation of repair cells in wound margin, and improve the growth of the granulation tissue in the R-W-CI model when compared with the conditions after si-NC intervention. Conclusion There exists a sustained high expression level of HO-1 during wound repair, and wound knockdown of HO-1 by siRNA can improve the lack of inflammation status and promote wound healing in R-W-CI mice.
